RESUMO

With the development of implant,more reliable way of the treatment of posterior teeth loss can be acheived.Dental implants present different biological and biomechanical characteristics from natural teeth and natural teeth with crown.It is important to realize these differences and occlusal adjustment accurately for the improvement of the treatment and prolongation of implant service life.This paper re-views the critical point with a clinical case.

RESUMO

Aim: To compare the amount of bone loss (if any) at the mesial and distal sides in delayed and immediate loaded dental implants. Study Design: Total 20 partially edentulous (anterior region of Maxilla) patients were randomized and equally divided into two groups. Group A received delayed loaded dental implants and group B received immediate loaded dental implants. Both the groups were monitored clinically and radiographically at 3, 6, 12 and 18 months. Methodology: We included 20 patients (11 men, 09 women; age range 20-48 years) with single or multiple edentulous areas in mouth. Clinical as well as all routine hematological examinations were done. Radiographs were taken sequentially as required for 18 months. Results: There was no significant statistical difference of bone loss mesially in both the groups at 3 (P=0.99) & 6 (P=0.25) months, but there was significant statistical difference of bone loss mesially seen in both the groups at 12 (P=0.03) & 18 (P=0.01) months. There was no significant statistical difference of distal bone loss in both the groups at 3 (P=0.22), 6 (P=0.38) and 12 (P=0.17) months, but there were significant statistical difference of distal bone loss seen in both the groups at 18 months (P=0.03). The bone loss was found more with the immediate loading type of implants at both mesial and distal sides of implant. Conclusion: The immediate loading implants may provide a lot of benefits over conventional delayed loading implants but the bone loss at Crestal (Marginal) level is higher as compared to delayed loading implants when seen in maxillary anterior impants which should be considered whenever selecting any particular method of dental implantation.

OBJECTIVE: The purpose of this study was to investigate the stability of mini-implants in relation to loading time. METHODS: A total of 48 mini-implants (ORLUS, Ortholution, Korea) were placed into the buccal alveolar bone of the mandible in 8 male beagle dogs. Orthodontic force (200 - 250 gm) was applied immediately for the immediate loading group while force application was delayed for 3 weeks in the delayed loading group. For the subsequent loading periods (3, 6, 12 weeks), BIC (bone implant contact) and BV/TV (bone volume/total volume) and mobility test were carried out. RESULTS: The immediate loading group showed no changes in BIC from 3 to 12 weeks, while the delayed loading group showed a significant increase in BIC between 3 and 12 weeks (p < 0.05). The BV/TV of the delayed loading group significantly increased from 6 to 12 weeks of loading (p < 0.05), while the BV/TV of the immediate loading group decreased from 3 to 12 weeks of loading. However, there was no significant difference in BV/TV between experimental groups. The mobility of the immediate loading group was not significantly different from that of the delayed loading group after 12 weeks of loading (p < 0.05). CONCLUSIONS: These results showed that immediate loading does not have a negative effect on the stability of mini-implants compared to the early loading method in both the clinical and histomorphometric point of view.

RESUMO

STATEMENT OF PROBLEM: It was reported high success rate of implant-supported fixed prostheses using with 5.6 implants on anterior mandible. Recently, immediate loading protocol was focused to overcome disadvantages of classic 2-stage delayed loading protocol. PURPOSE: This clinical study was to evaluate stability changes with time of immediately loaded and delayed loaded implants in edentulous mandible and to compare stability changes with time according to implantation sites. MATERIALS AND METHODS: Five or six implants were placed on anterior mandible depending on the arch shape. The immediately loading group was consisted of 8 patients received their prostheses within 24.48 hours after implantation. The delayed loading group was consisted of 8 patients received their definitive prostheses following classical prosthetic procedures after a healing period of 3 months. All patients were recalled every 6 months for check-up. The evaluations of radiographic examination, ISQ value measurement and recording of complication were done. To evaluate marginal bone level, intraoral periapical radiographs were taken with long cone paralleling technique. At every evaluation recall, all prostheses were removed and ISQ values were measured with Osstell(TM) on individual implants. RESULTS: 1. None of implants was failed. All implants showed stable marginal bone levels and ISQ values. 2. Marginal bone level changes with time showed statistically significant difference between immediately loading group and delayed loading group (P<0.001). 3. ISQ value changes with time did not show statistically significant difference between immediately loading group and delayed loading group (P=0.079). ISQ value decreased with time in both groups, however, all implants showed stable ISQ value at 30 months-recall evaluation. 4. Marginal bone level changes with time did not show statistically significant differences among implantation sites (P=0.604). 5. ISQ value changes with time showed statistically significant differences among implantation sites (P=0.047). ISQ values of most posterior implants decreased with time comparing to other implants. CONCLUSION: Although the marginal bone level of the terminal abutment didn't different with the other implants, ISQ value of the terminal abutment was lower than that of the other implants. Therefore, further clinical evaluation would be needed in this point of view.
