RESUMO

Background: Osteomyelitis is an infection of the bone that can occur from direct or indirect invasion by a pathogens, both of these types can potentially progress to subacute and chronic osteomyelitis that lasts longer than 4 weeks. This disease has important characteristics such as long-term clinical course, long periods of silence, the treatment of the recurrence of serious complications of the disease is difficult financially and it takes a lot of money epidemiology of chronic osteomyelitis in the Afghanistan is largely unknown. The aim of this study was epidemiologic study of chronic osteomyelitis in adult clients of Paktya city regional hospital and Wazir Akbar Khan Hospital in Kabul city of Afghanistan.Methods: This descriptive cross-sectional study was performed in patients' with chronic osteomyelitis who referred in this two hospitals in Paktya city regional hospital and the Wazir Akbar Khan hospital in Kabul During the March 2019 to March 2020, 70 patients were identified with chronic osteomyelitis. The information required for the study, such as general information, underlying diseases of the patients collected from these two medical centers. After encoding, the necessary information was entered into the computer and analyzed.Results: According to the findings of this study the prevalence of chronic bone infections in male were 64% (n=45) in female were 36% (n=25), current findings showed 44.3% of all patient involved with tibial (n=31) chronic osteomyelitis and the most common underlying disease causing chronic osteomyelitis is the direct entry of infection as a result of trauma.Conclusions: Our study showed the chronic osteomyelitis is higher in the male population than in the female population and highest incidence of chronic osteomyelitis site was tibia result of trauma because of humid climate, poor personal and hospital environmental health status and relatively poor medical facilities in Afghanistan may contribute to higher morbidity.

RESUMO

Relatou-se a ocorrência de fratura patelar associada à ruptura do tendão patelar em um cão sem raça definida, macho, atendido em um hospital veterinário escola. A técnica utilizada foi a combinação de fixação óssea com fio de Kirschner e aplicação de banda de tensão, recomendada em fraturas transversais da patela. Para reparar a lesão do tendão patelar, realizou-se a sutura tipo Kessler modificada com fio de náilon 0,80mm. No retorno do paciente, 150 dias após o procedimento cirúrgico, havia processo cicatricial ósseo avançado da patela. Concluiu-se que as medidas adotadas para reparação da lesão foram eficazes.


It is reported the occurrence of patellar fracture associated with rupture of the patellar tendon in a mongrel male dog referred to a Teaching Veterinary Hospital. The used technique was the combination of bone fixation with Kirschner wire and application of tension band recommended in most transverse fractures of the patella. To repair the damage to the patellar tendon, the modified Kessler suture with nylon 0.80mm was performed. When the patient returned 150 days after surgery, advanced bone healing process was observed in patella. It was concluded that the measures taken to repair the injury were effective.

RESUMO

Direct trauma to extraocular muscles is usually the result of penetrating injuries. Contusion, avulsion, or laceration of the extraocular muscles may occur. We have treated a 3-year-old boy with what appeared to be nasal conjunctival laceration by scissors. The injured eye showed approximately 50 prism diopter exotropia and slight limitation of adduction of the left eye. In orbital MRI focal hemorrhage and adhesion around left medial rectus muscle was noted but the discontinuity of left medial rectus muscle was not found in orbital MRI. After 6 months, the child underwent strabismus surgery under general anesthesia. We detected that the upper portion of the left medial rectus muscle was lacerated about 25% of total width of muscle from the insertion to 4mm distal to the insertion. We performed a recession-resection procedure to the left eye. 3 month after the operation, duction movement showed no limitation. The prism and cover test at near revealed orthophoria. We suggest that a strabismus following after a periocular laceration can be accompanied with a injury of extraocular muscle.
