Background: Evidence-based health policies are widely acknowledged as being essential for attaining ongoing improvements in health outcomes. This study represents a theoretical model to test and identify to identify the key drivers for adopting evidence-based healthcare practice (EBHP). Objectives: The goal of this study was to investigate the critical success factors for the adoption of evidence-based healthcare at a South African public hospital. Methods: Medical healthcare professionals were surveyed using a questionnaire developed by the researcher. The results were analysed with SPSS 23 and AMOS. A hypothetical model based on updated (D&M) IS Success Model, Technology, Organization and Environment framework (TOE) as well as Technology Acceptance Model (TAM) was developed and analysed using structural equation modelling (SEM). Results: The findings of this study indicate that EHR is crucial and the main construct influencing the adoption of EBHP. The findings of this study indicated a positive relationship between the construct electronic health records (EHR) with medical error reduction (MER), information quality (IQ) and knowledge quality (KQ), all showed a strong positive correlation towards the adoption of EBHP. Conclusions: Better coordination of patient care (BCP) typically results in lower health care costs. The results also show that using EBHP improves the accuracy of disease diagnosis and treatment, which lowers the burden of disease. Contribution: The developed framework will help healthcare systems that are strapped for money, particularly in developing countries.

@#Bicyclo [1.1.1] pentane (BCP) is a kind of bridged ring skeleton with a three-dimensional structure. As a bioisostere of benzene ring, tert-butyl and alkyne, it has been widely used in the field of medicinal chemistry.With the expansion of its application, the synthesis of BCP and its derivatives has become an increasingly attractive research hotspot for scientists.Based on this, the present article summarizes the main synthetic strategies and methods of BCP derivatives, in the hope of providing some reference for drug discovery researchers and some insight for new synthetic methods of such compounds.

B-cell precursor acute lymphoblastic leukemia (BCP-ALL) is characterized by genetic alterations with high heterogeneity. Precise subtypes with distinct genomic and/or gene expression patterns have been recently revealed using high-throughput sequencing technology. Most of these profiles are associated with recurrent non-overlapping rearrangements or hotspot point mutations that are analogous to the established subtypes, such as DUX4 rearrangements, MEF2D rearrangements, ZNF384/ZNF362 rearrangements, NUTM1 rearrangements, BCL2/MYC and/or BCL6 rearrangements, ETV6-RUNX1-like gene expression, PAX5alt (diverse PAX5 alterations, including rearrangements, intragenic amplifications, or mutations), and hotspot mutations PAX5 (p.Pro80Arg) with biallelic PAX5 alterations, IKZF1 (p.Asn159Tyr), and ZEB2 (p.His1038Arg). These molecular subtypes could be classified by gene expression patterns with RNA-seq technology. Refined molecular classification greatly improved the treatment strategy. Multiagent therapy regimens, including target inhibitors (e.g., imatinib), immunomodulators, monoclonal antibodies, and chimeric antigen receptor T-cell (CAR-T) therapy, are transforming the clinical practice from chemotherapy drugs to personalized medicine in the field of risk-directed disease management. We provide an update on our knowledge of emerging molecular subtypes and therapeutic targets in BCP-ALL.

Objective • To investigate the mechanism of spinal chemokine C-C motif receptor 2 (CCR2)-mediated maintenance of bone cancer pain (BCP) in rats. Methods • Fifty-four SD rats were divided into BCP group, sham operation group, BCP+INCB3344 (CCR2 specific antagonist) group, and BCP+vehicle control group. Walker256 breast cancer cells were injected into the tibia medullary cavity of rats in the BCP group to establish the BCP model, while the rats in the sham operation group were injected with the same amount of saline. The rats in the BCP+INCB3344 group received intrathecal injection of INCB3344 on the 14th day after the establishment of BCP model, while the BCP+vehicle control group rats were injected with the same amount of vehicle. The mechanical pain thresholds of BCP group rats and sham operation group rats were measured to judge the success of BCP model. The expressions of CCR2 in the dorsal horn of spinal cord in the sham operation group rats and the BCP group rats were detected by Western blotting. The effects of intrathecal administration of INCB3344 on the mechanical pain threshold of BCP rats were observed by mechanical pain behavior test. Whole-cell patch-clamp recordings were used to investigate the differences of spontaneous excitatory postsynaptic currents (sEPSCs), α-amino-3-hydroxy-5-methyl-4-isoxazole-propionicacid (AMPA) and N-methyl-D-aspartic acid (NMDA)-induced currents of spinal substantia gelatinosa (SG) neurons of rats in the BCP group, the BCP+INCB3344 group and the BCP+vehicle control group. Results • Compared with the sham operation group, the mechanical pain threshold of BCP group rats reduced significantly on the 14th day after operation (P=0.000), and the expression of CCR2 in ipsilateral spinal cord of BCP group rats increased significantly (P=0.009). After intrathecal injection of INCB3344 for 4 h, the mechanical pain threshold of BCP+INCB3344 group rats was significantly increased (P=0.002). The frequency and amplitude of sEPSCs and the amplitude of AMPA and NMDA-induced currents in SG neurons of BCP group rats were significantly higher than those of the sham operation group rats (all P=0.000), while intrathecal administration of INCB3344 could significantly inhibit the above-mentioned indices in the BCP+INCB3344 group (all P<0.05). In addition, extracellular perfusion of INCB3344 could also significantly inhibit the frequency (P=0.001) and amplitude (P=0.020) of sEPSCs in SG neurons in BCP rats. Conclusion • CCR2 expressing in the spinal cord mediates the enhancement of excitatory synaptic transmission efficacy in the spinal dorsal horn of BCP rats by enhancing the functions of AMPA and NMDA receptors, which may be an important mechanism for the maintenance of BCP.

Resumo O objetivo deste estudo é investigar a associação da posição socioeconômica e comorbidades com o autorrelato da deficiência. Dados provenientes de inquérito populacional em Belo Horizonte, entre 2008 e 2009. Amostragem foi probabilística, estratificada por conglomerados em três estágios: setor censitário, domicílio e indivíduos. A variável resposta foi deficiência, definida a partir do autorrelato de problemas nas funções ou nas estruturas do corpo. As variáveis explicativas foram: sexo, idade, morbidade referida e índice da posição socioeconômica que incluiu variáveis de escolaridade materna, do entrevistado e renda familiar. Empregou-se a análise fatorial para avaliar a composição do índice da posição socioeconômica e análise de regressão logística. A prevalência de deficiência foi de 10,43%. O autorrelato de deficiência associou-se à idade (OR = 1,02; IC 95%: 1,01-1,03), ao relato de duas ou mais doenças (OR = 3,24; 2,16-4,86) e ao índice da posição socioeconômica (OR = 0,96; IC 95%: 0,95-0,97). A pior posição socioeconômica e a ocorrência de doenças parecem contribuir para a ocorrência de deficiência. Esses resultados evidenciam as iniquidades em saúde entre as pessoas com deficiência e a relevância do BPC no atendimento a populações vulneráveis.


Abstract This study aims to investigate the association of socioeconomic status and comorbidities of self-reported disability. Data were obtained from a population survey in Belo Horizonte from 2008 to 2009. The sample was probabilistic and stratified by conglomerates in three stages: census tracts, households and individuals. The outcome variable was disability, defined by the self-reported problems in bodily functions or structures. The explanatory variables were gender, age, self-reported morbidity and socioeconomic status index that included variables mother and respondent schooling and household income. The factorial analysis was used to evaluate the socioeconomic status index and logistic regression. The prevalence of disability was 10.43% (95% CI: 9.1-11.7%). Self-reported disability was associated with age (OR = 1.02; 95% CI: 1.01-1.03) and reporting of two or more diseases (OR = 3.24; CI 95%; 2.16-4.86) and socioeconomic status index (OR = 0.96; 95% CI: 0.95-0.97). The worse socioeconomic status and occurrence of diseases appear to contribute to the occurrence of disability. These results show health inequities among people with disabilities, and BPC relevance supporting vulnerable populations.

Aim To assess the effects of Banqiao Codonopisis Pilosula(BCP)decoction on learning and memory dysfunction in AD model rats induced by high activity GSK-3β and its possible mechanism.Methods The SD rats(4 months old,♂)were divided into five groups,namely,sham-operated group(blank group),AD model group,BCP high-dose(2.16 g·kg-1·d-1)group,BCP medium-dose(1.08 g·kg-1·d-1)group,and BCP lower-dose(0.54 g·kg-1·d-1)group.Treatment group received BCP decoction by gavage once a day for 14 days,while other groups were offered drinking water by gavage once a day for 14 days.The autonomous behavior activities of all rats were observed and recorded after gavage.In the last seven days by gavage,Morris water maze test was used to test the spatial learning and memory ability of the five groups.After five days training,treatment groups and AD model group were injected wortmannin(WT,PI3K specific inhibitor)and GF-109203X(GFX,PKC specific inhibitor)(100 μmol·L-1 of each,total volume of 10 μL)into the right lateral ventricle of the rats.The blank group was only injected 2%DMSO.The spatial memory retention was detected by water maze 24 hours after lateral ventricle injection.Then,changes in the spatial learning memory of rats were observed.The level of Tau phosphorylation in SD rat hippocampus and the expression and activity changes of related protein kinase GSK-3β were detected by Western blot and immunohistochemistry.The changes of Nissl bodies in SD rat hippocampus were observed by Nissl′s staining.Results After intragastric administration of BCP,the rat autonomous behavior activities in each group all showed a declining trend,and the differences in low-dose and middle-dose groups had statistical significance compared with blank group.The Morris water maze tests showed that the latency navigation of model group was significantly longer than that of blank group(P<0.01),while that of the BCP three doses groups was shorter than that of model group(P<0.05).Compared with the same group,the latency navigation of the three groups after gavage BCP low,middle and high dose was significant shorter than that without gavage(P<0.05).Western blot results showed that the activity of GSK-3β in AD model group was up-regulated compared with the blank group.However,BCP inhibited activity of GSK-3β.Western blot and immunohistochemistry results showed the level of Tau phosphorylation in AD model group was increased compared with the blank group in the area of CA3(P<0.05).Compared with AD model group,the level of Tau phosphorylation was decreased in treatment group.Nissl′s staining results showed that dendritic spines in AD model group was significantly attenuated compared with the blank group(P<0.05).Far more dendritic spines were observed in treatment group than in AD model group.The number of Nissl′s bodies in neuron cells of hippocampus in hippocampal CA3 was obviously larger in treatment groups than in AD model group.These effect of BCP was dose-dependent.Conclusions BCP can prevent the learning and memory dysfunction in AD model rats induced by high activity of GSK-3β.The mechanism may be related to inhibiting GSK-3β activity and then reducing the level of phosphorylation of Tau and improving neural development.

BACKGROUND: Intrachromosomal amplification of chromosome 21 (iAMP21), defined as the presence of three or more RUNX1 signals on one marker chromosome, is a distinct cytogenetic subgroup of childhood B-cell precursor acute lymphoblastic leukemia (BCP-ALL) that is known to have a poor prognosis when treated with standard therapy. The aim of this study was to evaluate the clinical characteristics of Korean children with iAMP21. METHODS: The cytogenetic data from BCP-ALL children were reviewed. The ETV6/RUNX1 ES Dual Color Probe was used for fluorescence in situ hybridization (FISH). RESULTS: In total, 295 children were included. Of these, 10 patients (3.4%) had iAMP21. The median age of iAMP21 patients was 9 years, and the median value of white blood cell count was 5.09×10⁹/L. Slow early treatment response was observed more in iAMP21 patients. Patients with iAMP21 had a higher incidence of relapse and worse survival rates. In patients with iAMP21, the estimated 10-year cumulative incidence of relapse was 53.3%. The estimated 10-year event-free survival and overall survival rate were 46.7% and 64.8%, respectively. Most cases of leukemic relapse developed in the late period (median, 43 mo). In multivariate analysis, high risk group was the only factor that had a significant impact on death. CONCLUSION: The existence of iAMP21 was related to delayed treatment response and was likely to affect increased relapse and death in the late period. Further studies are needed to reveal its effect on BCP-ALL treatment outcomes and its role as an independent prognostic factor.

Globally, about 50% of liver cancer originates as a result of long term infection with hepatitis B virus (HBV), and some genotypes and mutations have been associated with an increased severity of infection. The aim of this study was to evaluate the genetic diversity of HBV in patients from Venezuela, with chronic infection, cirrhosis and hepatocellular carcinoma (HCC) and to compare the occurrence of mutations in all patient groups. Samples from patients with different pathologies of the liver, associated with HBV infection, were collected. The HBV S region was analyzed for genotype determination and, when available, the whole genome sequence was examined for mutations analysis. Genotype F was the most common genotype (87%). While the HBV subgenotype F3 was the most frequent genotype in the whole group of samples (44%), the subgenotype F2 predominated in HCC patients (56%). Mutations were more common in HCC and cirrhosis cases (p=0.01). The A1762T mutation was significantly associated with the advanced stage of liver disease (p=0.008). Additionally, mutations were more common in early stages of liver disease in HBV subgenotype F2- infected patients, and a significant association between this subgenotype and the emergence of T1753C, A1762T, A1762T/G1764A (p=0.04) and C1773T (p=0.001) mutations in chronic patients was found, when compared to the HBV subgenotype F3. By comparing F2 with all other HBV subgenotypes, a positive association for the three basal core promoter (BCP) mutants (A1762T, A1762T/G1764A p=0.01, G1764A p=0.04) was found. These results suggest that the HBV subgenotype F2 might be associated to more severe forms of liver disease in comparison with the HBV subgenotype F3.


Mundialmente, alrededor del 50% del cáncer de hígado se origina como consecuencia de la infección a largo plazo con el virus de la hepatitis B (VHB), y algunos genotipos y mutaciones han sido asociados con severidad incrementada de la infección. El objetivo de este estudio fue evaluar la diversidad genética del VHB en pacientes de Venezuela con infección crónica, cirrosis y carcinoma hepatocelular (CHC) y comparar la ocurrencia de mutaciones en los tres grupos de pacientes. Se reunieron muestras de pacientes con diferentes patologías de la enfermedad del hígado asociada a la infección por VHB. La región S del VHB fue analizada para la determinación del genotipo y cuando estuvo disponible, la secuencia del genoma completo fue examinada para análisis de mutaciones. El genotipo F de VHB fue el más frecuente (87%). Mientras que el F3 fue el subgenotipo más encontrado en el grupo completo de muestras (44%), el F2 fue predominante en pacientes con CHC (56%). Las mutaciones fueron más comunes en casos de pacientes con cirrosis y CHC (p=0,01). La mutación A1762T estuvo asociada significativamente con estado avanzado de la enfermedad del hígado (p=0,008). Adicionalmente, las mutaciones fueron más comunes en estados tempranos de la enfermedad del hígado en pacientes infectados con el subgenotipo F2, encontrándose una asociación significativa entre este subgenotipo y la ocurrencia de las mutaciones T1753C, A1762T, A1762T/ G1764A (p=0,04) y C1773T (p=0,001) en pacientes crónicos, en comparación con el subgenotipo F3. Por otro lado, al comparar F2 con los demás subgenotipos de VHB, se encontró una asociación positiva para las tres mutantes del promotor basal de la cápside (PBC) (A1762T, A1762T/G1764A p=0,01, G1764A p=0,04). Estos resultados sugieren que el subgenotipo F2 de VHB puede estar asociado a formas más severas de la enfermedad del hígado en comparación al subgenotipo F3.

Debido al pleomorfismo y variabilidad macroscópica que presenta el género Trichophyton, 1os métodos de identificación basados exclusivamente en caracteres morfológicos no son suficientes para su clasificación. El objetivo fue evaluar las características morfológicas y bioquímicas de aislados clínicos identificados como Trichophyton spp. Se estudiaron 98 cultivos, identificados previamente por macro y micromorfología y se reevaluaron por: microcultivo en lámina, perforación del pelo in vitro, hidrólisis de urea y uso de agares Trichophyton y BCP-MS-G. El análisis morfológico y las pruebas de hidrólisis de urea y perforación del pelo arrojaron 51% T. rubrum, 24% T. mentagrophytes, 16,6% T. interdigitale, 7% T. tonsurans y 2% Trichophyton spp. La prueba de urea evidenció un error de 9,4%. La evaluación mediante el BCP-MS-G permitió diferenciar T. rubrum tipo A algodonoso de T. interdigitale, e identificar Trichophyton spp. como T. rubrum tipo B granular. La concordancia entre la evaluación previa y la reevaluación fue de un 70% y entre las pruebas morfofisiológicas y BCP-MS-G fue de 96%. Se detectó un 30% de sobrediagnóstico de T. rubrum. Se sugiere el empleo del medio BCP-MS-G para diferenciar T. interdigitale de T. rubrum tipo A y otros biotipos de T. rubrum tipo B.


Due to the pleomorphism and macroscopic variability of the Trichophyton genus, identification methods based solely on the morphological characteristics are insufficient for its classification. The objective was to evaluate the morphological and biochemical characteristics of clinical isolates of Trichophyton spp. The study included 98 isolates previously identified by macro and micromorphology that were reassessed by means of microculture sheet, in vitro hair perforation, urea hydrolysis and culture on Trichophyton and BCP-MS-G agars. For morphological analysis, urea hydrolysis tests and in vitro hair perforation results were: T. rubrum 51%, T. mentagrophytes 24%, T. interdigitale 16.6%, T. tonsurans 7% and Trichophyton spp. 2%. The urea hydrolysis test showed 9.4% error. Culture on the BCP-MS-G medium allowed to differentiate T. rubrum cottony A type from T. interdigitale and the identification of Trichophyton spp. as T. rubrum granular B type. The agreement between the previous assessment and reassessment was 70% and when morphophysiological and BCP-MS-G culture were considered, agreement reached 96%. There was 30% overdiagnosis for T. rubrum. Cultures on BCP-MS-G agar are suggested to differentiate T. interdigitale from T. rubrum A type and other biotypes of T. rubrum B type.

As a vital component in an overall laboratory animal care and use program , development of a disaster plan plays a critical role for every research institution .Currently, most of domestic institutions would draw up an“emergency operation plan , EOP”, but ignoring a practicable “business continuity plan , BCP” in establishing a disaster plan.In this article, we will discusse about the definition of disaster , how to set up an EOP, and how to establish a thorough BCP , in order to show an integrated and professional disaster plan in laboratory animal care and use program .

Simple, rapid and sensitive spectrophotometric procedure is suggested for the determination of ranitidine hydrochloride (RNH) drug in pure form and in pharmaceutical formulations. The method was based on the ionpair formations of RNH with different dyestuff reagents such as methyl orange (MO), bromocrysol purple (BCP), eriochrome cyanine R (ECR) and alizaraine red S (ARS). The obtained ion-pairs were measured spectrophotometrically at 408, 420, 330 and 326 nm by using BCP, MO, ECR and ARS reagents, respectively. Beer’s plots were linear in the concentration range of 5-200, 20-350, 10-150 and 10-180 μg mL−1 RNH, with correlation coefficients not less than 0.9991, 0.9996, 0.9993 and 0.999 using BCP, MO, ECR and ARS reagents, respectively. The Sandell sensitivity was found to be 0.813, 0.462, 0.541and 0.630 μg cm−2 for BCP, MO, ECR and ARS, respectively. Standard deviation (SD = 0.024-0.028, 0.018-0.023, 0.016-0.021 and 0.023–0.029) and relative standard deviation (RSD% = 0.123-0.943, 0.0102-0.82, 0.118-0.145 and 0.132-0.178%) (n = 4) values using BCP, MO, ECR and ARS reagents, respectively, were obtained. These results were also confirmed with percent recovery of 99.78–100.52%, 99.86-101.12%, 99.82–100.31% and 100.18-101.25 % for BCP, MO, ECR and ARS reagents, respectively. This method was successfully applied for determination of RNH in aciloc tablet. The calculated t- and F- values (95% confidence limit) indicate no significant differences between the proposed and official methods.

Objective To investigate the clinical value of micro-array technique in the detection of HBV pre-core and basic core promoter region variants. Methods Four spot mutations of A1896(nt1896G→A) and A1899( nt1899G→A) in precore region and T1762A1764(nt1762A→T,nt1764G→A) in basic core promoter region in 46 patients of acute and chronic liver diseases were detected by gene chips to investigate the clinical value of micro-array technique. Results Micro-array technique had a high specificity in the detection of specific mutation, and the positive rate was 87.0%. A1896 mutation was found in 18 cases (45.0%), A1899 mutation in 10 cases (25.0%), T1762 and A1764 double variants in 30 cases (75.0%), and poly-sites variants in 14 cases (35.0%). PURPOSE: Buckwheat flour (BF) is known as a potent food allergen. Its sensitization usually occurs by ingestion, but also by inhalation due to occupational or domestic exposure. We underwent this study to identify an effect of buckwheat chaff stuffed pillow (BCP) exposure on sensitization to BF and clinical BF allergy in asthmatic children. METHOD: We obtained detailed history of BCP exposure in 36 asthmatic children (aged 0.7-14.2 years). We also performed RIA for specific IgE, BCP-elimination-provocation test. All subjects were divided into 3 groups, Group I (continuous BCP exposurers, n=13), Group II (previous exposurers, n=11), and Group III (non-exposurers to BCP, n=12) and all subjects had no history of ingestion of BF containing foods. RESULTS: In the 13 Group I cases, the durations of BCP exposure were 1-6 years, and 8 of them were users of BCP themselves, 5 were indirect exposurers by family members' BCP. The positive rates of BF specific IgE were 92.3, 36.4% and 8.3% in the Group I, II and III, respectively (Chi-square test, P<0.05). While the positive rates of house dust mites specific IgE were not significantly different among three groups. Twelve out of 13 Group I cases sensitized to BF, and 9 of those 12 were not sensitized to house dust mites. Eight out of 13 Group I cases were positive in BCP elimination-provocation test, during 6-24 month follow-up periods, 7 of them were managed effectively by BCP elimination only. CONCLUSION: Taken together, a small amount of BF attached to BCP can induce BF sensitization and BCP can be a major cause of childhood nocturnal asthma.

RESUMO

Objective: To study the preventive effect of bovine colostrum powder (BCP) on hyperglycemia in STZ-induced diabetic rats. Methods:The solutions of three different doses of BCP were administered orally to normal rats for 15 d. Then STZ (55 mg/kg ) were injected intraperitoneally to the rats. At D7 of STZ injection, body weight and blood glucose were measured, and at D14 , blood triglyceride, cholesterol, SOD、GSH-Px、NOS and MDA in liver and kidney were determined besides the indices mentioned above. Water intake was recorded at D6 and D13 after STZ injection.
