RESUMO

Clear cell syringoma has two distinct features from typical syringomas: histological predominance of clear cells and a correlation with diabetes mellitus. Otherwise it is similar to ordinary syringomas in age, sex, site, and clinical manifestations. We here report a 49-year-old male with clear cell syringoma accompanied with diabetes mellitus. Histological examination revealed numerous, variable-sized, well-defined tumor nests composed largely of clear cells that stained positive with PAS (Periodic Acid Schiff) before diastase digestion and negative after diastase digestion.

RESUMO

Dystrophic calcinosis cutis is diagnosed, when calcium is deposited in the previously damaged tissue by connective tissue disease, panniculitis, pseudoxanthoma elasticum or trauma. We report a case of dystrophic calcinosis cutis arising from the lesion of clear cell syringoma. A 73-year-old woman presented with asymptomatic, multiple, skin-colored to whitish flat papules on the forehead. A punch biopsy specimen revealed amorphous basophilic materials in the upper dermis, as well as a large number of nests of clear cells in the middle dermis.

Ansan, KoreaClear cell syringoma is a rare variant type of syringoma. It has two noteworthy features: histologic preponderance of clear cells and frequent coexistence with diabetes mellitus. There is no difference between clear cell syringoma and typical syringoma with respect to age, sex, and clinical features. We report a 48-year-old woman who presented with clear cell syringoma concomitant with diabetes mellitus. Histopathologic findings showed numerous various-sized, well-defined tumor cell nests composed largely of clear cells.

RESUMO

The subepidermal calcified nodule, also known as a form of calcinosis cutis, is characterized by solitary circumscribed deposits of insoluble calcium within the subepidermis. The disease is not associated with any systemic or cutaneous diseases, or any biochemical abnormalities. The pathogenesis is still unknown. A 55-year-old female presented with a 4-month history of solitary skin-colored, hyperkeratotic papule on the left upper eyelid. The lesion was asymptomatic and had increased slowly in size. It is interesting that excisional biopsy showed typical histopathologic features of the subepidermal calcified nodule and the clear cell syringoma.

RESUMO

Two cases of clear cell syringoma are reported in the women who were 39 ad 54 years old, respectively. They had had asymptomatic, ricegrain-sized skin colored to yellowish pink papules on the face, particularly on he lower eyelid for several years. Interestingly, both of them had suffered from diabetes mellitus for more than a decade and been placed on irregular antidiabetic medications. Routine laboratory findings in the elder patient(case 2), unfortunately not checked in the younger patient(cases 1) due to her refusal, were within normal limits except marked glucosuria. Histologic examination revealed numerous various-sized, well-defined tumor nests composed largely of clear cells with polygonal cellular boundary and eccentric nucleus. Special stain with Periodic acid-Schiff disclosed existance of glycogen in the clear cells, but other special stains such as alcian blue showed negative findings. Electron microscopic examination of the elder patient revealed many multivesicular bodies suggestive of lysosomes, and numerous droplets of glycogen dispersed between them in villi-rich periluminal cell.
