RESUMO

BACKGROUND: Transfusion guidelines are not only essential for the optimal use of blood products, but also help reduce transfusion-related adverse reactions and improve patients' outcomes. In this hospital, a transfusion-indication data-entry program based on the national transfusion guidelines was developed and applied to the electronic medical record system in 2016. All transfusion orders, except for emergencies, have been performed using this program since then. This study analyzed the reasons for the transfusion to monitor the blood product usage and provide feedback to clinicians.

As the adage, 揋arbage in, Garbage out� goes, data entry errors may lead to erroneous results and conclusions. Quality assurance during data entry is one of the most neglected components of research and is conspicuously missing in most of the reporting standards. In this study, we reviewed research studies published in Indian Journal of Public Health and Indian Journal of Community Medicine during 2014�16 and determined the proportion of papers reporting on quality assurance during data entry. Of 110 papers, only 6 (5.5%) papers explicitly included a statement about data quality assurance, with two studies reported to have performed double entry and validation, considered the gold standard in quality-assurance of data entry. This is highly unacceptable. We hereby appeal to the community of researchers, peer reviewers, and journal editors in India to pay attention to this important aspect of research and make reporting of quality assurance of data entry mandatory in every published paper.

RESUMO

Objective To explore how to use Epidata and SPSS software in process of coding,entry and statistical analysis of multiple choice conveniently and quickly.Methods On the basis of the hospital demands for nursing graduates behavior questionnaire,Epidata and SPSS software was adopted to realize information coding,entry and statistical analysis of multiple choice,directional selection problem,sort multiple-choice questions.Results Through examples show,a combination of the two software carried on reasonable coding,entry and processing in the three different types of multiple-choice questions,involved in the hospital demands for nursing graduates behavior questionnaire,and gave registration interface of Epidata software,multiple choice encoding and SPSS data analysis process.Conclusions The combination of the two kinds of software can greatly improve the speed of data entry,especially when the data sample is large.

El Centro de Inmunología Molecular ha venido desarrollando biomoléculas para el tratamiento del cáncer. En la medida que se avanza en el desarrollo de estos productos, se avanza en las fases I, II, III y IV de los ensayos clínicos, lo que trae aparejado un incremento en el número de pacientes a tratar y en el número de hospitales involucrados. Se cuenta con diez productos diferentes, implicados en más de 50 ensayos clínicos, nacionales y multinacionales, con un pronóstico de inclusión de 2 500 pacientes nuevos por año. En este proceso están incluidos alrededor de 30 hospitales en 13 provincias del país. Para mejorar el acceso a la información de ensayos clínicos por parte de los investigadores comprometidos, se creó un sitio Web formado por cuatro secciones fundamentales, relacionadas con los productos y sus indicaciones, buenas prácticas clínicas, la entrada remota de datos y la gerencia de los ensayos clínicos. Todo lo anterior mejora fundamentalmente la calidad de la información brindada a los investigadores clínicos involucrados y redunda en una mayor organización de la compleja actividad de los ensayos clínicos en Cuba


The Molecular Immunology Center has been developing biomolecules for cancer treatment. As the development of such products continues, phases I, II, III and IV of clinical assays also advance, which leads to an increase of the number of patients to be treated and the number of involved hospitals. There are ten different products involved in over 50 national and multinational clinical assays in which 2 500 patients are predicted to be included every year. Approximately 30 hospitals from 13 provinces are included in this process. For the purpose of improving the access of committed researchers to the clinical assay information, a new Website with four sections was devised. Those sections are related to products and their indications, good clinical practice, remote data entry and clinical assay management. All the above-mentioned improves the quality of the information given to involved clinical researchers and results in better organization of the complex activity of clinical assays in Cuba

OBJECTIVE: The raw material of quality improvement is information, whose building block is data. Data in an electronic medical record system have many secondary uses beyond their primary role in patient care, including research and organizational management. This study investigates the data quality of clinical observations recorded using a structured data entry format and assesses the impact of erroneous data. METHODS: A total of 4,580,846 input events from 3,348 inpatients, gathered over a three year period in a teaching hospital, were analyzed by using a 2-by-2 conceptual matrix framework for the appropriateness of data types and semantics. The data were classified into three categories: fully usable, partially usable, and not usable. RESULTS: The fully usable data constituted 88.6% of the correctly entered data the remaining 11.4% were erroneous. Among the erroneous data, 0.8% were partially usable (n=3,929), and the remaining 99.2% (n= 510,437) were identified as needing further assessment to improve their quality. CONCLUSION: Clinical information systems have increasingly used structured data entry or record templates, but the low quality of collected data has severely limited their secondary use potential.

Objetivo: Conocer el comportamiento de la velocidad de conducción nerviosa del nervio mediano a nivel del túnel del carpo asociado al tiempo de exposición al factor de riesgo de movimiento repetitivo de los dedos. Metodología: se realizó un estudio descriptivo transversal en digitadores de dos empresas de la ciudad de Medellín durante los meses de octubre y noviembre de 2005. Se investigaron las variables de edad, sexo, tiempo de exposición laboral, dominancia y los antecedentes patológicos asociados a STC. Se aplicaron las pruebas clínicas y se realizó la medición de la neuroconducción distal del nervio mediano. Resultados: Se evaluaron 55 trabajadores que corresponden a 110 manos. El 86% fueron mujeres, edad promedio de 28 años con un rango de 19 a 40 años; el 91% manifestó ser diestro y el 60% tenía un índice de Masa Corporal normal; el promedio de tiempo como digitador fue de 83 meses con un rango de 12 a 180 meses y una dedicación laboral de 48 horas semanales. Discusión: No se encontró correlación estadísticamente significativa entre el tiempo de exposición laboral y la neuroconducción del mediano, los resultados sugieren la hipótesis de que el hecho de tener como factor único de riesgo la alta frecuencia de movimientos de los dedos no es causal de alteración de la neuroconducción del nervio mediano en el túnel del carpo.


Objetive: This study was carried out in order to know the comportment of the median nerve’s impulse speed at the carpal tunnel associated to the exposure time to the fingers’ repetitive motion risk factor. Methodology: This study was applied to data-entry personnel at two companies in the city of Medellin, Colombia, during October and November of 2005. Age, gender, labour exposure time, dominant hand and pathological antecedents associated to the carpal tunnel syndrome variables were investigated. Clinical test and a median nerve conduction test were applied to the workers. Discussion: 55 data-entry workers were evaluated, for a total of 110 hands. The survey included ages between 19 and 40 years old and an average age of 28; 86% of the studied subjects were women; 91% of surveyed subjects declared themselves as right-handed and 60% had a normal weight. Subjects had worked between 12 and 180 months as data-entry personnel with an average time of 83 months, working 48 hours a week. No relevant statistical link was found between labour exposure time and median nerve conduction. Results suggest that doing repetitive movements at work as the only risk factor is not a cause of nerve conduction disorder in the median nerve.

RESUMO

Objective To focus on the expressiveness and flexibility of OpenSDE: an application that supports recording of structured narrative data. Methods OpenSDE enabled data entry with (customizable) forms based on trees of medical concepts. The relevant scope for data entry could be tailored per medical domain by construction of a domain-specific tree. OpenSDE was intended for structuring narrative data to make these available for both care and research. Results The OpenSDE application was used at two departments in aspects of radiology, neurology, pediatrics, and child psychiatry. Conclusion OpenSDE is superior to traditional text entry method in aspects of correct and full expression and clinical decision support.

Speech recognition as an input tool for electronic medical record enables efficient data entry at the point of care. We evaluated the speech recognition accuracy of IBM ViaVoiceTM for doctor-patient dialogues and for pronounced medical vocabularies. The recognition accuracy for doctor-patient dialogues was 95.4%, while that for pronounced medical vocabularies was 55.1%. In order to put speech-based electronic medical record to practical use, mis-recognized vocabulary must be significantly corrected. This paper describes a correction system for mis-recognized medical vocabulary for speech recognition-enabled electronic medical record. The correction system is composed of an extraction and a correction steps. In the extraction step, hamming distance between a parsed substring and the nearest medical vocabulary in the vocabulary database greater than 50% of the length of the substring was used to determine if the substring is a possible mis-recognized medical vocabulary. In the correction step, possible mis-recognized medical vocabularies are scored such that when both the code and location of a syllable is the same with those of a medical vocabulary found in our database, +5 is given and when the code is the same but the location is not, +1 is given. The medical vocabulary with the highest score in the database is used as the correction for the mis-recognized one. When 33 patient-doctor dialogues with 33 medical vocabularies were tested for three times by six testees (i.e., 33 x 6 x 3 = 594 sentences), 94% of the mis-recognized words were correctly detected and repaired. Poor recognition performance for hard medical vocabularies can be markedly improved by the mis-recognized medical vocabulary correction system.

RESUMO

The World Wide Web provides the means for theintegration of clinical computer systems. Extensible Markup Language (XML) and Extensible Stylesheet Language (XSL) are newly developed and standardized Internet protocols. Development of custom data entry forms requires significant programming resources in clinical information system development and management. Visual design tools and a modifiable, template-driven approach may facilitate this process. However, these approaches generally require the predefinition of data form element types. This paper describes an XML/XSL-based technology enabling post hoc definition of elementary and composite data entry form element types.

RESUMO

Objective To further develop SDE for structured documentation of clinical data based on medical ontology. Methods OpenSDE was customized for the broad domain of medical ontology: medical concepts and its descriptors from history taking and physical examination were modeled into a tree structure. Results An expandable EMR system with structured data entry was developed. Conclusion Structured data entry is significant to clinical decision based on EMR.
