Las enfermedades cardiovasculares son la principal causa de muerte a nivel mundial, manifestándose principalmente como enfermedad isquémica coronaria. El pronóstico y desenlace del Síndrome Coronario Agudo (SCA) depende en gran proporción de la disfunción endotelial asociado a este cuadro. Objetivo:Evaluar el efecto de los ácidos grasos omega­3, sobre la función endotelial en el Síndrome Coronario Agudo (SCA). Métodos: Se realizó estudio con 16 pacientes con síndrome coronario agudo (SCA) tratados con Omega-3 a dosis de 3 gramos/día+Terapia Convencional (O3+Tc) durante 30 días. Se les realizó a las 12 horas del ingreso y los a 30 días, perfil lipídico, proteína C reactiva ultrasensible (PCRus), Endotelina 1 (ET-1), y Péptido Natriurético cerebral (NTproBNP). Resultados: Durante 4 semanas de seguimiento, el uso de 3 gramos/día de ácidos grasos omega-3, añadido a la terapia convencional en el Síndrome Coronario Agudo, mostró una reducción significativa de la concentración plasmática de NTproBNP (p = 0,000), PCRus (p =0,000) y triglicéridos (p = 0,004). Conclusión: Tres gramos al día de ácidos grasos omega-3 contribuye a la reducción de la concentración plasmática de Triglicéridos y NTproBNP en pacientes con síndrome coronario agudo(AU)


Mainly as coronary ischemic disease. the prognosis and outcome of Acute Coronary Syndrome (ACS) depend largely on the endothelial dysfunction associated with this condition. Objective: To evaluate the effect of omega-3 fatty acids on endothelial function in Acute Coronary Syndrome (ACS). Method: A study was conducted with 16 patients with acute coronary syndrome (ACS) treated with Omega-3 at a dose of 3 grams/day+Conventional erapy (O3+Tc) for 30 days. Lipid profile, ultrasensitive C-reactive protein (usCRP), Endothelin 1 (ET-1), and Brain Natriuretic Peptide (NTproBNP) were performed 12 hours aer admission and 30 days later. Results: During 4 weeks of follow-up, the use of 3 grams/day of omega-3 fatty acids, added to conventional therapy in Acute Coronary Syndrome, showed a significant reduction in the plasma concentration of NTproBNP (p = 0.000), hsCRP (p =0.000) and Triglycerides (p = 0.004). Conclusion: three grams a day of omega-3 fatty acids contributes to the reduction of the plasmatic concentration of Triglycerides and NTproBNP in patients with acute coronary syndrome(AU)

Background: Autism Spectrum Disorder (ASD) is a disorder of neurodevelopment, which affects individuals across social, ethnic, and geographic groups. Autistic children have difficulty with gross motor and fine motor functioning difficulties, including a wide range of signs and symptoms. Toe walking due to TA tightness is commonly observed gait in autistic children altering foot posture in them. The knowledge about the abnormalities can be useful for the assessment and treatment planning of ASD children. We evaluated TA tightness, ROM of the ankle joint, and compare the effect of manual therapy (MET) and conventional therapy for improving TA flexibility and foot posture. Methods: An RCT included 20 diagnosed autistic children(13male,7female) as per inclusion criteria the subjects were divided into two groups, i.e., group A and B, the group A was given Conventional Therapy in the form of passive stretching whereas Group B was given Manual Therapy in the form Muscle Energy Technique. The participants were clinically examined and evaluate TA tightness in the form of Elastography, Range of motion, and foot posture. Data were taken as pre and after post-intervention. Results: There were significant changes in elastography readings, foot posture index, and range of motion in both groups post-intervention, but significant improvement was observed in group B as compared to group A, i.e., p>0.05. Conclusion: This has been concluded that there is a significant effect of Manual therapy in the form of muscle energy technique for improving TA flexibility and foot posture as compared to conventional treatment.

Background: Acne Vulgaris is chronic inflammatory disease of pilosebaceous units. Oral isotretinoin is recommended for moderate to severe acne vulgaris who are not responding satisfactorily to conventional therapies. Recent reports indicate that acne patients have been benefiting from the low dose treatment protocols. However, long term daily use of this drug results in frequent side effects such mucocutaneous and systemic side effects. Our aim was to assess and compare the various side effects and patient satisfaction of oral isotretinoin in low dose continuous and intermittent treatment of moderate to severe acne vulgaris.Methods: This was a prospective randomized open labeled comparative study carried out at outpatient department in the Department of Dermatology in Mandya Institute of Medical sciences, Mandya. Patients with moderate to severe acne were assigned equally (50 subjects each) to one of the two treatment regimens by using block randomization technique, Group A was given low dose continuous regimen-20 mg oral isotretinoin once daily for 4 months and Group B was given low dose intermittent regimen-20 mg oral isotretinoin once daily for 1 week out of every 4 weeks. The patients were followed up every 4th week during the treatment period. The patients were examined and side effects were noted in each visit. A six month follow-up evaluation was done to analyze patient satisfaction.Results: Muco-cutaneous dryness was most common adverse effect noted in both the groups A and B. Itching (42%), Alopecia (44%), Myalgia (36%) were seen most commonly in group A and Acne flaring (47%) was most common with group B. With regard to patient satisfaction, in group A 42% were satisfied and 20% were very satisfied, in group B 36% were satisfied and 14% were very satisfied.Conclusions: Study suggests that, Muco-cutaneous dryness was most common side effect in both treatment regimens. Side effects were more frequent with low dose continuous than low dose intermittent isotretinoin regimen. Patient satisfaction was better in continuous regimen.

@#AIM: To evaluate the effect of glass rod massage combined with conventional Meibomian gland dredging for Meibomian gland dysfunction(MGD). <p>METHODS: Totally 130 cases(220 eyes)of MGD treated in our hospital from August 2016 to June 2017 were analyzed retrospectively. The combined group(<i>n</i>=65, 108 eyes)were treated with glass rod massage combined with conventional Meibomian gland dredging; controlled group(<i>n</i>=65, 112 eyes)was given conventional therapy. The duration was 2wk.The ocular surface disease index scale(OSDI), ocular symptoms score \〖tear break-up time(BUT), Schirmer test(SⅠt), corneal fluorescein staining(CFS), limbi palpebralis scores, property of Meibomian secretion\〗, intraocular pressure, efficient rate were monitored. <p>RESULTS: Before treatment, the OSDI score, BUT, SⅠt, CFS, limbi palpebralis scores, property of Meibomian secretion and IOP were not significantly different between the two groups(<i>P</i>>0.05). OSDI score, CFS, limbi palpebralis scores, property of Meibomian secretion of the combined group was significantly lower than that of the control group(<i>P</i><0.01). The post-treatment BUT and SⅠt of the combined group were significantly higher than those of the control group(<i>P</i><0.01). No significant difference was found in post-treatment IOP between the two groups, either of the two groups higher than 21mmHg(<i>P</i>>0.05). The total effective rate of the combined group was 92.6%(100/108), while the total effective rate of control group was 78.6%(88/112, <i>P</i><0.05). <p>CONCLUSION: Glass rod massage combined with conventional Meibomian gland dredging in the treatment of MGD can effectively ameliorate the ocular surface symptoms, Meibomian secretion function, and promote tear secretion.

Objective To investigate the prognostic factors of young stroke patients with conventional treatment,and provide the basis for clinical diagnosis and treatment of stroke.Methods The clinical data of 72 cases of young stroke patients with conventional treatment were analyzed retrospectively.The prognostic factors were analyzed.Results In seventy-two cases of young stroke patients,18 cases of conventional treatment failed (25.00%,18/72).Univariate analysis showed that smoking,alcohol,underlying disease,dysphagia,barthel index (BI) score,U.S.national institutes of health stroke scale (NIHSS) score and Oxford handicap scale (OHS) score was closely related with the prognosis (P < 0.05 or < 0.01).Logistic analysis showed that the age,BI score,NIHSS score,OHS score and underlying diseases was the independent prognostic factor for young stroke patients.Eighteen cases who failed in conventional treatment fails accepted comprehensive rehabilitation treatment.Compared with that before treatment,1,3,6 months after treatment BI,NIHSS and OHS scores were significantly decreased (P < 0.05).Conclusions BI,NIHSS,OHS score and underlying diseases are the independent prognostic factor for young stroke patients.Surgery and postoperative comprehensive rehabilitation in young stroke patients who failed in conventional treatment can improve patient's outcomes and prognosis.

El Asma Casi Fatal (ACF) es la condición de un paciente en falla respiratoria progresiva debido a asma, en quien la terapéutica convencional ha fallado. La mortalidad se asocia típicamente a la falla en reconocer la severidad del episodio y al retardo en el tratamiento oportuno. Existen poblaciones que presentan mayor riesgo de morir durante las exacerbaciones de asma: una de ellas son los lactantes, debido a las características anatómicas y fisiológicas de sus pulmones. En los niños menores de 5 años la valoración del grado de severidad, depende más del examen físico. Se considera crisis severa a un valor en el Puntaje Pulmonar (Pulmonary Score) mayor o igual a 7 y/o una StcO2 < 91%. Los parámetros paraclínicos a tomar en cuenta en ACF son: los gases arteriales, la StcO2 y las pruebas de función pulmonar que pueden realizarse en mayores de 5 años. En esta revisión se hace énfasis en el tratamiento del ACF, especialmente en terapia convencional con nivel de evidencia y en el uso de las medidas terapéuticas no convencionales que han demostrado los mejores resultados, incluyendo las medidas de soporte de la vía aérea.


Almost Fatal Asthma (AFA) is the condition of a patient in progressive respiratory failure due to asthma, in whom conventional treatment has failed. Mortality is associated typically to failure in recognizing the severity of the episode and to retardation in opportune treatment. There are populations that present greater mortality risk during asthma episodes. One of these is the infant population, due to the anatomical and physiological characteristics of their lungs. In children under 5 years of age, the assessment of the severity of the episode depends more on physical examination. An asthma crisis is considered to be severe when the Pulmonary Score is greater or equal to 7 and/or the StcO2 is less than 91%. Laboratory parameters to be considered in AFA are: arterial gases, StcO2 and pulmonary function tests that can be performed in children over 5 years of age. In this review emphasis is focussed on treatment of AFA, specially on conventional therapy with level of evidence and on non conventional therapeutic measures that have demonstrated best results, including support measures for the respiratory airway.

PURPOSE: To study the effect of topical autologous serum in the treatment of recurrent punctate corneal erosions. METHODS: A total of 10 eyes from recurrent corneal erosion patients, who had suffered from recurrence after initial treatment with conventional therapy were treated using topical autologous serum from October 2001 to April 2003. We investigated the efficacy of this method over conventional therapy by retrospective review of their medical records. RESULTS: The recurrence rate of erosions before treatment with autologous serum eyedrops ranged from once every 2months to twice a month. Mean follow-up time was 11.0 +/- 4.6 months (range, 6~19 months). There was one case of recurrence 3 months after first treatment with autologous serum eyedrops which was effectively cured with autologous serum eyedrops. No side effects were noted in any of the treated patients. CONCLUSIONS: Topical autologous serum application may be considered as an effective and safe treatment modality in recurrent corneal erosions.

RESUMO

Current goals of ulcerative colitis treatment include rapid remission,steroid-sparing remission,endoscopic mucosal healing,reduction in hospitalization and surgery and improvement in quality of life.The treatment principle is rational medications according to the severity,activity and extent of the disease with individualized and comprehensive treatment.The medications conventionally used are 5-ASA,GCS and IS.Newly developed biological agents like Anti TNF-?(infliximab)are used in severe UC and refractory cases.Traditional Chinese Medicines have been used in more than 80% of UC cases and will play a great role in the future.The indications for surgery include acute perforation,massive hemorrhage,toxic megacolon,malignancy and refractory cases for medical therapy.

A burn of indeterminate depth is defined as a partial-thickness burn whose preferred management is difficult to select and whose prognosis is difficult to predict. The authors tried to determine the differences in the early morbidity and the late morbidity of early autograft and non-operative conventional therapies for burns of indeterminate depth less than 30% TBSA. The morbidity results were compared between 66 early autograft patients and 75 non-operative conventional therapy patients. In early morbidity, the early autograft group had a shorter average hospitalization, lower hospital cost, less wound contamination and sepsis, and less use of systemic antibiotics, such as cephalosporin and aminoglycoside, but more use of blood products than the non-operative conventional therapy group. The time away from work showed no significant difference when the two groups were compared. When long- term follow up was compared, the non-operative conventional therapy group showed superiority to the early autograft group in cosmetic outcomes, as wound surface character and blister formation. The joint function motion exhibited no difference between the two groups. We conclude that in otherwise healthy patients with dermal burns of indeterminate depth less than 30% TBSA, early autograft is the preferred form of treatment in early morbidity.

OBJECTIVE:To observe the efficacy and safety of adjuvant Shensong Yangxin Capsule(SYC)in patients with stable angina(SA)receiving conventional therapy.METHODS:Fifty-eight chronic stable angina(CSA)patients were randomly assigned to 2 groups:the SYC group(n=28)treated with SYC plus conventional therapy while the control group(n=28)with conventional therapy alone.The patients were allowed to take nitroglycerine if number of the angina attacks ≥ 2 per day.The course of treatment fro both groups was 12 weeks.RESULTS:The total ischemic time during 24 hours,the ST segment decreasing amplitude and the frequency of angina pectoris attacks in two groups were all improved significantly after treatment(P

RESUMO

OBJECTIVE: To observe the therapeutic effects of the conventional therapy plus Melitracen and Flupentixol in the treatment of functional dyspepsia(FD).METHODS: 320 FD cases were divided into gastrointestinal dyskinesis type(Type A,n=120 cases),ulceration type(Type B,n=100 cases),and nonspecific type(Type C,n=100 cases) based on their clinical symptoms.Patients of different clinical types were randomly assigned to receive conventional therapy alone or in combination with Melitracen and Flupentixol for 4 weeks.The curative effects were observed using gastrointestinal symptom rating scale(GSRS) and the total effective rate were computed.RESULTS: After treatment,the GSRS score of 3 different clinical types of FD in the combination therapy group versus the conventional therapy alone were 1.53?1.68 vs.3.10?1.78,2.02?1.63 vs.3.48?1.80 and 3.46?2.95 vs.5.18?3.21 respectively(P
