RESUMO

Objective To understand the epidemiological characteristics and clinical features of Zika virus disease,and to improve its prophylaxis and treatment.Methods The first case with imported Zika virus disease in China was retrospectively reported and analyzed.The literature of Zika virus infection in human was reviewed.Results This patient was the first case with imported Zika virus disease in China who presented with typical clinical characteristics and had clear epidemiological history.All the contacts were test negative for Zika virus nucleic acid.Literature retrieval showed evidence of Zika virus propagation in more than 40 countries in Africa,Asia,and Americas.The majority of patients presented with mild symptoms and the main prevention measures included mosquito control and improved awareness of personal protection.Conclusions Human infected with Zika virus often shows recessive infection.Only a small part develop disease and have generally good prognosis with supportive treatment.

Objective To evaluate the specificity and limitations of sacroiliac joint magnetic resonance imaging (MRI) in the diagnosis of axial spondyloarthritis (SpA)in patients with chronic low back pain.Methods We retrospectively analyzed clinical data of 390 patients with chronic low back pain in Department of Rheumatology,the PLA General Hospital from January 2013 to December 2015,including clinical manifestations,laboratory examinations and MRI data of sacroiliac joints.Results There were 238 men and 152 women recruited.A total of 326 cases were diagnosed as axial SpA,including 216 men and 110 women with mean age (27.10 ± 8.64) years and mean duration (7.64 ± 3.50) months.Among these 326 patients,243 (74.5%) were HLA-B27 positive.The other 64 patients were considered as diagnoses rather than SpA (non-SpA),consisting of 22 men and 42 women with mean age (31.29 ± 7.76) years and mean duration (5.75 ±2.90)months.Non-SpA group had 10 (15.6%) patients with HLA-B27 positive.There were 68.1% and 65.0% SpA patients showing bone marrow edema and bone erosion of sacroiliac joint in MRI imaging respectively.Although there were 25.0% non-SpA patients with bone marrow edema and 7.8 % with bone erosion in MRI of sacroiliac joint,the scores of bone marrow edema 0.00 (0.00,0.75) and bone erosion [0.00(0.00,0.00)] were significantly lower compared with those in axial SpA group [bone marrow edema scores 2.00 (0.00,4.00),bone erosion scores 1.00 (0.00,3.00);P < 0.05].The scores of fat infiltration [1.00 (0.00,4.25),1.00 (0.00,4.00)] and bone sclerosis [0.00 (0.00,1.00),0.00(0.00,1.75)] were not statistically different between two groups.Diagnostic sensitivity of bone marrow edema and bone erosion for axial SpA were 56.4% and 64.1% respectively,specificity were 93.8% and 92.2% respectively.The positive predictive value of bone marrow edema and bone erosion for axial SpA were 9.09 and 8.21,negative predictive value were 0.46 and 0.38.Diagnositic sensitivity of fatty infiltration and bone sclerosis for axial SpA group were 29.1% and 57.7%,specificity were 64.1% and 46.9%.The positive predictive value of fatty infiltration and bone sclerosis for axial SpA were 0.81 and 1.08,negative predictive valne were 1.10 and 0.90.Conclusion Sacroiliac joint MRI is a valuable method to diagnose axial SpA in patients with chronic low back pain.Yet it still has some limitations.Clinical presentations and spinal MRI would be helpful in some patients.

Child anxiety disorders are among the common mental health disorders,often with physical symptoms or physical disorders.The life time prevalence is relative high.Child anxiety disorders mainly include in subtype as separation anxiety,specific phobia,social anxiety disorder,panic disorder and selective mutism.In this paper,the prevalence rate of the above 5 subtypes,etiology,clinic manifestation,treatment and outcome were introduced.

Se realizó un estudio observacional, descriptivo y longitudinal sobre la evolución del tratamiento con moxibustión en 150 pacientes con sacrolumbalgia aguda que fueron diagnosticados en la consulta de Medicina natural y tradicional (MNT) del Policlínico Mantilla, durante el primer semestre del año 2008, con el objetivo de determinar su estado evolutivo según criterio clínico al cabo de la décima sesión, caracterizarlos en cuanto a edad, sexo y ocupación, y describir las manifestaciones clínicas de interés antes de someterse a la primera sesión y después de culminada la última. Se midieron las variables: edad, sexo, ocupación, realización de esfuerzo físico, dolor, intensidad, inflamación e irradiación. Se realizaron entrevistas para corroborar los aspectos cualitativos en la aplicación del tratamiento empleado y con los datos obtenidos se confeccionaron tablas para su mejor comprensión. Como resultado más relevante se obtuvo que 90 por ciento de los pacientes que llegaron a la consulta con dolor, refirieran no sentir dolor alguno al finalizar el tratamiento. Los resultados que se derivan de esta investigación se están utilizando en la preparación y actualización del personal médico y de enfermería en la Atención Primaria de Salud(AU)


It was carried out an observational, descriptive and longitudinal study on the evolution of the treatment with moxibustion in 150 patients with sharp sacrolumbalgy that were diagnosed in the consultation of natural and traditional Medicine of the Policlinic Mantilla, during the first semester of the year 2008 with the objective of determining their evolutionary state according to clinical approach after the tenth session, to characterize them as for age, sex and occupation, and to describe the clinical manifestations of interest before undergoing the first session and after having culminated the last one. The variables were measured: age, sex, and occupation, realization of physical effort, pain, intensity, inflammation and irradiation. They were carried out interviews to corroborate the qualitative aspects in the application of the used treatment and with the obtained data charts were made for their best understanding. As a result more excellent it was obtained that 90 percent of the patients that arrived to the consultation with pain, referred not to feel pain some when concluding the treatment. The results that they are derived of this investigation they are using in the preparation and the medical personnel's bring up to date and of infirmary in the primary attention of health(AU)

It was carried out an observational, descriptive, and traverse study on payees of Migraine, which went to the consultation of Natural and Traditional Medicine (NTM) of the Policlinic Mantilla with the objective of showing the studied population's clinical evolution, when applying digitopunture during the first semester of the year 2008. [Abstract] Objective To evaluate the clinical and endoscopic manifestations of abdominal type allergic purpura in adult patients as the evidences in reaching early diagnosis. Methods The clinical and endoscopic manifestations of 32 patients with abdominal type allergic purpura were analyzed retrospectively. Re-SllltS All the patients complained of paroxysmal abdominal colic, 59. 4% patients complained of nausea and vomiting, 50% patients had hematochezia and haematemesis, the occult blood test was positive in all patients, and 25% patients had elevated level of amylase in the blood and urine. Purpura was found in 2-10 days after the presentation of abdominal pain. Endoscopy found hyperaemia, edema, bleeding spots, erosion and ulcer in gastroenterologic mucosa. Duodenal, ileum and caecum had more severe mucous lesions, but the esophageal mucosas in all patients were normal. The clinic and endoscopic misdiagnosis rates were 87.5% and 85.7% respectively. Glucocorticoids were the especially effective medicine in its treatment, while those agents for relieving spasm or antacids were ineffective. Conclusion Many scattered bleeding spots and snow flower-like ulcers on the gastrointestinal mucosa are the most important findings in adult abdominal type allergic purpura patients. Clinic manifestations of these patients are very severe abdominal colic in indefinite sites and merely with mild abdominal objective signs is the important characteristic feature.

RESUMO

Objective:To find out the clinic characteristics of congenital syphilis(CS),to reduce the possibility of misdiagnosis.Methods:Fifty-five(aged at hedyanhour to three years)cases were analyzed the clinic manifestation retrospectively during 1999 to 2008.Results:There were 33 male and 22 female.of 55 cases,only 5 were diagnosed as CS at the beginning of entry hospital and the rest were misdiagnosed.The main complain showed 27 cases with mucocutaneous lesion(papula,papulopustule,bleb,desquamation),26 with jaundice,32 with hepatomegaly,14 with splenomegaly,12 with tachypnea,6 with osteomyolitis,20 with nose problem(rhinocleisis, epistaxis,rhinorrhea),7 with congenital deformity,5 with hemorrhage,4 with abdominal distension,1 with abdominal pain,2 with vomiting,2 with edema,1 with tic,1 with bad cry.Conclusion:In most newborns and infants,the onset of CP is heralded by nonspecific signs which involved multi-system.The rate of misdiagnosed for CP could be beyond 90%.
