RESUMO

Objective To investigate the factors that induce constipation in bedridden patients undergoing replantation of severed finger and find countermeasures. Methods A retrospective investigation was carried out in 92 patients undergoing replantation of severed finger, their constipation and factors influencing constipation were recorded. Results 61 patients suffered from constipation in total 92 cases, the rate being 66.30%. 76 patients reduced food intake, and 59 cases presented constipation (the rate being 77.63%). Factors influencing constipation included being unaccustomed to defecate in bed (the rate that present with constipation being 87.09%),reluctant to trouble others(75.47%), side effects of drugs(55.56%) and lack of dietary knowledge (71.87%), decreasing food intake was closely bound up with constipation. Conclusions Proper psychological counseling, strengthening of health propaganda, comfortable facility for defecation, improved family support and oral food intake can significantly reduce the proportion of constipation and increase comfort degree of patients.

@#Objective To study the test-retest reliability of grip strength assessment in the bed-ridden elders above 80 years old. Methods The left and right grip strength of 20 patients who were bed-ridden for more than 1 year, older than 80 years were measured on the lying position with the JAMAR Hand Dynamometer, and they were measured again in the same way by the same conner a week later. Results The intraclass correlation coefficient were 0.985 and 0.970 in the grip strength of left and right hands, respectively. Conclusion The assessment of grip strength showed good test-retest reliability in the bed-ridden elders above 80.

Objective To investigate the effect of family nursing intervention on family function and negative affect of bod-ridden senile patients. Methods 60 elderly bed-ridden patients were ran-domly divided into the observation group and the control group with 30 patients in each. Traditional nurs-ing was used as intervention method for the control group, while OPT (outcome present state testing mod-el) was chosen in the observation group. Questionnaires family care index (APGAR), activities of daily living scale (ADL), anxiety rating scale (SAS), geriatrie depression scale (GDS) and life satisfaction in-dex A (LSIA) were used to assess the nursing effect before and after the intervention using,X2 and t test. Results One year after the intervention, the score of each scale in the observation group was better than that of the control group. Conclusions Combination of OPT model with familial cooperation to give cor-responding nursing intervention for bed-ridden senile patients can effectively increase their family function, enhance the activity of daily life, relieve patients" negative emotion, raise their quality of life, all of these possess positive premotive effect on the life significance of patients.

RESUMO

PURPOSE: This study aims to identify the ADL and IADL of bed-ridden elderly. Also it is used to show fuctional status, and to investigate the content and the level of nursing services provided. METHOD: The subjects were 191 elderly who received visiting nurse service through public health centers in the Seoul Metopolitan and Chungnam Province. Data collection was conducted by public health center nurses during four months in 2000. Result: As for daily living activities, 100% of subjects had at least one difficulty in ADL and IADL. Among them, only 0.5% had moderate disabilities and 99.5% had severe disabilities by HFS, 27.9% were in a semi bed-ridden state and 72.1% were completely bed-ridden by JABC. The major service provided was a visiting nurse service which was preferable to the social welfare service. In the visiting nurse service, there was no significant difference according to the elderlys' functional status. In addition there was no standadization about the qualification of the visiting nurse, and single entry point for the nursing service. CONCLUSION: The researchers urgently suggest that a community based comprehensive service model has to be developed to respond to the needs of the elderly in Korea.
