Objective The purpose of this article is to summarize and review the current status of the construction of clinical research evaluation systems in domestic public hospitals,identify existing problems in the evaluation system,and propose development strategies and suggestions.Methods Retrieved relevant articles,dissertations and policies from the past five years(2018-2022),screened the titles,viewed the full texts of 52 selected papers and their references,and summarized them.Results The"five-only"indicators have long been an important indicator for evaluating clinical research in public hospitals,but in today's scientific research environment and policy environment,the"five-only"evaluation system has revealed its utilitarian draw-backs and gradually evolved into a hindrance to scientific research.It is urgent to break through the"five-only"orientation and establish a clinical research evaluation system oriented towards"transforming and applying transformation of scientific research achievements".Conclusion The evaluation system for clinical research should break the previous"five-only"evaluation model based on quantity-oriented scientific research evaluation.We can draw on the framework of the research output,influence,and environment indicators in the UK's REF Excellence Framework model,combine the American APT system and the Chinese STEM indicator dimensions,explore multi-outcome evaluation,integrate developmental indicators,and continuously improve the indica-tor system and application methods in practice to promote the development of clinical research in public hospitals.

Objective Under the background of"breaking the Five-Only"and the evaluation reform of healthcare professionals'titles,it analyzed the willingness of healthcare professionals in public hospitals to disseminate health knowledge and its incentives and constraints.Furthermore,it explored potential pathways to improve the willingness of healthcare professionals to popular science and disseminate health knowledge.Methods From November to Decem-ber 2022,a self-designed online questionnaire was used to survey healthcare professionals in four tertiary hospitals in Shanghai.The questionnaire included basic information of healthcare professionals,analysis of the current situa-tion and demands of healthcare professionals in health knowledge dissemination,willingness of healthcare profes-sionals to disseminate health knowledge and influencing factors.Results A total of 762 healthcare professionals partici-pated in this survey,79.9%(608/762)expressed willingness to promote the dissemination of health knowledge.A multiple-factor logistic regression analysis revealed that intermediate professional title,achieving personal value and so-cial responsibility,increasing patient resources,professional title promotion,science promotion-related awards,being included in performance appraisals,and difficulty in capturing science promotion skills were the influential factors affecting the dissemination of health knowledge among healthcare professionals(P<0.05).Conclusion The willing-ness of healthcare professionals to disseminate health knowledge was strong,and hospitals should motivate health-care professionals to disseminate health knowledge through building a long-term incentive mechanism,strengthening training in science popularization ability,and improving humanistic qualities.

The cultivation of graduate students' scientific research ability is an important task of colleges and universities. The evaluation standards of cultivation achievements play an important guiding role. In recent years, quantitative academic evaluation based on the "five-only" has caused some negative effects on the cultivation of talents in colleges and universities. A new evaluation system needs to be established. During the transition period between the old and new academic evaluation systems, the evaluation criteria for the cultivation of graduate students' scientific research capabilities will inevitably need to be reformed. This study attempts to reform the existing evaluation standards by investigating the evaluation standards for the cultivation of scientific research capabilities in various colleges and universities. The aim is to improve the evaluation of the achievements in the cultivation of scientific research ability of graduate students and improve the quality of the cultivation of scientific research ability of graduate students.

ABSTRACT Medical specialties have recognized that breaking bad news assists clinical practice by mitigating the impact of difficult conversations. This scenario also encourages various studies on breaking bad news in ophthalmology since certain ocular diagnoses can be considered bad news. Thus, the objective is to review the scientific literature on breaking bad news in ophthalmology. The literature databases like MEDLINE/PUBMED, EMBASE, LILACS, SCOPUS, COCHRANE, and SCIELO, were screened for related research publications. Two independent reviewers read all the articles and short-listed the most relevant ones. Seven articles, in the formats of original article, review, editorial, oral communication, and correspondence, were reviewed. Conclusively it reveals that ophthalmologists are concerned with communicating bad news effectively but lack related studies. Nevertheless, there is a growing realization that training in breaking bad news can increase physicians' confidence during communication, thus, benefiting the therapeutic relationship with the patient and his family. Therefore, it would be valuable to include breaking bad news training in the curriculum of residencies.
RESUMO O reconhecimento sobre a comunicação de más notícias como mitigadora de conversas difíceis por outras especialidades médicas, incentiva o estudo desta temática na oftalmologia. Sendo assim, o objetivo deste estudo é revisar a produção de pesquisas científicas sobre a comunicação de más notícias em oftalmologia. Para isso, foi realizada uma revisão de literatura. As bases de dados utilizadas foram MEDLINE/PUBMED, EMBASE, LILACS, SCOPUS, COCHRANE e SCIELO. Dois revisores independentes leram todos os artigos e selecionaram a amostra final. Sete artigos foram escolhidos nos formatos de artigo original, revisão, editorial, comunicação oral e correspondência. Os oftalmologistas estão preocupados em comunicar as más notícias de forma eficaz, mas faltam estudos sobre o tema. No entanto, há uma crescente percepção de que o treinamento de comunicação de más notícias aumenta a confiança dos médicos na comunicação, beneficiando a relação terapêutica. Portanto, seria valioso incluir este treinamento no currículo das residências.

Click chemistry has been proven to be very useful in drug delivery. Due to the availability of a large number of click reactions with a various characteristics, selection of appropriate chemistry for a given application is often not a trivial task. This review is written for pharmaceutical researchers who are interested in click chemistry applications and yet may not be click chemistry experts. For this, the review gives an overview of available click reactions organized by application types. Further, the general understanding of click reactions being fast and high yielding sometimes overshadows the need to analyze reaction kinetics in assessing suitability of a given reaction for certain applications. For this, we highlight the need to analyze the relationship among reaction kinetics, concentration effects, and reaction time scales, knowing that lack of such analysis could easily lead to failures. Further, possible issues such as chemical stability with various click reagents are also discussed to aid experimental designs. Recent examples and extensive references are also provided to aid in-depth understanding of technical details. We hope this review will help those interested in using click chemistry in drug delivery to select the appropriate reactions/reagents and minimize the number of pitfalls.

A wound is a break in the integrity of the skin or tissues often which may be associated with disruption of the structure and function. Globally, the magnitude of acute wounds as a healthcare problem is sharply rising mainly due to industrialization. There is still a need to revisit the ancient healing methods by using traditional medicine aiming to discover a wound healing drug with better protection, rapid granulation changes, least side effects, good bio-availability and low cost. A folk medicinal plant Humboldtia vahliana Wight. belongs to the family Fabaceae, is a medium sized tree with rough, dark brown bark mottled with white. Its stem bark is used for wound healing among the tribes. Aims and objectives: The study aims to evaluate the in-vivo wound healing action of Humboldtia vahliana Wight. through excision and incision animal wound models. Methodology: 36 male Wistar albino rats weighing 150-250gm. were selected and made into 3 groups each for both incision and excision wounds, i.e., control, standard and trial group -each containing 6 rats. The trial drug Humboldtia vahliana Wight. was applied over the wound area in the form of ‘Kalka’. In excision wound model, percentage of wound contraction was the parameter for the study and was achieved by using Planimetry. In incision wound model, tensile strength was the parameter and was achieved by Local Tensiometer. Results: In case of both Incision as well as Excision wound contraction study, the data shows a statistically non-significant increase in percentage wound contraction of Trial group when compared to Control and Standard groups. Interpretation and Conclusion: The stem bark of Humboldtia vahliana Wight. showed better wound healing action in Wistar albino rats when percentage of efficacy or mean values were compared

Background: Nutrient restriction is a common strategy to prevent metabolic disorders in fast growing broiler chickens, but limited information is available regarding the impact of diets with low protein and energy on leg abnormalities Objective: Two experiments were conducted to evaluate the effect of varying crude protein (CP) and metabolizable energy (ME) levels on gastrocnemius tendon (GTeBS) and tibia breaking strength (TiBS), gait score (GS) and valgus/varus angulation (VAng). Methods: In Experiment 1, eight-d-old Ross 308 broilers (n=90) were randomly assigned into three treatments: 13/2,900 (13% CP and 2,900 kcal ME kg-1 of diet), 17/3,000 (17% CP and 3,000 kcal ME kg-1 of diet), and 21/3,025 or control (21% CP and 3,025 kcal ME kg-1 of diet). In Experiment 2, six-d-old Ross 308 chickens (n=192) were randomly distributed into two treatments: 16/3,000; 16% CP and 3,000 kcal ME kg-1 of diet and 21/3,000 or control; 21% CP and 3,000 kcal ME kg-1 of diet. In both experiments data were analyzed as one-way ANOVA. Results: In Experiment 1, broilers in the 17/3,000 and control treatments had similar (p>0.05) GTeBS (202 and 224 N, respectively), and TiBS (338 and 332 N, respectively). Birds in the 13/2,900 treatment showed higher GS (lower walking ability; 80% of birds with score >3), and greater VAng (53% of birds with score >2) than broilers in the control treatment (0% of birds with GS score >3 and 0% of birds with VAng score >2). Experiment 2, broilers in the 16/3,000 showed similar VAng, GS and TiBS than birds in the control treatment. However, birds in the 16/3,000 treatment showed lower (-26%) GTeBS than birds in the control treatment (p<0.05). Conclusions: Feeding broilers with 16% CP and 3,000 kcal ME kg-1 of diet did not affect tibia breaking strength, gait score and valgus/varus angulation. A diet containing 16% CP or less and 2,900-3,000 kcal ME kg-1 reduces tendon and tibia breaking strength.
Antecedentes: La restricción de nutrientes es una estrategia común para prevenir trastornos metabólicos en pollos de engorde, sin embargo, se dispone de información limitada sobre el impacto de las dietas bajas en proteína y energía en las anomalías de piernas. Objetivo: Se llevaron a cabo dos experimentos para evaluar el efecto de niveles variables de proteína cruda (PC) y energía metabolizable (EM) en la fuerza de rotura del tendón del gastrocnemio (GTeBS) y tibia (TiBS), la evaluación de marcha (GS) y la angulación en valgus/varus (VAng). Métodos: En el Experimento 1, pollos de engorde Ross 308 de ocho días de edad (n = 90) se asignaron al azar en tres tratamientos: 13/2.900; 13% PC y 2.900 kcal ME kg-1 de dieta, 17/3.000; 17% PC y 3.000 kcal ME kg-1 de dieta y 21/3.025 o control; 21% PC y 3.025 kcal ME kg-1 de dieta. La dieta formulada con 16% de PC o menos y 2.900-3.000 kcal de EM kg-1 reduce la resistencia a la ruptura del tendón y de la tibia.

Métodos: Experimento 1, foram utilizados 308 frangos de corte machos da linhagem Ross com oito dias de idade (n = 90) distribuidos aleatoriamente em três tratamentos: dieta controle com 21% PB e 3.025 kcal EM kg-1 13/2.900; dieta con 13% PB e 2.900 kcal EM kg-1, y 17% PB e 3.000 kcal EM kg-1 . Experimento 2, foram utilizados 308 frangos de corte da linhagem Ross com seis dias de idade (n = 192) distribuídos aleatoriamente em dois tratamentos: dieta controle con 21% PB e 3.000 kcal EM kg-1 e dieta com 16% PB e 3.000 kcal EM kg-1 . Entretanto, as aves do tratamento 16/3.000 apresentaram GTeBS mais baixo (-26%) em comparacao as aves do tratamento de controle (p<0,05). Conclusões: A dieta com 16% PB e 3.000 kcal EM kg-1 não afetou a forca de ruptura da tíbia, gaite score marcha e angulação em valgus/varus de frangos de corte. Uma dieta com níveis de 16% de PB ou menos e 2.900-3.000 kcal EM kg-1 reduziu a resistência à ruptura do tendão e a resistência de tibia de frangos de corte.

Neuroscientists have emphasized visceral influences on consciousness and attention, but the potential neurophysiological pathways remain under exploration. Here, we found two neurophysiological pathways of heart-brain interaction based on the relationship between oxygen-transport by red blood cells (RBCs) and consciousness/attention. To this end, we collected a dataset based on the routine physical examination, the breaking continuous flash suppression (b-CFS) paradigm, and an attention network test (ANT) in 140 immigrants under the hypoxic Tibetan environment. We combined electroencephalography and multilevel mediation analysis to investigate the relationship between RBC properties and consciousness/attention. The results showed that RBC function, via two independent neurophysiological pathways, not only triggered interoceptive re-representations in the insula and awareness connected to orienting attention but also induced an immune response corresponding to consciousness and executive control. Importantly, consciousness played a fundamental role in executive function which might be associated with the level of perceived stress. These results indicated the important role of oxygen-transport in heart-brain interactions, in which the related stress response affected consciousness and executive control. The findings provide new insights into the neurophysiological schema of heart-brain interactions.

Objective:To explore the efficacy of breaking blood expelling stasis method accelerates hematoma resolution after intracerebral hemorrhage (ICH) and its potential mechanism.Methods:63 ICH patients confirmed by computer tomography (CT) scan from August 2019 to February 2020 were selected as the research objects and randomly divided into control group ( n=29, routine treatment plus placebo) and observation group ( n=34, routine treatment plus breaking blood expelling stasis granules). The changes of neurological function and hematoma volume were observed before and after treatment. At the same time, the ICH rat model was constructed to observe the changes of neurobehavior and hematoma volume after the intervention of breaking blood expelling stasis granules. The expressions of peroxidase proliferator-activated-receptor γ(PPARγ), CD11b and CD36 in the surrounding tissues of hematoma were detected by Western blot on the third day after the intervention. Results:After two weeks of treatment, the National Institutes of Health Stroke Scale (NIHSS) score and hematoma volume of the two groups decreased (all P<0.05), and the NIHSS score and hematoma volume of the observation group were significantly lower than those of the control group (all P<0.05). In addition, the changes of NIHSS score and hematoma volume in the observation group before and after treatment were significantly greater than those in the control group ( P<0.01). In animal experiments, the hematoma volume in the breaking blood expelling stasis group on the 14th day after ICH was significantly smaller than that in the ICH group [(9.8±4.9)mm 3 vs (17.6±6.4)mm 3,P<0.05], and the reduction of hematoma in the breaking blood expelling stasis group on the 7th and 14th day was significantly larger than that in the ICH group [(4.6±2.9)mm 3 vs (-2.1±1.6)mm 3, (14.3±3.8)mm 3 vs (4.2±2.8)mm 3, all P<0.01]. The percentage of right turn on 3rd, 7th and 14th day and the modified Neurological Severity Score (mNSS) on 7th and 14th day in the breaking blood expelling stasis group were lower than those in the ICH group (all P<0.05). Western blot analysis showed that the expressions of CD11b, CD36 and PPARγ in the breaking blood expelling stasis group on the third day after ICH were significantly higher than those in the ICH group (CD11b: 0.78±0.12 vs 0.49±0.11, P<0.05; CD36: 1.16±0.16 vs 0.80±0.11, P<0.05; PPARγ: 0.78±0.11 vs 0.37±0.10, P<0.01). Conclusions:Breaking blood expelling stasis can effectively accelerate intracerebral hematoma clearance and improve neurological outcome after ICH, and the mechanism maybe probably mediated by activating PPARγ and enhanced CD36, CD11b upregulation on microglia/macrophages, which resulting in facilitating erythrocyte endogenous phagocytosis.

@#<p style="text-align: justify;"><strong>Background:</strong> Exercising discretion is vital when making decisions. While decision-making implies having to choose between predefined alternatives, discretion requires sifting through several solutions and critically choosing the desired one that implies a choice between many alternatives without rigid guidelines pointing to one direction or the other. While previous studies have constantly investigated the nature and dynamics of creative insubordination or discretionary insubordination in primary and secondary education little is known as to how such a construct operates in higher education.</p><p style="text-align: justify;"><strong>Purpose:</strong> This study was undertaken to address the central question: "What attributes define creative insubordination as a decision-making strategy among Philippine medical school deans?"</p><p style="text-align: justify;"><strong>Design:</strong> Anchored on descriptive phenomenology, 21 medical deans who have been in the position for at least 4.5 years (range 4.5-26 years) were purposively selected for in-depth semi-structured interviews. Guided by the rules of cool and warm analyses, transcribed texts were phenomenologically reduced and interpreted, and validated via member checking procedure.</p><p style="text-align: justify;"><strong>Findings:</strong> Five interesting elements that typify the modes of behavior in the practice of creative insubordination surfaced, namely: cognitional elasticity, emotional sensitivity, conational sincerity, relational mobility and axiological reflectivity.</p><p style="text-align: justify;"><strong>Conclusion:</strong> This study provides eidetic images of the multidimensional facets of individual attributes necessary for the praxis of discretionary decision-making by a select group of Philippine medical deans in varied instances.</p>

Talent evaluation is the premise of development management and the use of human resources. The establishment of the talent classification and evaluation mechanism of university affiliated hospitals plays an important role in setting up the correct employment orientation, stimulating the career development of talents and promoting the high-quality development of public hospitals. The authors took the perspective of university affiliated hospitals, summed up the construction process of talent evaluation system in China, analyzed the existing problems in evaluation indicators, evaluation methods and operation mechanism of talent evaluation system, and came out with suggestions that the national level should strengthen the top-level design and collaborative management of talent evaluation, the hospital level should refine the evaluation indicators, improve the evaluation methods and establish the concept of developmental evaluation, at meanwhile, schools should improve the talent management mechanism and strengthen the function of audit and supervision.

Resumo: Introdução: Comunicação de más notícias refere-se às informações médicas que rompem de maneira adversa e negativa as expectativas de um paciente. Trata-se de um momento delicado da relação médico-paciente. O role playing com pacientes padronizados (PP) é uma alternativa para o ensino da comunicação, porém é desafiador construir roteiros e cenários semelhantes com a realidade, especialmente quando não se dispõe de atores profissionais. Objetivo: Este estudo teve como objetivo descrever a construção de PP a partir da experiência dos estudantes para o treinamento da comunicação com outros discentes de Medicina. Método: Estudantes de Medicina participaram voluntariamente da formação teatral e se dedicaram à construção de PP, com supervisão de docentes médicos e de atrizes, para oficinas de comunicação de más notícias. Para a construção dos PP, criaram-se diagnósticos médicos desfavoráveis, e estabeleceram-se os seguintes tópicos: perfil emocional, historicidade, instruções, progressões e possíveis desfechos, objetivos primário e secundário, e elementos facilitadores das cenas. Ensaiou-se exaustivamente para que os PP apresentassem a mesma reação em diferentes atitudes nas simulações. A participação nas oficinas foi aberta aos estudantes de Medicina interessados. Aferiu-se a qualidade das cenas pela sensação de realidade proporcionada aos participantes das oficinas. Resultado: Dez estudantes com idades entre 19 e 26 anos, de diferentes semestres, participaram do treinamento, do desenvolvimento de PP e das oficinas de simulação. Desenvolveram-se quatro cenas: Mariana recebeu diagnóstico de HIV e revelou violência sexual (60 cenas), Caroline e Marcelo receberam diagnóstico de câncer e necessidade de cirurgia (53 cenas), Thaís recebeu diagnóstico de insensibilidade androgênica e presença de gônadas masculinas (31 cenas), e Roger ou Rosana recebeu diagnóstico de retinose pigmentar e cegueira progressiva (22 cenas). Todos os PP proporcionaram sensação de realidade aos diferentes participantes das oficinas. Conclusão: Foi possível construir PP com estudantes de Medicina, apoiados por docentes, com a finalidade de participação em oficinas de comunicação de más notícias para outros discentes de Medicina que apontaram, em sua maioria, sensação de realidade com as cenas. Os estudantes atores se engajaram no processo e desenvolveram habilidades de comunicação e de empatia, características necessárias para a prática médica.
Abstract: Introduction: Breaking bad news is giving medical information that adversely and negatively disrupts a patient's expectations. This is a delicate moment in the doctor-patient relationship. Role playing with standardized patients (SP) is one method for teaching such communication skills, but it is challenging to construct scripts and scenarios that reflect reality, especially when professional actors are not used. Objective: To describe the construction of SP based on the students' experience for communication skills training with other medical students. Method: Medical students voluntarily participated in theatrical training, and worked on developing SPs, under the supervision of medical professors and actors, for breaking bad news workshops. For the construction of the SPs, unfavorable medical diagnoses were created, establishing an emotional profile, history, instructions, progressions and possible outcomes, primary and secondary objectives, and facilitating elements of the scenes. It was exhaustively tested so that the SP presented the same reaction in different attitudes in the simulations. Participation in the workshops was open to interested medical students. The quality of the scenes was measured by the sense of reality given to the workshop participants. Result: Ten students aged between 19 and 26 years, from different semesters, participated in the training, the SP development and role-play workshops. Four scenes were developed: Mariana was diagnosed with HIV and revealed sexual violence (60 scenes), Caroline and Marcelo who were diagnosed with cancer and needed surgery (53 scenes), Thais received a diagnosis of androgen insensitivity and presence of male gonads (31 scenes) and Roger or Rosana with retinitis pigmentosa and progressive blindness (22 scenes). All the SPs gave a sense of reality to the different workshop participants. Conclusion: It was possible to develop SPs with medical students, supported by professors, with the purpose of participating in workshops to communicate bad news to other medical students who perceived, for the most part, a sense of reality in the scenes. SUMMARY This study aimed at evaluating effective methods for breaking the hard and insoluble spores of Ganoderma lucidum to recover functional biomolecules. Rupture techniques were evaluated such as manual maceration (RM), maceration with spheres of various materials (BR), and microwave exposure plus maceration with steel/ chrome spheres (MBR1). Spore rupture was evaluated using UV-Vis spectroscopy, which showed vibrations of 2955, 1642, 1240, 1080 and 1746 cm-1 corresponding to changes in spore walls. The MBR1 extract contained the largest amounts of carbohydrates (19.80 mg.g-1 spores) and polyphenols (2.21 mg.g-1 spores), whereas the BR extract had higher antioxidant activity (57.22%Inb DPPH). The MBR1 and BR extracts contained 62.2 and 73.5% glucose, respectively. Both methods also involved significant extraction of carbohydrates and proteins. The best way to extract biomolecules from spore walls is to perform a microwave heat treatment and break the walls with steel/chrome spheres; this produces large quantities of carbohydrates with antioxidant properties.
RESUMEN El objetivo de este estudio fue evaluar varios métodos de ruptura de las esporas de Ganoderma lucidum y extraer sus propiedades bioactivas. Para este propósito se evaluaron diferentes técnicas de rompimiento como: la maceración manual (RM), la maceración con esferas de diversos materiales (BR) y la exposición a microondas junto la maceración de las esporas con esferas de acero/cromo (MBR1). La ruptura de las esporas fue evaluada por espectroscopia UV-Vis, la cual mostró que las vibraciones 2955, 1642, 1240, 1080 y 1746 cm-1 correspondieron a cambios estructurales en las paredes de las esporas. El extracto MBR1 presento el mayor contenido de carbohidratos (19,80 mg.g-1) y polifenoles (2,21 mg.g-1), mientras que el extracto BR tuvo una mayor actividad antioxidante (57,22% Inb DPPH). Los extractos MBR1 y BR también presentaron en el análisis de monosacáridos un 62,2 y 73,5% de contenido glucosa. Como conclusión la mejor metodología para extraer biomoléculas de las paredes de las esporas de G. lucidum fueron el tratamiento térmico con microondas y la ruptura de las paredes con esferas de acero/cromo, porque este proceso permitió la extracción de una mayor cantidad de carbohidratos con posibles propiedades antioxidantes.

BACKGROUND: The morbidity rate of thoracolumbar burst fracture is high; however, the simple posterior distraction and reduction technique has poor patient satisfaction on maintaining vertebral height and reducing complications. Therefore, we attempt to explore a better therapeutic regimen. OBJECTIVE: To assess the efficacy of inverse arch roof breaking technique combined with pedicle screw and bone graft in treatment of thoracolumbar burst fractures. METHODS: This was a retrospective study of 78 patients with thoracolumbar burst fractures. All the patients suffered from fresh closed fractures, and all of them were operated by posterior approach. The time from injury to surgery ranged from 4 to 14 days, with an average of 7.8 days. They were randomly assigned to two groups. The 38 cases in the simple distraction group were treated with simple vertebra pedicle screw-rod system distraction and reduction fixation. The 40 cases in the inverse arch roof breaking and bone graft group were treated with inverse arch roof breaking technique combined with pedicle screw and bone graft in fractured vertebra. All patients signed the informed consent. The study was approved by the Hospital Ethics Committee. Operation time, intraoperative blood loss, fracture healing time, anterior height ratio of injured vertebrae, Cobb angle, visual analogue score, Barthel Index and postoperative complications were measured between the two groups. RESULTS AND CONCLUSION: (1) The follow-up period for all patients was 10-22 months. (2) Operation time and intraoperative blood loss were better in the simple distraction group than in the inverse arch roof breaking and bone graft group (P < 0.01). (3) The ratio of anterior height of injured vertebra and Cobb angle were significantly different between the two groups (P < 0.01). Above indexes were better in the inverse arch roof breaking and bone graft group than in the simple distraction group. (4) There were significant differences in fracture healing time and life activity function (Barthel index) between the two groups (P < 0.01), and above indexes were better in the inverse arch roof breaking and bone graft group than in the simple distraction group. (5) No deep infection was found in both groups. There were no complications such as internal fixation failure and excessive loss of vertebral height in arch roof breaking and bone graft group. In the simple distraction group, there were 3 cases of screw pull-out because of failed internal fixation, 2 cases of titanium rod breakage, and 10 cases of obvious vertebral height loss. (6) These findings suggest that compared with simple distraction and reduction fixation, inverse arch roof breaking technique combined with pedicle screw and bone graft can provide bony support to compression center of fractured vertebra. The efficacy was identified in reconstructing the height of anterior and middle columns. This method has the advantages of high mechanical strength, strong vertebral height maintenance, high bone healing rate and few complications, which will be the ideal choice in the treatment of thoracolumbar burst fracture.

ABSTRACT Introduction Breaking bad news althoughfrequent among healthcare professionals and their patients is still considered a very difficult task. These communication skill main determinants in the physician-patient relationship. Objective In view of the need to promote academic spaces that provide opportunities to learn breaking bad news , thisstudy aims to evaluate the medical residentsin relation to their interest in learning communication skills, as well as their skills in breaking bad news, before and after a workshop on the topic. Methods All the medical residents in the first year ofnternal edicine of a public general hospital in Belém-Pará, Brazil inwere invited to answer a questionnaire sociodemographic data and questions about their communication skills in clinical practice, as well as the Communication Skills Attitude Scale (CSAS), which addressed their interest in learning communication skills. The questionnaire. This research uses both uantitative and qualitative methods. The quantified data were statisticallyanalyzed by the Wilcoxon test (),Chi-Square test G-test adherence (quantitative variables of the questionnaire on communication skills). The qualitative evaluation Content Analysis based on Bardin. Results Ten residents attended the workshop. The results show that after taking part in the workshop, the resident's perceptions of the practice of breaking bad news had improved (in 80% of the participants), as well as their attitudes to learning communication skills ( CSAS = 99.5 and 105, before and after the course, respectively p = 0.0039). Conclusion Eighty percent of residents (n = 08) considered their communication skills have improved as a result of the workshop (p = 0.0078). Most of the participantsbecame more aware of the importance of considering the patient's perspective, and admitted positive changes following the course. Conclusion A positive effect on the participants' perceptions of communication skills and on their interest in learning these skills, were identified after an intervention focused on the context of breaking bad news.
RESUMO Introdução A comunicação de má notícia, embora frequente entre os profissionais de saúde e seus pacientes, ainda é considerada tarefa extremamente difícil. Essa habilidade de dar informação é um dos principais determinantes da relação médico-paciente. Objetivo Considerando a necessidade de promover espaços acadêmicos que proporcionem a aprendizagem da comunicação de notícia difícil, o presente estudo objetivou avaliar a percepção de um grupo de residentes de Clínica Médica quanto ao interesse pelo ensino e aprendizagem de comunicação, assim como suas habilidades de dar notícia difícil, antes e após uma oficina de comunicação de má notícia. Métodos Todos os médicos residentes do primeiro ano de Clínica Médica de um hospital-geral, em Belém, Pará (Brasil), foram convidados a responder a um questionário composto por dados sociodemográficos e perguntas sobre a sua prática de comunicação, além da versão da Escala de Atitudes e Habilidades de Comunicação (Communication Skills Attitude Scale) que aborda o interesse pela aprendizagem das habilidades comunicacionais. O questionário e a escala foram aplicados em dois momentos: antes e após a oficina teórico-prática de comunicação de má notícia, elaborada para esta pesquisa. Trata-se de uma pesquisa quanti-qualitativa. Os dados quantificados foram tratados estatisticamente por meio do teste de Wilcoxon (avaliação do escore do CSAS) e Qui-Quadrado e Teste G de aderência (variáveis quantitativas do questionário sobre habilidades de comunicação). A avaliação qualitativa foi feita pela análise de conteúdo de Bardin. Resultados Dez médicos residentes participaram da pesquisa. Os resultados demonstraram, após a participação na oficina, melhora na percepção dos residentes quanto à prática de comunicação de notícia difícil (em 80% dos participantes) e nas atitudes relacionadas ao interesse pela aprendizagem de comunicação (CSAS = 99,5 e 105, antes e após a oficina, respectivamente - p = 0,0039). Conclusão Um efeito positivo na percepção dos participantes quanto às habilidades de comunicação e seu interesse pelo aprendizado de tais habilidades foi identificado após uma intervenção focalizada no contexto da comunicação de má notícia.

Parents are often concerned about their children's nonnutritive sucking (NNS) habits and may institute mechanisms to try to break them. Aim: The study identified various methods instituted by mothers resident in a suburban Nigeria to break NNS habits of children, reasons for wanting to break the habit, and the association between instituted methods and sociodemographic profile of the mothers. Materials and Methods: The data of 129 mothers of children aged 4 years to 12 years who had NNS habits at the time of conducting a household survey were analyzed. Statistical Analysis: Tests of associations were conducted to determine the association between maternal age and level of education and methods employed to break child's NNS habits. Results: Eighty-four mothers (65.1%) made efforts to break the habit. Habit persisting to older age was the main concern. The most common method employed for breaking habits was punishing the child (54.8%).Only 7 (20.0%) mothers who were concerned about NNS habit(s) sought professional advice. A greater though insignificant percentage of mothers in the 25–34 years age group (57.2%; P = 0.48) and those with secondary school level of education (56.0%; P = 0.12) made efforts to break their children's NNS habits. Conclusions: The majority of mothers were concerned about their children's NNS habits. However, very few concerned mothers sought professional advice. Efforts need to be made to improve the public awareness about the availability of professional services for managing NNS habits and potential impact of employing nonprofessional methods to break habits.

Objective To study the correlation between the particle size and cell -wall breaking ratio ,disso-lution rate of Zhichuanwu powder .Methods The particle size and cell breaking rate of four kinds of Chuanwu powder (40 mesh,60 mesh,80 mesh and 300 mesh) were determined by optical microscope .The dissolution characteristics of the above four kinds of powders were investigated by paddle method .Results The results showed that the particle size distribution,fracture rate and dissolution rate of the Chuanwu powder were different ,within 0-15 hours,with the increase of particle size of the prepared aconite root divided ,the dissolution rate increased from 81.2% to 90.5%, and the cell breaking rate and dissolution rate increased with the decrease of the particle size .Conclusion Moderate changes in the degree of crushing can improve the rate of cell -wall breaking and rate of dissolution of active cells .

Objective: To investigate the ethical decision-making process of breaking confidentiality when counselors dealing with self-inflicted injury and suicide issues in college situation. Methods: A semi-structural interview was addressed to 10 counselors from 7 college counseling centers in Beijing, among whom with (10 ± 8) years of experience on average in this field. Content analysis method was used to transcription of the interviewing data. Results: Totally 8 counselors had received ethical training more or less, and attached great importance to ethical codes. There were still some conflicts between school regulations and confidentiality rules in 7 university counseling centers. Different counselors varied greatly in decision-making on breaking confidentiality when facing college students' self-inflicted injury and suicide. Faced with conflicts between college demands and confidentiality principles, counselors could take the professional standpoint and consider more of the interests of students. Conclusion: The decision-making process on self-inflicted injury and suicide confidentiality breakthrough needs to be standardized. College's attention and support to the counseling work should be strengthen and enhance ethical awareness.

Objective To observe the clinical effects of breaking blood expelling stasis method of traditional Chinese medicine (TCM) combined with edaravone on patients with acute hypertensive intra-cerebral hemorrhage and preliminarily discuss its protection mechanism on this disease. Methods Ninety-two patients with hypertensive intra-cerebral hemorrhage within 72 hours after occurrence admitted to Zengcheng District People's Hosipital of Guangzhou from May 2013 to December 2017 were enrolled, they were divided into conventional therapy group and combined treatment of TCM and western medicine group by random numbers produced by a computer, 46 cases in each group. The conventional therapy group was treated with conventional therapy, and the combined treatment of TCM and western medicine group was treated with conventional therapy, additionally the treatment of Poxue Zhuyu decoction (the ingredients of the decoction: leech 10 g, gradfly 10 g, rhubarb 15 g, cattail pollen 5 g, trichosanthes fruit 10 g, radix notoginseng 5 g, colla plastri testudinis 10 g, grassleaf sweetflag rhizome 10 g, ground beetle 3 g, dried lacquer 3 g, peach seed 10 g) and edaravone for 10 days. The levels of interleukin-6 (IL-6) and tumor necrosis factor-α (TNF-α) in serum were tested by enzyme-linked immunosorbent assay (ELISA) before and 7 days after treatment in the two groups; and the differences in National Institutes of Health stroke scale score (NIHSS) before and 14 days, 90 days after treatment as well as the changes of brain edema around the hematoma 14 days after treatment were compared between the two groups, and the incidence of adverse reaction was observed. Results After treatment, the levels of TNF-α, IL-6 and intra-cerebral hemorrhage quantities in the two groups were all decreased compared with those before treatment, and the degrees of decrease of TNF-α and cerebral hemorrhage volume in combined treatment of TCM and western medicine group were more significant than those in the conventional treatment group [TNF-α (ng/L): 21.00±6.10 vs. 29.40±11.33, cerebral hemorrhage volume (mL): 5.23±0.60 vs. 8.50±0.64, both P <0.05]. The IL-6 in the two groups were recovered to approximately normal levels after the treatment (ng/L: 13.60±5.36 vs. 15.40±6.13, P > 0.05). With the prolongation of therapeutic time, the scores of NIHSS of the two groups were significantly lowered than those before treatment, and the degree of reduction in the combined TCM and western medicine group on 90 days after treatment was more obvious than that of the conventional treatment group (4.34±0.67 vs. 7.73±0.61, P < 0.05). The volumes of edema around hematomas were increased after treatment in the two groups, but the degree of increase in combined treatment of TCM and western medicine group was slower than that of the conventional group (cm3: 7.57±0.64 vs. 10.16±0.60, P < 0.05). There was no statistical significant difference in the incidence of adverse reactions in comparison between the combined treatment of TCM and western medicine group and conventional therapy group [18.2% (8/44) vs. 20.5% (9/44), P > 0.05]. Conclusions Using breaking blood expelling stasis and edaravone for treatment of acute hypertensive intra-cerebral hemorrhage can accelerate the absorption of brain hematoma and improve the neurological function, and its mechanism may be relevant to the inhibition of some inflammatory factors.
