RESUMO

RESUMEN La queratopatía bullosa pseudoafáquica o edema corneal crónico pseudoafáquico del paciente operado por cataratas con implante de lente intraocular es un proceso en el que la córnea se edematiza y pierde su función óptica, con síntomas dolorosos e irritativos de lagrimeo, ojo rojo, probabilidad de inflamación y ulceración corneal por infección secundaria. Se observan en esta afección multicausal dos grupos de pacientes. En uno de ellos el daño estructural corneal y ocular ha sido excesivo y no existe expectativa de mejora visual. El otro grupo aún conserva la potencialidad de recuperación de la agudeza visual. En el intento de solucionar esta afección se han empleado diversos tratamientos con fundamentos diferentes y resultados acordes con el desarrollo técnico y científico en que fueron aplicados. Es propósito de esta actualización describir y adecuar las posibilidades que los oftalmólogos puedan aplicar como terapéutica en sus respectivos niveles de actuación(AU)


ABSTRACT Pseudophakic bullous keratopathy or chronic pseudophakic corneal edema in patients undergoing cataract surgery with intraocular lens implantation is a process in which the cornea edematizes and loses optical function, with painful and irritating lacrimation symptoms, red eye, probable inflammation and corneal ulceration due to secondary infection. Sufferers of this multicausal condition are divided into two groups: In one of them structural corneal and ocular damage has been extreme and no visual improvement is expected, whereas in the other visual acuity recovery is still possible. In an attempt to heal this condition various treatments have been used with different rationales and results according to the scientific and technological development of the setting where they were applied. The present update is aimed at describing and adjusting the possible therapies that ophthalmologists may use in their respective fields of activity(AU)

RESUMO

PURPOSE: To report four cases of split cornea transplantation involving separate Descemet membrane keratoplasty and Deep anterior lamella keratoplasty from a single cornea. CASE SUMMARY: Four donor corneas were separated into the endothelium and other layers. The endothelial layers were transplanted into 4 pseudophakic bullous keratopathy patients, and the other layers were stored in corneal storage media. Deep anterior lamella keratoplasties were performed with the stored corneas in 2 lipid keratopathy and 2 keratoconus patients. Postoperatively, all grafted corneas were stable. CONCLUSIONS: The authors report the first 4 cases of split cornea transplantation in Korea, which is experiencing a shortage of donated corneas. Split cornea transplantation will be of benefit to a large number of patients by separating a single cornea into separate layers to be implanted into two patients.

OBJETIVO: Avaliar o conhecimento de estudantes de medicina sobre o processo de doação de córneas, comparando-o entre aqueles que já cursaram a disciplina de Oftalmologia com os demais. MÉTODOS: Foi aplicado um questionário a estudantes de medicina do interior de São Paulo, contendo dados como: idade, sexo, ano de graduação e 10 perguntas de múltipla escolha sobre o tema. As questões abordaram o limite de idade para doação, contra-indicações para o transplante, tempo para retirada das córneas, dentre outras. Foi comparado o conhecimento ( por cento de acertos) entre aqueles que haviam cursado a disciplina de Oftalmologia (Grupo A) com os demais (Grupo B). RESULTADOS: Foram entrevistados 402 estudantes. Destes, 140 eram do grupo A e 262 do grupo B. Não houve diferença estatisticamente significante entre os dois grupos (p=0,8328). CONCLUSÃO: O conhecimento sobre o processo de doação de córneas mostrou-se insuficiente entre os alunos entrevistados, mesmo para aqueles que haviam cursado a disciplina de Oftalmologia, apontando a necessidade de aperfeiçoamento do conhecimento divulgado nas escolas médicas.


PURPOSE: To compare knowledge of medical students about the cornea donation process among those who already studied Ophthalmology and the others. METHODOS: A questionnaire containing data as: age, sex, graduation year, and 10 multiple-choice questions about the subject was applied to medical students from the interior of São Paulo state, Brazil. The questions were: age for donation, contraindication for transplants, the time limit to remove the cornea, among others. The knowledge regarding cornea donation was compared between the two groups: students who already studied Ophthalmology (Group A) and the others (Group B). RESULTS: The study group was composed of 402 students, of whom 140 were of group A and 262 of group B. Knowledge between the two groups was different, but not statistically significant (p=0.8328). CONCLUSION: Knowledge about the cornea donation process among the interviewed seemed to be insufficient, even those who had studied Ophthalmology. Information and education about transplants in Medical Schools should be improved.

RESUMO

Combined with the clinical practice of Xi'an Eye Bank for over four decades,this article focuses on the ethical issues related to the practice of eye bank,such as voluntary cornea donation free of charge,the relationship between donors and recipients,the choice of operation indication and application of heterogeneous cornea,in order to promote the all-round development of eye bank system.
