RESUMO

Objective:To correlate neutrophil-to-lymphocyte ratio (NLR), platelet-to-lymphocyte ratio (PLR), and D-dimer (DD) with the severity of acute cholangitis.Methods:The clinical data of 96 patients with acute cholangitis who received treatment in Panjin Central Hospital from September 2019 to March 2021 were retrospectively analyzed. These patients were divided into three groups according to the severity of acute cholangitis: 36 patients with mild acute cholangitis (group A), 35 patients with moderate acute cholangitis (group B), and 25 patients with severe acute cholangitis (group C). The correlation between age, sex, NLR, PLR, DD, and the severity of acute cholangitis was compared among the three groups.Results:In groups A and B, the area under the receiver operating characteristic curve (AUC) showing the performance of DD, NLR, and PLR levels in predicting acute cholangitis was 0.800, 0.838, and 0.721, respectively, with the optimal cut-off value of 1.985 mg/L, 9.589, and 154.410, respectively. Among them, NLR had the largest AUC, and the highest sensitivity (82.9%), and had a high diagnostic value. In groups B and C, the AUC for DD, NLR, and PLR was 0.967, 0.915, and 0.543, respectively, with the optimal cut-off value of 6.000 mg/L, 22.390, and 264.220, respectively. DD and NLR had a diagnostic significance (both P < 0.05), but PLR had no diagnostic significance ( P > 0.05). The AUC for DD was the largest, and therefore DD had a great diagnostic significance. When NLR, PLR, and DD were jointly detected, the AUC was the highest and the diagnostic value was the highest. The AUC in groups A and B was 0.866, and that in groups B and C was 0.977. Conclusion:The levels of DD, NLR, and PLR increase in patients with acute cholangitis, which are related to the severity of the disease. DD, NLR, and PRL can be used as indicators to evaluate mild and moderate acute cholangitis, and NLR has the highest diagnostic value. DD and NLR can be used as indicators to evaluate moderate to severe acute cholangitis, and the effect of DD is superior to that of NLR. The combined detection of the three indicators can increase the value to evaluate the severity of acute cholangitis, and its effect is superior to that of a single detection. The combined detection of NLR, PLR, and DD is helpful for the clinical diagnosis and treatment of acute cholangitis.

Active targeted drug delivery methods facilitate effective uptake of functionalized nanoparticles through receptor-mediated transcytosis. In recent years, albumin-nanoparticle interaction has been critically examined so that this functionalized nanoparticle can be efficiently loaded with drugs. The present investigation aims at understanding the adsorption of Bovine Serum Albumin (BSA) on Silver Nano-particle (SNP) surface, preparation of soft conjugates (SC) and hard conjugates (HC) of BSA-functionalized SNP (SNP-BSA), and their interaction with curcumin (CUR). HC contains tightly bound BSA whereas SC involves tightly and loosely bound BSA. Increase in the hydrodynamic radii of conjugates was observed upon SNP incubation with increased concentration of BSA. Three different SNP-BSA conjugate ratios were selected to study their interaction with CUR. Fluorescence spectroscopy showed a strong association between CUR and SNP:BSA conjugates. However, binding varied with a change in the conjugate ratio. Circular Dichroism (CD)/Fourier Transform Infrared (FTIR) spectroscopy revealed the alterations in the secondary structure of BSA upon CUR binding to the conjugates. Zeta potential data indicated stable conjugate formation. CUR in SNP:BSA conjugate was found to have a higher half-life as compared to the control. We believe that this is the first biophysical characterization report of conjugates that can be effectively extrapolated for targeted drug delivery.

Objective To investigate the effect of curcumin (Cur)on AngⅡ-induced proliferation and oxidative stress of vascular smooth muscle cells (VSMCs).Methods Primary rat VSMCs were cultured and divided into control group,AngⅡ group,AngⅡ+Cur 5μmol/L group,AngⅡ+Cur 10μmol/L group,AngⅡ+Cur 20μmol/L group,and Cur 20μmol/L group.The proliferation of AngⅡ-induced VSMCs was measured by MTT assay.The mRNA and protein expressions of inducible nitric oxide synthase (iNOS)and p47phox were detected by real-time PCR and Western blot.Nitric oxide (NO)production was measured by Griess reaction.Production of intracellular reactive oxygen species (ROS)was measured by DCFH-DA staining,and the activities of superoxide dismutase (SOD)and glutathione peroxidase (Gpx)were detected by xanthine oxidase assay and visible spectrophotometer. small interfering RNA (siRNA)was used to silence the expression of p47phox to further explore the mechanism for Cur inhibiting the proliferation of AngⅡ-induced VSMCs and oxidative stress.Results VSMCs activities were not significantly affected by Cur at the concentration between 0 and 80μmol/L.Cur (5,10 and 20μmol/L)significantly inhibited AngⅡ-induced proliferation of VSMCs.Cur had an inhibitory effect on the overexpression of NO,iNOS, p47phox and ROS in VSMCs and upregulated the activities of SOD and Gpx in a concentration-dependent manner. AngⅡ-induced ROS production in VSMCs was significantly attenuated by pretreatment with p47phox specific siRNA.Conclusion Cur can inhibit the proliferation and oxidative stress of AngⅡ-induced VSMCs.

Objective To understand the attitude and current situation of medical English stud y as well as its relevant factors among long schooling medical students so as to provide first hand information for constructing students-centered medical English teaching model. Methods A self-de-signed questionnaire including basic information of students, attitude and current situation of medical English students was given to 134 seven or eight year program clinical medical graduates from grade 2006 to grade 2008 in Tongji University. Questionnaires were collected timely and an-alyzed with SPSS 19.0 and results were described by percentage. Results Totally 132 copies of questionnaire were recovered with an recovering rate of 98.51%. 56.82%(75/132) students had interest in medical English study and 71.97%(95/132)students had a will to go abroad for learning exchange. Main pur-poses for students to learn medical English were ‘professional literature reading’(68.94%,91/132),‘SCI papers writing’(59.09%,78/132)and ‘international communication’(58.33%, 77/132). Main influencing factors of medical English study were ‘boring teaching methods ’ ( 45 . 45%, 60/132 ) ,‘difficult medical English’(41.67%,55/132) and ‘insufficient teaching time’(40.91%,54/132).Conclusions Medical students with long schooling have demand for medical English study;however, most of them have some difficulties in practical application. Measures should be taken from three as-pects including students, teachers and curriculum setting to improve students' medical English.

RESUMO

A high-speed counter-current chromatography (HSCCC) method was successfully developed for the preparative separation and purification of deoxyschizandrin from Schisandrae Sphenantherae Fructus in one step. The purity of deoxyschizandrin was 98.5%, and the structure was identified by MS, UV and NMR. This method was simple, fast, convenient and appropriate to prepare pure compound as reference substances for related research on Schisandrae Sphenantherae Fructus.

PURPOSE: To investigate the accommodating potency of the 1CU accommodative posterior chamber intraocular lens (PC IOL), according to pilocarpine-induced ciliary muscle contraction, near visual acuity, and distance-corrected near visual acuity after implantation of the IOL. METHODS: In a prospective study, 9 eyes of 7 patients (mean age 48+/-14.9 years [range 23 to 71 years]) had phacoemulsification and PC IOL (1CU(R), Humanoptics). Distance refraction, distance visual acuity (VA), distance-corrected VA (DCVA), near visual acuity (NVA), and distance-corrected near visual acuity (DCNVA) were determined at 2 week, 1, 2, 6, and 12 months after surgery. Anterior chamber depth (ACD) was measured 3 times with the Orbscan(R) (Orbtek, U.S.A.), and averaged. Then ACD was repeatedly measured within 90 minutes after administration of pilocarpine 2% eyedrops 3 times at 10 minutes interval. RESULTS At 2 weeks, 1, 2, 6 and 12 months after surgery, the mean DCNVA was 0.55+/-0.23, 0.37+/-0.14, 0.40+/-0.22, 0.51+/-0.20, and 0.56+/-0.20, respectively. The mean ACD decrease (mm) was 0.57+/-0.77, 0.91+/-0.69, 0.17+/-0.45, 0.35+/-0.40 and 0.59+/-0.31, respectively, using Orbscan(R) after pilocarpine 2% eyedrops. There was statistically significant correlation between the pilocarpine-induced IOL movement and DCNVA (r=0.439, P=0.012). CONCLUSIONS: 1CU accommodative PC IOL provided relatively good DCNVA and accommodating potency for up to 1 year. It is necessary to investigate accommodative change for a longer time period.

Many software systems have been developed concerning the collection and collation of pharmaceutical information data through microcomputers and the initiation of clinical drug-use consulta- tion","clinical anti-biotic drug-use consultation","emergency treatment of drug toxication","consulta- tion on injection compatibility","data control of clinical drug-use monitoring",etc.These systems are charaterized by their large information storage,rapid retrieval,practical use,simple operation and easy control.They highly satixfactory roles in raising the level of rational clinical drug-use.But a unified or- ganization and planning is lacking in the software devdlopment of drug-use consultation.Scattering and overlapping are not uncommon.A lag in the innoration of softwares and in the replenishment of data does dxist.The functions of consultation are many while those of statistical analysis are not enough. Those shortcomings should be gradually overcome.

RESUMO

The effect of OQP, a pormula of traditional Chinese medieine belonging to the category "Precription of the Reinforeing Qi and Enriching Blood for keep the Original Qi", on cell strain of human hepatic carcinoma SMMC-7721 was observed. The results showed that OQP depressed the incorporation of both ~3H-TdR and ~(14)C-UR in cells of hepatic carcinoma. This indicated that OQP inhibited the synthesis of DNA and RNA, and the growth of the cancer cell. Such effect may result from the low ratio Cu/Zn and the high content of Se in OQP. In conclusion, in this study it has been demonstrated that OQP can not only improve the general condition of patients in advanced liver cancer and lcukopenia of patients in chemotherapy and radiotherapy, but also can inhibite the growth of the cell of hepatic carcinoma directly.
