RESUMO

Aim: To investigate the feasibility of recombinant adeno-associated virus serotype9 (rAAV9) mediated nerve growth factor (NGF) transfection in diabetic rats by intramyocardial injection, and to confirm the protective effect of this method on diabetic cardiac autonomic neuropathy. Methods: At 4th week after the establishment of diabetic model, recombinant adeno-associated virus (rAAV9-NGF) carrying NGF gene was injected into the heart of rats. Five weeks later, the distribution of NGF, calcitonin gene-related peptide (calcitonin gene-related peptide, CGRP) and nerve fibers were detected by immunofluorescence. Hemodynamic parameters were used to evaluate cardiac function, and ELISA method was used to detect the expression of NGF, CGRP in myocardium. Results: The exogenous gene NGF mediated by rAAV9 could be stably transfected and over-expressed in the myocardium of diabetic rats. The left ventricular systolic pressure, heart rate, and maximal rate of increase of left ventricular pressure significantly decreased, the left ventricular end-diastolic pressure and the maximal decrease rate of left ventricular pressure significantly increased, and the cardiac function was improved. NGF, CGRP protein in myocardial tissues were up-regulated, and immunofluorescence showed that the decrease of NGF, CGRP and nerve fibers in myocardial tissues could be partially reversed after transfection. Conclusions: Intramyocardial injection of NGF gene can effectively enhance the expression of NGF and promote the growth of nerve fiber, exerting a protective effect in diabetic rat heart.

Objective To observe the therapeutic effect and effects on nerve conduction velocity of α-lipoic acid combined with monosialoteterahexosyl ganglioside sodium (GM 1) on diabetic peripheral neuropathy (DPN).Methods Ninety-six cases patients with DPN were randomly divided into the observation group (50 cases) and the control group(46 cases).The control group received GM1 40 mg daily though travenous drip,the observation group received α-lipoic acid 600 mg and daily though travenous drip.The course of treatment was 2 weeks in two groups.The effect was observed after 2 weeks,the motor nerve conduction velocity (MNCV) and sensory nerve conduction velocity (SNCV) of median nerve and common peroneal nerve were detected by EMG.Results The total effectiveness of observation group was 94.00%(47/50),higher than that of the control group(71.74% (33/46),x2=7.0205,P< 0.05).After the treatment,MNCV and SNCV of median nerve of observation group and the control group were (47.6813.34)m/s and (43.82±3.10)m/s,(40.23±2.76)m/s,(37.31 ±2.79)m/s,and M NCV and SNCV of common peroneal nerve of observation group and the control group were(42.86±3.16)m/s and (39.47±2.17)m/s,(35.67±2.61) m/s and (32.16±2.50) m/s,all improved compared to before treatment(observation grou:t =7.876,8.428,6.923,7.234;control grou:t =3.572,3.483,3.413,4.182;P<0.05),and the magnitude of improvement of the observation group was more significant,and the difference was significant compared to control group after six months (t =5.133,3.827,4.634,4.545;P<0.05).Conclusion α-lipoic acid combined with GM1 in the treatment of DPN can significantly improve the clinical symptoms of patients and improve nerve conduction velocity,is better than the single use of gangliosides,it is worthy of clinical application.

@#Objective To explore the mechanism of the nerve growth factor (NGF) accommodated by electro-acupuncture in rats with diabetes peripheral neuropathy (DPN).MethodsThe rat DPN model was established with streptozotocin (STZ). The rats were randomly divided into the normal group, model group, western medicine group and electro-acupuncture group. The animals of the electrol-acupuncture group were treated with electrol-acupuncture on "Zusanli" and "Shenshu" after DPN model established. The nervous tissues of all animals were examined with the techniques of immunity and fluorescent quantitation PCR to detect the albumen and mRNA expression of NGF.ResultsThe albumen and mRNA expression of NGF of the animals in the model group, western medicine group and electro-acupuncture group were significantly lower than that in the normal group ( P<0.05~0.01), and the albumen and mRNA expression of NGF was up-regulation in the electro-acupuncture group after electro-acupuncture treatment and was significantly higher than that in the model group ( P<0.01).ConclusionElectro-acupuncture on "Zusanli" and "Shenshu" can up-regulate the albumen and mRNA expression of NGF, and improve reparation of the sciatic nerve in DPN animals.

Objective To observe the effect of monochromatic infrared energy(MIRE)on diabetic periphe- ral neuropathy(DPN).Methods Seventy-four subjects with diabetic peripheral neuropathy who were tested by Semmes-Weinstein monofilaments(SWM)were randomized into 2 groups:a conventional management group and a conventional management plus MIRE group.Then the patients'sensory function and other DPN symptoms were evalu- ated by the SWME and the score of Michigan Neuropathy Screening Instrument.Results After treatments,there was a decrease(P<0.01)in the number of the sites insensitive to SWME(grade 5.07),and MNSI scores were sig- nificantly decreased(P<0.01).The MIRE management was more effective than conventional management.Con- clusion Monochromatic infrared energy is perhaps a safe,non-pharmaceutical and non-invasive method for the treat- ment of diabetic peripheral neuropathy.
