RESUMO

Diabetic neuropathic pain (DNP) is one of the common complications of diabetic peripheral neuropathy, which is difficult to be cured. The clinical manifestations of DNP include reduced pain threshold, neuropathic pain in extremities, and abnormal pain. Studies have demonstrated that nuclear factor-kappa B (NF-κB), mitogen-activated protein kinase (MAPK), Toll-like receptor 4 (TLR4), nuclear factor erythroid 2-related factor 2 (Nrf2), and phosphatidylinositol 3-kinase (PI3K)/protein kinase B (Akt) signaling pathways are involved in the treatment of DNP by inhibiting the activation and release of inflammatory cytokines [e.g., tumor necrosis factor-alpha (TNF-α) and interleukin (IL)], regulating inflammation-mediating factors [e.g., reactive oxygen (ROS), inducible nitric oxide synthase (iNOS), cyclooxygenase (COX-2), and NF-κB inhibitor (IκB)], suppressing neurogenic inflammation, destroying the persistent damage of peripheral nerves, and repairing damaged peripheral nerves. The literature of modern medical research has demonstrated that the treatment of DNP with Chinese herbal medicines is associated with the signaling pathways above. They can treat DNP by regulating the expression of key proteins in the signaling pathways to control neurogenic inflammation and repair diabetic peripheral neuropathic damage. By summarizing the available studies, this review aims to provide a reference for the in-depth research on Chinese herbal medicines in the treatment of DNP and give new insights into the protection of diabetic peripheral nerves and the treatment of DNP with Chinese herbal medicines.

OBJECTIVE@#To observe the occurrence time of neuralgia and the expression of purinergic ligand-gated ion channel 7 receptor (P2X7R) in the dorsal horn of the spinal cord after intraperitoneal injection of streptozotocin (STZ) in diabetic rats, and to explore the effect of electroacupuncture (EA) and pretreatment of EA on the heat pain threshold and expression of P2X7R in the spinal dorsal horn in rats with diabetic neuropathic pain (DNP), and to explore the possible mechanism of EA for DNP.@*METHODS@#PartⅠ: Thirty male SD rats were randomly selected from 64 male SD rats as the control group; the remaining rats were given intraperitoneal injection of STZ (10 mg/mL) at a dose of 65 mg/kg to establish the diabetes model, and 30 rats were successfully modeled as the model group. The control group and the model group were divided into three subgroups respectively at 7, 14 and 21 days, with 10 rats in each subgroup. Body mass, fasting blood glucose (FBG) and thermal pain threshold were recorded at 7, 14 and 21 days after injection; the expression of P2X7R in spinal dorsal horn was detected by Western blot. PartⅡ: Eight SD rats were randomly selected from 35 male SD rats as the blank group, and the remaining 27 rats were given intraperitoneal injection of STZ (10 mg/mL) at a dose of 65 mg/kg to establish the diabetes model. The 24 rats with successful diabetes model were randomly divided into a DNP group, an EA group and a pre-EA group, 8 rats in each group. Fifteen to 21 days after STZ injection, the EA group received EA at "Zusanli" (ST 36) and "Kunlun" (BL 60), continuous wave, frequency of 2 Hz, 30 min each time, once a day; the intervention method in the pre-EA group was the same as that in the EA group. The intervention time was 8 to 14 days after STZ injection. The body mass, FBG and thermal pain threshold were recorded before STZ injection and 7, 14 and 21 days after STZ injection; the expression of P2X7R in spinal dorsal horn was detected by Western blot 21 days after injection.@*RESULTS@#PartⅠ: Compared with the control group, in the model group, the body mass was decreased and FBG was increased 7, 14 and 21 days after STZ injection (P<0.01), and the thermal pain threshold was decreased 14 and 21 days after STZ injection (P<0.05), and the expression of P2X7R in spinal dorsal horn was increased 7, 14 and 21 days after STZ injection (P<0.05, P<0.01). PartⅡ: Compared with the blank group, in the DNP group, the body mass was decreased and fasting blood glucose were increased 7, 14 and 21 days after STZ injection (P<0.01). Compared with the DNP group, in the pre-EA group, the heat pain threshold was increased 14 and 21 days after STZ injection (P<0.05), while in the EA group, the heat pain threshold was increased 21 days after STZ injection (P<0.01), and the expression of P2X7R in the dorsal horn in the EA group and the pre-EA group was decreased (P<0.01).@*CONCLUSION@#The diabetic neuropathic pain is observed 14 days after STZ injection. EA could not only treat but also prevent the occurrence of DNP, and its mechanism may be related to down-regulation of P2X7R expression in the dorsal horn of the spinal cord.

AIM To investigate the protective effects of polysaccharides from Dendrobium nobile Lindl.on cerebra ischemia/reperfusion injury in rats and its possible mechanism of action.METHODS One hundred and five SD rats were divided into sham group,model group,Nimodipine group (10 mg/kg) and treatment groups with low-,medium-,and high dose (50,100,and 200 mg/kg) of polysaccharides from D.nobile.The right middle cerebral artery of rats was occluded by inserting a.thread through internal carotid artery for 2 h,and was sampled after reperfusion for 24 h.The rats received gavage once a day for 7 d before operation.Neurological deficits score,brain index,brain water content and infarct size in rats were conducted at the end of reperfusion;the activity of superoxide dismutase (SOD),glutathione peroxidase (GSH-Px),myeloperoxidase (MPO) and the content of malondialdehyde (MDA) of brain tissue and blood serum were measured by chemical colorimetry;the infiltration of neutrophile granulocyte in rat ischemic cortex were detected with immune-fluorescence staining.RESULTS Sham group had no neurological deficit,but the model group showed severe neurological deficits,meanwhile,the infarct size,brain index and brain water content rose markedly,the content of MDA and the activity of MPO of the brain tissue and the blood serum increased remarkably,while the activity of SOD and GSH-Px apparently decreased;compared with the model group,neurological deficits of rats were improved significantly in polysaccharides from D.nobile dose groups,moreover,the infarct size,brain index and brain water content markedly declined,the MDA level and the activity of MPO significantly decreased,but the activity of SOD,GSH-Px increased remarkably;the infiltration of neutrophile granulocyte in the high dose group significantly decreased compared with the model group.CONCLUSION Polysaccharides from D.nobile has some neuroprotective effects on local cerebral ischemia/reperfusion injury,and that may be related to the remove of oxygen free radicals and the decrease in inflammation reaction.

Oxidative stress can be regarded as an imbalance between the amount of reactive oxygen species (ROS) and the ability of a biological system to eliminate the toxic species and repair the resulting damages. Since the germinating seeds and the resulted seedlings are rich in enzymes, whereas the treatment with chemicals affects much the seed germination, producing also ROS, we evaluate here the influence of 2,4-dinitrophenol (DNP) and potassium iodate (KIO3) on wheat germination (Triticum aestivum L.) and seedlings growth. Germination rate, the masses and heights of the 7 day old seedlings, as well as the activity of some enzymes involved in the oxidative stress such as peroxidase, catalase and superoxide dismutase were measured seven days after the chemical treatment. The treatment of the wheat seeds with 10-5 - 10-3 M solutions of DNP resulted in a relative concentration-dependent inhibition of the germination, with a concomitant stimulation of the weight and height of viable seedlings. The Gasparom variety treated with 10-5 M KIO3 showed a slight increase in the germination rate in comparison with the control batch. The two tested substances determined a significantly modified response of the oxidative stress enzymes, especially in the seeds treated with 10-4 and 10-3 M solutions.

Objective To study the pedicle of lumbosacral developmental patterns and aging characteristics by means of studying the ado-lescents aging from 12~18 years old,and provide the basis for image and three-dimensional analysis of the treatment of spinal diseases. Meth-ods Select the 30 normal spines of adolescents without any the problems of nervous system,who did thin spiral CT scan of lumbosacral verte-brae. The original data in the form of DICOM were put into three-dimensional software to do the relevant measurement and analyzed according to gender and age. Results There were no significant differences between the pedicle of lumbosacral E/F angle and DSP /DNP and these differ-ences were not influenced by age and gender(P>0. 05). The physical development of human increases gradually with age and has some signifi-cant differences. Conclusion A regular change process and operation in the region value in patients with lumbarsacrum is displayed. But it must be combined with the results of individual imagine and the technology of reverse engineering and the support of rapid prototype manufactur-ing. Only in this way can it meet the satisfaction of individualized treatment.

Objective To observe the effect of treating diabetic peripheral neuropathy(DPN) with methylcobalamin and Chinese medicines with the funetious of nourishing Yin and promoting blood circulation. Methods 72 patients with DPN were randomly recruited into a control group and a treatment group.Symptoms, NCV,value of blood routine examination,blood biochemistry check and ECG were compared before and after the treatment. Results The total effective rate was 88.0% and 68.2% in the treatment group and the control group respectively,showing significant difference (t=3.14, P<0.01).The value of blood routine examination, blood biochemistry check (except blood sugar level) and ECG showed no obvious changes before and after the treatment in both groups.Conclusion Therapy of methylcobalamin Tablets with Chinese medicines of nourishing Yin and promoting blood circulation is effective on DPN and worth of clinical promotion.

El presente artículo busca adelantar un análisis libre de la evolución del desarrollo del Sistema General de Seguridad Social en Salud, desde su implementación mediante la promulgación de la Ley 100 de 1993 y las posteriores normas que la han complementado.En su primera parte realiza un breve recorrido por el sistema, tomando como puntos de referencia los principios fundamentales establecidos para el mismo, tanto en la constitución nacional, como dentro de la misma Ley 100, para luego continuar con una visión crítica de la misma, buscando determinar las principales fallas y puntos álgidos por donde la corrupción la ha flagelado, al igual que ubicando los actores que se han beneficiado, lo mismo que los grandes perdedores tanto en el desarrollo del sistema como en sus resultados.Una vez concluido el proceso de crítica y análisis, el artículo deja en claro una posición respecto al Sistema General de Seguridad Social, en cuanto a las lógicas estructurales que lo fundamentan y adicionando un juicio digno de un mas amplio debate, sobre el papel adelantado por los profesionales de la salud, tanto en la ejecución del mismo, como en su participación como agentes activos generadores de cambio.


This article wants to make a free analysis about thedevelopment of the General Social Security Systemfrom the moment it was implemented by virtue ofLaw 30, 1993 and further complementaryregulations. First of all, it briefly analysis the system,taking as a reference the established principles, theNational Constitution, and the Law 100; then, itcontinues with a critical view of it in order to determinethe main failures and decisive points wherecorruption has severely criticized it; it has alsolocated some actors who have taken advantage of it,and some others who have been losers both in thedevelopment of the system and in its results. Oncethe process of criticism and analysis has beenconcluded, this article presents a clear position aboutthe General Social Security Health System, regardingstructural logic it is based on, and it also adds ajudgment worthy of a broader discussion on therole of the health professionals, both in their performanceand as active actors to generate a change.

In the present study, we have investigated the effect of metabolic inhibition on the inward rectifer K current (IK1). Using whole cell patch clamp technique we applied voltage ramp from +80 mV to -140 mV at a holding potential of -30 mV and recorded the whole cell current in single ventricular myocytes isolated from the rabbit heart. The current-voltage relationship showed N-shape (a large inward current and little outward current with a negative slope) which is a characteristic of IK1. Application of 0.2 mM dinitrophenol (DNP, an uncoupler of oxidative phosphorylation as a tool for chemical hypoxia) to the bathing solution with the pipette solution containing 5 mM ATP, produced a gradual increase of outward current followed by a gradual decrease of inward current with little change in the reversal potential (-80 mV). The increase of outward current was reversed by glibenclamide (10 muM), suggesting that it is caused by the activation of KATP. When DNP and glibenclamide were applied at the same time or glibenclamide was pretreated, DNP produced same degree of reduction in the magnitude of the inward current. These results show that metabolic inhibition induces not only the increase of KATP channel but also the decrease of IK1. Perfusing the cell with ATP-free pipette solution induced the changes very similar to those observed using DNP. Long exposure of DNP (30 min) or ATP-free pipette solution produced a marked decrease of both inward and outward current with a significant change in the reversal potential. Above results suggest that the decrease of IK1 may contribute to the depolarization of membrane potential during metabolic inhibition.

TTH and DNP-BSA delayed hyper-sensitivity(DH)were transfered tononimmune human recipients withGoose derived transfer factor(TF-G)prepared from blood and spleen ofGoose immunized with TTH or DNP-BSA.Only human recipients injec—ted with immune TF-G showed DHskin reactivity by skin test.TTHDH response were only transferedby TTH specific TF-G but not DNPBSA specific TF-G and the DNP-BSAdermal reactivity could only be ob-served when DNP-BSA specific TF-G were employed.The fact that TF-G was able to tranfer donor specificDTH to human recipients suggestedthat there may be no absolute speciesbarriers for DTH transfer with TF.
