RESUMO

Objective To investigate the effects of post-operative analgesia with oxycodone or morphine for patients undergoing colon cancer radical surgery on platelet activation and cellular immunity.Methods Forty colon cancer patients scheduled for radical surgery, 23 males and 17 females, ASA physical status Ⅰ or Ⅱ, were randomly divided into 2 groups (n=20 each): oxycodone group (group O) and morphine group (group M).Patient-controlled intravenous analgesia (PCIA) was used for post-operative analgesia.PCIA solution contained oxycodone 1 mg/kg and tropisetron 6 mg in 100 ml normal saline in group O or morphine 1 mg/kg and tropisetron 6 mg in 100 ml normal saline in group M.Blood samples were obtained from the patients at 5 min before anesthesia induction (T0), 4 h after surgery (T1), 24 h after surgery (T2) and 48 h after surgery (T3).The levels of glycoprotein (GP)Ⅱb/Ⅲa, P-selection (CD62P), natural killer (NK) cells, NKT cells, and natural Treg (nTreg) cells were detected.The platelet aggregation rate (PAR) was determined.Results Compared with T0, the levers of GPⅡb/Ⅲa, CD62P, PAR and nTreg cells were significantly higher at T1 in group O and at T1, T2 in group M (P<0.05).Compared with T0, the levels of NK and NKT cells were decreased significantly at T1 in group O and at T1-T3 in group M (P<0.05).The levels of GPⅡb/Ⅲa, CD62P, PAR and nTreg cells at T2 and T3 in group O were decreased significantly as compared with group M (P<0.05).The levels of NK cells, NKT cells at T2 and T3 in group O were significantly higher than those in group M.Conclusion Post-operative analgesia with oxycodone for patients undergoing colon cancer radical surgery exhibits a more significant effect of decreasing platelets activity and presents a less disturbance on cellular immunity as compared with morphine.

Objective To investigate the result of colonoscopic positioning in laparoscopic treatment of colon tumors . Methods A retrospective analysis was made in 16 cases of colon tumors treated with laparoscopic colon resection from January 2014 to November 2015.During the operation, the lesion could not be locatized and the resection range could not be determinated because the serous layer was not involved or the lesion was located in the lateral abdominal wall .Transanal colonoscopic positioning of the lesion with light transmission method was carried out . Results The pathological changes were found under colonoscopy in all the 16 cases and the laparoscopic resection was successfully completed without conversion to open operation .No missed resection or incorrect resection occurred.The operation time was (112.5 ±31.0) min and the blood loss was (22.8 ±11.2) ml.Among 9 cases of preoperative pathological diagnosis of benign tumors , there were 6 cases of malignant tumors after surgery , including 4 cases of moderately differentiated adenocarcinoma , 1 case of moderate to severe atypical hyperplasia with cancerous lesions , and 1 case of moderately differentiated adenocarcinoma with mucous adenocarcinoma .A total of 15 patients were discharged from hospital at 12-24 days after the operation , whereas 1 patient was dismissed 1 month after the operation due to anastomotic fistula .The 16 patients were followed up for 3 months with no recurrence under colonoscopic examiantion . Conclusions During laparoscopic colon resection , if the tumor can not be locolized , colonoscopic positioning can help prevent missed resection or incorrect resection , which increases the indication and safety of laparoscopic and colonoscopic minimally invasive treatment .It has good clinical application value and deserves further promotion .

Intestinal lipomas can occur anywhere in the gastrointestinal tract, and these tumors are more frequent in the colon. By being largely asymptomatic, colonic lipomas are usually found incidentally, as findings in colonoscopy examinations, in association with biopsy. Endoscopic or surgical resection is the therapeutic option, depending on the size of the tumor, its location, and the presence or absence of symptoms. In this study, we present a case of a 59-year old woman, with a descending colon lipoma histologically diagnosed only after surgical resection of the lesion. The approach was adopted according to the patient's clinical picture (intestinal bleeding, vomiting and weight loss), in addition to the occlusion of 80% of the colonic lumen observed in a colonoscopy.


Os lipomas intestinais podem ocorrer em qualquer parte do trato gastrointestinal, sendo mais frequente no cólon. Por serem em grande parte assintomáticos, os lipomas colônicos são usualmente encontrados acidentalmente como achados de exame de colonoscopia associada à biópsia. Como opções de tratamento, há a ressecção endoscópica ou cirúrgica, a depender do tamanho do tumor, sua localização e presença (ou não) de sintomas. Nesse relato, é apresentado um caso de uma mulher de 59 anos com lipoma de cólon descendente, diagnosticado histologicamente apenas após ressecção cirúrgica da lesão. A conduta foi adotada pelo quadro clínico de enterorragia, vômitos e perda ponderal, além da oclusão de 80% da luz do cólon observada em exame de colonoscopia.

Objective To investigate the effect of eicosapentaenoic acid(EPA)on the apoptosis of human colon cancer SW480 cells and the mechanisms.Methods Mitochondrial membrane potential was detected,the quan-tity of cytochrome C was analyzed by Elisa kit,and the expression of cleaved caspase -9 and caspase -3 was detected by Western Blot.Results After treatment with EPA (0μg/mL,42.1μg/mL,84.2μg/mL,168.4μg/mL),the mitochondrial membrane potential(Δψm)of SW480 cells were declined (P <0.05 ),the values were (99.71 ± 0.04)%,(95.04 ±0.10)%,(88.65 ±0.41)% and (73.60 ±1.20)%(t =5.161,6.302,4.601,5.198,all P <0.05).The quantity of cytochrome C in cytosol was increased significantly compared with no treatment group,and the values were (12.8 ±1.2)ng/mL,(115.5 ±3.5)ng/mL,(290.5 ±5.2)ng/mL and (262.0 ±12.5 )ng/mL in different EPA treatment groups(t =6.345,6.013,5.846,4.613,all P <0.01).The expression of cleaved caspase -9 and caspase -3 were significantly increased.Conclusion EPA inhibits SW480 cells growth and induces apoptosis in a dose dependent manner.This action may be mediated by mitochondria -mediated intrinsic apoptosis pathway,cyto-chrome C release,and caspase -9 and caspase -3 activation.

RESUMO

Objective To study the effect of ShenQi FuZheng injection on side effects of chemotherapy and the patients' life quality after operation of colon cancer. Methods 60 patients were recruited into a control group and a treatment group, with 30 patients in each group. The treatment group was treated with conventional chemo-medicines plus ShenQi FuZheng injection, while the control group was treated with conventional chemo-medicines exclusively. Side effects and life quality were compared between the two groups after two courses of treatment. Results The side effects and mark of the life quality in the treatment group were both obviously better than the control group (P<0.05) after the treatment. Condusion ShenQi FuZheng injection can decrease side effects of chemotherapy and improve patients' life quality.

RESUMO

"Pseudosarcomatous change" occurs when sarcoma-like atypical cell hyperplasia is grossly indistinguishable from malignant tumor. Pseudosarcoma often exists in those areas of the gastrointestinal track where ulcer or erosion commonly takes place. This may indicate atypical granulomatous cell proliferation during a period of healing, as well as benign atypical change in the mesenchyme due to chronic inflammatory reaction. Both the visual appearance and intestinal involvement of tuberculous colitis are variable, and tuberculosis may result in reactive mesenchymal change associated with ulcer or polyp, which is that's induced by chronic inflammation. We report here on a case of a young female pulmonary tuberculosis patient who suffered with a pseudosarcoma of the ascending colon, and this was associated with tuberculous colitis. The patient underwent successful sugical resection. We report on this case, along with a review of the relevant literature.

RESUMO

Flat colon lesions, which include superficial colon tumor and depressed colon tumor, have close relationship with colon cancers. The main type of flat colon tumor is laterally spreading tumor (LST) while the main type of depressed tumor is depressed early colon cancer (IIc type). Little has been reported about the flat colon tumor because it is difficult to be detected and has not yet been broadly recognized by clinicians. Recently, clinicians with the Nanfang Hospital have innovated some novel colonoscopic approaches to improve the diagnosis and treatment of flat colon tumor and accumalate experiences in this regard. Their conclusions are: 1. Improvement in recognizing the physical characteristics of the lesion and the application of chromoscopic colonoscopy are the key points to reveal and identify the flat colon tumors. 2. The best theraputic methods for flat colon lesions are endoscopic mucosal resection and piecemeal endoscopic mucosal resection.

Purpose:To investigate the expression of vascular endothelial growth factor (VEGF) and its relationship with proliferating cell nuclear antigen (PCNA) and the prognosis of colon cancer patients. Methods:The VEGF and PCNA as well as p53 expression were examined using immunohistochemistry with monoclonal antibody in 70 surgical samples of colon cancer. Purpose:To study the relationship between Fas/FasL expression and apoptosis of colon adenocarcinoma cell lines induced by chemotherapeutic drugs and to screen out hypersensitive drugs. Methods:Colon adenocarcinoma cell lines LS174T and LoVo were incubated respectively in cultural media with DDP,MMC,5 FU and EPI at concentrations of PPC(plasma peak concentration),1/10 PPC,1/5 PPC,5 PPC and 10 PPC. Fas/FasL expression and apoptosis were assessed by FACScan, and DNA ladder was examined by agarose gel electrophoresis. Results:In LS174T cells, Fas expression and apoptosis of DDP group, MMC group and EPI group reached their highest peaks at 1/5 PPC or PPC,and Fas expression and apoptosis were positively correlated by relativity analysis. There was no significant correlation between Fas expression and apoptosis in LoVo cells. Conclusions:We could find out hypersensitive drugs and their appropriate dosage according to apoptosis, partly according to Fas expression. The different apoptosis paths between LS174T cells and LoVo cells demonstrate the importance of individualized selection of chemotherapeutic drugs.
