Resumen: Introducción: Un tema en común que ha caracterizado la evolución de las metodologías cualitativas en las últimas décadas es un "giro transdisciplinario" en forma de la incorporación de técnicas derivadas de las artes. Objetivo: Realizar un análisis crítico de esta tendencia en el campo de la salud y la salud mental, a través de la descripción y explicación de los alcances de dos dispositivos, el artístico y el poético. Método: El trabajo se desarrolla desde la duo-etnografía, entendida como un proceso dialógico que genera nuevos significados a los fenómenos sociales y constructos epistemológicos. Resultados: Los dispositivos descritos tienen un impacto significativo en la relación de la expresión artística con los fundamentos de la investigación cualitativa. Conclusiones: Las artes han impulsado la investigación cualitativa para ir más allá del logocentrismo en las preocupaciones representacionales. Como consecuencia se ha dejado de pensar el uso del arte como una herramienta, sino como una vivencia de construcción de la subjetividad que permite una auténtica experiencia de construcción de la realidad con la implicación activa y crítica por parte del investigador en sus proyectos.

Objetivo: Realizar uma análise crítica desta tendência no campo da saúde e da saúde mental, através da descrição e explicação do alcance de dois dispositivos, o artístico e o poético. Método: O trabalho é desenvolvido a partir da duo-etnografia, entendida como um processo dialógico que gera novos significados para os fenômenos sociais e construções epistemológicas. Resultados: Os dispositivos descritos têm um impacto significativo sobre a relação da expressão artística com os fundamentos da pesquisa qualitativa. Conclusões: As artes têm impulsionado a pesquisa qualitativa para ir além do logocentrismo nas preocupações de representação. Como consequência, o uso da arte não é mais pensado como uma ferramenta, mas como uma vivência de construção da subjetividade que permite uma experiência autêntica de construção da realidade com o envolvimento ativo e crítico do pesquisador em seus projetos.


Abstract: Introduction: A common theme that has characterized the evolution of qualitative methodologies in recent decades is a "transdisciplinary turn" in the form of the incorporation of techniques derived from the arts. Objective: To carry out a critical analysis of this trend in the field of health and mental health, through the description and explanation of the scope of two dispositifs, the artistic and the poetic. Method: The work is developed from duo-ethnography, understood as a dialogic process that generates new meanings to social phenomena and epistemological constructs. Results: The described dispositifs have a significant impact on the relationship of artistic expression with the foundations of qualitative research. Conclusions: The arts have pushed qualitative research to move beyond logocentrism in representational concerns. As a consequence, the use of art is no longer thought of as a tool, but as an experience of construction of subjectivity that allows an authentic experience of construction of reality with the active and critical involvement of the researcher in their projects.

Objective:To investigate the clinical efficacy of pancreaticoduodenectomy (PD) for periampullary diseases.Methods:The retrospective cohort study was conducted. The clinico-pathological data of 2 019 patients with periampullary diseases who underwent PD in the First Affiliated Hospital of Nanjing Medical University from January 2016 to December 2020 were collected. There were 1 193 males and 826 females, aged 63(15) years. Observation indicators: (1) surgical situations; (2) postoperative conditions; (3) postoperative pathological examinations; (4) prognosis of patients with periampullary carcinoma. Regular follow-up was conducted by telephone interview and outpatient examination once every 3 months within the postoperative first year and once every 6 months thereafter to detect the survival of patients with periampullary carcinoma. The follow-up was up to December 2021. Measurement data with skewed distribution were represented as M(IQR) or M(range), and comparison between groups was analyzed using the rank sum test. Count data were described as absolute numbers and (or) percentages, and comparison between groups was analyzed by the chi-square test or Fisher exact probability. Kaplan-Meier method was used to draw survival curves and calculate survival rates, and Log-Rank test was used to conduct survival analysis. Results:(1) Surgical situations: of 2 019 patients, 1 116 cases were admitted from 2016-2018 and 903 cases were admitted from 2019-2020. There were 1 866 cases undergoing open PD and 153 cases undergoing laparoscopic or robot-assisted PD. There were 1 049 cases under-going standard PD and 970 cases undergoing pylorus-preserved PD. There were 215 cases combined with portal mesenteric vein resection, 3 cases combined with arterial resection. The operation time of 2 019 patients was 255(104)minutes and the volume of intraoperative blood loss was 250(200)mL. The intraoperative blood transfusion rate was 31.401%(623/1 984), with the blood transfusion data of 35 cases missing. The proportions of pylorus-preservation, combination with portal mesenteric vein resection, intraoperative blood transfusion were 585 cases(52.419%), 97cases(8.692%), 384 cases(34.941%) for patients admitted in 2016-2018, versus 385 cases(42.636%), 118 cases(13.068%), 239 cases(27.006%) for patients admitted in 2019-2020, showing significant differences between them ( χ2=19.14,10.05,14.33, P<0.05). (2) Postoperative conditions: the duration of postoperative hospital stay of 2 019 patients was 13 (10) days. One of 2 019 patients lacked the data of postopera-tive complications. The overall postoperative complication rate was 45.292%(914/2 018), of which the incidence rate of grade B or C pancreatic fistula was 23.439%(473/2 018), the rate of grade B or C hemorrhage was 8.127%(164/2 018), the rate of grade B or C delayed gastric emptying was 15.312%(309/2 018), the rate of biliary fistula was 2.428%(49/2 018) and the rate of abdominal infection was 12.884%(260/2 018). The reoperation rate of 2 019 patients was 1.932%(39/2 019), the in-hospital mortality was 0.644%(13/2 019), the postoperative 30-day mortality was 1.238%(25/2 019), and the postoperative 90-day mortality was 2.675%(54/2 019). There were 541 cases(48.477%) with overall postoperative complications, 109 cases(9.767%) with grade B or C hemorr-hage, 208 cases(18.638%) with grade B or C delayed gastric emptying , 172 cases(15.412%) with abdominal infection, 39 cases(3.495%) with postoperative 90-day mortality of 1 116 patients admitted in 2016-2018. The above indicators were 373 cases(41.353%), 55 cases(6.098%), 101 cases(11.197%), 88 cases(9.756%), 15 cases(1.661%) of 902 patients admitted in 2019-2020, respectively. There were significant differences in the above indicators between them( χ2=10.22, 9.00, 21.30, 14.22, 6.45 , P<0.05). The in-hospital mortality occurred to 11 patients(0.986%) of 1 116 patients admitted in 2016-2018 and to 2 cases(0.221%) of 903 patients admitted in 2019-2020, showing a significant difference between them ( P<0.05). (3) Postoperative pathological examinations. Disease area of 2 019 patients reported in postoperative pathological examinations: there were 1 346 cases(66.667%) with lesions in pancreas, including 1 023 cases of carcinoma (76.003%) and 323 cases(23.997%) of benign diseases or low potential malignancy. There were 250 cases(12.382%) with lesions in duodenal papilla, including 225 cases of carcinoma (90.000%) and 25 cases(10.000%) of benign diseases or low potential malignancy. There were 174 cases(8.618%) with lesions in bile duct, including 156 cases of carcinoma (89.655%) and 18 cases(10.345%) of benign diseases or low potential malignancy. There were 140 cases(6.934%) with lesions in ampulla, including 134 cases of carcinoma (95.714%) and 6 cases(4.286%) of benign diseases or low potential malignancy. There were 91 cases(4.507%) with lesions in duodenum, including 52 cases of carcinoma (57.143%) and 39 cases(42.857%) of benign diseases or low potential malignancy. There were 18 cases(0.892%) with carcinoma in other sites. Postoperative pathological examination showed carcinoma in 1 608 cases(79.643%), benign diseases or low potential malignancy in 411 cases(20.357%). The histological types of 1 608 patients with carcinoma included adenocarcinoma in 1 447 cases (89.988%), intra-ductal papillary mucinous carcinoma in 37 cases(2.301%), adenosquamous carcinoma in 35 cases(2.177%), adenocarcinoma with other cancerous components in 29 cases(1.803%), neuroendocrine carcinoma in 18 cases(1.119%), squamous carcinoma in 1 case (0.062%), and other histological malignancies in 41 cases(2.550%). The histological types of 411 patients with benign or low poten-tial malignancy included intraductal papillary mucinous neoplasm in 107 cases (26.034%), chronic or autoimmune inflammatory disease in 62 cases(15.085%), neuroendocrine tumor in 58 cases(14.112%), pancreatic serous cystadenoma in 52 cases(12.652%), pancreatic solid pseudopapillary tumor in 36 cases(8.759%), gastrointestinal stromal tumor in 29 cases(7.056%), villous ductal adenoma in 20 cases(4.866%), pancreatic mucinous cystadenoma in 2 cases(0.487%), pancreatic or duodenal trauma in 2 cases(0.487%) and other histological types in 43 cases(10.462%). (4) Prognosis of patients with periampullary carcinoma. Results of survival analysis of 1 590 patients with main locations of periampullary carcinoma showed that of 1 023 patients with pancreatic cancer, 969 cases were followed up for 3.0-69.6 months, with a median follow-up time of 30.9 months. The median overall survival time, 1-year, 3-year and 5-year survival rates of pancreatic cancer patients were 19.5 months [95% confidence interval ( CI) as 18.0-21.2 months], 74.28%, 29.22% and 17.92%. Of 225 patients with duodenal papillary cancer, 185 cases were followed up for 3.0-68.9 months, with a median follow-up time of 36.7 months. The median overall survival time, 1-year, 3-year and 5-year survival rates were unreached, 94.92%, 78.87% and 66.94%. Of 156 patients with distal bile duct cancer, 110 cases were followed up for 3.0-69.5 months, with a median follow-up time of 25.9 months. The median overall survival time, 1-year, 3-year and 5-year survival rates were 50.6 months (95% CI as 31.4 to not reached), 90.37%, 56.11% and 48.84%. Of 134 patients with ampullary cancer, 100 cases were followed up for 3.0-67.8 months, with a median follow-up time of 28.1 months. The median overall survival time, 1-year, 3-year and 5-year survival rates were 62.4 months (95% CI as 37.8 months to not reached), 90.57%, 64.98% and 62.22%. Of 52 patients with duodenal cancer, 38 cases were followed up for 3.0-69.5 months, with a median follow-up time of 26.2 months. The median overall survival time, 1-year, 3-year and 5-year survival rates were 52.0 months (95% CI as 30.6 months to not reached), 93.75%, 62.24% and 40.01%.There was a significant difference in overall survival after PD between patients with different locations of periampullary malignancies ( χ2=163.76, P<0.05). Conclusions:PD is safe and feasible in a high-volume pancreas center, but the incidence of overall postoperative complications remains high. With the increase of PD volume, the incidence of overall postoperative complications has significantly decreased. There is a significant difference in overall survival time after PD among patients with different locations of periampullary malignancies. The 5-year survival rate after PD for duodenal papillary cancer, ampullary cancer, duodenal cancer and distal bile duct cancer is relatively high, whereas for pancreatic cancer is low.

Objective: To evaluate fracture resistance and survival rate of IPS Empress CAD versus Polished Celtra Duo ceramic laminate veneers. Material and Methods:Thirty-six ceramic laminate veneers were fabricated for maxillary anterior teeth. The patients were divided into two groups according to the material Group 1(control group) fabricated from IPS Empress CAD laminate veneers and group 2 (intervention group) fabricated from Polished Celtra Duo laminate veneers. Standardized the same preparation with butt joint design and chamfer finish line located supra gingival were performed for all the teeth. The fabrication of the veneers was performed using Cad\Cam (Ceramill motion) machine, with software (Exocad). The veneers surfaces were treated and silanated according to the manufacture instruction of each ceramic and enamel surfaces were etched where total etch adhesive protocol was obeyed using BISCO. Follow up sessions were done every two months up to one year for each patient using dental probe and operator vision to evaluate the fracture, survival rate, marginal adaptation, sensitivity and caries. according to USPHS criteria (United States Public Health Service). This was performed by an experienced, blinded investigator. Results: Fracture resistance, marginal adaptation, retention, caries and sensitivity were evaluated according to the criteria of USPHS and we found there is no significant difference as both groups scaled zero score. Conclusion: Both IPS Empress Cad and Polished Celtra Duo laminate veneers revealed successful clinical performance in terms of fracture resistance, marginal adaptation, retention, and sensitivity after one year follow up period (AU)


Objetivo: Avaliar a resistência à fratura e a taxa de sobrevivência de laminados de cerâmica IPS Empress CAD versus Celtra Duo polidos. Material e Métodos: Trinta e seis facetas cerâmicas laminadas foram confeccionadas para dentes anteriores superiores. Os pacientes foram divididos em dois grupos de acordo com o material Grupo 1 (grupo controle) confeccionado com laminados IPS Empress CAD e grupo 2 (grupo intervenção) confeccionado com laminados Celtra Duo polidos. A mesma forma de preparo e acabamento de chanfro localizado supra-gengival padronizados foram realizados para todos os dentes. A confecção das facetas foi realizada em máquina Cad \ Cam (Ceramill motion), com software (Exocad). As superfícies laminadas foram tratadas e silanizadas de acordo com as instruções do fabricante de cada cerâmica e as superfícies de esmalte foram condicionadas seguindo o protocolo adesivo de condicionamento ácido total com BISCO. Sessões de acompanhamento foram realizadas a cada dois meses durante um ano para cada paciente usando sonda exploradora e visão do operador para avaliar a fratura, taxa de sobrevivência, adaptação marginal, sensibilidade e cárie; de acordo com os critérios USPHS (Serviço de Saúde Pública dos Estados Unidos). Isso foi realizado por um investigador experiente e cego. Resultados: a resistência à fratura, adaptação marginal, retenção, cárie e sensibilidade foram avaliadas de acordo com os critérios da USPHS e não encontramos diferença significativa, pois ambos os grupos escalonaram pontuação zero. Conclusão: As facetas laminadas IPS Empress Cad e Celtra Duo polido revelaram desempenho clínico bem-sucedido em termos de resistência à fratura, adaptação marginal, retenção e sensibilidade após um período de acompanhamento de um ano. RESUMEN Antecedentes: la pandemia de COVID-19 ha introducido cambios drásticos en el sistema de salud. Las cirugías electivas son una de las actividades quirúrgicas que más han descendido durante la pandemia. Objetivo: analizar el impacto de la pandemia de COVID-19 en la cirugía pancreática en una institución pública y otra privada. Se comparó, en cada institución, con el número de cirugías en el mismo período del año pasado. Material y métodos: se revisaron en una base prospectiva los pacientes que recibieron una cirugía pancreática en las dos instituciones entre el 10/3/20 y el 24/6/20. Se determinaron los datos epide miológicos, el tipo de resección pancreática, el diagnóstico anatomopatológico, la morbilidad y la mor talidad. Se compararon con los pacientes en ambas instituciones que recibieron cirugía pancreática durante el período 10/3/19 al 24/6/19. Resultados: durante la pandemia se realizaron 23 resecciones pancreáticas (13 duodenopancreatec tomías cefálicas, 9 pancreatectomías izquierdas y 1 pancreatectomía total). El 70% (16/23) fueron adenocarcinomas. La morbilidad alcanzó el 34,7% y no se registró mortalidad. Ningún paciente ni miembro del equipo quirúrgico se infectó con coronavirus. La pandemia no tuvo impacto en el núme ro de cirugías en el centro privado (22 vs. 20, p = 0,88), mientras que en el centro público hubo una reducción significativa en el número de cirugías (14 vs. 3, p = 0,009). Conclusión: la cirugía pancreática se puede hacer con seguridad durante la pandemia. En el centro privado se mantuvo el número de cirugías pancreáticas. En el centro público, con máxima prioridad para pacientes con COVID-19, hubo un descenso significativo.


ABSTRACT Background: The COVID-19 pandemic has introduced dramatic changes in the health system. Elective surgeries are the surgical activities with greater decline during the pandemic. Objective: The aim of this paper is to analyze the impact of the COVID-19 pandemic in pancreatic sur gery in a public and a private institution. The number of surgeries performed in each institution was compared with those performed in same period of the previous year. Material and methods: Data from a prospective database of all the patients who underwent pancrea tic surgery between March 10, 2020, and June 3, 2020, were analyzed. The epidemiological data, type of pancreatic resection, pathology diagnosis, morbidity and mortality were determined in each insti tution and compared with patients who underwent pancreatic surgery in both institutions between March 3, 2019, and June 24, 2019. Results: 23 pancreatic resections were performed during the pandemic (13 cephalic pancreaticoduo denectomies, 9 left pancreatectomies and 1 total pancreatectomy); 70% (16/23) were adenocarcino mas. There were 34.7% complications and no deaths were reported. None of the patients was infected with coronavirus. The pandemic had no impact on the number of pancreatic resections in the private institution (22 vs. 20, p = 0.88), while the number of pancreatic surgeries was significantly lower in the public center (14 vs. 3, p = 0.009). Conclusion: Pancreatic surgery can be safely performed during the pandemic. The number of pancrea tic surgeries did not decline during the pandemic. The priority for treating patients with COVID-19 at the public center resulted in a significant decrease in pancreatic surgeries.

Objective To discuss the risk in duo paternity testing. Methods 22 fictitious duo families formed by 22 pairs of unrelated individuals having one or zero inconsistent locus were selected after detected by GoldeneyeTM 20A Kit. The 22 fictitious duo families were further tested with STRtyper-10G kit and/or AGCU 21+1 STR kit until there were more than 3 inconsistent loci and the cumulative paternity index(CPI) value was less than 0.0001. According to the three excluding rules, ① number of inconsistent loci>3; ② CPI ≤ 0.0001; ③ accord with both ① and ② , using multiple STR systems, such as 19 STR loci, 26 STR loci, 39 STR loci and 46 STR loci to test and discuss whether there is difference among the excluding result of unrelated individual. Results Among those 22 fictitious duo families, using three excluding rules, None was excluded by 19 STR loci, and all was excluded by 39 STR loci. Conclusion Duo paternity tests may get a wrong result using only 19 loci system. To reduce the error risk 39 STR-loci systems would be suggested.

Aim: The aim of this study is to investigate whether dental nanocomposite Filtek Z350 XT and Ceram X-duo can benefit from electron beam irradiation in order to achieve increased flexural strength Place and Duration of Study: Department of Conservative Dentistry and Endodontics, A. B. Shetty Memorial Institute of Dental Sciences, Nitte University, Deralakatte, Mangalore, India and Microtron Centre; Department of Physics Mangalore University; Mangalore, India between May 2011 to March 2014. Materials and Methods: Materials were prepared on rectangular bar shaped specimens of 25-×2-×2-mm according to ISO standard -4049. Electron beam irradiation dose selected for the study were 1KGy, 3KGy and 5KGy. In total, 48 specimens were fabricated and divided into 4 groups based on radiation dose group I (non-radiated), group II (1KGy), group III (3KGy), group IV (5KGy) with 12 specimens in each. After 24 hours, specimens were subjected to 3-point bend test on a universal uniaxial servo mechanical testing machine. Statistical Analysis: Performed using one way ANOVA and inter group comparisons were done using tukeys multiple comparision. ‘P’ value<0.05 was considered statistically significant. Results: Flexural strength of Filtex Z350 XT before radiation was170.89±9.07.Flexural strength of Filtex Z350 XT after irradiation with 1KGy, 3KGy and 5KGy was found to be 269.06±94.91, 326.53±54.74 and 377.64±74.5 respectively. P value statistically significant (P<0.0001). Flexural strength of Ceram X duo before radiation was120.14±7.12.Flexural strength of Ceram X duo after irradiation with 1KGy, 3KGy and 5KGy was found to be 206.27±76.38, 177.31±67.35 and 229.33±59.15 respectively. P value statistically significant (P<0.0001). Conclusion: It can be concluded that electron beam irradiation can be used as a tool to modify the present day dental materials for enhancing their mechanical properties.

Objective To explore the efficacy of duo positive airway pressure ventilation in treating preterm infant with neonatal respiratory distress syndrome (RDS). Methods 65 preterm infants admitted to the neonatal intensive care unit from December 2012 to December 2013 were randomly divided into DuoPAP group (34 cases) or NCPAP (31 cases) group. Blood gas analysis(pH value, PaO2, PaCO2 and OI) at 1h, 12h, 24h, and 72h, and incidence of apnea, pulmonary air leak, repeated application of pulmonary surfactant and non-invasive ventilation failure at 72h in the two groups were measured and compared after using intubate surfactant extubate (INSURE). Results OI after non-invasive ventilation at 1h, 12h, and 24h was signiifcantly higherin DuoPAP group than that in NCPAP group P<0.01). PaCO2 after non-invasive ventilation at 1h, 12h, 24hand 72h was signiifcantly lower in DuoPAP group than that in NCPAP group (P<0.01). PaO2 after non-invasive ventilation at 1h and 12h was higher in DuoPAP group than that in NCPAP group, and there was signiifcant difference between the two groups (P<0.01). The incidence of non-invasive ventilation failure, apnea in 72h was lower in DuoPAP group than that in NCPAP group, and there was signiifcant difference between the two groups (P<0.05). Conclusions Compared with NCPAP, DuoPAP is more feasible for improving oxygenation, decreasing the need for mechanical ventilation in 72h in preterm infants with neonatal RDS.

Direct composite veneers can be used to mask tooth discolorations and/or to correct unaesthetic tooth forms and/or positions. However, these type of restorations are often regarded as one of the most challenging in aesthetic dentistry presumably due to the extent of natural tooth structure that must be recreated. This paper discusses easy application techniques and tips for Ceram.X Duo™, a nano ceramic composite restorative material. Its natural shading system allows the restoration of tooth with both dentine and enamel shade and transforms it into a final direct veneer restoration that mimics a natural tooth.

The article summed up academic thinking of LOU Duo-feng professor in treating rheumatism.Professor LOU thought that deficiency,pathogen and stasis were the fundamental pathogenesis of rheumatism,and suggested that we should handle deficiency,pathogen and stagnation as a whole in treating rheumatism.There were four treatment principles should be obey in treating rheumatism:firstly,strengthening the genuine-qi to eliminate the evil-qi,simultaneous treatment of principal and subordinate symptoms;Secondly,application of diffusing and obstruction-removing,choosing different medicine according to different location of disease;Thirdly,treatment in accordance with three types of disease causes,treating combing similarities and differences;Lastly,changes of prescriptions and principals according to different courses of disease.In this article,the author list Tongbitang,the basic prescription of Professor LOU in treating rheumatism,and his clinical experience in treating deficiency heat type of obstinate arthralgia.

Objective Estimate the paternity exclusion probability for 15 short tandem repeats (CODIS set plus Penta D and Penta E) in duo paternity tests. Methods 644 random individuals were paired to con-struct 207046 fictitious duo families. Cases of non-exclusion or with only one exclusionarylocus were count-ed. Results 27 cases could not be excluded by the 15 STR loci. The combined probability of exclusion was 0.999869. Number of cases showing only one exclusionary locus was 384 (0.185%). Conclusion To con-firm relationship in duo cases with only 15 STRs should be careful. Extensive genetic markers is needed to discriminate one locus exclusion or mutation.

RESUMO

The purpose of this study was to investigate the color stability of composite resins used widely as esthetic restorative material.Surefil(Dentsply,U.S.A.),Esthet X(Dentsply,U.S.A.),Filtek P60(3M,U.S.A.),Filtek Z250(3M,U.S.A.),Synergy Compact(Coltene,Switzerland),and Synergy Duo(Coltene,Switzerland)were chosen for this study.Thirty six specimens(7mm x2mm) of each composite resin were fabricated and polished after polymerization.Treatment conditions designed for this study were thermocycling,methylene blue staining,and filtered coffee staining. The color changes before and after treatment were measured by colorimeter(colorimeter,Model Tc-6Fx,Tokyo Denshoku Co.)and analyzed. The followings were drawn from this study: 1.The color changes of Filtek Z250,Surefil,Filtek P60 and Esthet X after thermocycling for 3 weeks were greater than those of the others. 2.The color changes of Surefil and Esthet X after methylene blue staining and those of Surefil, Filtek Z250 and Filtek P60 after coffee staining were great. 3.The color changes of Synergy Duo and Synergy Compact after thermocycling and coffee staining were relatively lesser than those of other composite resins.

RESUMO

The results of the gastroscopic examinations on 3973 patients of the aged,pre-aged,middle-aged,and adolescent groups were analyzed.It was found that the incidences of chronic atrophic gastritis (CAG),gastric cancer(GC),and gastric ulcer(Gu) in the aged group were 26.15%,16%,and 15.0% respectively,and that of intestinal metaplasia(IM) and dysplasia(DP) was 30.67%,all of which were higher than those of the pre-aged,middle-aged and adolescent groups(P
