RESUMO

BACKGROUND: The appropriate procedures and equipment for the pretransfusion test are fundamental to a safe blood transfusion. The present study aimed to assess the current status of procedures and equipment for pretransfusion tests at small- and medium-sized medical institutions, as well as to use this basic raw data to better manage blood transfusions at these institutions. METHODS: Offline and online questionnaire surveys were performed at institutions that used between 24 and 1,000 units of blood products in 2014. A total of 338 institutions participated, and the survey results were subsequently analyzed. RESULTS: Among 307 institutions where on-site ABO blood typing was performed, 15.0%, 2.1%, and 43.5% did not conduct ABO serum typing, RhD typing, and irregular antibody screening tests, respectively, and 12.8% only conducted the saline phase for crossmatching. Moreover, among 338 institutions, only 66.7% of blood banks had centrifuges, 84.5% had 37℃ incubators, 41.1% had slide view boxes; in addition, 66.1% and 18.6% had refrigerators and deep freezers, respectively, for blood storage. CONCLUSION: Certain small- and medium-sized institutions did not have the essential equipment required to operate as blood banks. Moreover, they also needed to improve their testing procedures. To address these issues, the initiation of systematic training programs and the employment of institutional strategies are necessary to enhance testing procedures and equipment, respectively.

Objective To investigate the application values of 3 kinds of crossmatching method saline medium method,polybrene test and microcolumn gel test in the neonatal blood transfusion security.Methods 174 newborns needing blood transfusion were simultaneously performed the isotype crossmatching by the saline method,polybrene test and microcolumn gel test.The irregular antibody and the direct antiglobulin test were routinely conducted,the samples with positive irregular antibody were further per-formed the antibody specificity identification.Results 14 cases (8.05%)of crossmatching incompatibility were found by the saline method,1 case (0.57%)by the polybrene test and 62 cases (35.63%)by the microcolumn gel test;among which,31 cases were secondary side crossmatching incompatibility caused by positive direct antiglobulin test,1 was the incompatibility of 3 methods caused by anti-D antibodies.The difference between the polybrene test and the microcolumn gel test was of statistical significance (P <0.05).Conclusion The blood crossmatching by the microcolumn gel test in blood transfusion of newborns requires quite few amount of blood sample,is simple to operate and easy to be standardized.With results clearly and easily measured and sensitivity higher than that of polybrene test,its result is clear and easy to be judged,its sensitivity is higher than that of the polybrene test, which has the important significance for ensuring the neonatal blood transfusion safety.

RESUMO

Objective To evaluate the influence of blood transfusion or pregnancy on blood cross-matching results.Methods 1 000 specimens with blood transfusion or pregnancy history were collected and performed the blood group typing,blood cross matching,irregular antibody screening and identification by the micro-gel method with the specimens of first time transfuse blood and without pregnancy history as the control.The inconsistency rate of positive negative blood grouping,unqualified rate of blood cross matching and the positive rate of irregular antibody were compared between the two groups by the t-test.The causes influen-cing the blood cross-matching results were analyzed.Results The inconsistency rate of blood cross-matching in the blood transfu-sion or pregnancy group was significantly higher than that in the control group(P 0.05);the positive rate of irregular antibody screening in the blood transfu-sion or pregnancy group was also significantly higher than that in the control group(P <0.05),indicating that the main causes in-fluencing blood cross matching was the existence of irregular antibodies in the patients in vivo.Conclusion Blood transfusion or pregnancy may stimulate the body to produce the irregular antibodies.Therefore the irregular antibody screening should be per-formed before blood transfusion and blood without corresponding antigen will be selected for transfusion according to the antibody identification results for ensuring safe and effective blood transfusion.
