RESUMO

Objective:To establish an evaluation index system for the refinement management of respiratory specialties in four dimensions: medical care, teaching, scientific research and personnel training, to develop a refinement management system based on medical big data, and evaluate the effectiveness of its application. Methods:Based on evaluation indexes and literature of several domestic respiratory specialty influence rankings, an index system covering four dimensions (primary indexes), 16 secondary indexes and 73 tertiary indexes covering medical treatment (30%), teaching (15%), scientific research (40%) and personnel training (15%) was devised using brainstorming, Delphi, and hierarchical entropy weighting. Data from 13 professional groups and 248 respiratory discipline members integrated by the system in 2018 and 2019 were statistically analyzed using the refined index system embedded in the system to understand the system's application after a year of use with comparing the four dimensions of the discipline and the changes among specialty groups before and after implementation. Results:In 2019, respiratory medicine′s comprehensive score grew 30% compared to 2018. The subscores also grew, with scientific research showing the largest growth rate of 136% and talent cultivation showing the second highest. In 2019, ten of the 13 professional groups grew by 2% to 135%.Conclusions:An improved management system for respiratory medicine based on big data technology has provided a comprehensive, intuitive and quantitative view of the discipline′s development in four dimensions and the differences among specialty groups. It will be an effective decision-making tool for ensuring high-quality and balanced discipline development.

RESUMO

Objective@#To construct and implement a calculation model of clinical nursing post classification.@*Methods@#Between October to December 2018, head nurses of the hospital were screened as consultants, and Delphi method was used to determine the indicators, while the weights were assigned from the aspects of nursing workload, work quality, patient satisfaction and number of nursing night shifts. Combined with HIS data, the calculation model of clinical nursing post classification was constructed to classify clinical nursing units into different categories and levels.@*Results@#After two rounds of expert enquiry, 82 nursing work items were identified and the objective weight assignment was determined ranging from 0.80 to16.14. According to the established calculation model and HIS data, nursing posts in clinical departments were classified into 6 levels and 3 categories, and the accurate management of clinical nursing post classification was achieved.@*Conclusions@#The construction of a calculation model is scientific and rigorous, which provides a scientific basis for dynamic nurse performance management and rational allocation of human resources. In addition, it provides a useful reference for accurate nursing management.

RESUMO

Critical care medicine is an important part of modern medicine and has become an important comprehensive second-level discipline of clinical medicine. The department of critical care medicine of the Affiliated Hospital of Guizhou Medical University was established in 1994. After 24 years of development, there are currently 90 beds, 6 sub-specialties (including comprehensive ICU A, B, C 3 subspecialties, internal ICU, emergency ICU, pediatric ICU) of the third-level intensive medical discipline development model, involve severe nervous system, severe circulatory system, severe environmental disorders, severe trauma, severe digestion, severe kidney, severe immunity, severe endocrine, severe respiratory disease, severe infection, severe obstetric disease, severe poisoning, and there are corresponding talent echelons. The three-level discipline construction model has been explored and operated for more than three years. The hospital's critical care medicine discipline has established a larger professional discipline in southwestern China. The rapid and standardized development of critical care medicine in an all-round way was promoted, so as to lead the rapid development of critical care medicine in hospitals, cities, provinces and even surrounding provinces, and to achieve mutual learning, complementary advantages, resource sharing, win-win cooperation and coordinated development.

RESUMO

Objective: To design a management system of medical consumables that has concise interface in good taste, simple operation, rational construction and well usability so as to achieve informatization and delicacy management for medical consumables. Methods: Through utilized computer networking technology(CNT), and adopted a special architectural pattern of browser and servicer(B/S), and depended on the process of purchase, application and usage, to design and establish a logistics management system of hospital, and to construct a scientific and reasonable management platform for logistics informationization. Result: The application of the logistics management system has solved many problems happened in formerly medical consumables management, such as confusion of qualification management, cumbersome signing approval, the problems of "running, emitting, dripping and leaking" that existed in part of materials and others. Conclusion: The design and application of logistics management system have established a networking approval process which can extremely enhance working efficiency and provide a digital platform for achieving a new model of informatization and delicacy management of medical consumables.
