Introducción: el prolapso rectal es una invaginación del recto que produce su exteriorización a través del ano. Se denomina completo cuando está constituido por todo el espesor de la pared rectal. En su incidencia están vinculados factores como el sexo femenino, los pacientes añosos y el parto. Etiopatogenia: es multifactorial, se plantean varias teorías. Entre ellas, un fondo de saco peritoneal recto vaginal o recto vesical anormalmente profundo, la falta de fijación normal del recto a su lecho, la consecuencia de una invaginación idiopática de la parte superior del recto y un estado de relajación y atonía de la musculatura del suelo de la pelvis y del canal anal. Caso Clínico: mujer, 64 años. Prolapso rectal completo por debilidad de la musculatura del piso pélvico y canal anal. Se plantean la valoración diagnóstica y exámenes paraclínicos preoperatorios. Se describe el procedimiento de Delorme como solución técnica a su prolapso rectal. Se destaca que la paciente evolucionó favorablemente. Conclusiones: el objetivo del tratamiento es solucionar el prolapso y restaurar la función anorrectal sin provocar efectos nocivos. El prolapso que presenta esta paciente es multifactorial e incluye el conjunto de las siguientes anomalías, afecta a las mujeres de edad avanzada y multíparas, está exteriorizado permanentemente y forma parte de una enfermedad degenerativa difusa del periné. El mismo fue solucionado según el procedimiento de Delorme. La evolución de la paciente fue satisfactoria, no se objetivaron complicaciones ni recidiva en el post operatorio precoz o alejado.


Introduction: rectal prolapse is an invagination of the rectum that produces its exteriorization through the anus. It is called complete when it is constituted by the whole thickness of the rectal wall. Factors such as female sex, elderly patients and childbirth are linked to its incidence. Etiopathogenesis: it is multifactorial, and several theories have been put forward. Among them, an abnormally deep peritoneal rectal vaginal or rectal vesical fornix, the lack of normal fixation of the rectum to its bed, the consequence of an idiopathic invagination of the upper part of the rectum and a state of relaxation and atony of the muscles of the pelvic floor and the anal canal. Case report: female, 64 years old. Complete rectal prolapse due to weakness of the pelvic floor muscles and anal canal. The diagnostic evaluation and preoperative paraclinical examinations are presented. The Delorme procedure is described as a technical solution to her rectal prolapse. It is emphasized that the patient evolved favorably. Conclusions: The goal of treatment is to resolve the prolapse and restore anorectal function without causing harmful effects. The prolapse presented by this patient is multifactorial and includes all of the following anomalies, affects elderly and multiparous women, is permanently externalized and is part of a diffuse degenerative disease of the perineum. It was solved according to the Delorme procedure. The patient's evolution was satisfactory, no complications or recurrence were observed in the early or distant postoperative period.


Introdução: O prolapso retal é uma invaginação do reto que resulta em sua externalização através do ânus. É chamado completo quando consiste de toda a espessura da parede retal. Fatores como sexo feminino, pacientes mais velhos e parto estão ligados a sua incidência. Aetiopatogênese: é multifatorial, e várias teorias foram apresentadas. Estes incluem um fórnix peritoneal rectovaginal ou retal vesical anormalmente profundo, a falta de fixação normal do reto em seu leito, a conseqüência da invaginação idiopática da parte superior do reto e um estado de relaxamento e atonia dos músculos do assoalho pélvico e do canal anal. Relato de caso: mulher, 64 anos de idade. Prolapso retal completo devido à fraqueza dos músculos do assoalho pélvico e do canal anal. São apresentados a avaliação diagnóstica e os exames paraclínicos pré-operatórios. O procedimento Delorme é descrito como uma solução técnica para seu prolapso retal. Observa-se que o paciente evoluiu favoravelmente. Conclusões: O objetivo do tratamento é resolver o prolapso e restaurar a função anorretal sem causar efeitos nocivos. O prolapso apresentado por esta paciente é multifatorial e inclui todas as seguintes anomalias, afeta mulheres idosas e multíparas, é externalizado permanentemente e faz parte de uma doença degenerativa difusa do períneo. Foi tratado de acordo com o procedimento Delorme. A evolução do paciente foi satisfatória, não foram observadas complicações ou recidivas no período pós-operatório precoce ou distante.

Objective:To evaluate the safety and efficacy of Delorme procedure for adults with full-thickness rectal prolapse.Methods:Clinical data of 17 adult patients suffering from full-thickness rectal prolapse undergoing Delorme procedure from June 2014 to May 2018 in Hangzhou Third Hospital were retrospectively analyzed. Patient characteristics, operative data, postoperative complications, recurrence of rectal prolapse, continence state and constipation state were evaluated.Results:Eleven patients were female, 6 patients were male with a mean age of (68 ± 9) years. Operations were successfully performed in these 17 cases. The operation time was (88 ± 16) minutes. The estimated blood loss during operation was (23 ± 9) ml. The postoperative time of hospital stay was (8 ± 1) d. Two complications in two patients were observed. There was no treatment related death. One recurrent case was observed during (16 ± 2) months follow-up. The preoperative and postoperative mean constipation score of five patients with fecal constipation were (23 ± 2) and (11 ± 3) respectively ( t = 9.51, P<0.01). The mean fecal incontinence score of six patients with fecal incontinence, before and after Delorme procedure, were (14 ± 2) and (6 ± 2) respectively ( t = 9.09, P<0.01). Conclusions:The Delorme procedure for adults with full-thickness rectal prolapse is a safe and effective surgery with less complications and low recurrence rate. The Delorme procedure may be one of the preferred option of perineal approach for adults with full-thickness rectal prolapse, but the long-term outcome of Delormer procedure and its effect on postoperative anal function need to be further studied.

Objective@#To evaluate the quality of life after Altemeier and Delorme procedures for rectal prolapse patients.@*Methods@#A retrospective cohort study was performed. Clinical data of patients with full-thickness rectal prolapse undergoing surgical treatment in the Sixth Affiliated Hospital, Sun Yat-sen University from February 2013 to January 2018 were retrospectively analyzed. Patients who had no preoperative imaging data, who suffered from internal rectal intussusception, or who did not undergo Altemeier and Delorme operations were excluded. Sixty-seven patients were enrolled, including 32 males with median age of 20.5 (13, 34) years and 35 females with median age of 65.0 (50, 77) years. According to different procedures, patients were divided into the Altemeier group (48 cases) and the Delorme group (19 cases), who received standard Altemeier and Delorme operations respectively. The maximal prolapse length of preoperative squat position, the Longo constipation score, Wexner incontinence score, EQ-5D-5L score, postoperative complications and recurrence rate were analyzed and compared between two groups.@*Results@#The maximal prolapse length of preoperative squat position in Altemeier group and Delorme group was (7.3±3.3) cm and (4.9±2.1) cm respectively with significant difference (t=2.907, P=0.005). The operations in both groups were successfully completed. The operation time and postoperative hospital stay of Altemeier group were longer than those of Delorme group [(112.3±47.0) minutes vs. (80.7±35.4) minutes, t=2.637, P=0.010; (11.3±5.0) days vs. (8.6±3.0) days, t=2.177, P=0.033]. The median follow-up period was 26 (13, 45) months. In the last follow-up, compared to pre-operation, the Longo constipation score [9.0 (6.0, 14.0) vs 4.0 (1.0, 6.5), Z=-4.989, P<0.001], Wexner incontinence score [0 (0, 5.5) vs. 0 (0, 2.0), Z=-3.325, P<0.001] and EQ-5D-5L score [45.0 (40.0, 57.5) vs. 80.0 (70.0, 87.5), Z=-5.587, P<0.001] were all improved obviously in the Altemeier group, meanwhile Longo constipation score [6.0 (5.0, 14.0) vs. 3.0 (1.0, 7.0), Z=-2.186, P=0.029], Wexner incontinence score [0 (0, 12.0) vs. 0 (0, 4.0), Z=-2.325, P=0.020] and EQ-5D-5L score [50.0 (35.0, 60.0) vs. 75.0 (65.0, 90.0), Z=-3.360, P=0.001] in the Delorme group were all improved obviously as well. The postoperative morbidity of complication between the two groups was not significantly different [10/48 (20.8%) vs. 4/19 (21.1%), χ2=0.049, P=0.826]. Sixteen patients (28.0%) relapsed after operation, including 10 patients in the Altemeier group and 6 patients in the Delorme group, without statistically significant difference (P=0.134).@*Conclusions@#Both the Altemeier and Delorme procedures are effective treatments for rectal prolapse, which can improve the postoperative quality of life. Delorme procedure has the advantages of shorter operation time and faster postoperative recovery in patients with mild prolapse.

PURPOSE: The laparoscopic rectopexy has become increasingly popular with verified stability, surgical route selection should be tailored to individual patient characteristics rather than operative risk. The perineal approach is useful in young male patients who need to preserve fertility. This study aimed to compare the characteristics of men and women who underwent Delorme-Thiersch procedures and analyze the postoperative outcomes of the perineal approach by sex. METHODS: We retrospectively reviewed the medical records of 293 patients who underwent Delorme-Thiersch operations in Seoul Song Do Colorectal Hospital between January 2011 and September 2017. Patient clinical characteristics and postoperative complications were analyzed by sex. We analyzed surgical outcomes with preoperative and 3-month postoperative incontinence questionnaires, constipation levels, and anal manometry. RESULTS: In this study, men with rectal prolapse were younger than women with the same condition. American Society of Anesthesiologists physical status classifications were higher in women and women had more L-spine X-ray and pudendal nerve terminal motor latency test abnormalities. Anorectal manometry pressures were higher in men. Men also had longer operation times and hospital stays and more postoperative complications (8 T ring infections, 6 patients with bleeding, 3 with strictures, 2 with severe pain, and 2 with rectal perforations). The recurrence rate was higher among women. CONCLUSION: Men with rectal prolapse were younger, healthier, and had relatively better anorectal function than women. The Delorme-Thiersch operation in men promoted lower recurrence rates and was advantageous in preserving the fertility of young patients, but the incidence of complications was also higher in men. Adequate counseling and preparation for the possibility of complications are needed.

PURPOSE: Although there are more than a hundred techniques, including the transabdominal and the perineal approaches, for the repair of the rectal prolapsed, none of them is perfect. The best repair should be chosen not only to correct the prolapse but also to restore defecatory function and to improve fecal incontinence throughout the patient's lifetime. The aim of this retrospective review is to evaluate clinical outcomes of the Delorme's procedure for the management of the complete rectal prolapse. METHODS: A total of 19 patients (13 females and 6 males) with complete rectal prolapses were treated by using the Delorme's procedure in St. Vincent's Hospital, The Catholic University of Korea, from February 1997 to February 2007. Postoperative anal incontinence was evaluated using the Cleveland Clinic Incontinence Score. RESULTS: All 19 patients had incontinence to liquid stool, solid stool, and/or flatus preoperatively. Three (15.8%) patients reported recurrence of the rectal prolapse (at 6, 18, 29 months, respectively, after the operation). Information on postoperative incontinence was available for 16 of the 19 patients. Twelve of the 16 patients (75%) reported improved continence (5 [31.3%] were improved and 7 [43.7%] completely recovered from incontinence) while 4 patients had unchanged incontinence symptoms. One (6.3%) patient who did not have constipation preoperatively developed constipation after the operation. CONCLUSION: The Delorme's procedure is associated with a marked improvement in anal continence, relatively low recurrence rates, and low incidence of postoperative constipation. This allows us to conclude that this procedure still has its own role in selected patients.

RESUMO

The aim of treatment of rectal prolapse is to control the prolapse, restore continence, and prevent constipation or impaired evacuation. Faced with a multitude of options, the choice of an optimal treatment is difficult. It is best tailored to patient and surgeon. Numerous procedures have been described and are generally categorized into perineal or abdominal approaches. In general, an abdominal procedure has associated with lower recurrence and better functional outcome than perineal procedures. The widespread success of laparoscopic surgery has led to the development of laparoscopic procedures in the treatment of complete rectal prolapse. In Korea, there has been a trend toward offering perineal procedures because of the high incidence of rectal prolapse in young males and its being a lesser procedure. Delorme-Thiersch procedure has appeal as a lesser procedure for patients of any age or risk category, especially for elderly low-risk patients, patients with constipation or evacuation difficulties, young males, and patients with symptomatic hemorrhoids or mucosal prolapse. Laparoscopic suture rectopexy is recommended for either low-risk female patients or patients who are concerned with postoperative aggravation of their incontinence.

PURPOSE: This retrospective study was designed to review and analyze the results of Delorme's procedure for rectal prolapse. METHODS: Between 1990 and 1999, twenty-nine patients with rectal prolapse underwent Delorme's procedure. These patients had had no previous operation for rectal prolapse. This study was proceeded retrospectively through the out-patient clinic and by telephone questionnare. RESULTS: Twelve cases (41%) were males and 17 cases (59%) were females. Mean age was 55.5 years (range, 23~86 years). The duration of the symptoms was ranged from 3 months to 60 years, with the mean period of 12.2 years. The follow-up period after the operation was from 3 months to 10 years (mean follow-up, 45 months). The internal rectal prolapses were 11 cases (38%), and the complete rectal prolapses were 18 cases (62%). The common preoperative bowel habits were incontinence with 6 cases (21%) and constipation with 10 cases (34%). After the operation, incontinence and constipation were improved in 4 cases (67%) and 6 cases (60%) respectively. Additional 2 cases of constipation occurred among 19 cases who hadn't had it preoperatively but the use of laxative helped in improving the symptom. The mean operation time was 71 minutes and in 24 cases (83%), the operation was proceeded with spinal anesthesia. In 27 cases (93%), the amount of bleeding during the operation was less than 100 cc, and in 1 case (3.4%), blood transfusion was needed because the amount was more than 400 cc. The three patients (10%) had postoperative complications(one perianal abscess due to anastomotic dehiscence and two urinary retention). There was one case of recurrence (3.4%) after the operation and no postoperative mortality. CONCLUSIONS: Delorme's procedure has the short operation time, causes less bleeding and is possible with regional anesthesia. Delorme's procedure has low complication rate, results in good bowel function and has a low recurrence rate. Therefore, Delorme's procedure can be performed with satisfactory outcome in elderly patients and the poor general conditioned patients as well as younger patients. As recurrence rates is low and continence is improved, this procedure may be the preferred initial treatment of all patients with rectal prolapse.
