RESUMO

Crossed occlusion can be treated either by overdenture and telescopic denture or by placing an implant at the edentulous area to reestablish the support on the occlusion. If alveolar bony support is sufficient and an the environment where an implant is inserted is favorable to restoring the masticatory and aesthetic function of a patient, the implant-supported fixed prosthesis can provide more definitive occlusal support and more aid for other oral functions. In this case report, a patient with a severe residual alveolar bone resorption following the extraction of teeth and who had a crossed occlusion was treated with sinus bone graft and alveolar bone augmentation in order to place the implants at prosthetically position. The definitive restoration was made to reflect the patient's occlusal and aesthetic function using the CAD/CAM double scanning method. Finally, the treatment had the masticatory and aesthetic function adequately restored, which is reported here.

RESUMO

In removable partial denture with crossed occlusion, it is difficult to meet the satisfaction of patient due to variable movement of denture and unfavorable stability under loading. In a case with few unilaterally teeth remaining, additional treatments such as implantation or residual root are required to assure bilaterally stable support. However, due to the medical condition of the patient with hypertension, angina and old age etc., removable partial denture was planned as treatment in this case. Proper diagnosis, accurate analysis and full understanding of the adaptation for neuromuscular system are necessary to recover the vertical dimension of patient from a decreased vertical dimension due to crossed occlusion.

When attempting to restore the oral function of a partially edentulous patient, there are a number of prosthetic treatment options available, depending on the structure of remaining teeth. For example, when only one set of maxillary and mandibular teeth are diagonally in place across from each other, it is difficult to gain stable occlusion. In this case, implants can be put in place at the corresponding edentulous area to achieve balance. By doing so, a stable occlusion can be achieved. For this case report, a patient with crossed occlusion after extraction was treated with maxillary RDP (removable dental prosthesis) and mandibular implant-supported RDP (removable dental prosthesis). Moreover, an implant fixture was placed under the posterior molar of the distal extension base diagonally across from the remaining maxillary teeth. Then, magnetic attachment was implemented. According to the patient who received the treatment, the result was functionally and aesthetically satisfactory.

RESUMO

Conventional removable partial dentures (RPDs) with distal extensions are associated with some problems, including lack of stability that calls for frequent relining, and cantilever actions of claps that can produce excessive loading to abutment teeth, and the need for unesthetic retentive arm clasps. Therefore, IARPDs (Implant-assisted RPD) that use implants to support or retain RPDs has been reported to improve stability, esthetics and masticatory performance of RPDs. Also, an IARPD that has zirconia occlusal table can prevent the incongruity of occlusal plane and the extrusion of antagonistic tooth. In this case of partially edentulous patient with crossed occlusion, each edentulous area was restored with implant fixed prosthesis and implant retained partial denture to suit each situation. Through the procedure, satisfactory outcomes were achieved both in functional and esthetic aspects.

RESUMO

When losing many of the remaining teeth, the relation among them can be put into dynamically unfavorable situation. In the case that the patient without any incompatibilityto the denture, overdenture can be considered. Moreover, we can overcome the dynamical disadvantage, and improve the support, stability, and retention of the partial denture by using implant at the edentulous area. In this case, patient with crossed occlusion between few remaining teeth was treated with maxillary and mandibular partial denture. Single implant fixture was placed at the edentulous space, opposing to the maxillary teeth occluded, and Locator(R) attachment was connected. The patient's esthetic satisfaction was improved by ideally adjusting the tilted occlusal plane. Since the patient was satisfied both esthetically and functionally, and maintained stable during the clinical observation for 6 months after the treatment, we would like to report about it.
