Objective:To investigate the collection efficiency of peripheral blood hematopoietic stem cells and explore its influencing factors.Method:The parameters of the collection process, blood routine indexes and the number of MNC and CD 34+ cells of the product were detected by Fresenius blood cell separator, Mindray blood cell analyzer and BD flow cytometer. A retrospective analysis was performed on 72 patients who underwent autologous peripheral blood hematopoietic stem cell transplantation in Southwest Hospital of Army Medical University from January 2013 to January 2021, including the correlation among gender, age, blood routine indexes, collection circulation volume and MNC and CD 34+ cell count in these cases, and influence of various factors on collection efficiency of peripheral blood stem cells. The correlation among gender, age, blood routine indexes, collection circulation volume and MNC and CD 34+ cell count in 72 cases of autologous transplantation patients, and influence of various factors on collection efficiency of peripheral blood stem cells were analyzed retrospectively. Results:There were no significant differences in collecting efficiency of peripheral blood stem cells among patients with different age, sex and disease type ( P>0.05). The collected MNC count of all patients was positively correlated with the collection cycle count ( r = 0.33, P<0.001) and WBC count after mobilization ( r = 0.41, P<0.001). The number of CD 34+ cells collected was positively correlated with MNC count after mobilization ( r = 0.38, P<0.001) and the amount of white membrane collected ( r = 0.48, P<0.001). Logistic multivariate regression analysis showed that MNC count after mobilization: P<0.001, 95% CI 0.07(0.05 - 0.09), collection cycle amount [ P<0.001, 95% CI 0.00(0.00 - 0.00)] and postharvest total amount [ P<0.001, 95% CI 0.07(0.05 - 0.10)] were the influencing factors of the collected MNC number. Meanwhile, these factorswere also the influencing factors of the collected CD 34+ number (MNC count after mobilization: P<0.001, 95% CI 0.09(0.04 - 0.14); collection cycle amount: P = 0.003, 95% CI 0.00(0.00 - 0.00); postharvest total amount: P = 0.005, 95% CI 0.08(0.03 - 0.14)). Conclusions:The collection efficiency of peripheral blood stem cells varies greatly among individuals. The more MNC counts after mobilization, the more peripheral blood stem cells could be collected. In order to obtain high collection efficiency, it is necessary to adjust the parameters of peripheral blood MNC before collection, and pay attention to the collection circulation quantity, postharvest total amount and white membrane volume.

Background:Harvest of hematopoietic progenitor cells via leukapheresis is being used increasingly for autologous transplantation. Adequate yield of cells per kilogram body weight of recipient is required for a successful engraftment. Collection efficiency(CE)is a useful parameter to assess quality of peripheral blood stem cell (PBSC) collection program. In this study, we report a 25-year experience in a tertiary care Hospital in Italy.Patients and Methods:1,026 consecutives autologous PBSC collection procedure, performed in 763 patients, from January 1996 to December 2020 were retrospectively considered.Data regarding patients, Blood Cells Separators (BCS) , apheresisprocedures and PBSC products were collected in our database. In these 25 years different BCSwere adopted in our Apheresis Unit (AU). In the first period (1996-1999) we used Fresenius Com.Tec, in the central period (2000-2013) we used Cobe Spectra and inthe last period (2014-2020) Spectra Optia. Results:As regards the evaluation of patients before leukapheresis, the most significant data was the increasing number of CD34+ cells. Considering the PBSC collection procedure, there was a progressive increase in the processed blood volume with a shorter apheresis duration. Data related to the PBSC collection demonstrated an increasing CD34+ cell yield and efficiency a raise in CE that was 43% using Fresenius COM-TEC BCS, 49% using Cobe Spectra BCS and 53% using Spectra Optia BCS. . Conclusions:These results were observed considering a 25-year period, thus a great number of factors likely contributing to the observed results, including technological improvement of the instrumentation for leukapheresis,increased experience of the team operating in the Apheresis Unit, improved mobilization protocols, better criteria for patients’ selection. Focusing our attention on CE we observed quite satisfactory results with a median which rose from 43% to 53% with an increase of 10% in the observation period.

【Objective】 To explore the effects of blood routine parameters on the peripheral blood hematopoietic stem cell collection of healthy donors, and predict collection timing based on these parameters. 【Methods】 The blood routine parameters pre-donation and the total number of mononuclear cells post-donation of 249 donors who applied blood cell separator to collect peripheral blood hematopoietic stem cells in our hospital from January 2018 to August 2020 were collected. Taking total nucleated cells of circulating blood per litre as the main evaluation index, and its collection with blood routine parameters pre-collection was analyzed. The relevant influencing factors were analyzed using multiple linear regression analysis. The blood routine parameters of healthy donors who donated peripheral blood hematopoietic stem cells in our hospital from September 2020 to October 2020 were substituted into the equation to obtain the predicted values, which were then compared with the actual values obtained from actual product using t test for verification. 【Results】 The analysis showed that the parameters of Hb, RBC, Hct, leukocyte count, neutrophil, lymphocyte, monocyte and Plt were statistically correlated with the total number of mononuclear cells of circulating blood per liter volume (P<0.05). There was a linear relationship between lymphocyte, monocyte, Plt and leukocyte count and the total number of mononuclear cells of circulating blood per liter. The total number of mononuclear cells of circulating blood per liter was set to (Y), and the variables such as lymphocyte (X1), monocyte (X2), Plt (X3), leukocyte count (X), and neutrophil were used as dependent variables for multiple linear regression, and the equation was: Y=9.814+ 3.131X1+ 1.666X2+ 0.020X3+ 0.124X4. There was no statistical difference between the predicted value and the calculated value (P>0.05). 【Conclusion】 The blood routine parameters of lymphocyte, monocyte, platelet count and leukocyte count of donors before collection can effectively predict the collection efficiency, therefore help predict the collection time.

Objective To contrastively analyze the performance of two kinds of blood cell separators for apheresis platelets . Methods 80 blood donors were selected from 4 234 healthy apheresis platelet donors and their data before and after apheresis platelets were respectively collected by using two kinds of different blood cell separators Trima and Amicus .The product quality during the collection process ,residual WBC and RBC count ,acquisition time ,collection efficiency ,scrapping situation and safety were performed the comparative analysis between the two kinds of blood cell separator .Results In the case of the difference of the basic parameters before apheresis platelet having no statistical significance ,the two kinds of blood cell separator had no differences in the aspects of product quality ,processing blood amount and the use amount of anticoagulants(P>0 .05);but there were statisti‐cal differences in the aspects of acquisition time ,collection efficiency and pipeline residual blood amount (P<0 .05) .Conclusion Two kinds of blood cell separators have their own advantages and disadvantages during the use process ,so the blood cell separator should be selected according to the physical quality and their own characteristics of blood donors .

Objective To study the relationship of the collection efficiency and physical parameters,such as the height of exhaust hood,baffle,ventilation conditions of these parameters on the effect of control.Methods The computational fluid dynamics software FLUENT was used to form the model,and to maximize production in line with the actual operating conditions for physics experiments,to take air numerical simulation and physical model of re-testing the method of combining.Results Without the lagging conditions,the air flow system should be chosen 1 320 m~3/h,with the lagging conditions,the air flow system should be chosen 1 078 m~3/h.The system in a certain air volume,lower installation height could increase the average wind speed of breathing zone.At a certain suction cover installation height and air flow,the baffle could improve the collection efficiency.Without the baffle,the collection efficiency and control distance(dimensionless)showed a relation of cubic polynomial.Conclusion The physical parameters of suction on the hood may impact the collection efficiency at breathing zone measuring point.

BACKGROUND: It is necessary to protect patient from white blood cells (WBC) caused side effects of platelet transfusion by reducing the WBC contamination in single donor platelets (SDPs). Objective of the new software is to improve WBC depletion performance and collection efficiency. Revised software version, LDP Rev. C2 was installed in our MCSR+ (Hemonetics, USA). We compared the newly introduced software version with the previous software LDP Rev. C. METHOD: SDPs were collected from registered and random repeat donors who visited our blood center. After 49 single needle collections by MCSR+ (software LDP Rev. C) were performed, revised software (LDP Rev. C2) was installed and 48 single needle collections were carried out. The platelet count of donors were measured electronically. The target platelet yields were 3.0x10(11). All units of SDPs were tested for platelet yields and residual WBC. And other parameters were also evaluated. RESULTS: The MCSR+ LDP collected platelets with mean platelet yields of 3.3x10(11)(Rev. C) and 3.4x10(11)(Rev. C2). The total processing blood volume and collection time were significantly reduced in Rev. C2. The collection efficiency was also significantly improved in Rev. C2 (64% vs 57%). Residual WBC in all product collected from software Rev. C2 were below 1 106 and 71% of the products revealed residual WBC below 1 105, respectively. Citrate toxicity was not observed during the apheresis by Rev. C2. CONCLUSION: Revised software LDP Rev. C2 in MCSR+ showed improved collection efficiency and leukocyte depletion performance compared to the Rev. C. And optional control of citrate re-infusion rate seemed to reduce donor citrate reactions during the apheresis.

BACKGROUND: Transplantation of allogeneic peripheral blood stem cells (PBSCs) may have advantage over bone marrow transplantation with regards to the speed of hematopoietic and immunologic recovery. Recently to overcome the need for multiple leukaphereses to collect enough PBSCs for autologous transplantation, large volume leukaphereses (LVL) are used to process multiple blood volumes per session. Experience with this technique in healthy individuals after mobilization with colony stimulating factor (CSF) is limited. We have investigated the efficacy and safety of LVL for the collection of G-CSF and GM-CSF mobilized PBSCs from healthy donors. METHODS: This study was done on 40 healthy donors who were mobilized with G-CSF and GM-CSF for allogeneic peripheral blood stem cells transplantation (allo-PBSCT). After 5 days of mobilization treatment, PBSCs were collected by LVL with Fenwal CS-3000 Plus (Baxter Co, USA). In LVL, heparin was administered in addition to ACD-A. Patients underwent of LVL procedures daily to obtain a target cell dose of >or= 4x10(8)/kg MNCs and >or= 6x10(6)/kg CD34+ cells. RESULTS: 66 LVL procedures were done on 40 donors. Of these donors, 31 (77.5%) reached the collection target with one leukapheresis. The product per LVL contained a mean 5.79+/-2.47 10(8)MNCs/kg and 11.6+/-10.62x10(6) CD34+ cells/kg respectively. Mean percentages of MNC were 79.88+/-22.15% and collection efficiencies of MNCs were inversely related to the starting MNC count (r=-0.536, P<0.001). After LVL, although none of the donors exhibited bleeding complications, platelets decreased from 187.4+/-52.68x10(3)/microL to 74.88+/-13.7x10(3)/microL and activated partial thromboplastin time (APTT) prolonged from 29.13+/-3.77 seconds to 67.51+/-54.26 seconds. CONCLUSION: We conclude that LVL after mobilization treatment with G-CSF and GM-CSF in normal healthy donors was tolerable and efficient methods for PBSCs collection, but long-term risk of adverse effects in normal donors needs to be carefully addressed by individual institutions as well as national and international registries.
