Objective@#To investigate the potential caries prevention mechanism of the Xinjiang Mori cortex and to analyze its effect on the main cariogenic bacteria.@*Methods@#The active components of the Xinjiang Mori cortex and the main targets were predicted and screened using the TCMSP database. The GeneCards, DisGENET and TTD databases were used to obtain caries-related targets. The common targets were derived, and core genes were screened. The enrichment analysis was performed using the DAVID data platform. Molecular docking was performed using AutoDock software. In in vitro antibacterial experiments, first, the 50% minimum inhibitory concentration (MIC50) and the minimum bactericidal concentration (MBC) of the Xinjiang Mori Cortex extract against Streptococcus mutans, Streptococcus sanguis and Actinomyces viscosus were determined and the growth curves were measured. The effects of the Xinjiang Mori Cortex extract on acid production, polysaccharide production and adhesion ability of Streptococcus mutans, Streptococcus sanguis and Actinomyces viscosus in the planktonic state were determined. The 50% minimum biofilm inhibition concentration (MBIC50) and 50% minimum biofilm reduction concentration (MBRC50) were determined by crystal violet staining, and biofilm morphology was visualized using scanning electron microscopy (SEM).@*Results@#The main active components of the Xinjiang Mori cortex included quercetin, kaempferol, and β-sitosterol. Tumor necrosis factor (TNF), interleukin-6 (IL-6), and interleukin-1beta (IL-1β) could be the most important targets of the Xinjiang Mori cortex for the prevention of dental caries. The enrichment analysis results showed that Mori cortex extract may have effects on the AGE-RAGE signaling pathway, IL-17 signaling pathway, and TNF signaling pathway. The antibacterial experiment results showed that the MIC50 values of Xinjiang Mori Cortex extract against Streptococcus mutans, Streptococcus sanguis and Actinomyces viscosus were 0.5, 0.5 and 0.25 mg/mL, respectively, and the MBCs were 4.0, 2.0 and 1.0 mg/mL, respectively. The inhibitory effect of Xinjiang Mori Cortex extract on the acid production, polysaccharide production and adhesion ability of three major cariogenic bacteria in the planktonic state was stronger than that of the control group, and the differences were statistically significant (P<0.05). The MBIC50 was 1.0, 1.0, and 0.5 mg/mL, and the MBRC50 was 4.0, 4.0, and 2.0 mg/mL. SEM observation showed that the amount of biofilm formation decreased with the drug concentration compared with the control group.@*Conclusion@#Xinjiang Mori cortex extract can prevent caries through quercetin, kaempferol, and β-sitosterol active ingredients, TNF、IL-6、IL-1β key targets and multiple pathways and inhibit the growth, acid production, polysaccharide production, and adhesion ability of three major cariogenic bacteria in the planktonic state and has some inhibitory effect on corticogenic biofilm formation.

A imaturidade do sistema imunológico, associado a Determinantes Sociais de Saúde (DSS), promove doenças na infância. Especificamente, na cavidade oral, os DSS, representados pelo consumo elevado de açúcar, limitado acesso aos serviços de saúde e deficiência na higiene bucal, favorecem transtornos locais e sistêmicos. Assim, o estudo objetivou associar os DSS, no contexto das condições socioeconômicas, do acompanhamento pelo serviço de saúde e dos aspectos relacionados à saúde bucal de crianças atendidas em Unidades Básicas de Saúde (UBS) de um município cearense. Trata-se de estudo observacional, analítico, transversal e de abordagem quantitativa, conduzido com crianças e suas mães em Acarape - CE. Após consentimento, essas preencheram um questionário. Os dados foram analisados. Das 70 mães, 87,14% e 90,00% tinham idade inferior ou igual a 30 anos e renda de até um salário mínimo, respectivamente. Das 70 crianças, 87,14% tinham seus dentes/gengiva higienizados por seus pais ou responsável. Do total, 94,29% nunca se submeteram a atendimento odontológico. Observou-se associação significativa entre a mãe ter escolaridade superior ao ensino fundamental incompleto e higienizar os dentes/gengiva do filho com escova dental e dentifrício. Constatou-se associação significativa entre a criança ingerir bolacha doce/recheada, não consumir refrigerante e usar escova dental e dentifrício na higienização oral. Conclui-se que as crianças eram acompanhadas nas UBS regularmente; no entanto, esse serviço não esteve relacionado ao atendimento odontológico. Apesar da ausência desse tipo de acompanhamento e do consumo de alimentos cariogênicos, as mães se preocupavam com a saúde bucal das crianças, higienizando a cavidade oral diariamente, com meios adequados.


The immaturity of the immune system, associated with Social Determi- nants of Health (SDH), promotes diseases in childhood. Specifically, in the oral cavity, SDH, represented by high sugar consumption, limited access to health services, and poor oral hygiene, favors local and systemic disorders. Thus, the study aimed to associate the SDH, in the context of socioeconomic conditions, monitoring by the health service and aspects related to children's oral health assisted in Basic Health Units (BHU) of a muni- cipality in Ceará. This is an observational, analytical, cross-sectional study with a quan- titative approach conducted with children and their mothers in Acarape - CE. After con- sent, they filled out a questionnaire. Data were analyzed. Of the 70 mothers, 87.14% and 90.00% were aged less than or equal to 30 years and had income up to one minimum wage, respectively. Of the 70 children, 87.14% had their teeth/gums cleaned by their pa- rents or guardian. Of the total, 94.29% never underwent dental care. There was a signifi- cant association between the mother having higher education than incomplete elementary school and cleaning the child's teeth/gums with a toothbrush and toothpaste. A significant association was found between the child eating sweet/stuffed biscuits, not consuming soft drink, and using a toothbrush and dentifrice for oral hygiene. It is concluded that the chil- dren were regularly monitored at the BHU; however, this service was not related to dental care. Despite the absence of this type of follow-up and the consumption of cariogenic foods, the mothers were concerned about their children's oral health, cleaning the oral cavity daily with adequate means.


La inmadurez del sistema inmunológico, asociada a los Determinantes So- ciales de la Salud (DSS), promueve enfermedades en la infancia. Específicamente, en la cavidad bucal, los DSS, representados por el alto consumo de azúcar, el acceso limitado a los servicios de salud y la mala higiene bucal, favorecen los trastornos locales y sisté- micos. Así, el estudio tuvo como objetivo asociar el DSS, en el contexto de las condicio- nes socioeconómicas, el seguimiento por el servicio de salud y los aspectos relacionados con la salud bucal de los niños atendidos en las Unidades Básicas de Salud (UBS) de un municipio de Ceará. Se trata de un estudio observacional, analítico, transversal con enfo- que cuantitativo, realizado con niños y sus madres en Acarape ­ CE. Después del consen- timiento, completaron un cuestionario. Los datos fueron analizados. De las 70 madres, 87,14% y 90,00% tenían edad menor o igual a 30 años e ingresos hasta un salario mínimo, respectivamente. De los 70 niños, al 87,14% se les limpió los dientes/encías por sus pa- dres o tutores. Del total, el 94,29% nunca realizó atención odontológica. Hubo una aso- ciación significativa entre la madre con educación superior a la primaria incompleta y la limpieza de los dientes/encías del niño con cepillo y pasta dental. Se encontró una asoci- ación significativa entre el niño que come galletas dulces/rellenas, no consume gaseosas y usa cepillo de dientes y dentífrico para la higiene bucal. Se concluyó que los niños eran monitoreados periódicamente en la UBS; sin embargo, este servicio no estaba relacionado con el cuidado dental. A pesar de la ausencia de este tipo de seguimiento y del consumo de alimentos cariogénicos, las madres se preocupan por la salud bucal de sus hijos, reali- zando la limpieza de la cavidad bucal diariamente, con medios adecuados.

Introducción: Las enfermedades bucales se encuentran entre las cinco de mayor demanda de atención en los servicios de salud a nivel mundial. Materiales y métodos: Se realizó una investigación observacional analítica retrospectiva de casos y controles, con el objetivo de determinar el grado de asociación entre la caries dental y los factores de riesgo en la población infantil de 5 a 12 años del Consultorio del Médico de la Familia no. 3, pertenecientes al Policlínico Docente Belkis Sotomayor Álvarez, del municipio Ciego de Ávila, durante el período de octubre de 2021 a marzo de 2022. El grupo de casos estuvo integrado por 62 niños, al igual que el grupo control. Se utilizó la técnica de independencia basada en la distribución de Chi-cuadrado, para determinar la presencia de asociación estadística entre variables cualitativas, y se realizó el cálculo del Odd ratio para la cuantificación de riesgo. Resultados: El grupo de 5 a 8 años fue el de mayor número de niños con caries dental, para un 59,7 %. El 62,9 % solo se cepillaban los dientes con una frecuencia de dos o menos veces al día. El antecedente de caries dental, la enfermedad gingival y la ingestión de alimentos azucarados, estuvieron relacionados con la aparición de caries dental. Conclusiones: La dieta cariogénica resultó ser el factor de riesgo con mayor puntaje en la incidencia de la caries dental en la muestra estudiada.


Introduction: Oral diseases are among the five diseases demanding more care in health services worldwide. Materials and methods: A descriptive, analytic, observational research of cases and controls was carried out, with the objective of determining the association degree between dental caries and risk factors in the child population aged from 5 to 12 years from the Family Physician Consultation Nr. 3, belonging to the Teaching Polyclinic Belkis Sotomayor Alvarez, in the municipality Ciego de Avila, during the period from October 2021 to March 2022. The case group consisted of 62 children, the same as the control group. The independence technique based on the Chi-square distribution was used to determine the presence of statistical association between qualitative variables, and the Odd ratio was calculated for risk quantification. Results: The group from 5 to 8 years was the one with higher number of children with dental caries, for 59.7%. 62.9% only brushed their teeth with a frequency of two or less times a day. History of dental caries, gingival disease and sugary food ingestion were related to the appearance of dental caries. Conclusions: The cariogenic diet proved to be the risk factor with the higher score in the incidence of dental caries in the sample studied.

@#Caries is a chronic infectious disease that occurs in the hard tissues of teeth. The interaction of oral microorganisms, host, food and time leads to the occurrence and progression of caries. Free sugar is an important food factor in the occurrence of dental caries. This paper reviews the research progress on the relationship between free sugar and caries in recent years, providing a reference for further clarifying the mechanism of the occurrence and progression of caries caused by free sugar and exploring the methods of caries prevention. The cariogenic mechanism of free sugar is multifaceted. Free sugar can not only be used by oral microorganisms in dental plaque biofilms to produce acid and synthesize intracellular and extracellular polysaccharides but also cause an imbalance in oral microecology and enhance gene expression related to the cariogenic toxicity of oral microorganisms. Based on the correlation between free sugar and caries, it is important to limit the intake amount and frequency of free sugar to prevent caries. This can be achieved mainly by formulating public health policies for reducing sugar, creating a low-sugar family environment with good eating habits, using sugar substitutes and using fluoride. There are few studies on the relationship between free sugar and oral microecology. More research is needed to better understand the effect of free sugar on oral microecology and its mechanism and to validate the caries-preventing effect of various sugar reduction measures. We believe such studies would open up new avenues for the effective prevention of caries.

@#Antimicrobial peptides have antibacterial effects on various pathogenic microorganisms, including natural antimicrobial peptides and synthetic antimicrobial peptides. According to the structure of natural antimicrobial peptides, synthetic antimicrobial peptides can be obtained by recombining different functional domains, adjusting the original amino acid sequence, or completely redesigning the peptides from scratch. Antimicrobial peptides can inhibit the growth of various cariogenic microorganisms and the formation of microbial biofilms. They also reduce acid production and acid resistance of microorganisms. Natural antimicrobial peptide genes can be used as genetic susceptibility markers for predicting the development of caries, thus, showing potential applications in the prevention and treatment of dental caries. The instability of natural antimicrobial peptides and the inability to achieve targeted sustained release limit their application in the prevention and treatment of oral caries. Synthetic antimicrobial peptides can enhance their stability and the antibacterial effect. Synthetic antimicrobial peptides can also be polymerized with common oral adhesives to reduce the incidence of microleakage after filling treatment for caries and to prevent the occurrence of secondary caries. The pH-sensitive antimicrobial peptides are slowly released to promote remineralization in the process of caries. However, the safety and biocompatibility of synthetic antimicrobial peptides are worse than those of natural antimicrobial peptides. Moreover, the combined effect of antibacterial peptides and anticaries drugs, such as fluoride, is still uncertain. Therefore, in this paper, we will review the design methods, application and underlying mechanisms of antimicrobial peptides to introduce novel methods and ideas for the prevention and treatment of dental caries.

@#The oral microecological balance is closely associated with the development of dental caries. Oxidative stress is one of the important factors regulating the composition and structure of the oral microbial community. Streptococcus mutans is closely related to the occurrence and development of dental caries. The ability of S. mutans to withstand oxidative stress affects its survival competitiveness in biofilms. The oxidative stress regulatory mechanisms of S. mutans include the synthesis of reductase, the regulation of iron and manganese uptake by metalloregulatory proteins, transcription regulator Spx, extracellular uptake of glutathione and other related signal transduction systems. The current research focuses on how S. mutans adapts to a complex external environment through an oxidative stress response and its influence on oral microecology. We can design targeted small molecular compounds for key signaling pathways to inhibit oxidative stress and weaken the virulence of S. mutans, which is important for oral microecological modulation and dental caries prevention and treatment.

ABSTRACT Objective: To evaluate the characteristics and factors associated with the intake of ultra-processed cariogenic foods (UFC) by preschoolers during the COVID-19 pandemic. Material and Methods: This is a cross-sectional study involving parents of 672 children from two to five years old enrolled at public schools in Curitiba, Brazil. Parents answered a questionnaire about socioeconomic and demographic data, their behavior regarding the dietary education of their children (Parent Mealtime Action Scale - translated and validated for use in Brazil), and children's food intake (qualitative food frequency questionnaire - list of foods based on a report from the Pan American Health Organization). The data were analyzed using Poisson regression analysis (α=0.05). Results: About 43% of parents/guardians reported changes in their children's diet during the pandemic, being that diet got worse and better in 19% and 24% of the cases, respectively. The ultra-processed cariogenic foods with the highest daily intake frequencies were sweetened juices/sweetened drinks (0.52), followed by cookies (0.37), and candies (0.35). Parents with a lower level of education reported a daily frequency of UCF intake 1.36 times higher (PR=1.359; CI 95%: 1.106-1.669) in their children compared to those with a higher level of education. On the other hand, parents'/guardians' report of higher intake and greater offer of fruits and vegetables to children was associated with low UCF intake (PR=0.716; CI 95%: 0.592-0.866). Conclusion: The lower level of formal education of parents/guardians and lower availability of fruits and vegetables were related to higher consumption of ultra-processed cariogenic foods by children.

Introducción: La salud oral tiene un impacto durante todo el proceso del ciclo vital del ser humano. Los hábitos alimenticios inciden directamente en una buena salud bucal, donde la dieta cariogénica es de interés fundamental. Objetivo: Describir el nivel de conocimiento sobre dieta cariogénica de las madres con niños entre 3 y 6 años, bailarines de la Academia Stone en Ecuador. Método: Se realizó un estudio descriptivo trasversal, el universo lo constituyeron las madres con niños en edades entre 3 y 6 años, se trabajó con todo el universo (n=25). Se definieron las variables: criterios que tomaban en cuenta para elegir los alimentos, cuidado de higiene bucal en los menores, frecuencia de ingesta de hidratos de carbonos, primera consulta odontológica, diagnóstico dado y motivo para no asistir a la consulta. Para la recolección de la información se utilizó un cuestionario tipo Likert. Resultados: El 75 % de las madres se basó en una tabla nutricional al momento de elegir un alimento. El 25 % refirió no leer una tabla nutricional por falta de tiempo y el 32 % refirió consumir alimentos fuera de casa los fines de semana. El 20 % no cepillaba los dientes de sus niños. En la primera consulta el 60 % indicó que ya contaban con caries y el 50 % no había asistido a dicha consulta, de estos últimos el 30 % debido a limitaciones económicas. Conclusiones: El nivel de conocimiento sobre dieta cariogénica en madres con niños de 3 a 6 años es insuficiente.


Introduction: Good oral health has an essential impact throughout the human life cycle. The correct eating habits have proven to be effective on good oral health, where cariogenic diet is of fundamental interest. Objective: To describe the level of knowledge about cariogenic diet of mothers with children between 3 and 6 years old, dancers at the Stone Dance Academy in Ecuador. Method: A descriptive cross-sectional study was carried out; the universe was constituted of mothers (n=25) with children between 3 and 6 years of age, involved all of them in the research. The following variables were defined: criteria they took into account when choosing food for children, oral hygiene care they maintained in children, frequency of carbohydrates, first dental consultation, diagnosis given at the dentist's office, and which was the reason why the mothers not taken their children to their first dental visit. A Likert-type questionnaire was used for the information collected. Results: The 75% of mothers relied on a nutritional table when choosing the food for their children intake. The 25% reported not reading a nutritional table due to lack of time and 32% reported consuming food away from home on weekends. The 20% 20 % did not brush their children's teeth. At the first dental check-up, 60% indicated that they already had caries, 50% had never been attended such a consultation, where from them 30% reported never been attend due to low income. Conclusions: The level of knowledge about cariogenic diet in mothers with children from 3 to 6 years of age is insufficient.


Introdução: A saúde oral tem impacto em todo o processo do ciclo de vida do ser humano. Os hábitos alimentares afetam diretamente a boa saúde bucal, onde a dieta cariogênica é de fundamental interesse. Objetivo: Descrever o nível de conhecimento sobre dieta cariogênica de mães com filhos de 3 a 6 anos, bailarinas da Academia Stone do Equador. Método: Realizou-se um estudo descritivo transversal, o universo foi constituído por mães com filhos entre os 3 e os 6 anos, trabalhamos com todo o universo (n=25). Foram definidas as variáveis: critérios tidos em conta para a escolha dos alimentos, cuidados de higiene oral nos menores, frequência da ingestão de hidratos de carbono, primeira consulta dentária, diagnóstico dado e motivo da não comparência à consulta. Para a coleta de informações, foi utilizado um questionário do tipo Likert. Resultados: 75% das mães se basearam na tabela nutricional para escolher um alimento. 25% relataram não fazer a leitura da tabela nutricional por falta de tempo e 32% relataram consumir alimentos fora de casa nos finais de semana. 20% não escovavam os dentes dos filhos. Na primeira consulta, 60% indicaram que já tinham cárie e 50% não compareceram à consulta, destes 30% por limitações econômicas. Conclusões: O nível de conhecimento sobre dieta cariogênica em mães com filhos de 3 a 6 anos é insuficiente.

Aim: The consumption of foods rich in sugar is linked to several non-communicable diseases, including dental caries. Coconut sugar has systemic benefits due to its lower glycemic indexes (GI) than other table sugars. However, there is currently no data regarding its ca-riogenic potential. This study aimed to evaluate the effect of coconut sugar on acidogenicity and adhesion of Streptococcus mutans in vitro, compared to sugarcane products. Materials and methods: Aliquots of cultu-res of S. mutans UA159 were resuspended in a buffer solution enriched with coconut sugar, crystal sugar (refined sugar), and minimally processed sugarcane (demerara light brown sugar and maskavo dark brown sugar), as well as positive (sucrose) and negative con-trols. The decrease in pH and its corresponding area under the curve (AUC; cm2) were evaluated for the analysis of acidogenicity. S.mutans was incubated in BHI supplemented with each sugar and the percentages of microbial adhesion were calculated. After testing data normality, the one-way ANOVA test (Bonferroni post hoc) was used to compare the AUC and the pro-portion of adhesion of each group. Results: Regarding the acidogenic potential, statistical differences were found only between the negative control versus all other groups (p<0.001). Likewise, no significant difference in adhesion was found between the tested sugars (p>0.05). Discussion: Although the tested sugars are marketed as "healthy products," their amount and frequency of usage should be controlled. Conclusion: Coconut su-gar presents a similar cariogenic potential to that of sugarcane products when acidogenicity and adhesion are evaluated. Coconut sugar is not indicated as a substitute for sucrose in the control of cariogenic activity.


Objetivo: O consumo de alimentos ricos em açúcar está associado a diversas doenças não transmissíveis, incluindo a doença cárie. O açúcar de coco tem benefícios sistêmi-cos devido aos seus índices glicêmicos (IG) mais baixos do que outros açúcares de mesa. No entanto, atualmente não há dados sobre seu potencial cariogênico. Esse estu-do teve como objetivo avaliar o efeito do açúcar do coco na acidogenicidade e adesão de Streptococcus mutans in vitro, em comparação com produtos derivados da cana--de-açúcar. Materiais e métodos: alíquotas de culturas de S. mutans UA159 foram suspensas em solução tampão enriquecida com açúcar de coco, açúcar cristal (açúcar refinado) e cana-de-açúcar minimamente processada (açúcar mascavo demerara claro e açúcar mascavo), além de controles positivo (sacarose) e negativo. A diminuição do pH e correspondente área sob a curva (AUC; cm2) fo-ram avaliadas na análise de acidogenicidade. S. mutansfoi incubado em BHI suplementado com cada tipo de açúcar e as porcentagens de adesão microbiana foram calculadas. Após testar a normalidade dos dados, o teste ANOVA de uma via (Bonferroni post hoc) foi utilizado para comparar a AUC e a proporção de adesão de cada grupo. Resultados: Em relação ao potencial acidogênico, diferenças estatísticas foram encontradas apenas entre o controle negativo versus todos os outros grupos (p <0,001). Não foi encontrada diferença significativa na adesão en-tre os açúcares testados (p> 0,05). Discussão: Apesar dos açúcares testados serem comercializados como "produtos saudáveis", sua quantidade e frequência de consumo deve ser controlada. Conclusão: O açúcar do coco apresenta potencial cariogênico semelhante à dos produtos da cana-de-açúcar quanto a acidogenicidade e a adesão. O açúcar de coco não é indicado como substituto da sacarose no controle da atividade cariogênica.

Objective @#To investigate the correlation between vitamin D levels and the risk of early childhood caries (ECC), and to provide a reference for the primary prevention of ECC in children.@*Methods @# A total of 389 children aged 6 months to 48 months were enrolled in this study. Data of feeding patterns and oral health behaviors were collected, and decayed-missing-filled teeth (dmft) were recorded. The children were divided into an ECC group (dmft > 0, n= 146) and a caries-free group (dmft=0, n=243). Peripheral blood of fingertips was collected to detect the active form of vitamin D--25(OH)D in vivo, and the cariogenic activity of dental plaque was analyzed with the Cariostat test. Logistic regression analysis was carried out to analyze the correlation among caries related factors such as 25(OH)D levels and caries risk in patients with ECC.@*Results@#There was no significant difference in the prevalence of dental caries among 25(OH)D deficiency, insufficiency and normal children (χ2=2.320, P=0.313). There was no correlation between dmft and 25(OH)D levels (dmft=1-3, r < 0.001, P > 0.05; dmft > 3, r= 0.009, P > 0.05). The risk factors for ECC were age (OR=1.082, 95% CI: 1.045-1.121, P < 0.001), breastfeeding within 6 months after birth (OR=2.789, 95% CI: 1.581-4.921, P < 0.001), nighttime milk consumption or sleeping with a nipple (OR=4.187, 95% CI: 1.938-9.048, P < 0.001), and a high Cariostat value of 1.5-3.0 (OR=4.173, 95% CI: 2.014-8.646, P < 0.001). @*Conclusion@#There was no correlation between 25(OH)D level and caries in children aged 6-48 months. The risk factors for ECC are age, breastfeeding before 6 months old, nighttime milk consumption or sleeping with a nipple and high cariogenic activity (Cariostat value of 1.5-3.0). It is necessary to establish good feeding habits and oral hygiene habits for children while promoting breastfeeding.

El blanqueamiento dental está considerado como pieza fundamental en el embellecimiento de los seres humanos, ya que permite la restauración de la "sonrisa perfecta". Este proceso en sí, es poco invasivo y juega como un gran papel como aliado en la restauración satisfactoria de la sonrisa y autoestima del paciente. Es un procedimiento que debe ser aplicado cuidadosamente para lograr los efectos positivos del mismo. El fundamento de esta técnica es aclarar la tonalidad que han sufrido los dientes por diversos factores: extrinsecos, intrínsecos y decoloraciones internas. Durante el procedimiento, es usual el uso de peróxido de hidrógeno (H2O2) en concentraciones que van del 10 al 32 % en volumen o el peróxido de carbamida, un compuesto conformado por peróxido de hidrógeno y urea concentraciones del 10 al 22 %. El uso de peróxido de hidrógeno se lleva a cabo fundamentalmente en los consultorios, mientras que el uso del peróxido de carbamida es un procedimiento doméstico. A pesar de los excelentes resultados que se obtiene al usar ambos blanqueadores, su uso puede ocasionar erosiones dentales y sensibilidad dentaria. El primer caso, puede llevar a la adherencia de bacterias cariogénicas como el Strepctococus mutans responsable de caries. Los resultados obtenidos, demostraron que el peróxido de hidrógeno es un agente más agresivo que el peróxido de carbamida, lo cual origina mayor sensibilidad dentaria y un mayor control bacteriano; en cambio el peróxido de carbamida fue mejor blanqueador y originó menor sensibilidad dental(AU)


Tooth whitening is considered a fundamental piece in the beautification of human beings, since it allows the restoration of the "perfect smile". This process itself is minimally invasive and plays a great role as an ally in the satisfactory restoration of the patient's smile and self-esteem. It is a procedure that must be carefully applied to achieve its positive effects. The basis of this technique is to clarify the shade that the teeth have suffered due to various factors: extrinsic, intrinsic and internal discoloration. During the procedure, the use of hydrogen peroxide (H2O2) in concentrations ranging from 10 to 32% by volume or carbamide peroxide, a compound made up of hydrogen peroxide and urea concentrations of 10 to 22%, is usual. The use of hydrogen peroxide is mainly carried out in offices, while the use of carbamide peroxide is a home procedure. Despite the excellent results obtained by using both whiteners, their use can cause dental erosion and tooth sensitivity. The first case can lead to the adherence of cariogenic bacteria such as Streptococcus mutans responsible for caries. The results obtained showed that hydrogen peroxide is a more aggressive agent than carbamide peroxide, which causes greater dental sensitivity and greater bacterial control; On the other hand, carbamide peroxide was a better whitener and caused less dental sensitivity(AU)

Introducción: El tipo de dieta influye directamente en el valor del pH salival, el cual es un factor importante para evitar la formación de caries. Objetivo: Evaluar el cambio del pH salival por ingesta cariogénica y no cariogénica en preescolares de una institución educativa de Huaura, Perú. Métodos: Esta investigación fue de tipo observacional, comparativo y longitudinal. El tamaño de muestra fue de 30 preescolares. Después que los niños consumieron dieta cariogénica y no cariogénica, se evaluó su pH a diferentes tiempos. Para el contraste de resultados se utilizó la prueba T Student y U de Mann-Whitney. Resultados: El pH salival con dieta no cariogénica a los cinco minutos fue 7,11 ± 0,19 y 7,09 ± 0,20, y, a los 40 minutos, 7,46 ± 0,19 y 7,42 ± 0,22, en mujeres y hombres, respectivamente. El pH salival con dieta cariogénica en mujeres fue 6,56 ± 0,21 a los cinco minutos y 7,15 ± 0,12 a los 60 minutos; mientras que, en los hombres, para iguales rangos de tiempos fue 6,47 ± 0,32 y 7,23 ± 0,22, respectivamente. Al realizar las comparaciones entre grupos de dieta cariogénica y no cariogénica se observaron diferencias muy significativas (p < 0,01). Conclusiones: Para la dieta cariogénica el pH disminuye como máximo a los cinco minutos y se restablece a partir de los 60 minutos. Para la dieta no cariogénica disminuye en el mismo tiempo pero se recupera a los 40 minutos, siendo este proceso similar en ambos géneros(AU)


Introduction: Salivary pH, an important factor in caries prevention, is under the direct influence of diet type. Objective: Evaluate salivary pH changes caused by cariogenic and non-cariogenic food intake in preschoolers from an educational institution in Huaura, Peru. Methods: An observational longitudinal comparative study was conducted of a sample of 30 preschoolers. After the children consumed cariogenic and non-cariogenic food, their salivary pH was evaluated at various times. Student's t-test and the Mann-Whitney U test were used to contrast results. Results: Salivary pH with a non-cariogenic diet at five minutes was 7.11 ± 0.19 and 7.09 ± 0.20; whereas at 40 minutes it was 7.46 ± 0.19 and 7.42 ± 0.22, in female and male participants, respectively. Salivary pH with a cariogenic diet in women was 6.56 ± 0.21 at five minutes and 7.15 ± 0.12 at 60 minutes, whereas values in men for the same time ranges were 6.47 ± 0.32 and 7.23 ± 0.22, respectively. Comparison between the cariogenic and non-cariogenic diet groups revealed very significant differences (p < 0.01). Conclusions: With a cariogenic diet, pH is reduced at five minutes maximum, and is restored as of 60 minutes. With a non-cariogenic diet, pH is reduced in the same time period, but is restored at 40 minutes. This process is similar in the two genders(AU)

Abstract The objective was to investigate the experience of dental caries and associated factors from the children's and adolescents' medical records who visited the Children's Dental Clinic of a College of southern Brazil. A cross-sectional study was carried out by analyzing 262 medical records. For data collection, information on demographic data, oral hygiene, food, breastfeeding and oral clinical data were used to investigate. A multivariate analysis was used to estimate the odds ratios (OR) and their 95% confidence intervals, by the exposure variables in a Binary Logistic Regression model (p-value< 0.05). Of the sample, 86.3% had some teeth with experience of dental caries, with only 13.7% free of caries. The average dental caries was 5.3 ± 3.1 in children between 6 and 8 years and 2.0 ± 1.9 between 9 and 12 years. There was a statistically significant association between age and dental caries, with the largest age group being a protective factor for dental caries (OR 0.30; 95%CI 0.17-0.53). Children who were breastfed naturally had a protective factor for caries (OR 0.29; 95%CI 0.10-0.88). The experience of dental caries in children was high, especially in primary teeth, with a higher average of decayed teeth. Breastfeeding and age were a protective factor for dental caries. (AU)


Resumo O objetivo foi investigar a experiência de cárie dentária e fatores associados em prontuários de crianças e adolescentes que procuraram o Ambulatório de Odontologia Infantil de uma Faculdade do Sul do Brasil. Realizou-se um estudo transversal por meio da análise de 262 prontuários. Para a coleta de dados, foram utilizadas informações sobre dados demográficos, higiene bucal, alimentação, aleitamento materno e dados clínicos bucais para investigação. Uma análise multivariada foi utilizada para estimar os odds ratios (OR) e seus intervalos de confiança de 95%, pelas variáveis de exposição em um modelo de Regressão Logística Binária (p-valor <0,05). Da amostra, 86,3% tinham alguns dentes com histórico de cárie dentária, e apenas 13,7% livre de cárie. A média de cárie dentária foi de 5,3 ± 3,1 em crianças entre 6 e 8 anos e 2,0 ± 1,9 entre 9 e 12 anos. Houve associação estatisticamente significativa entre idade e cárie dentária, sendo a maior faixa etária fator de proteção para cárie dentária (OR 0,30; IC95% 0,17-0,53). Crianças amamentadas naturalmente apresentaram fator de proteção para cárie (OR 0,29; IC95% 0,10-0,88). O histórico de cárie dentária em crianças foi elevado, principalmente nos dentes decíduos, com maior média de dentes cariados. O aleitamento materno e a idade foram fatores de proteção para cárie dentária. (AU)

RESUMEN Introducción: El confinamiento por el covid 19 alejó a los niños del acompañamiento odontológico preventivo que favorece a las practicas que disminuyen el riesgo de instalación y desarrollo de la caries dental. Objetivo: Describir características de la dieta, en cuanto a consumo de azucares libres y la higiene bucal, de niños escolares de la ciudad Concepción, Paraguay, durante el confinamiento por el Covid 19. Materiales y Métodos: Se realizaron entrevistas vía telefonía celular a padres o encargados y a un hijo/a matriculado en el 1er o 2do ciclo de la institución, luego de obtener consentimiento informado del adulto responsable, a través de un mensaje de texto. Fueron realizadas las entrevistas hasta completar el tamaño de la muestra, establecido en 110, con un intervalo de confianza del 95%. Los encuestadores fueron calibrados en 3 encuentros virtuales en el manejo del instrumento de encuesta/recuento de alimentos de 24 horas, adaptado para este trabajo. Resultados: En cuanto a la dieta, 108 niños consumían azúcares libres "AL" en diferentes horarios, 60 de ellos (55%) = o > a 4 momentos de AL/día, esto mostró asociación con el nivel de educación primario de los padres o encargados (p=.03). A su vez, 56 niños (51%) consumían AL entre comidas, con más frecuencia, en las siguientes presentaciones: confitería 48%; jugos artificiales 39%. Se cepillan los dientes 2 a 3 veces al día (85%). Conclusión: Poco más de la mitad de los niños e consumía 4 o más veces alimentos con azúcares libres durante el día; el 51% de niños lo consumen entre comidas, principalmente en las siguientes presentaciones: productos de confitería y jugos azucarados, estas características de la dieta podrían poner a los niños en riesgo de caries dental.


ABSTRACT Introduction: Confinement due to Covid 19 kept children away from preventive dental care, which favors practices that reduce the risk of dental caries development. Objective: To describe the dietary intake patterns, in terms of free sugar consumption, and oral hygiene, of school-aged children in the city of Concepción, Paraguay, during confinement by Covid 19. Materials and Methods: Interviews were conducted via cell phone with parents or guardíans and a child enrolled in the 1st or 2nd cycle of elementary education, after obtaining informed consent from the responsible adult, through a text message. Interviews were carried out until the sample size was completed, established at 110, with a 95% confidence interval. The interviewers were trained during 3 virtual meetings on how to handle the 24-hour food survey / counting instrument, adapted for this study. Results: Regarding diet, 108 children consumed free sugars (FS) at different times, 60 of them (55%) > 4 servings of FS / day, this was associated with the parents' or caregivers' level of primary education (p = .03). At the same time, 56 children (51%) consumed FS between meals, more frequently, in the following presentations: Baked goods 48%; artificial juices 39%. Subjects brushed their teeth 2 to 3 times a day (85%). Conclusion: Just over half of children ate 4 or more foods with free sugars during the day; 51% of children consumed them between meals, mainly in the following presentations: baked goods and sugary juices. These dietary patterns could put children at risk of dental caries.

@#Among many factors affecting dental caries, bacteria are its initiating factor. From the perspective of ecological plaque hypothesis, the application of modern molecular biology methods enable scholars to deeply explore the relationship between the microbial population distribution, biodiversity, microbial community changes of dental plaque biofilm and the occurrence of dental caries in children. The decrease in microflora diversity is closely related to the occurrence of dental caries, which can be detected as early as 6 months before the occurrence of dental caries, providing an important basis for caries monitoring and caries risk assessment. Differential bacteria such as Lactobacillus and Veillonella may be used as biomarkers of caries in children to judge the risk of caries in children. However, current studies have shown that the dominant bacteria in the microbial community structure vary in different processes of caries occurrence and development, that the interaction between different flora is still not clear, and that the specific cariogenic mechanism of the oral plaque microbial community in the process of caries occurrence and development has not been clarified. Further research is still needed to explore the changes in plaque microbial diversity and composition during the development of dental caries and to screen specific dental caries biomarkers. This paper also summarizes the related research findings.

ABSTRACT Objective: To investigate the influence de two instruments for evaluation of cariogenic foods consumption by schoolchildren. Material and Methods: The convenience sample comprised of 30 active-caries children (7-10 years old) attended in a public dental clinic and their mothers, who have not received dietary orientation before study. A trained evaluator administered the Previous Day Food Questionnaire (PDFQ) to children and other examiner applied the 24-hour dietary recall with mothers. Internal consistency between PDFQ and 24-hour dietary recall regarding frequency of sugar intake was calculated using intraclass correlation coefficient. Bivariate analyses (Mann-Whitney test) were performed to investigate the factors associated with frequency of sugar intake when using both instruments (α=0.05). Results: A moderate correlation (ICC = 0.51; 95%CI: -0.04-0.77; p=0.03) was found between instruments. Mean frequency of sugar intake reported by mothers and children was 3.5 (± 1.8) and 3.7 (± 1.5) times a day, respectively. When using the PDFQ, the frequency of cariogenic foods consumption varied depending on frequency of toothbrushing, i.e., children that had higher frequency of sugar intake brushed their teeth at least two times a day (p=0.016). Conclusion: Previous Day Food Questionnaire applied with schoolchildren could substitute the mothers' reports of cariogenic foods consumption when dietary interventions for controlling dental caries are necessary. If doubts remain regarding the children's response, mothers should be consulted.

ABSTRACT Objective: To evaluate different concentrations of Galla chinensis extract (GCE) added to poly(methyl methacrylate) (PMMA), which is widely used for fabrication of removable orthodontic appliances, regarding the effectiveness of this herbal extract on antimicrobial effect and flexural strength of PMMA. Methods: Acrylic resin samples containing 0.4%, 0.8% and 1.6% GCE were prepared. Flexural strength was investigated via three-point flexural strength test for the 15 acrylic resin blocks of each concentration. Disk diffusion test was used to evaluate antibacterial effects of incorporating the same concentrations of GCE into acrylic resin. All these three groups were compared with the control group, with no added GCE, regarding flexural strength and antibacterial properties. Results: Comparison of flexural strength between the three study groups and the control group showed significant differences between the groups (P=0.018). However, there was no significant difference between the groups containing GCE. There were significant differences in antimicrobial activity between the four groups (P=0.026). Conclusion: Within the limitations of this study, it is suggested that incorporation of GCE into PMMA would be beneficial for antimicrobial activity and flexural strength of PMMA, but further studies on other physical properties and antimicrobial effects on other bacterial strain would be beneficial prior to clinical investigations.


RESUMO Objetivo: Avaliar se diferentes concentrações de extrato de Galla chinensis (EGC) adicionado ao polimetilmetacrilato (PMMA), que é amplamente utilizado para a fabricação de aparelhos ortodônticos removíveis, interferem no efeito antimicrobiano desse extrato e na resistência à flexão do PMMA. Métodos: Foram preparadas amostras de resina acrílica com concentrações de 0,4%, 0,8% e 1,6% de EGC. Para a avaliação da resistência à flexão, utilizou-se o teste de flexão em três pontos para as 15 amostras de resina em cada concentração. O teste de disco-difusão foi utilizado para avaliar os efeitos antibacterianos da incorporação das mesmas concentrações de EGC na resina acrílica. Esses três grupos foram comparados ao grupo controle, sem adição do EGC, em relação à resistência à flexão e quanto às propriedades antimicrobianas. Resultados: As comparações dos três grupos com o grupo controle mostraram diferenças significativas (p=0,018) para a resistência à flexão. Entretanto, não houve diferença significativa entre os grupos contendo EGC. Foram encontradas diferenças significativas na atividade antimicrobiana entre os quatro grupos (p=0,026). Conclusão: Dentro das limitações desse estudo, parece que a incorporação de EGC no PMMA seria benéfica para a atividade antimicrobiana e a resistência à flexão do PMMA. Porém, estudos adicionais sobre outras propriedades físicas e sobre os efeitos antimicrobianos contra diferentes cepas de bactérias seriam interessantes antes de se fazer pesquisas clínicas.

Objetivo: objetivou-se avaliar a experiência de cárie e seus fatores determinantes e moduladores em pré- -escolares de um centro de educação infantil localizado no estado do Ceará. Método: trata-se de um estudo exploratório, descritivo e de abordagem quantitativa realizado com crianças do infantil IV e V do Centro de Educação Infantil Francisca Arruda de Pontes (Redenção, CE) e suas mães. Após consentimento, as mães responderam um questionário contendo perguntas desde os aspectos socioeconômicos e acompanhamento pelo serviço de saúde a conhecimento das doenças bucais. Posteriormente, as crianças foram submetidas à avaliação odontológica para registro do índice CEO-D, o qual corresponde à soma do número de dentes decíduos cariados (c), com extração indicada (e) e restaurados (o). Os dados foram tabulados e descritos como frequência relativa e absoluta. Resultados: das mães, 80,75% tinham renda de até 1 salário mínimo, 61,53% utilizavam creme dental na higiene oral de seu filho e 48,07% desconheciam as doenças bucais. Das crianças, 65,38% não eram atendidas pelo cirurgião-dentista e 50,00% consumiam diariamente bolacha doce. Das crianças avaliadas, 52,50% tinham dentes cariados, 70,00% não apresentavam dentes com extração indicada e nenhuma tinha dentes obturados/restaurados. A média do índice CEO-D foi 1,325. Conclusão: conclui-se que as crianças, apesar de terem baixa prevalência de cárie, apresentam, como fatores de risco determinantes, uma dieta cariogênica e, como fatores de risco moduladores, um reduzido conhecimento sobre doenças bucais, higiene oral inadequada, renda familiar insuficiente, baixa escolaridade e limitada procura por serviço de saúde, especialmente o odontológico.(AU)


Objective: to evaluate the experience of caries and its determining and modulating factors in preschoolers at an early childhood education center located in the state of Ceará. Method: this is an exploratory, descriptive study with a quantitative approach carried out with children from infant IV and V of the Francisca Arruda de Pontes Early Childhood Center (Redenção, CE) and their mothers. After consent, the mothers answered a questionnaire containing questions from the socioeconomic aspects and monitoring by the health service to knowledge of oral diseases. Subsequently, the children underwent dental evaluation to record the dmf Index, which corresponds to the sum of the number of decayed primary teeth (d), missing (m) and filled (f). The data were tabulated and described as relative and absolute frequency. Results: of the mothers, 80.75% had an income of up to 1 minimum wage, 61.53% used toothpaste in their child's oral hygiene and 48.07% were unaware of oral diseases. Of the children, 65.38% were not attended by the dentist and 50.00% consumed sweet cookies daily. Of the children evaluated, 52.50% had decayed teeth, 70.00% did not have teeth with indicated extraction and none had filled/ restored teeth. The average of the dmf Index was 1.325. Conclusion: it is concluded that children, despite having a low prevalence of caries, present, as determining risk factors, a cariogenic diet and, as modulating risk factors, a reduced knowledge about oral diseases, inadequate oral hygiene, family income insufficient, low education and limited demand for health services, especially dental care.(AU)

Introducción: La caries de la temprana infancia (CTI) es una de las enfermedades infecciosas más comunes en infantes y niños preescolares. Objetivo: Evaluar la caries de la temprana infancia y los factores de riesgo asociados con prácticas dietarías y el estado nutricional en niños de 10-48 meses de edad. Materiales y Métodos: Se realizó un estudio transversal en 142 niños de nivel socioeconómico medio-bajo de un centro infantil gubernamental en Cuenca, Ecuador. La salud dental, estado nutricional y patrones dietarios se evaluaron mediante exámenes clínicos, mediciones antropométricas y recordatorios de 24 horas, respectivamente. Resultados: La prevalencia de caries de la temprana infancia fue alta (95.8%) mayoritariamente en etapa avanzada (83.1%). No se observaron asociaciones con el estado nutricional. El consumo diario de alimentos cariogénicos fue significativamente mayor entre semana (11,1 ± 1,4) (p<0,001). La dieta fue categorizada como de bajo riesgo cariogénico (<34). La edad del niño (OR=1.1; IC 95%: 1.041, 1.160, p=0.001) y el índice de placa (OR=4.9; IC 95%: 1.331, 17.82; p=0.017) fueron determinantes positivos de caries avanzada. Conclusión: El deterioro de la salud dental de esta población se evidenció, empeorando a edades mayores sin asociación con los patrones dietarios.


Introduction. Early childhood caries (ECC) is one of the most common infectious diseases for infants and preschool children. Objective: To evaluate early childhood caries and risk factors associated with dietary practices and nutri- tional status in children of 10-48 months old. Materials and Methods: A cross-sectional study was performed on 142 children of medium-low so- cio-economic status from a governmental child-care center in Cuenca, Ecuador. Dental health, nutritional status and dietary patterns were assessed through clinical examination, anthropometric measurements and 24-hours dietary recalls, respectively. Results: Early childhood caries prevalence was high (95.8%) mostly of advanced stage (83.1%). No as- sociations with nutritional status. Daily consumption of cariogenic foods was significantly higher during weekdays (11.1 ± 1.4) (p<0.001). The diet was categorized as low cariogenic risk (<34). Child's age (OR=1.1; 95% CI: 1.041, 1.160, p=0.001) and the plaque index (OR=4.9; 95% CI: 1.331, 17.82; p=0.017) were positive determinants for advanced caries. Conclusions: The deteriorated dental health of this population was evidenced, being worse at older ages without any association with dietary patterns.


Introdução. A cárie da primeira infância (CPI) é uma das enfermidades infecciosas mais comuns em crianças pre-escolares. Objetivo: Avaliar a cárie da primeira infância e fatores de risco associados com práticas dietárias e estado nutricional de crianças de 10-48 meses. Materiais e métodos. Realizou-se um estudo transversal em 142 crianças de nível socioeconômico médio-baixo do centro infantil governamental em Cuenca, Equador. A saúde dental, estado nutricio- nal e patroes dietários se avaliaram mediante exames clínicos, medições antropométricas e lembretes de 24 horas respectivamente. Resultados: A prevalência de cárie da primeira infância foi alta (95.8%) principalmente em estágio avançado (83.1%). Não se observaram relações com o estado nutricional. O consumo diário de ali- mentos cariogênicos foi significativamente maior entre semana (11,1 ± 1,4) (p<0,001). A dieta foi categorizada como de baixo risco cariogênico (<34). A idade das crianças (OR=1.1; IC 95%: 1.041, 1.160, p=0.001) e o índice de placa (OR=4.9; IC 95%: 1.331, 17.82; p=0.017) foi determinante em cárie avançada positiva. Conclusão: O deterioro da saúde dental dessa população se evidenciou, piorando as idades mais velhas sem relação com os patroes dietários.

Background: The oral bacteria, mutans streptococci (MS), are an etiological agent of dental caries. Of MS, Streptococcus downei are rarely isolated bacteria. Aim: The aim of this study was to isolate and characterize S. downei from caries-active subjects. Materials and Methods: In all, 65 dental plaque samples were collected from dental caries-active subjects. All the isolates were further identified and characterized using 16S rDNA sequencing, biochemical tests, antibiogram, and minimum inhibitory concentration (MIC). Results: Five isolates have been identified as S. downei using 16S rDNA sequencing. Phylogenetic analysis showed that S. downei was closely related to S. sobrinus. The biotype traits of these five isolates were IV (n = 3), V (n = 1), and variants (n = 2). The study proposed one new biotype, classified as biotype VIII for the variant strain. The antibiogram tests revealed that all the strains of S. downei were susceptible to all the antibiotics used in the study with higher sensitivity to penicillin and ampicillin. The MIC of ampicillin and erythromycin against S. downei was 0.047 and 0.39 μg/mL, respectively. Conclusion: The study reports the prevalence of S. downei in caries-active subjects and recommends further investigations to determine its role in the disease.
