Introdução: Os arcos branquiais são os precursores embriológicos da face, pescoço e faringe. As anomalias dos arcos branquiais são a segunda lesão congênita mais comum de cabeça e pescoço em crianças. Entre essas anomalias, estão os cistos de arcos branquiais (BCC), que surgem devido a uma incorreta obliteração das fendas branquiais, ainda no período embrionário. Os BCC podem ser assintomáticos, apenas percebidos incidentalmente, e não se manifestar até a idade adulta. Resultados: Anomalias do segundo arco branquial devem ser consideradas como um dos possíveis diagnósticos diferenciais de massas cervicais, especialmente as que se manifestam como um abaulamento em região lateral do pescoço. Os BCC são formações de revestimento epitelial, sem aberturas externas. Após seu diagnóstico, o tratamento é cirúrgico, usualmente por meio de uma incisão cervical transversa e cuidadosa dissecação das estruturas, com o objetivo de extirpar toda a lesão. Conclusão: O método descrito, de excisão da lesão, por meio de incisão transversa em região cervical, dissecção tecidual por planos e ressecção de massa cística, é uma opção para o tratamento dessa deformidade, com adequado resultado estético e boa reprodutibilidade.


Introduction: The branchial arches are the embryological precursors of the face, neck, and pharynx. Branchial arch anomalies are the second most common congenital head and neck lesions in children. Among these anomalies are branchial arch cysts (BCC), which arise due to incorrect obliteration of the branchial slits, still in the embryonic period. BCCs may be asymptomatic, only noticed incidentally, and not manifest until adulthood. Results: Anomalies of the second branchial arch should be considered as one of the possible differential diagnoses of neck masses, especially those that manifest as a bulge in the lateral region of the neck. BCCs are epithelial lining formations without external openings. After diagnosis, treatment is surgical, usually through a transverse cervical incision and careful dissection of the structures, with the aim of extirpating the entire lesion. Conclusion: The method described of excision of the lesion through a transverse incision in the cervical region, tissue dissection in planes, and resection of the cystic mass is an option for the treatment of this deformity, with adequate aesthetic results and good reproducibility.

O cisto epidermóide (CE) é uma má formação cística de desenvolvimento incomum. Ocorre mais comumente na linha média do assoalho da boca, ocasionalmente localiza-se lateralmente ou em outros sítios. A etiologia do CE ainda é incerta, mas acredita-se que esteja associado a remanentes do ectoderma durante a fusão do primeiro e segundo arcos branquiais. A lesão cresce lentamente sem provocar dor, apresentando-se como massa de consistência semelhante à de borracha ou à massa de pão. Seu diagnóstico se dá por meio de exame clínico e exames complementares de imagem como tomografia computadorizada, ressonância magnética e/ou ultrassonografia, entretanto somente com o exame histopatológico é possível um diagnóstico definitivo. A excisão cirúrgica do cisto é o tratamento de escolha. Quando o diagnóstico é precoce e a intervenção correta, o índice de recidiva torna-se raro. Diante da raridade de casos na região maxilo facial e a dificuldade relacionada ao diagnóstico, o presente trabalho tem como objetivos descrever a ocorrência do cisto epidermóide bem como seu diagnóstico através de exame histopatológico/exames de imagem, tratamento cirúrgico e contribuir com a literatura já existente por meio de um relato de caso que foi diagnosticado no Serviço de Patologia Bucal da Universidade de Gurupi, campus Gurupi/TO.


Epidermoid cysts (EC) is an unusually developing cystic malformation that occurs most frequently in the midline of the floor of the mouth, occasionally located laterally or elsewhere. The etiology of the (EC) still uncertain, but it is believed to be associated with remnants of the ectoderm during the fusion of the first and second branchial arches. The lesion grows slowly without causing pain, presenting itself as a mass with a consistency like of rubber or bread dough. Diagnosis of (EC) is made through complementary imaging tests such as computed tomography, magnetic resonance imaging and/or ultrasound, only histopathological examination it is possible to obtain a definitive diagnosis. Surgical excision of cyst is the treatment of choice. When the diagnosis is made early and the intervention is correct, the recurrence rate is rare. Given the rarity of cases in the maxillofacial region and the difficulty related to diagnosis, this study aims to describe epidermoid cysts occurrence and your diagnostic through of histopathologic/imagine exams, surgical treatment and contribute to the existing literature, through a review and description of a clinical case that was diagnosed at the Oral Pathology Outpatient Clinic at the Faculty of Dentistry - University of Gurupi - UNIRG. Gurupi -Tocantins. Brazil.


El quiste epidermoide (CE) es una malformación quística de desarrollo infrecuente. Ocurre más comúnmente en la línea media del piso de la boca, ocasionalmente se localiza lateralmente o en otra parte. La etiología de la FB aún es incierta, pero se cree que está asociada con restos del ectodermo durante la fusión del primer y segundo arcos branquiales. La lesión crece lentamente sin causar dolor, apareciendo como una masa con una consistencia similar a la de la goma o la masa de pan. Su diagnóstico se realiza a través del examen clínico y pruebas de imagen complementarias como tomografía computarizada, resonancia magnética y/o ultrasonografía, sin embargo, solo con el examen histopatológico es posible un diagnóstico defi nitivo. La escisión quirúrgica del quiste es el tratamiento de elección. Cuando el diagnóstico es temprano y la intervención es correcta, la tasa de recurrencia se vuelve rara. Dada la rareza de los casos en la región maxilofacial y la difi cultad relacionada con el diagnóstico, el presente trabajo tiene como objetivo describir la ocurrencia del quiste epidermoide así como su diagnóstico a través del examen histopatológico/pruebas de imagen, tratamiento quirúrgico y contribuir a la literatura existente a través de un reporte de caso que fue diagnosticado en el Servicio de Patología Oral de la Universidad de Gurupi, campus Gurupi/TO.

Introducción: El quiste dentígero es un quiste odontogénico, casi siempre asociado a la corona de un diente retenido. Sus ubicaciones más frecuentes son la zona de tercer molar inferior y canino superior. Objetivo: Caracterizar la presentación de un quiste dentígero en un paciente adulto. Presentación de caso: Se presentó el caso de una paciente de 27 años de edad que acude a consulta por presentar sensación de opresión, dolor e inflamación en región de tercer molar inferior izquierdo. Al examen oral se observa tejido dentario en correspondencia con molar 38 parcialmente brotado con ligero edema de la mucosa a su alrededor. En radiografía panorámica se observa imagen radiolúcida en relación con cara mesial del 38 con discreto halo radiopaco, redondeada, compatible con un quiste dentígero. Conclusiones: Un diagnóstico a tiempo de estas lesiones constituye la clave para un tratamiento exitoso y menos invasivo, lo cual evitaría complicaciones mayores como la malignización, el crecimiento exagerado que puede ocasionar deformidad facial y la disfunción masticatoria permanente(AU)


Introduction: Dentigerous cyst is an odontogenic cyst, almost always associated with the crown of a retained tooth. Its most frequent locations are the areas of the lower third molar and the upper canine. Objective: To characterize the presentation of a dentigerous cyst in an adult patient. Case presentation: The case is presented of a 27-year-old female patient who came to the clinic with a sensation of pressure, pain and inflammation in the region of the lower left third molar. Oral examination permits to observe some dental tissue corresponding with molar 38 partially erupted with slight edema of the surrounding mucosa. Panoramic radiograph shows a radiolucent image related to the mesial side of 38, with a discrete radiopaque halo, rounded, compatible with a dentigerous cyst. Conclusions: A timely diagnosis of these lesions is the key to a successful and less invasive treatment, which would avoid major complications, such as malignization, the exaggerated growth that can cause facial deformity and permanent masticatory dysfunction(AU)

Introducción: el queratoquiste hace parte de los quistes odontogénicos del desarrollo, derivado de los residuos de la lámina dental con un comportamiento biológico similar al de una neoplasia benigna. Presenta un alto grado de recidiva y agresividad, con lo que ocasiona una destrucción ósea masiva. Puede aparecer en cualquier zona de los maxilares y estar relacionado con dientes retenidos. Imagenológicamente, presenta una lesión radiolúcida multilocular o poliquística, que muestra un borde cortical delgado. Objetivos: demostrar la importancia de un diagnóstico correcto, una detección temprana y acertada, así como inferir la necesidad de utilizar la solución de Carnoy como primera línea de tratamiento, y determinar el momento oportuno para hacerlo. Presentación de caso: hombre de 34 años de edad, sin antecedentes personales relevantes. Tres años atrás había presentado una lesión radiolúcida localizada en los órganos dentarios 34 y 35, motivo por el que le realizaron endodoncia de estos y le removieron la lesión. Ocho meses después, en la tomografía Cone Beam, se evidenció una lesión hipodensa de mayor tamaño que las iniciales en la hemiarcada inferior izquierda. Conclusión: la marsupialización, enucleación más adyuvancia, así como la correcta evaluación de las características clínicas, radiográficas e histopatológicas, permitieron que se llevara a cabo un tratamiento quirúrgico más conservador sin utilizar la solución de Carnoy. Aun así, siempre se recomienda un seguimiento postoperatorio a largo plazo. Por otra parte, no se descarta el uso de solución de Carnoy en casos aislados y según el criterio del operador


Introduction: Keratocyst is a developmental odontogenic cyst originating from dental lamina residues. Its biological behavior is similar to that of a benign neoplasm. Furthermore, it is aggressive and has a high recurrence rate, causing massive bone destruction. It can appear in any area of the jaw and is associated with impacted teeth. On imaging, it appears as a multilocular or polycystic radiolucent lesion with a thin cortical border. Objective: To show the importance of a precise diagnosis, an early and accurate detection, and to determine the use and appropriate time of the application of Carnoy's solution as first-line treatment. Case presentation: A 34-year-old man, with no relevant personal history, who 3 years ago presented with a radiolucent lesion located at the level of teeth 34 and 35. He under- went endodontic treatment and lesion removal. Approximately 8 months later, cone-beam computed tomography revealed a hypodense lesion larger than the initial lesion at the level of the lower left hemi- arch. Conclusion: Marsupialization; enucleation along with adjuvant therapy; and correct evaluation of clinical, radiographic, and histopathological characteristics enabled a conservative surgical treatment to be performed without using Carnoy's solution; nevertheless, postoperative follow-up is recommended. However, in the long term, the use of Carnoy's solution is not eliminated in isolated cases and will be based on the operator's criteria.


Introdução: o queratocisto faz parte dos cistos odontogênicos de desenvolvimento, derivados dos resíduos da lâmina dentária com comportamento biológico semelhante ao de uma neoplasia benigna. Apresenta alto grau de recidiva e agressividade, causando destruição óssea maciça. Pode aparecer em qualquer área dos maxilares, e estar relacionado a dentes retidos. Por meio de análise de imagem, apresenta uma lesão radiolúcida multilocular ou policística que mostra uma borda cortical fina. Objetivo: demonstrar a importância de um diagnóstico correto, uma detecção precoce e correta, bem como inferir a necessidade de usar a solução de Carnoy como primeira linha de tratamento e determinar o momento oportuno para fazê-lo. Apresentação do caso: paciente do sexo masculino, 34 anos, sem antecedentes pessoais relevantes, apresentou há 3 anos uma lesão radiolúcida localizada ao nível dos órgãos dentários 34 e 35, pelo que foi realizada endodontia nos mesmos e retirada da lesão. Depois de 8 meses, a tomografia Cone Beam revelou uma lesão hipodensa maior que as iniciais ao nível da hemiarcada inferior esquerda. Conclusão: a marsupialização, enucleação mais adjuvante, bem como a correta avaliação das características clínicas, radiográficas e histopatológicas permitiram a realização de um tratamento cirúrgico mais conservador sem o uso da solução de Carnoy, mesmo assim, um acompanhamento pós-operatório é sempre recomendado. Já a longo prazo, não está descartada a utilização da solução de Carnoy em casos isolados e a critério do cirurgião

Background: Epidermoid cysts (ECs) are uncommon benign cystic lesions derived from the germinative epithelium. Head and neck ECs constitute only 7% of all ECs whereas only 1.6% are seen intraorally. The floor of the mouth is the commonest intraoral site whereas tongue, lips, buccal mucosa, and jaws are less commonly involved intraoral sites. To date, very few large case series of ECs of head and neck have been published. To the best of our knowledge, this is the third-largest case series of 11 intraoral ECs along with 2 extra-oral cases in the pre-auricular region. Aims: To highlight the typical and atypical features of ECs in the common as well as rare sites and draw attention to its consideration as a differential diagnosis for head and neck masses. Settings and Design: Archival data of 13 histopathological cases identified as ECs were analyzed from the Department of Oral Pathology at a tertiary dental hospital and college in New Delhi from 2007 to 2020. Materials and Methods: The demographic, clinical, radiographic, histopathological features, and treatment modalities were recorded and analyzed. Statistical Analysis Used: Appropriate statistical tests were used. Results: The study found strong male predilection in the ratio of 10:3 with an average age of presentation as 28 years. The pre-auricular region and floor of the mouth were the common sites involved followed by buccal mucosa, lips, and jaws. All patients presented with slowly growing swelling with dysphagia, dyspnea, and dysphonia seen in larger cysts on the floor of the mouth. Microscopically, all cases were lined with stratified squamous epithelium filled with laminated layers of keratin. Two cases showed the presence of melanin. One case showed recurrence even after complete surgical excision. Conclusion: ECs, though a rare entity, should be considered in differential diagnosis for head and neck masses and require close follow-up due to their potential for malignant transformation.

As lesões odontogênicas epiteliais benignas constituem um grupo heterogêneo de lesões. A proteína CLIC4 atua na regulação dos processos de parada de crescimento e apoptose, participando também do processo de transdiferenciação dos fibroblastos em miofibroblastos que passam a expressar α-SMA. Além disso, a expressão de CLIC4 pode interferir no processo de transição epitélio-mesenquima (TEM) em neoplasias. Este trabalho avaliou a imunoexpressão de CLIC4, α-SMA, E-caderina e Vimentina em ameloblastomas (AM) (n = 16), ceratocistos odontogênicos (n = 20) e tumores odontogênicos adenomatóides (TOA) (n = 8). A análise da expressão imunoistoquímica das proteínas CLIC4, E-caderina e vimentina no componente epitelial das lesões e de CLIC4 e α-SMA no tecido conjuntivo foi realizada de forma semi-quantitativa por um avaliador previamente calibrado. A expressão no componente epitelial de CLIC4 foi analisada separadamente no núcleo e no citoplasma, bem como a marcação de E-caderina que foi avaliada na membrana e no citoplasma. As comparações dos percentuais de imunorreatividade em relação aos grupos estudados foram realizadas por meio dos testes não paramétricos de Kruskal-Wallis e Mann-Whitney. Possíveis correlações entre a expressão de CLIC4, α-SMA, E-caderina e Vimentina foram avaliadas por meio do teste de correlação de Spearman. O nível de significância foi estabelecido em 5% (p < 0,05). Foram observados diferentes padrões de marcação entre os grupos analisados, observando-se que a imunoexpressão exclusivamente citoplasmática da CLIC4 no componente epitelial dos AM (p < 0,001) e TOA (p < 0,001) foi significativamente superior a dos CO, não demonstrarando significância estatística entre os AM e TOA. A imunoexpressão (nuclear e citoplasmática) da CLIC4 no revestimento epitelial CO foi significativamente superior à encontrada no componente epitelial dos AM (p < 0,001) e dos TOA (p < 0,001). A imunoexpressão estromal de CLIC4 foi significativamente superior nos AM (p = 0,009) e CO (p = 0,004) quando comparados aos TOA. A imunoexpressao de α-SMA significativamente maior em AM (p = 0,016) e CO (p = 0,034) quando comparados aos TOA. Para a imunoexpressão membranar da E-caderina em CO foi significativamente superior em comparação à encontrada nos AM (p = 0,009) e nos TOA (p = 0,024). Foi observada maior imunoexpressão de E-caderina (membranar e citoplasmática) nos COs, quando comparados aos AM (p < 0,001) e aos TOAs (p < 0,001). A expressão de Ecaderina citoplasmática foi significativamente maior nos AM e TOA (p < 0,001) quando comparados aos CO. Observou-se diferença estatisticamente significativa na imunoexpressão de vimentina entre os casos de AM e os casos de TOA (p = 0,038) e CO (p < 0,001), bem como entre o TOA e CO (p < 0,001). As correlações testadas entre os escores das proteínas estudadas evidenciou que no grupo dos AM foi possível evidenciar moderada correlação positiva e estatisticamente significativa (r = 0,527; p = 0,036) entre a expressão citoplasmática da CLIC4 e a expressão citoplasmática da E-caderina. Também foi verificada fraca correlação negativa e estatisticamente significativa (r = -0,499; p = 0,049) entre a expressão núcleo-citoplasmática da CLIC4 e a expressão citoplasmática da E-caderina nos AM. Além disso, uma moderada correlação positiva e estatisticamente significativa entre a expressão estromal da CLIC4 e a expressão da α-SMA nos AM (r = 0,648; p = 0,007) e nos CO (r = 0,541; p = 0,014). Foi observada forte correlação negativa e estatisticamente significativa (r = -0,813; p < 0,001) entre a expressão da E-caderina e a expressão da vimentina nos AM. Os resultados deste estudo sugerem um potencial envolvimento de CLIC4 no processo de transdiferenciação de miofibroblastos, e que a presença destas células é mais frequentemente associada a lesões de comportamento biológico mais agressivo como os AM e CO, além de uma possível atuação desta proteína na regulação do ciclo celular e na TEM nas lesões estudadas (AU).


Benign epithelial odontogenic lesions constitute a heterogeneous group of lesions. the CLIC4 protein acts in the regulation of growth arrest and apoptosis processes, also participating in the process of transdifferentiation of fibroblasts Into myofibroblasts that begin to express α-SMA. Furthermore, CLIC4 expression can interfere with the epithelialmesenchymal transition (EMT) process in neoplasms. This work evaluated the immunoexpression of CLIC4, α-SMA, e-cadherin and vimentin in ameloblastomas (AM) (n = 16), odontogenic keratocysts (OK) (n = 20) and adenomatoid odontogenic tumors (AOT) (n = 8). The analysis of the immunohistochemical expression of the proteins CLIC4, ecadherin and vimentin in the epithelial component of the lesions and of CLIC4 and α-SMA in the connective tissue was carried out in a semi-quantitative way by a previously calibrated evaluator. Expression in the epithelial component of CLIC4 was analyzed separately in the nucleus and cytoplasm, as well as e-cadherin labeling, which was evaluated in the membrane and cytoplasm. Comparisons of the percentages of immunoreactivity in relation to the studied groups were carried out using the nonparametric kruskal-wallis and mann-whitney tests. Possible correlations between the expression of CLIC4, α-SMA, e-cadherin and vimentin were evaluated using the spearman correlation test. The significance level was set at 5% (p < 0.05). Different staining patterns were observed between the groups analyzed, observing that the exclusively cytoplasmic immunoexpression of CLIC4 in the epithelial component of AM (p < 0.001) and AOT (p < 0.001) was significantly higher than that of OK, not demonstrating statistical significance between the AM and AOT. The immunoexpression (nuclear and cytoplasmic) of CLIC4 in the co epithelial lining was significantly higher than that found in the epithelial component of AM (p < 0.001) and AOT (p < 0.001). Stromal CLIC4 immunoexpression was significantly higher in AM (p = 0.009) and OK (p = 0.004) when compared to AOT. The immunoexpression of α-SMA is significantly higher in AM (p = 0.016) and OK (p = 0.034) when compared to AOT. For e-cadherin membrane immunoexpression in co was significantly higher compared to that found in AM (p = 0.009) and AOT (p = 0.024). Greater immunoexpression of e-cadherin (membrane and cytoplasmic) was observed in OK, when compared to AM (p < 0.001) and AOT (p < 0.001). Cytoplasmic ecadherin expression was significantly higher in AM and AOT (p < 0.001) when compared to OK. A statistically significant difference in vimentin immunoexpression was observed between cases of AM and cases of AOT (p = 0.038) and OK (p < 0.001), as well as between AOT and OK (p < 0.001). The correlations tested between the scores of the proteins studied showed that in the am group it was possible to demonstrate a moderate positive and statistically significant correlation (r = 0.527; p = 0.036) between the cytoplasmic expression of clic4 and the cytoplasmic expression of e-cadherin. A weak and statistically significant negative correlation (r = -0.499; p = 0.049) was also found between the nucleus-cytoplasmic expression of clic4 and the cytoplasmic expression of e- cadherin in AM. Furthermore, a moderate positive and statistically significant correlation between the stromal expression of CLIC4 and the expression of α-SMA in AM (r = 0.648; p = 0.007) and OK (r = 0.541; p = 0.014). Additionally, a strong negative and statistically significant correlation (r = -0.813; p < 0.001) was observed between the expression of ecadherin and the expression of vimentin in AM. The results of this study suggest a potential involvement of CLIC4 in the myofibroblast transdifferentiation process, and that the presence of these cells is more frequently associated with lesions with more aggressive biological behavior such as AM and OK, in addition to a possible role of this protein in the regulation of cell cycle and EMT in the lesions studied (AU).

O objetivo deste artigo é relatar um caso de COG em sínfise mandibular, tratado com enucleação e osteotomia periférica. Relato de Caso: Paciente do sexo feminino, 58 anos de idade, foi encaminhada para avaliação de uma lesão mandibular, apresentando-se assintomática no momento da consulta. Foi observado um leve aumento de volume vestibular na região da sínfise mandibular, sem sensibilidade ao toque. Na tomografia foi observada imagem hipodensa, multiloculada, bem delimitada, na região de sínfise, próxima as raízes dentárias e a basilar mandibular, além de um dente incluso intralesional. Foi realizada biópsia incisional e exame histopatológico, através do qual foi estabelecido o diagnóstico de COG. Como forma de tratamento, o paciente foi submetido a enucleação com curetagem de toda lesão e a osteotomia periférica, além da remoção do dente incluso. Após um ano de acompanhamento, a paciente encontra-se livre de recorrências. Conclusão: Por fim, este caso destaca a importância de um tratamento eficaz de COG com a osteotomia periférica, considerando o tamanho da lesão, suas características e sua localização, a fim de reduzir suas chances de recidiva... The aim of this article is to report a case of COG in mandibular symphysis, treated with enucleation and peripheral osteotomy. Case Report: A 58-yearold female patient was referred for evaluation of a mandibular lesion, instrument was asymptomatic at the time of consultation. An increase in vestibular volume was observed in the region of the mandibular symphysis, without sensitivity to touch. The tomography showed a hypodense, multiloculated, well-delimited image in the symphysis region, close to tooth roots and a mandibular basilar, in addition to an intralesional impacted tooth. An incisional biopsy and histopathological examination were performed, through which the diagnosis of COG was established. As a form of treatment, the patient underwent enucleation with curettage of the entire lesion and peripheral osteotomy, in addition to removal of the impacted tooth. After a year of follow-up, the patient is free from recurrences. Conclusion: Finally, this case highlights the importance of an effective treatment of COG with peripheral osteotomy, considering the size of the lesion, its characteristics and its location, in order to reduce its chances of recurrence... El objetivo de este artículo es reportar un caso de COG en la sínfisis mandibular, tratado con enucleación y osteotomía periférica. Caso Clínico: Paciente femenino, de 58 años de edad, fue remitida para valoración de lesión mandibular, presentándose asintomática al momento de la consulta. Se observa ligero aumento de volumen vestibular en la región de la sínfisis mandibular, sin sensibilidad al tacto. En la tomografía se observó una imagen hipodensa, multiloculada, bien delimitada en la región de la sínfisis, próxima a las raíces dentarias y la base de la mandíbula, además de un diente intralesional. Se realizó biopsia incisional y examen histopatológico, a través del cual se estableció el diagnóstico de GOC. Como forma de tratamiento, el paciente fue sometido a enucleación con curetaje de toda la lesión y osteotomía periférica, además de extracción del diente impactado. Después de un año de seguimiento, un paciente está libre de recurrencias. Conclusión: Finalmente, este caso destaca la importancia de un tratamiento efectivo de la GOC con osteotomía periférica, considerando el tamaño de la lesión, sus características y su ubicación, para reducir sus posibilidades de recurrencia... La enfermedad poliquística renal autosómica dominante es considerada la enfermedad renal genética más frecuente y es la cuarta causa de enfermedad renal crónica a nivel mundial. Afecta a cerca de 1 por cada 1000 nacidos vivos. La enfermedad poliquística hepática es la manifestación extrarrenal más frecuente de la enfermedad poliquística renal. La cirrosis hepática constituye un hallazgo tardío en esta enfermedad y su presentación es más común en el anciano, y la descompensación de la enfermedad con insuficiencia hepática como causa de muerte. Presentamos el caso de un paciente masculino de 60 años con antecedentes patológicos personales de la enfermedad poliquística renal con ascitis a tensión, y edemas en ambos miembros inferiores con datos de insuficiencia hepática y evolución desfavorable hasta su fallecimiento. Aunque la presencia de la falla hepática se asocia a un pronóstico precario, la detección precoz puede suponer el inicio de un tratamiento oportuno y apropiado que puede ser beneficioso.


Autosomal dominant polycystic kidney disease is considered the most common genetic kidney disease and the fourth leading cause of chronic kidney disease worldwide. It affects about 1 in 1,000 live births. Polycystic liver disease is the most common extrarenal manifestation of polycystic kidney disease. Liver cirrhosis is a late finding in this disease and its presentation is more common in the elderly. Decompensation of the disease, with liver failure as the cause of death, is rare in polycystic liver disease. We report the case of a 60-year-old male patient with a personal pathological history of polycystic kidney disease with tense ascites and edema in both lower limbs, with evidence of liver failure and unfavorable evolution until his death. Although the presence of liver failure is associated with poor prognosis, its early detection may mean the initiation of timely and appropriate treatment that may be beneficial.

Introduction: This case report analyzes the regeneration potential of advanced-platelet rich fibrin (A-PRF) in large bony lesions. Advanced-platelet rich fibrin provides various growth factors which aids in faster healing. Materials and Methods: Patient presented with peri-apical radiolucency. CBCT showed bony radiolucency in teeth 31, 32, 41, 42. A full mouth mucoperiosteal flap was raised and a cyst lining was enucleated. Apicoectomy was done with respect to 31, 32, 41, 42 along with and retrograde with a preparation of APRF clots were placed in the bony cavity. Results: Post-op CBCT at 6 months showed a significant difference in size and bone density of the lesion. Conclusions: Advanced-platelet rich fibrin has shown promising results in reducing the size of bony defect and periapical lesion in this case.


Introducción: Este caso clínico demuestra el potencial de regeneración de la fibrina rica en plaquetas avanzada (A-PRF) en lesiones óseas de gran tamaño. La fibrina rica en plaquetas avanzada proporciona varios factores de crecimiento que ayudan a una curación más rápida. Materiales y Métodos: El paciente se presentó con radiolucencia periapical. Tomografía computarizada de haz cónico mostró radiolucencia ósea en los dientes 31, 32, 41, 42. Se levantó un colgajo mucoperióstico de boca completa y se enucleó el revestimiento del quiste. Se realizó apicectomía con respecto a 31, 32, 41, 42, y se colocó una preparación de APRF se colocaron coágulos en la cavidad ósea. Resultados: La tomografía computarizada de haz cónico postoperatorio a los 6 meses mostró una diferencia significativa en el tamaño y la densidad ósea de la lesión. Conclusión: La fibrina rica en plaquetas avanzada ha mostrado resultados prometedores en la reducción del tamaño del defecto óseo y la lesión periapical en este caso.

Resumen Objetivo: Describir el manejo de un caso de quiste odontogénico glandular, con tratamiento conservador, terapias adyuvantes y rehabilitación implantoasistida a los 7 años de seguimiento. Caso clínico: Se presenta un caso clínico de un paciente masculino con una lesión mandibular con diagnóstico de quiste odontogénico glandular. El tratamiento que se realizó fue una enucleación con tratamiento de superficie con solución de Carnoy y ostectomía periférica. Se realizó seguimiento clínico y radiográfico durante 7 años verificando ausencia de recidiva, momento en cual se realizó mediante planificación virtual la colocación de implantes con guías quirúrgicas de precisión para rehabilitación implanto asistida.


Abstract Aim: To describe the management of a case with a glandular odontogenic cyst, treated with a conservative treatment, adyuvant therapy and guided implantologic rehabilitation at 7 years of follow up. Clinical case: A case report of a male patient with a mandibular lesion with a diagnose of glandular odontogenic cyst is presented. The treatment that was performed was an enucleation with surface treatment with Carnoy's solution and peripheral ostectomy. Clinical and radiographic follow up was carried out for 7 years, verifying the absence of recurrence. At that time, it was done placement of implants with virtual planning and precision surgical guides for implant-assisted rehabilitation.

Background: Mesenteric cysts are rare, benign, fluid-filled tumors that occur in the mesentery, a thin layer of tissue that supports the intestines. The management of mesenteric cysts in children typically involves surgical intervention. The aim of this study was to evaluate the prevalence, the clinical presentation and management of mesenteric cysts in children. Material & Methods: This retrospective study was conducted in the Division of Pediatric Surgery, Bangladesh Shishu Hospital & Institute, Dhaka from July 2016 to June 2022. A total of 18,326 patients admitted in the Division of Pediatric Surgery, hospital records of 55 patients diagnosed with mesenteric cysts were evaluated and included in this study. Results: The prevalence of mesenteric cyst in our study was almost 1 per 333 pediatric surgical admissions. In our study, the mean (± SD) age of the patients was 36.5±13.5 months ranged between 2 days to 12 years. Majority of the patients were male (58.2%). Of the total of 55 patients, 18 patients required emergency surgery and 37 underwent elective surgery. Abdominal pain was the main presenting symptom, present in 52.7% of the patients. Fifteen of the emergency surgery were done on preoperative diagnosis of appendicitis or perforated appendicitis with peritonitis. Majority of patients (76.4%) had multiple numbers of cysts. The cysts were located in the small bowel mesentery in 23 cases, the base of the mesentery having retroperitoneal extension in 18 cases, the transverse mesocolon in 14 cases. Complete cyst excision was done in majority of the patients (65.5%) and complete excision with intestinal resection was required in 19 (34.5%) patients. With a mean follow-up of 9 months, we encountered no recurrences. Some patients are still under follow-up. Conclusion: Mesenteric cysts are usually symptomatic and CT scan is the investigation of choice. Complete surgical excision is the optimal treatment. Prognosis is excellent after complete surgical excision; long-term follow-up is needed due to the possibility of recurrence, even in adulthood. Though mesenteric cysts are a rarity in children in the literature, but it is not so uncommon in our experience.

Introdução: Os cistos e tumores odontogênicos são lesões que apresentam comportamento biológico heterogêneo e patogênese ainda não totalmente esclarecida. A Yes-associated protein (YAP) atua como um regulador transcricional de genes envolvidos na proliferação celular e na apoptose, participando da ativação de vias associadas ao crescimento cístico e à progressão neoplásica. Objetivo: Analisar a expressão imuno-histoquímica da proteína YAP e correlacioná-la com marcadores envolvidos na proliferação celular e na apoptose em lesões odontogênicas epiteliais benignas. Metodologia: A amostra consistiu de 95 casos de lesões odontogênicas - 25 cistos dentígeros (CDs), 30 CO não sindrômicos (COs), 30 AMB convencionais (AMB-Cs) e 10 AMB unicísticos (AMB-Us) -, além de 10 espécimes de folículo dentários (FD). Foi realizada coleta dos dados clinico-demográficos dos casos, bem como análise morfológica para melhor caracterização da amostra. Os cortes histológicos foram submetidos à técnica imuno-histoquímica através da utilização dos anticorpos YAP, ciclina D1, Ki-67 e Bcl-2, e a análise da expressão destes foi realizada quali-quantitativamente, mediante metodologia adaptada. Os dados coletados seguiram para análise descritiva e estatística (p ≤ 0,05). Resultados: Houve discreta predileção por mulheres (n = 55; 57,6%) e por indivíduos na faixa etária dos 21 aos 40 anos (n = 50; 47,6%), sendo a região posterior de mandíbula mais afetada (64%). A análise da imunoexpressão de YAP revelou maiores níveis de expressão em COs, especialmente nas camadas basal e parabasal, seguido dos AMB-Us e AMB-Cs, que demonstraram moderada imunorreatividade, predominantemente nas células periféricas. Além disso, houve diferenças significativas quanto à imunoexpressão de YAP entre os grupos analisados, com existência de correlações positivas e estatisticamente significativas entre YAP e ciclina D1 em CDs e AMB-Us, e entre YAP e Ki-67 em AMB-Us (p < 0,05). Todavia, entre a imunoexpressão YAP e Bcl-2, foi verificada ausência de correlação estatisticamente significativa. Conclusões: A YAP pode exercer influência sobre a proliferação celular do epitélio de cistos e tumores odontogênicos, auxiliando, assim, na progressão das diferentes lesões odontogênicas (AU).


Background: Odontogenic cysts and tumors present heterogeneous biological behavior, and their etiopathogenesis is not fully understood yet. Yes-associated protein (YAP) acts as a transcriptional regulator of genes involved in cell proliferation and apoptosis, activating pathways associated with cystic growth and neoplastic progression. Objective: To analyze the immunohistochemical expression of YAP protein and correlate it with markers involved in cell proliferation and apoptosis in benign epithelial odontogenic lesions. Methods: The sample consisted of 95 cases of odontogenic lesions - 25 dentigerous cysts (DCs), 30 non-syndromic odontogenic keratocyst (OKCs), 30 conventional AMB (C-AMBs), and 10 unicystic AMB (UAMBs) -, in addition to 10 specimens of dental follicles (DF). Clinicodemographic data collection was carried out, as well as morphological analysis for better characterization of the sample. The histological sections were submitted to the immunohistochemical technique using YAP, cyclin D1, Ki-67, and Bcl-2 antibodies, and their immunoexpression analysis was performed qualitatively and quantitatively, through an adapted methodology. The collected data were submitted for descriptive and statistical analysis (p ≤ 0.05). Results: There was a slight predilection for women (n = 55; 57.6%) and individuals aged between 21 and 40 years (n = 50; 47.6%), with the posterior region of the mandible as the most affected site (64%). Analysis of YAP immunoexpression revealed higher expression levels in OKCs, especially in the basal and parabasal layers, followed by U-AMBs and C-AMBs, which showed moderate immunoreactivity, predominantly in peripheral cells. In addition, there were significant differences in YAP immunoexpression between the analyzed groups, with positive and statistically significant correlations between YAP and cyclin D1 in DCs and U-AMBs, and between YAP and Ki-67 in U-AMBs (p < 0.05). However, between YAP and Bcl-2 immunoexpression, there was no statistically significant correlation. Conclusions: YAP may influence on the cell proliferation of odontogenic cysts and tumors epithelium, thus helping with the progression of the different odontogenic lesions (AU) .

Introducción: La cavidad ósea de Stafne es una variante anatómica poco frecuente, radiolúcida y bien delimitada, que usualmente se presenta en la región molar cerca del ángulo mandibular y por debajo del canal para el nervio dentario inferior. Es frecuente que sea erróneamente diagnosticada con otras entidades de carácter patológico. Objetivo: Determinar la frecuencia de la cavidad ósea de Stafne en las radiografías panorámicas del Servicio de Radiología Oral y Maxilofacial del Centro Dental Docente "Cayetano Heredia", desde 2015 hasta 2019. Métodos: Se realizó un estudio observacional, descriptivo, transversal y retrospectivo de una muestra de 17875 radiografías panorámicas. Se consideraron las variables demográficas como el sexo, la edad, la localización y la forma, posteriormente se realizaron tablas de contenido para el análisis de los datos. Resultados: Entre los 17875 pacientes, solo 24 (0,13 por ciento) presentaban cavidad ósea de Stafne, incluidos 16 hombres y 8 mujeres. La octava década de vida presentó la mayor cantidad de casos con 6 (0,4 por ciento). La localización posterior derecha contó con 13 (54,17 por ciento), la posterior izquierda con 7 (29,17 por ciento) y la anterior con 4 (16,67 por ciento). La forma ovalada con 23 (95,83 por ciento) y la redonda solo con 1 (4,17 por ciento). Conclusiones: La frecuencia de la cavidad ósea de Stafne fue de 0,13 por ciento con predilección del sexo masculino, la octava década de vida, la localización posterior derecha y la forma ovalada(AU)


Introduction: Stafne's bone cavity is a rare, radiolucent, well-demarcated anatomic variant that usually occurs in the molar region near the mandibular angle and below the canal for the inferior dental nerve. It is frequently misdiagnosed with other pathological entities. Objective: To determine the frequency of Stafne's bone cavity in panoramic radiographs of the Oral and Maxillofacial Radiology Service of the Teaching Dental Care Center "Cayetano Heredia", from 2015 to 2019. Methods: An observational, descriptive, cross-sectional and retrospective study was performed on a sample of 17875 panoramic radiographs. Demographic variables such as gender, age, location and shape were considered; subsequently tables of contents were performed for data analysis. Results: Among the 17875 patients, only 24 (0.13 percent) had Stafne's bone cavity, including 16 males and 8 females. The eighth decade of life presented the highest number of cases with 6 (0.4 percent). The right posterior location accounted for 13 (54.17 percent), the left posterior with 7 (29.17 percent) and the anterior with 4 (16.67 percent). The oval shape with 23 (95.83 percent) and round with only 1 (4.17 percent). Conclusions: The frequency of Stafne's bone cavity was 0.13 percent with male sex predilection, eighth decade of life, right posterior location and oval shape(AU)

ABSTRACT Primary cardiac hydatid cyst is a rare and fatal pathology, especially when involving the left ventricular free wall. A 44-year-old male was diagnosed with large intramural left ventricular hydatid cyst with wall thickness of 6 mm at the thinnest point. Cyst was accessed through pleuropericardial approach (left pleura opened, followed by entry into cyst directly through adjacent pericardium without removing the pericardial adhesions) which resulted in easy entry into the cyst, mitigating the risk of mechanical injury. This case report highlights that with detailed evaluation, cardiac hydatidosis can be addressed with off-pump technique, reducing the anaphylaxis risks and cardiopulmonary bypass-related effects.

Background:Immunohistochemistry have had a huge impact on oral and maxillofacial pathology diagnosis. As a method it determines distribution and amount of certain cellular molecules via specific antigen-antibody reaction. Whereas in most cases a definitive diagnosis is achieved based on detailed hematoxylin and eosin cytomorphological analysis, along with clinical and radiological features, some challenging and equivocal neoplasms need to be further assessed with immunohistochemistry. Objective:This article reviews and updates immunohistochemistry technique fundamentals, its role and relevance in the diagnosis of common oral and maxillofacial lesions encountered in daily practice. Materials and methods: A literature review on the topic was carried out by searching pertinent and available papers on PubMed, ClinicalKey and Scielo platforms with no date restriction, up to 2022. Conclusion: Immunohistochemistry is an important tool that has been integrated into conventional histopathology and provides diagnostic assistance in the interpretation of common but equivocal neoplasms


Antecedentes: El uso de la inmunohistoquímica ha tenido un gran impacto en el diagnóstico de patología oral y maxilofacial. Como técnica, determina la distribución y la cantidad de ciertas moléculas celulares a través de una reacción antígeno-anticuerpo específica. Aunque en la mayoría de los casos se logra obtener un diagnóstico definitivo basado en el análisis cito morfológico con hematoxilina y eosina, junto con las características clínicas y radiológicas, algunas neoplasias microscópicamente equívocas deben evaluarsemás a fondo con inmunohistoquímica. Objetivo: Este artículo revisa los fundamentos básicos actuales de la técnica y su relevancia en el diagnóstico de algunas lesiones orales y maxilofaciales frecuentemente tratadas en la práctica clínica diaria. Materiales y Métodos: Se realizó una búsqueda y revisión de artículos científicos relacionados con el uso immunohistoquímica en patología oral y maxilofacial en PubMed, ClinicalKey y Scielo. Conclusión: La immunohistoquimica es una herramienta importante que ha sido integrada a la histopatología convencional y brinda asistencia diagnostica en la interpretación de neoplasias comunes pero equívocas

RESUMO

The Hippo signaling pathway is highly conserved in evolution, and participates in the regulation of cell proliferation, differentiation, and tissular dynamic balance, and plays an important role in regulating tissue, organ size, and cell number. Autosomal dominant polycystic kidney disease (ADPKD) is the most common hereditary kidney disease and one of the most common causes of end-stage renal disease. Emerging studies have identified the Hippo signaling pathway is closely related to the occurrence and development of ADPKD. The abnormal activity and expression of the main members of the pathway affect the cilia and cell polarity of renal tubular epithelial cells and induce the formation of renal cysts. The review summarizes the potential mechanism of the Hippo pathway in the pathogenesis of ADPKD, the crosstalk with other signaling pathways, and the variances in different species, and discusses the strategies for the treatment of ADPKD based on the Hippo signaling pathway to provide new strategies for the treatment.

Objective:To investigate the effect of laparoscopic ovarian cystectomy on anti-Mullerian hormone (AMH) level, ovarian response to gonadotropin stimulation and pregnancy rate for in vitro fertilization (IVF) patients with benign ovarian cysts.Methods:Patients with benign ovarian cysts who were admitted for cystectomy and had undergone IVF treatment were enrolled in the study. There were 373 participants with ovarian cysts underwent laparoscopic ovarian cystectomy in the experimental group. According to duration of post-surgery, there were four sub-groups: 1 year post-surgery (1Y POST), 2 years post-surgery (2Y POST), 4 years post-surgery (4Y POST) and ≥5 years post-surgery (≥5Y POST) in the experimental group. According to histopathologic types of ovarian cysts, there were two sub-groups: ovarian endometriotic cysts and ovarian non-endometriotic cysts. Two hundreds and three patients with no history of ovarian cysts and ovarian surgery were in the control group. The level of AMH and basic concentrations of follicle stimulating hormone (FSH), luteinizing hormone (LH), progesterone (P), estradiol (E 2) were measured. Antral follicle counts (AFC) were calculated. There were other study variables: total dose of gonadotropins, duration of ovarian stimulation, the number of oocyte retrieved, the number of embryo obtained, blastocyst transfer rate and pregnancy rate. Results:The control group was matched as closely as possible to the experimental group, including age, body mass index and menstrual cycle (all P>0.05). Compared to the women in control group, the women in ovarian endometriotic cystectomy sub-group had significantly higher levels of basal FSH and basal P, lower level of AMH (all P<0.05); the women in ovarian endometriotic cysts sub-group had significantly higher dose of gonadotropins (all P<0.05); the women in ovarian endometriotic cysts ≥5Y POST sub-group had significantly lower number of oocyte retrieved, lower number of embryo obtained, lower blastocyst transfer rate, and lower pregnancy rate (all P<0.05). Compared to the women in control group, the women in ovarian non-endometriotic cysts sub-group had a significantly higher level of basal FSH and basal P (all P<0.05). The women in ovarian non-endometriotic cysts sub-group had lower level of AMH, higher dose of gonadotropins, lower number of oocyte retrieved, lower number of embryo obtained, lower rate of blastocyst transfer and lower rate of pregnancy than the control group but there were no statistically significant differences among them (all P>0.05). The women with unilateral ovarian endometriotic cysts had significantly lower number of oocyte retrieved on the side of surgery than another side ( P<0.05). Conclusions:In short term laparoscopic ovarian cystectomy has no significant effect on ovarian reserve. But with long-term follow-up ovarian reserve, ovarian response to gonadotropin stimulation and pregnancy rate are decreased. The effect of laparoscopic ovarian cystectomy in benign cysts on ovarian is associated with whether or not it is the surgical side.

Objective:To evaluate the effectiveness and safety of ultrasound-guided percutaneous polidocanol sclerotherapy of the thyroglossal duct cysts in children.Methods:A retrospective analysis of 26 children who were treated with ultrasound-guided percutaneous puncture sclerosis for thyroglossal duct cysts in Beijing Children′s Hospital, Capital Medical University from October 2019 to April 2022, the size of the cyst before treatment was recorded, and the cyst volume in accordance with V(ml) =1/6 πabc cyst volume was calculated. The follow-up time was the 1st, 3rd, 6th, and 12th months after sclerotherapy. And the cyst volume and volume reduction ratio at different time points was calculated. According to changes in the cystic volume, the treatment was considered effective if the cyst volume reduction rate was≥50%. Cosmetic grading scores (CGS) were performed pre-treatment and at the last follow-up after sclerotherapy using the WHO grading system. Complications were recorded during the follow-up period.Results:The cyst volume before treatment was 2.67 (3.78)ml, and the cyst volume at the 1st, 3rd, 6th and 12th month after treatment were 0.66(1.83), 0.45(0.87), 0.40(0.70), 0.38 (0.63)ml, respectively, there were significant differences between pre-treatment and each time point after-treatment (all P<0.001); the volume reduction rate at the last follow-up was (81.48±14.57)%. The reduction rate of cyst volume was more than 50% at the last follow-up in 22 children, the treatment efficiency was 84.62% (22/26). The cosmetic grading scores at the last follow-up after sclerotherapy was 1.74(1.50), and it decreased significantly compared with the pre-treatment score 3.85(0)( P<0.001). There was no skin pigmentation, pain in the injection area, local urticaria and blistering after sclerotherapy, no adverse reactions such as cyst bleeding and drunkenness-like reaction, and no serious complications occurred. There were 2 cases of cysts that presented infection with fever, and no serious complications occurred. Conclusions:Ultrasound-guided percutaneous sclerotherapy is a safe and effective minimally invasive treatment for thyroglossal duct cysts in children.

【Objective】 To investigate the efficacy of flexible ureteroscopic holmium laser lithotripsy combined with cyst wall incision and drainage in the treatment of renal calculi with ipsilateral renal cyst. 【Methods】 A total of 70 patients with renal calculi complicated with ipsilateral renal cyst (cyst diameter >40 mm, maximum diameter of stone 0.05). 【Conclusion】 Flexible ureteroscopic holmium laser lithotripsy combined with cyst incision and drainage has the advantages of short operation time, small trauma, few adverse reactions, good stone clearance effect and satisfactory efficacy. It can be used as a preferred surgical method for renal calculi complicated with ipsilateral renal cyst.

RESUMO

The advent of enhanced radiological imaging techniques has facilitated the diagnosis of cystic liver lesions. Concomitantly, the evidence base supporting the management of these diseases has matured over the last decades. As a result, comprehensive clinical guidance on the subject matter is warranted. This guideline covers the diagnosis and management of hepatic cysts, mucinous cystic neoplasms of the liver, polycystic liver disease, caroli disease, caroli syndrome, biliary hamartomas and peribiliary cysts. On the basis of in⁃depth review of the relevant literature, this guideline provides recommendations to navigate clinical dilemmas followed by supporting text. The recommendations are graded according to the Oxford Centre for Evidence⁃based Medicine system and categorized as "weak" or "strong". This guideline aims to provide the best available evidence to aid the clinical decision⁃making process in the diagnosis and treatment of patients with cystic liver diseases, and presents the readers with translations and summarizations of the above mentioned recommendations.
