Objective To evaluate the application effect of ciprofol-alfentanil in short urological surgery.Methods A total of 80 patients who were to undergo urological general anesthesia surgery in this hospital were divided into two groups by random number method:ciprofol-alfentanil group(group C)and propofol-alfentanil group(group P).Group C was injected with ciprofol 0.4 mg/kg,group P was injected with propofol 1.5-2.0 mg/kg,and when the bispectral index(BIS)was<60,the intravenous injection of alfen-tanil(10 μg/kg)and rocuronium bromide(0.6 mg/kg)was continued.When the modified alertness/sedation score(MOAA/S score)was 0,the laryngeal mask was placed and mechanical ventilation was used.During the maintenance period,ciprofol 0.8-1.2 mg·kg-1·h-1 was infused intravenously in group C,and propofol 4-6 mg·kg-1·h-1 was infused intravenously in group P.The heart rate(HR),blood pressure(BP),oxygen saturation(SpO2),end-tidal carbon dioxide(PetCO2),BIS and MOAA/S score were recorded at the begin-ning of anesthesia induction(T0),laryngeal mask insertion(T1),ureteroscopy entry(T2),10 min after sur-gery(T3)and the end of surgery(T4).The consciousness disappearance time,operation time,anesthesia re-covery time,drug dosage,injection pain during induction,hypotension,bradycardia and other adverse reactions during the operation were recorded.Results There was no significant difference in HR,SpO2,PetCO2,BIS value,MOAA/S score,operation time,consciousness disappearance time,and anesthesia recovery time be-tween the two groups at each time point(P>0.05).The dosage of sedative drugs in group C was less than that in group P(P<0.05).Compared with group P,systolic blood pressure and diastolic blood pressure at T1-T3 and diastolic blood pressure at T4 increased in group C(P<0.05).Compared with T0,systolic blood pressure at T1-T4 in group C and group P decreased,diastolic blood pressure at T2-T4 in group C de-creased,and diastolic blood pressure at T1-T4 in group P decreased(P<0.05).Compared with group P,the injection pain and the incidence of intraoperative hypotension were reduced in group C(P<0.05).Conclusion Cipro-fol-alfentanil is superior to propofol-alfentanil in short urological surgery.

Objective To compare the anesthetic effect and safety of Ciprofol and Propofol combined with Fentanyl in painless bronchoscopy for conscious patients in intensive care unit(ICU).Methods 60 conscious patients who underwent painless bronchoscopy from October 2022 to January 2023 were selected.According to the random number table method,30 cases were divided into the control group(Propofol 1.5 mg/kg + Fentanyl 1 μg/kg)and 30 cases were divided into the experimental group(Ciprofol 0.4 mg/kg + Fentanyl 1 μg/kg).Systolic blood pressure,diastolic blood pressure,heart rate,respiratory frequency,and percutaneous arterial oxygen saturation(SpO2)were recorded before anesthesia(T1),during the examination(T2),and at the completion of the examination(T3)in the two groups,and the success rate of induction of general anesthesia,the time of induction of anesthesia,the time of bronchoscopy,the time of eye-opening,the total dosage of medications used,the incidence of injection pain,and the incidence of intraoperative adverse reactions(hypotension,respiratory depression,bradycardia,and bucking)in the two groups were compared.Results Analysis of the data revealed significant group,time,and interaction effects for systolic blood pressure,diastolic blood pressure,heart rate,and respiratory frequency at different time points(P<0.05).The experimental group had a shorter induction time of anesthesia[1.40(1.10,1.62)min]in comparison to the control group[1.60(1.30,2.10)min](P<0.05),and the experimental group required a less amount of drug to achieve the desired outcome[(21.40±1.82)mg]compared to the control group[(78.75±6.71)mg](P<0.05).Furthermore,the incidence of injection pain(3.33%)and respiratory depression(6.67%)was significantly reduced in the experimental group compared to the control group(36.67%)and(30.00%)(P<0.05).Conclusion The use of Ciprofol in combination with Fentanyl has been found to provide effective anesthesia during bronchoscopy,while also increasing safety and decreasing the incidence of adverse events.It is worthy of clinical application.

Objective To observe the effect of opioid-free anesthesia with esketamine and ciprofol for the elderly underwent colonoscopy.Methods 160 elderly patients underwent colonoscopy were included in this study.Those patients were assigned to esketamine combined with ciprofol group(group E,n = 80)and sufentanil combined with ciprofol(group S,n = 80).Patients were intravenously injected respectively with esketamine 0.2 mg/kg in group E and sufentanil 0.1 μg/kg in group S.Then,all the patients were intravenously injected with ciprofol(0.2 mg/kg).Mean arterial pressure(MAP),heart rate(HR)and percutaneous arterial oxygen saturation(SpO2)were recorded before injection of test drug(T0),when modified observer's assessment of alertness/sedation(MOAA/S)score was 0 points(T1),when colonoscopy through hepatic flexure(T2)and colonoscopy was removed after examination(T3).The induction time of anesthesia,time of fully awake,rate of successful anesthesia,additional dose of ciprofol,satisfaction score and incidence of adverse events were recorded.Results Compared with T0,MAP at T1,T2 and T3 in two groups was decreased,HR at T1 in group E,HR at T1,T2 and T3 in group S and SpO2 at T1 and T2 in group S were decreased,the differences were statistically significant(P<0.05).Compared with T1,MAP and HR at T2 in two groups were increased,the differences were statistically significant(P<0.05).Compared with group E,MAP in group S at T1,T2 and T3,HR and SpO2 at T1 and T2 in group E were decreased,the differences were statistically significant(P<0.05).Compared with the group E,the additional dosage of ciprofol,the incidence of hypoxemia and hypotension were increased,the satisfaction of anesthesiologists was decreased in group S,the differences were statistically significant(P<0.05).Conclusion For opioid-free anesthesia,esketamine is feasible for colonoscopy anesthesia in elderly patient,and the incidence of hypoxemia and hypotension is low.

Objective To research the efficacy and safety of ciprofol in combined intravenous and inhalation anesthesia in elderly patients undergoing thoracoscopic surgery.Methods A retrospective analysis was conducted on 806 elderly patients who underwent thoracoscopic surgery from January 2021 to July 2022,416 males and 390 females,aged 65-85 years,BMI 16-30 kg/m2,ASA physical status Ⅰ-Ⅲ.Patients were divided into two groups according to different intravenous anesthesia drugs during the sur-gery:ciprofol group(group C,n=229)and propofol group(group P,n=577).The preoperative and intraoperative indexes of the two groups were balanced by 1∶1 propensity score matching(PSM).Multiva-riate logistic regression was used to analyze the correlation between intravenous anesthetics and the incidence of adverse reactions(hypotension,bradycardia,hypoxemia during recovery,and postoperative nausea and vomiting).Multivariate linear regression was used to analyze the correlation between intravenous anesthetics and short-term recovery indicators after anesthesia(extubation time,postanesthesia care unit residence time,and length of postoperative hospitalization).Results Compared with group P,the total dosage of va-soactive drugs in group C were significantly reduced(P<0.05),extubation time and PACU residence time were significantly shortened(P<0.05).Multivariate logistic regression analysis showed that the use of cip-rofol was associated with a reduced incidence of intraoperative hypotension(OR=0.378,95%CI 0.236-0.603,P<0.001)and hypoxemia during recovery(OR=0.542,95%CI 0.336-0.874,P=0.012).Multivariate linear regression analysis showed that the use of ciprofol was associated with the reduction of ex-tubation time and PACU residence time(P<0.001).Conclusion Compared with propofol,combined in-travenous and inhalation anesthesia with ciprofol can significantly reduce the incidence of anesthetic adverse reactions in elderly patients,and is conducive to short-term recovery after anesthesia.

Objective:To evaluate the effect of gender on anesthetic potency of ciprofol for gastroscopy when combined with fentanyl.Methods:American Society of Anesthesiologists Physical Status classification Ⅰ or Ⅱ patients, aged 18-50 yr, with body mass index of 18-25 kg/m 2, undergoing elective gastroscopy with intravenous anesthesia, were divided into 2 groups according to gender: male group (M group) and female group (F group). After fentanyl 1.5 μg/kg was intravenously injected, ciprofol was given by the Dixon′s up-and-down method, with the initial dose of 0.4 mg/kg followed by dose increment/decrement of 0.04 mg/kg. The ED 50 and 95% confidence interval of ciprofol for gastroscopy anesthesia were calculated by the probit regression analysis. Results:The ED 50 (95% confidence interval) of ciprofol for gastroscopy was 0.33 (0.32-0.34) mg/kg in F group and 0.27 (0.26-0.28) mg/kg in M patients when combined with fentanyl 1.5 μg/kg. There was no significant difference between the two groups ( P>0.05). Conclusions:There is no significant gender difference in the anesthetic potency of ciprofol for gastroscopy (ED 50: female 0.33 mg/kg, male 0.27 mg/kg) when combined with fentanyl (1.5 μg/kg).
