Resumen: Objetivo: Describir una técnica de conservación ósea de uso común en neurocirugía en un procedimiento ortopédico. Material y métodos: Se describe el caso de una paciente que se somete a artroplastía primaria de cadera con un resultado no satisfactorio inicial, planteándose previo al cierre la necesidad de una revisión con reconstrucción acetabular. Conservando cabeza femoral de paciente en tejidos blandos para realizar reconstrucción en segundo tiempo. Resultados: Tras seguimiento por seis meses se encuentra completa osteointegración de injerto de cabeza femoral, con buena evolución clínica y radiológica de la paciente. Las técnicas de conservación ósea en colgajos óseos han demostrado buenos resultados en la osteointegración de los injertos en otras áreas como neurocirugía. Conclusiones: La conservación de colgajos óseos en tejido celular subcutáneo para posterior uso como injerto es una opción viable de tratamiento también en la cirugía ortopédica.


Abstract: Objective: To describe a bone preservation technique commonly used in neurosurgery in an orthopedic procedure. Material and methods: We describe the case of a patient who undergoes primary hip arthroplasty with an initial unsatisfactory result, the need for a revision with acetabular reconstruction is considered before the wound closure. Keeping the patient's femoral head in soft tissues for second-time reconstruction. Results: After six months of follow-up, complete osseointegration of the femoral head graft was found, with a good clinical and radiological evolution of the patient. Bone conservation techniques in bone flaps have shown good results in the grafts osseointegration in other areas such as neurosurgery. Conclusions: The conservation of bone flaps in subcutaneous tissue for later use as a graft is a viable treatment option also in orthopedic surgery.

OBJECTIVES: To investigate the incidence of non-responder to hormone therapy (HT) and to evaluate the bone response to HT according to basal bone mineral density(BMD) in postmenopausal women. METHODS: A total of 167 postmenopausal women received either continuous combined estrogenprogestogen replacement (n=102) or estrogen replacement (n=65) for 1 years. BMD at the lumbar spine and femoral neck was measured by dual energy X-ray absorptiometry (DXA) before and 1 year after HT. RESULTS: The incidence of non-responder (women with >3% bone loss per year) to HT was 8.3% in the lumbar spine, and 22.1% in femoral neck. non-responder group had a higher basal BMD at the lumbar spine than responder group, and showed bone loss rate of 7.7% per year. After 1 year of HT, postmenopausal women with osteoporosis showed a higher rate of increase in BMD at the lumbar spine and femoral neck than women with normal BMD or osteopenia. CONCLUSIONS: The non-responders to HT have a higher basal lumbar BMD, compared with responders. The higher basal BMD at the lumbar spine is, the less bone conservation effect of HT is.

RESUMO

Se presenta la Osteotecnia en peróxido de hidrógeno y ácido acético como una herramienta para la conservacón de material óseo para fines docentes en colegios y universidades o de exhibición en museos o instituciones afines. Esta técnica de conservación de material óseo permite además rescatar piezas y estructuras óseas de difícil adquisición. Es un método rápido y muy baja toxicidad, fácil de aplicar y de un costo relativamente bajo en comparación con otros descritos en la literatura.


An Osteotechnique is presented on hidrogen peroxide and acetic acid as a tool for the conservation of bone material with teaching purposes on high schools and colleges, museum exhibitions or similar institutions. This bone preservation technique allowds to rescue pieces and bone structures that are difficult to adquire. It`s a simple and low toxicity method, easy to apply and with relatively low cost compared to other techniques described on the literature.
