RESUMO

Dermal melanocytosis is characterized by the presence of ectopic melanocytes in the dermis. There are several clinical forms, such as blue nevus, mongolian spot, nevus of Ota, nevus of Ito and dermal melanocyte hamartoma. Differential diagnostic features of these dermal melanocytic lesions depend on onset, course, distribution and histologic feature. A 24-year-old woman presented with an asymptomatic 6x3.5cm bluish-to-grayish speckled patch on the left calf, which had been there since birth. Histopathologic findings, using Hematoxylin & eosin staining, revealed marked increase in the infiltrate of pigment cells mainly in the upper dermis. The patient was treated twice with a Q-switched Nd-YAG laser (Medlite Co., USA). Clinical improvement was observed. Herein, we report a case of dermal melanocyte hamartoma treated with a Q-switched Nd-YAG laser.

RESUMO

Dermal melanocytosis is characterized by the presence of ectopic melanocytes in the dermis. Dermal melanocytosis assumes several morphologic forms, including the Mongolian spot, the blue nevus, the nevus of Ota, the nevus of Ito and dermal melanocytic hamartoma. Differential diagnostic features of these dermal melanocytic lesions depend on onset, course, distribution, histologic features, uniformity of pigmentation, etc. In our case, an 18-year-old girl had asymptomatic 5x3cm sized ill-defined bluish patch on left forearm since birth. Histologic examinations revealed marked increased infiltrate of pigment cells mainly in the upper dermis in Hematoxylin & eosin stain . Fontana-Masson stain and Gp 100 stain revealed marked infiltrate of melanin-containing cells in upper dermis. We report a rare case of dermal melanocyte hamartoma.

Dermal melanocytosis is characterized by the presence of ectopic melanocytes in the dermis and shows several morphological forms, including the mongolian spot, the blue nevus, the nevus of Ota, the nevus of Ito, and dermal melanocyte hamartoma. A 30-month-old boy had a gray blue patch on the left deltoid region, upper back, sacral area, the entire length of the right arm, and right hand. There was a bluish speckled patch on the right palm. The lesions were present at birth, and no appreciable change in appearance had occurred. He also had blue to brownish pigmentation on the right side of his face with dark bluish pigmentation of the right sclera. His Mental and physical development had been normal. The histopathological examination revealed dendritic dermal melanocytes, mainly in the upper and mid dermis. Clinically and pathologically, the patient was diagnosed as having Ota nevus, Ito nevus, mongolian spot, and dermal melanocyte hamartoma. To our knowledge, dermal melanocytosis with 4 different morphological forms in a patient has not previously been reported. We report this unique case of dermal melanocytosis with various morphological forms.

RESUMO

A 26-year-old man presented with a bluish speckled patch on the dorsum of the left hand. It had apperaed at birth as a bluish spot. Since adolescence the lesion extended and at the time of visiting hospital it covered the most part of the dorsum of the left hand and even some palmar aspect. The histopathological and ultrastructural examination revealed numerous dermal melanocytes, mainly in the upper dermis. Clinically and pathologically, the patient was diagnosed as having dermal melanocyte hamartoma, a distinct type of dermal melanocytosis.
