RESUMO

Shenmai Injection is a Chinese medicinal injection prepared from Ginseng Radix et Rhizoma Rubra and Ophiopogonis Radix, which is widely used in clinical practice for the treatment and adjuvant therapy of cardiovascular diseases with significant pharmacological effects. Proton nuclear magnetic resonance spectroscopy(~1H-NMR) has the advantages of simple and nondestructive sample pretreatment, fast analysis, abundant chemical information, quantification and no need to follow the standard curve. It is widely used in the analysis and research of complex mixtures of traditional Chinese medicine, clinical blood and urine samples. In this study, the ~1H-NMR fingerprint of Shenmai Injection was established. Thirty-two chemical components were identified, including seven amino acids, eight small molecular organic acids, one alkaloid, four sugars, two nucleosides, seven saponins, and three other components. Pearson's correlation coefficient and multivariate analysis of variance(principal component analysis combined with hierarchical cluster analysis) were applied based on the ~1H-NMR fingerprint to evaluate the quality consistency. The results showed high-quality consistency of 82 batches of Shenmai Injection. This study confirms that the ~1H-NMR fingerprint has great potential in the application of quality control of Chinese medicinal injection.

RESUMO

Chinese medicinal injection, made of active components extracted from Chinese medicine or Chinese medicinal compound, is a novel dosage form of Chinese patent medicine in China and is pivotal in the traditional Chinese medicine(TCM) industry. The quality control standard of Chinese medicinal injection determines its safety and efficacy. The quantitative nuclear magnetic resonance(qNMR) spectroscopy is a non-targeted, non-invasive, and non-destructive technique with high reproducibility, short measurement time, convenient sample preparation, a broad range of linearity, and no requirement on the reference substance of tested components, which is advantageous as compared with traditional chromatographic methods, and it can provide information about the molecular composition of the tested samples. Therefore, in light of multiple challenges in the quality control of Chinese medicinal injection, such as complex composition, difficulties in quantitative analysis, and the shortage of reference substances, the application of qNMR spectroscopy combined with chemometrics techniques was proposed for the quality evaluation of Chinese medicine reference substances, Chinese medicinal injection, and intermediates in the production process, as well as for the stability analysis of Chinese medicinal injection. This study is expected to provide references for the application of qNMR spectroscopy in the quality control of Chinese medicinal injection.

ObjectiveTo systematically evaluate the clinical effectiveness and safety of Chinese medicinal injection (CMI) in the treatment of unstable angina pectoris (UAP). MethodEight databases, i.e., China National Knowledge Infrastructure (CNKI),VIP,Wanfang Data,CBM,PubMed,EMBASE,The Cochrane Library,and Web of Science were searched for randomized controlled trials (RCT) of conventional treatment combined with CMI (treatment group) versus conventional treatment (CT)(control group)in the treatment of UAP published from database inception to March 31th 2021. Stata 16.0 was used for network Meta-analysis. ResultThirty-nine RCT involving 3 407 patients were included. As revealed by the results of network Meta-analysis, in terms of the total effective rate in angina pectoris improvement, the therapeutic protocols were ranked as Tanreqing injection(TRQI)+CT>Xiangdan injection(XDI)+CT>Ciwujia injection(CWJI)+CT=Shengmai injection(SMI)+CT>Xuesaitong injection(XSTS)+CT>Breviscapine injection(BI)+CT>Shuxuetong injection(SXTI)+CT>Kudiezi injection(KDZI)+CT>Shuxuening injection(SXNI)+CT>Danshen injection (DSI)+CT>Guanxinning injection(GXNI)+CT>Dengzhanxixin injection(DZXXI)+CT>Xueshuantong injection(XSTI)+CT>Gualoupi injection(GLPI)+CT>CT;for the total effective rate in ECG improvement, SXTI+CT>XDI+CT>TRQI+CT>CWJI+CT>XSTI+CT>BI+CT>XSTI+CT>SXNI+CT>GXNI+CT>KDZI+CT>DZXXI+CT>GLPI+CT>CT>SMI+CT;for the adverse reactions, DZXXI+CT>XDI+CT>DSI+CT>BI+CT>SMI+CT>SXNI+CT>CT>GLPI+CT>GXNI+CT>SXTI+CT>KDZI+CT>CWJI+CT;for the reduction of fibrinogen (FIB), BI+CT>SXTI+CT>XSTI+CT>CT>KDZI+CT;for the reduction of C-reactive protein (CRP), DSI+CT>DZXXI+CT>XSTI+CT>CT;for the reduction of high-sensitivity C-reactive protein (hs-CRP), SXNI+CT>KDZI+CT>SXTI+CT>DZXXI+CT>GLPI+CT>TRQI+CT>XSTI+CT>CT. The results of subgroup analyses were consistent with those of the overall Meta-analysis. ConclusionCMI combined with CT can improve angina pectoris and ECG,reduce adverse reactions,and also improve FIB,CRP,and hs-CRP to varying degrees. However,due to the differences in the quality and quantity of CMIs in RCTs,clinical application should be performed based on the specific conditions.

RESUMO

Angina pectoris is a common belonging to the diseases of "Chest Bi-syndrome", "precordial pain" or "syncopal precordialgia" in traditional Chinese medicine. Clinically Shengmai injection, Acanthopanex injection, Breviscapine injection, Fleabane injection, Safflower injection, Astragalus injection, Shuxuetong injection, Xin-ding injection and Panax notoginsenoside injection were commonly used to treat the disease with good clinical curative effect.

OBJECTIVE:To study the change of particles of Chinese medicinal injection added to different kinds of infu-sions fluid before and after filtration.METHODS:Particle analysator was adopted to measure the particles of Xueshuantong injection,Yinzhihuang injection,Yuxingcao injection before and after filtration and30minutes after being mixed with0.9%Sodium Chloride injection,5%glucose injection,and10%glucose injection respectively.RESULTS:The number of particles in all kinds of unfiltered mixture increased significantly while those of filtered ones remarkably decreased.CONCLUSION:It is recommended that when intravenous drip of Chinese medicinal injection in the clinic was used,particles addition should be considered.

OBJECTIVE:To evaluate the cost-effectiveness of 3 kinds of traditional Chinese medicinal injections in the treatment of angina pectoris caused by coronary heart disease(CHD).METHODS:223 CHD patients with angina pectoris were divided into 3 groups:group A(Xuesaitong injection),group B(Breviscapine for injection) and group C(Shuxuening injection). The curative effects were observed and the cost-effectiveness analyses of the 3 groups were conducted as well.RESULTS:The costs for the 3 groups were 1 345.50 yuan,1 768.125 yuan,1 770.00 yuan,respectively; the total effective rates were 83.75%,82.09% and 85.53%,respectively; the cost-effectiveness ratios were 16.07,21.54 and 20.69,respectively;the incremental cost-effectiveness ratios of group B and C were 254.60 and 238.48,respectively as against group A. CONCLUSION:Xuesaitong injection(group A) is the preferred therapy for angina pectoris caused by CHD.
