OBJECTIVE@#To compare the clinical efficacy differences between WANG Ju-yi 's meridian diagnosis method combined with Bobath rehabilitation training and Bobath rehabilitation training alone for post-stroke shoulder-hand syndrome (SHS) typeⅠ.@*METHODS@#A total of 106 patients with post-stroke SHS typeⅠwere randomly divided into an observation group (53 cases, 2 cases dropped off ) and a control group (53 cases, 3 cases dropped off ). The patients in the both groups were treated with medications for basic diseases and conventional acupuncture at Waiguan (TE 5), Shousanli (LI 10) and Jianyu (LI 15) on the affected side. In addition, the patients in the control group were treated with Bobath rehabilitation training, 20 minutes each time; on the basis of the control group, the patients in the observation group were treated with WANG Ju-yi's meridian diagnosis method to adjust the abnormal parts in meridians of the hand taiyin and hand yangming on the affected side, 20 minutes each time. Both groups were treated once a day, 5 times a week for 8 weeks. The scores of visual analogue scale (VAS), upper-limb Fugl-Meyer assessment (FMA) and Barthel index (BI) were recorded before and after treatment as well as 6 weeks after treatment (follow-up), and the clinical efficacy of the two groups was evaluated after treatment.@*RESULTS@#Compared before treatment, the VAS scores were reduced and the scores of upper-limb FMA and BI were increased in the two groups after treatment and in the follow-up (P<0.05). The VAS score in the observation group was lower than that in the control group (P<0.05), and the scores of upper-limb FMA and BI in the observation group were higher than those of the control group (P<0.05). The total effective rate in the observation group was 82.4% (42/51), which was higher than 62.0% (31/50) in the control group (P<0.05).@*CONCLUSION@#WANG Ju-yi 's meridian diagnosis method combined with Bobath rehabilitation training could effectively treat post-stroke SHS typeⅠ, reduce pain symptoms and improve joint motor dysfunction, and improve the quality of life. Its curative effect is better than Bobath rehabilitation training alone.

Objective:Effects of pole-specific acupuncture combined with Bobath on upper limb function, daily life ability and nerve function after traumatic brain injury were observed.Methods:A total of 142 patients with cerebral hemiplegia after traumatic brain injury from January 2019 to December 2020, were divided into the Bobath group (47 cases), the pole-specific acupuncture group (47 cases) and combination group (48 cases) by the random number method. Bobath group received Bobath rehabilitation, the pole-specific acupuncture group received pole-specific acupuncture rehabilitation, and combination group was given pole-specific acupuncture rehabilitation and Bobath treatment. The overall rehabilitation efficiency, limb function Fugl-Meyer scale score, Barthel index of daily life ability, nerve function, and other indicators were observed and compared.Results:After treatment, the overall recovery efficiency (86.96%) in combination group was significantly higher than that of the Bobath group (65.96%) and acupuncture group (64.44%)( χ2=5.84, P=0.016). After treatment, the limb function Fugl-Meyer scale (including upper limb and lower limb function scores)( F=19.38, 24.83, all Ps<0.01), daily life ability Barthel index (including cognitive ability situation score, language ability score, self-care ability score, social adaptability score and total score) of combination group were significantly higher than those in the Bobath group and acupuncture group ( F=14.91, 15.87, 18.71, 18.88, 32.62, all Ps<0.001), while the NIHSS score of combination group was significantly lower than that of the Bobath group and acupuncture group ( F=31.71, P<0.01). After treatment, the NE[(58.29±9.82)μg/L vs. (86.29±12.35)μg/L, (88.34±12.87)μg/L, F=33.39], DA[(204.29±20.26)μg/L vs. (278.72±27.56)μg/L, (281.14±27.82)μg/L, F=55.50], 5-HT[(231.27±20.12)μg/L vs. (294.74±29.34)μg/L, (298.19±28.73)μg/L, F=13.86], E[(21.85±3.19)μg/L vs. (28.37±4.07)μg/L, (28.26±4.14)μg/L, F=9.34] of combination group were significantly lower than those in the Bobath group and acupuncture group ( P<0.01). Conclusion:Magnetic pole-specific acupuncture combined with Bobath can improve the function of limbs, daily quality of life and nerve function of the patients with traumatic craniocerebral injury with cerebral palsy.

Objective:To evaluate the efficacy of task-oriented training for rehabilitation of stroke patients with hemiplegia in the community.Methods:Eighty stroke patients with hemiplegia receiving rehabilitation in Shanghai Babsongyuan Subdistrict Community Health Service Center from January 2017 to December 2019 were enrolled in the study. According to the rehabilitation methods, the patients were divided into the observation group (42 cases) and the control group (38 cases). Both groups were given routine Bobath technique and motor relearning method for 3 months; on this basis the observation group was given additional task-oriented training. The Brunnstrum stage was used to evaluate the limb function, Bobath balance scale was used to evaluate the sitting and standing balance function, and modified Barthel Index (MBI) was used to evaluate the activities of daily living.Results:The Brunnstrum stage of upper limbs, hands and lower limbs, Bobath sitting and standing balance function classification and MBI score of the two groups were significantly improved after treatment (the observation group: Uc=5.037, 4.567, 5.228, 2.538, 6.508, t=36.215, P<0.05; the control group: Uc=2.483, 2.311, 2.335, 2.200, 4.102, t=29.260, P<0.05). While the Brunnstrum stage of upper limb, hand and lower limb, Bobath sitting and standing balance function grade of observation group were significantly better than those of control group, MBI score was significantly higher than that of control group ( Uc=2.592, 2.243, 2.897, 2.018, 2.825, t=8.375, P<0.05). Conclusion:Bobath technique and motor relearning method combined with task-oriented training can significantly enhance the recovery of limb motor function and the improvement of activities of daily living of stroke patients.

RESUMO

Bobath concept improves walking and motor function in stroke patients with hemiplegia by promoting posture control and establishing correct body mapping. The use of the Bobath concept alone or in combination with other treatments, such as knee stability training, Botulinum toxin injection, and Traditional Chinese Medicine, can effectively improve the walking ability of stroke patients. The Bobath concept has not been widely used, and its clinical effect is difficult to assess, which is the major problem at the current stage in China.

Objective To explore the influence of fixed-foot stance Yunshou combined with raising handclasp of Bobath on the recovery of upper extremity in post- stroke patients with hemiplegia. Methods Meeting the criteria 59 cases of stroke patients with hemiplegia were randomly divided into experimental group (29 cases) and control group(30 cases) according to random number table.All the patients in both groups were received routine therapy and nursing,besides,the patients in the control group were given raising handclasp of Bobath,while the patients in experimental group were treated with fixed-foot stance Yunshou combined with raising handclasp of Bobath.The intervention were once a day,five days a week,lasting eight weeks.The Fugl-Meyer Assessment of Upper Extremity(FMA-UE)and Modified Barthel Index(MBI) as well as Simple Test for Evaluating Hand Function(STEF) were used to assess the patients′condition before,8 weeks after treatment respectively. Results After 8weeks intervention,the scores of FMA-UE,MBI,STEF were 40.69±8.67,76.89±1.79,59.31±7.89 and before intervention were 24.17 ± 11.98,57.14 ± 13.93,31.83 ± 5.41, the difference between the experimental group before and after intervention was statistically significant(t=13.222,8.755,18.311,P<0.01).At 8 weeks,the scores of FMA-UE, MBI, STEF in the control group were 35.47 ± 9.68,73.17 ± 2.82,49.47 ± 8.78, and were17.38 ± 4.10,37.38 ± 4.30, 74.62 ± 11.22 respectively before intervention,all the measures in control groups had significantly improved than those before training (t=-4.797, 7.372, 17.139, P<0.01).Score of FMA-UE and STEF in the experimental group were higher than those in the control group,and the difference was statistically significant (t=2.180, 4.525, P<0.05 or 0.01), while the MBI had no statistically difference between two groups (P>0.05). Conclusion Compared with training on raising handclasp of Bobath alone,fixed-foot stance Yunshou combined with raising handclasp of Bobath is more effective On the recovery of upper limb function in stroke patients with hemiplegia.

Se presentó un paciente de un año de edad con síndrome Cornelia de Lange, único recibido en el Servicio de Rehabilitación del Policlínico Docente René Vallejo Ortiz del municipio Manzanillo, provincia Granma, con signos evidentes de retraso del desarrollo psicomotor, remitido al servicio por su médico de familia cuando tenía seis meses de nacido. Para su tratamiento se emplearon técnicas específicas de la estimulación temprana, se lograron pequeños avances en su desarrollo motor y cognitivo, aunque no solo depende de la rehabilitación, sino también del entorno familiar en el cual se va desarrollando el niño. Se lograron pequeños avances motores en el paciente como: sostén cefálico, abertura de las manos y seguimiento de los objetos con la mirada, el paciente continúa rehabilitándose en el servicio de fisiatría.


A one -year-old patient with Cornelia de Lange syndrome (SCdL) was presented in this article, who was the only one attended in the service of rehabilitation of the René Vallejo Ortiz teaching polyclinic of Manzanillo municipality, Granma province. The patient had evident signs of late psychomotor development, so he was referred to this service by the family doctor when he was six months of age. For the treatment, specific techniques of early stimulation were used; small advances have been achieved regarding motor and cognitive development, although it not only depends on the rehabilitation, but also depends on the family environment, in which the infant is developing. Not many motors progresses were achieved in the patient such as: cephalic support, opening of the hands and objects pursuit followed by the eyes. The patient continues rehabilitating in the physiotherapy service.

<p><b>OBJECTIVE</b>To compare the difference in the clinical efficacy on post-stroke upper limb spasmodic hemiplegia between the combined therapy ofneedling technique and Bobath technology and simple Bobath technology.</p><p><b>METHODS</b>Sixty patients were randomized into an observation group and a control group, 30 cases in each one. The usual medication of neurological internal medicine was used in the two groups. In the control group, Bobath facilitation technology was applied to the rehabilitation training. In the observation group, on the basis of the treatment as the control group,needling technique was used to stimulate Zhongfu (LU 1), Tianfu (LU 3), Chize (LU 5), Quchi (LI 11), Jianshi (PC 5) and Daling (PC 7). The treatment was given once a day; 5 treatments made one session and totally 4-week treatment was required in the two groups. The modified Ashworth scale, the modified Fugle-Meyer assessment (FMA) and the Barthel index (BI) were adopted to evaluate the muscular tension, the upper limb motor function and the activities of daily living (ADL) before and after treatment in the two groups. The clinical efficacy was compared between the two groups.</p><p><b>RESULTS</b>Compared with those before treatment, the modified Ashworth scale, Fugl-Meyer score and BI score were all improved after treatment in the two groups (all<0.01). The results in the observation group were better than those in the control group (all<0.01). The total clinical effective rate was 93.3% (28/30) in the observation group and was 80.0% (24/30) in the control group. The efficacy in the observation group was better than that in the control group (<0.05).</p><p><b>CONCLUSIONS</b>Theneedling technique combined with Bobath therapy achieve the superior efficacy on post-stroke upper limb spasmodic hemiplegia as compared with the simple application Bobath therapy. This combined treatment effectively relieve spasmodic state and improve the upper limb motor function and the activities of daily living.</p>

Objective To determine the therapeutic effect of electroacupuncture combined with the Bobath technique on the lower limb motor function in stroke patients.Methods Eighty-seven patients with stroke were included and randomly divided into groups A,B,and C (n =29 in each group).The patients were treated with the Bobath technique in group A,electroacupuncture in group B,and electroacupuncture combined with the Bobath technique in group C.The Fugl-meyer Assessment (FMA),Modified Barthel Index (MBI),Motor Assessment Scale (MAS),and Functional Ambulation Category (FAC) were used to assess lower limb movement before and after treatment.Results Before treatment,no significant differences in FMA and MBI scores were found among the three groups (P > 0.05).After treatment,the FMA and MBI scores in the three groups were significantly higher than those before the treatment,and the scores in group C were significantly higher than those in groups A and B (P < 0.05).No significant differences in MAS and FAC grading were found among the three groups before treatment (P > 0.05).After treatment,there were significant differences in MAS grading between group C and groups A and B and FAC grading between group C and group A (P < 0.05).No significant difference in FAC grading was found between group C and group B (P > 0.05).Conclusion The recovery of lower limb motor function was better in the stroke patients treated with electroacupuncture combined with the Bobath technique,which can significantly improve the ability of daily living,lower limb motor function,walking ability,and limb spasticity,compared with the simple electroacupuncture or Bobath technique.

OBJECTIVE: To investigate the recognition about clinical application of "neurodevelopmental treatment (NDT)". METHOD: We surveyed on the recognition for Bobath and NDT concepts and its training programs provided by Korean Academy of Rehabilitation Medicine from Jan 2009 to Feb 2009. The survey was made for physiatrists and physical therapists (PTs), separately. One hundred twelve physiatrists and 322 PTs have participated. RESULTS: In physiatrist, interest in NDT was very high (98%), and the need for further knowledge of NDT was also high (95%). Though the NDT was mainly used techniques in neuro-rehabilitation setting, the expectation about effectiveness of it was moderate (56%) and the requirement of changing concepts of the NDT was very high (93%). PTs have carried out NDT most frequently with Bobath's concepts, but they also did not regard NDT as the best procedure. PTs emphasized the importance of their handling technique in NDT practices and also had high interest in the other maneuvers beside Bobath's. CONCLUSION: Physiatrists and PTs regard NDT as one of the most important treating method for patients with brain lesions, but it is not approved as the most effective method yet. Nowadays NDT is changing or adapting, with discarding some of the old Bobath's concepts and adding new modern scientific knowledge. Therefore we suggest that physiatrists to give PTs the revised NDT concepts and evidence based the other therapeutic methods. And the NDT needs to be verified its therapeutic efficacy with evidence based view points through proper studies.

@#ObjectiveTo investigate the effect of Bobath approach combined with acupuncture on spastic cerebral palsy. Methods30 children with cerebral palsy treated with acupuncture combined with Bobath approach were as study group, other 30 children treated with Bobath approach only as the control group. ResultsThere were 27 cases (93%) were effective in study group, and 21 cases (80%) in control. The scores of Gross Motor Function Measure (GMFM) improved in both groups (P<0.05), but more in the study group (P<0.05) after treatment. The spasticity alleviated in both study group (76.7%) and control group (50%). ConclusionThe combination with acupuncture is more effective on spastic cerebral palsy than Bobath approach alone.

@#Objective To observe the effect of Bobath approach applied in early stage on hemiplegia after cerebral infarction. Methods 174 cases of cerebral infarction were treated with Bobath approach and assessed using Fugl-Meyer Assessment (FMA) and Modified Barthel Index (MBI), and the patients who accepted rehabilitation within or over a month after stroke were compared. Results After treatment the scores of FMA and MBI were improved more in the patients within a month than those over a month(both P<0.01). Conclusion The Bobath approach is effective on the locomotor function and the activity of daily living in patients with cerebral infarction. The earlier it starts, the better the effect should be.

@#Objective To explore the affect of acupuncture cooperating with Bobath approach on the spasticity after stroke.Methods 83 patient were divided into treatment group (42 cases) and control group (41 cases). On basis of conventional treatment, the treatment group accepted acupuncture and Bobath approach while the control group accepted a Bobath approach only. They were assessed with Modified Ashworth Scale before and 8 weeks after treatment. Results 8 weeks after treatment, the spasticity reduced in both groups, but more in the treatment group (P<0.05).Conclusion Acupuncture can facilitate the efficacy of Bobath approach on spasticity after stroke.

O objetivo deste estudo foi apresentar a evolução de uma criança com paralisia cerebral submetida a tratamento baseado no conceito Bobath. A avaliação foi realizada antes e depois do tratamento pela análise das posturas fundamentais, fotografias e pela escala GMFM. A intervenção foi realizada em período de sete semanas. Os resultados mostraram melhor simetria de cabeça e tronco, redução da flexão de membros superiores e maior controle estático desses membros na postura supina. O maior ganho foi na postura ortostática. A avaliação inicial detectou dependência de apoio para manter a postura, assimetria de cabeça, elevação dos ombros e nenhuma funcionalidade com membros. Após o tratamento houve redução da dependência de apoio do terapeuta e das compensações posturais, simetria de tronco e uso funcional dos membros. Com relação aos resultados da GMFM houve melhora em todas as dimensões da escala (A = 41 para 60, B = 15 para 35, C = 2,3 para 3, 3, D = zero para 12,8, E = zero para 13, 8, Escore total = 11,6 para 25). Os ganhos mais evidentes foram obtidos nas dimensões D e E, referentes a manter-se em pé, andar, correr e pular. Tendo em vista as limitações impostas pela patologia, considera-se que o tratamento baseado no conceito Bobath permitiu aquisições importantes para o desempenho motor deste paciente.


The aim of this study was to present the evolution of a child with cerebral palsy submitted to treatment based on Bobath concept. The evaluation was conducted before and after treatment, and was focused on postural analysis, photographs and GMFM results. Intervention was carried out seven weeks. The results showed a better symmetry in head and trunk, reduction of upper limbs flexion and a good static control in supine position. The better gain was found in the orthostatic posture. In the initial evaluation, the need of a help to maintain the posture, head asymmetry, shoulder elevation, and no function of the upper limbs were observed. After treatment, there was little need for help and a reduction of compensations. The trunk was more symmetric, and the upper limbs exhibited a functional use. The GMFM changed considerably in all scale dimensions (A = 41 to 60, B = 15 to 35, C = 2.3 to 3.3 , D = 0 to 12.8, E = 0 to 13.8, total score= 11.6 to 25). More evident improvement was noted in dimensions D and E (orthostatic, walking, running, shout). Considering the limitations of the pathology, the treatment permitted important acquisitions to the motor development of the patient.

Este estudo considerou os aspectos envolvidos no adulto com paralisia cerebral em relação aos vários sistemas do corpo e analisou o aspecto motor e a presença de dor. Também enfocou as alternativas para o adulto jovem para a continuidade de habilitação, prevenção ou minimização dos efeitos secundários da paralisia cerebral e da idade avançada. Para tanto, foram avaliados nove jovens adultos (18 a 28 anos) e um adulto (51 anos) durante período de dois anos, utilizando-se a Medição da Função Motora Grossa (GMFM). Durante esse período os indivíduos participaram de terapias individuais baseadas no Conceito Bobath, e de terapias em grupo também baseadas no Conceito Bobath, mas associado com aspectos da Educação Condutiva (Método Petõ). Noventa por cento apresentaram ganhos no resultado final do GMFM que variaram de 1,5% a 15,1 %, com média de ganho de 6,2%. Os resultados, portanto, demonstraram que a continuidade de atividade dirigida a mobilidade, controle de postura e dos movimentos integradas a atividades funcionais permitem que o processo de deterioração seja contido ou minimizado para adultos jovens com paralisia cerebral.


This study has considered aspects of several body systems in adults with cerebral palsy emphasizing motor and pain conditions. Options for keeping on the habilitation process, preventing from deterioration, and minimizing secondary effects of cerebral palsy in young adults were also discussed. For this purpose, nine young adult (18 to 28 years of age) and one adult (51 years of age) were assessed during two years by the aplication of the Gross Motor Function Measure (GMFM). During these two years the individuals participated in individual therapies based on Bobath approach as well as in therapeutic group activities also based on Bobath approach associated with some aspects of Conductive Education (Petõ Method). Improvements in GMFM total results varying from 1.5% to 15.1 % (mean of 6.2%) were observed in 90% of the patients. The results have shown, therefore, that the continuity of directed activity aimimg at motor control integrated with functional activities can constrict or minimize the deterioration process in young adults with cerebral palsy.

@#ObjectiveTo observe the effect of rehabilitation on post stroke depression with different treatment methods.Methods68 post stroke depression patients were randomly divided into treatment group, who accepted Bobath approach combined with Chinese herbs and acupuncture, and control group, who accepted Bobath approach only. Evaluation was conducted before treatment and 3 and 6 weeks after treatment with Hamilton Assess Depression Scale (HAMD), and activity of daily living (ADL).ResultsThere was no significant difference before the treatment between these two groups. 3 weeks after treatment, the score improved, but no significant enough in the both groups. 6 weeks after treatment, the scores of MBI and HAMD in treatment group significantly improved compared with that before treatment(P<0.05). ConclusionBobath approach combined with Chinese medicine is effective on improving depressive symptoms and ADL in post stroke depression patients.

Neste trabalho e apresentada uma revisão bibliográfica de artigos que pesquisaram os efeitos do exercício físico resistido como abordagem de tratamento ao portador de Paralisia Cerebral com espasticidade. A avaliação preliminar das condições do paciente quanto a real força muscular, à espasticidade, as limitações articulares e ao condicionamento cárdio-respiratório e necessária, a fim de eleger esta abordagem como forma mais apropriada de tratamento com resposta direta sobre a funcionalidade. As razões para a fraqueza muscular, juntamente com a perda de controle motor seletivo, a fatigabilidade, o reduzido equilíbrio e os déficits sensoriais, todos fatores determinantes do prognóstico locomotor, tem implicações diretas no tratamento. O treino de força com exercícios em cadeia cinética fechada com peso corporal ou num grupo que tem A disposição estações de trabalho que incluam atividades funcionais com equilíbrio e treinamento isocinético com enfoque em membros inferiores promove efeitos benéficos as condições gerais do paciente desde que seja mantida boa postura durante a execução das atividades e inibidas as reações associadas, conforme o Conceito Bobath.


The purpose of this paper is to present a research about strength training effect as a management technique in spastic cerebral palsy. A preliminary evaluation of the patient's conditions regarding muscle strength, spasticity, joint limitations and cardiorespiratory fitness is necessary to elect this technique as the most appropriate with direct response for functionality. The causes of muscle weakness, loss of selective motor control, fatigability, reduced balance and sensorial deficit, all determining factors to the locomotor prognosis, have direct implications in the treatment. Strength training with closed-kinetic-chain exercises and with body weight or in a group that has acess to workstations including functional activities with balance and isokinetic training in lower limb produces gains in general conditions provided that a good posture during activities is kept and the associated reactions are inhibited as prescribed by the Bobath Concept. Physical fitness in cerebral palsy should be aimed always taking into consideration the functional conditions of each patient.

RESUMO

OBJECTIVE: To introduce a Modified triangular Bobath sling (MTBS) and a MTBS with distal support (MTBSDS) as a new slings and to compare with previous slings in the therapeutic effects of shoulder subluxation in stroke patient. METHOD: Using a simple X-ray with and without slings, the vertical and horizontal distances were measured for 17 stroke patients. MTBS and MTBSDS were compared with 3 conventional slings, which were hemisling, Bobath sling and Extension-type sling, in the effects of correction. The questionaire was used to evaluate preference of patients. RESULTS: In two developed slings, inferior subluxation showed significant correction and lateral displace was less increased than a Bobath sling although it was not statistically significant (p<0.05). MTBS had the preference during non-walking because of comfort and easiness for donning and doffing, whereas MTBSDS had the preference during walking because of reciprocal arm swing and protecting the hemiplegic upper extremity from trauma. CONCLUSION: The results indicated MTBS and MTBSDS were more effective on shoulder subluxation in stroke patients. Further study on the development of slings for decrement of lateral displacement is recommended.

RESUMO

@#95 children with cerebral palsy (CP), aged from 4 months to 5 years, were treated with Bobath,Vojta and Wilson therapies. The developmental scores of motor function was evaluated. The motor function of children suffering from CP was severely retarded before the treatment. The average score is 1. 28 ± 0. 34 permonth, and they were singificantly improved after the treatment with 8 scores in average per month.
