Resumo O artigo discute questões sobre o futuro da humanidade ante as ameaças que rondam a saúde das populações, cujo impacto vem se exacerbando no curso das desigualdades em todas as partes do mundo, pari passu o desenvolvimento global no modelo hegemonizado a partir do século passado. A pandemia de COVID-19 foi tomada como um caso que bem ilustra essa dessintonia entre desenvolvimento e desigualdades. Formulam-se perguntas a serem postas em debate sobre a construção do futuro da sociedade mundial, com base na acepção sobre o caráter evolucional da vida no planeta vis-à-vis os males que acometem grandes contingentes populacionais e representam poderosos riscos para esse processo evolutivo. São indagações que apontam para a discussão em torno da participação social na definição e no controle das políticas públicas, em contrapartida à hegemonia dos interesses privados na formulação e execução dessas políticas, tanto nos cenários de cada país como no contexto internacional.


Abstract This article discusses questions concerning the future of humanity in the face of threats to the health of populations, whose impact has been exacerbated in the course of inequalities in all parts of the world, pari passu with global development in the hegemonized model since last century. The COVID-19 pandemic is a good example that illustrates this dissonance between development and inequalities. Questions were formulated to be debated about the construction of the future of world society, based on the understanding of the evolutionary character of life on the planet vis-à-vis the evils that affect large contingents of the population and represent powerful risks for this evolutionary process. These questions call attention to the discussion around social participation in the definition and control of public policies, as opposed to the hegemony of private interests in the formulation and execution of these policies, both in the scenarios of each country and in the international context.

So far, the monoclonal hypothesis of tumor occurrence and development cannot be justified. The genetic diversity selection hypothesis for the occurrence and development of lung cancer links Mendelian genetics with Darwin's theory of evolution, suggesting that the genetic diversity of tumor cell populations with polyclonal origins-monoclonal selection-subclonal expansion is the result of selection pressure. Normal cells acquire mutations in oncogenic driver genes and have a selective advantage over other cells, becoming tumor initiating cells; In the interaction with the tumor microenvironment (TME), the vast majority of initiating cells are recognized and killed by the human immune system. If immune escape occurs, the incidence of malignant tumors will greatly increase, and subclonal expansion, intratumour heterogeneity, etc. will occur. This article proposed the hypothesis of genetic diversity selection and analyzed its clinical significance.

Simmering method is one of the traditional processing methods of Chinese materia medica, which has been documented in the herbal literature and medical books of the past dynasties and has a great variety, but at present, there are not many specific varieties of Chinese materia medica involved, and there are few related researches. By reviewing the ancient and modern related information, the authors have organized and analyzed the historical evolution, processing purpose, modern representative Chinese materia medica(processing technology, quality evaluation, pharmacological research) of simmering method. After sorting out, it was found that the simmering method was widely used in ancient times, which was first seen in Huashi Zhongzangjing of the Eastern Han dynasty, and was enriched and developed through the Tang, Song, Jin, Yuan, Ming and Qing dynasties, and entered its heyday in Ming and Qing dynasties along with the economic prosperity and development of the Ming dynasty, involving as many as 159 varieties of Chinese materia medica, and gradually perfecting the processing theory of the simmering method. However, the number of varieties that still use the simmering method in modern times significantly decreased. The main purposes of using simmering method in modern Chinese materia medica are to reduce adverse reactions, moderate medicinal properties, enhance therapeutic effects, remove non-medicinal parts, and facilitate further processing, etc. This paper combed the key information of simmering methods for Chinese materia medica from ancient to modern times, which can provide a literature basis for the clinical application and modern research of simmered products of Chinese materia medica.

Tuoli Xiaodusan is the 65th formula in the Catalogue of Ancient Famous Classical Formulas(First Batch). In this study, the bibliometric method was used to summarize and verify the ancient books about Tuoli Xiaodusan in terms of its historical origin, composition and dosage of the formula, indications, decoction and administration method, and processing, etc. According to the research, there is no definite date of the formation of Tuoli Xiaodusan, the earliest can be traced back to Lizhai Waike Fahui in Ming dynasty, which has been widely circulated, with many versions of prescription composition, and the modern influential version is from Waike Zhengzong in Ming dynasty, which is made up of 12 Chinese herbs including Ginseng Radix et Rhizoma(3.73 g), Chuanxiong Rhizoma(3.73 g), Paeoniae Radix Alba(3.73 g), Astragali Radix(3.73 g), Angelicae Sinensis Radix(3.73 g), Atractylodis Macrocephalae Rhizoma(3.73 g), Poria(3.73 g), Lonicerae Japonicae Flos(3.73 g), Angelicae Dahuricae Radix(1.87 g), Glycyrrhizae Radix et Rhizoma(1.87 g), Gleditsiae Spina(1.87 g), Platycodonis Radix(1.87 g). The herb origins almost follow the 2020 edition of Chinese Pharmacopoeia, except that Angelica dahurica var. formosana is only recommended as the origin of Angelicae Dahuricae Radix, and Glycryyhiza uralensis is only recommended as the origin of Glycyrrhizae Radix et Rhizoma. All the herbs are recommended to be used in the raw products. As for the preparation method, it is recommended to decoct with water, add 400 mL of water, boil until 160 mL, and take 2-3 times a day. The formula has the functions of nourishing Qi and nourishing blood, detoxifying and draining pus, and was mainly used to treat ulcerative diseases with the syndrome of syndrome of healthy Qi deficiency and pathogenic factors excess in ancient times, and in modern times, it is used for a wide range of treatment, involving the skin and soft tissues, bones, digestion and many other systemic diseases, and is also mainly used for syndrome of healthy Qi deficiency and pathogenic factors excess. In this study, the ancient and modern applications of Tuoli Xiaodusan were summarized, and its key information was identified, providing a basis for its wider clinical application, in-depth research and formulation development.

Yigongsan is derived from Xiaoer Yaozheng Zhijue written by QIAN Yi in the Northern Song dynasty, which is the No. 3 formula in the Catalogue of Ancient Famous Classical Formulas(The Second Batch of Pediatrics) released by the National Administration of Traditional Chinese Medicine(TCM) in September 2022, and it can be developed as a class 3.1 new TCM drug. By referring to ancient medical books and modern literature, this study conducted herbal textual research on Yigongsan from five aspects, including historical evolution, origin and processing, dosage conversion, usage and preparation methods, and functional application, then formed the key information table of this formula, in order to provide reference for the development of reference samples and preparations of Yigongsan. Based on the results of the study, it is recommended that Panax ginseng should be removed the basal part of stem(rhizoma), Poria cocos should be removed the peel, Citrus reticulata should be cut into shreds and Glycyrrhiza uralensis should be used. According to 4.13 g/Qian(钱), 1 g/slice for ginger, 3 g for each jujube and 300 mL/Zhan(盏), the doses of Ginseng Radix, Poria, Atractylodis Macrocephalae Rhizoma, Glycyrrhizae Radix et Rhizoma, Citri Reticulatae Pericarpium, Zingiberis Rhizoma Recens, Jujubae Fructus were 1.652, 1.652, 1.652, 1.652, 1.652, 5, 6 g, and the total amount was 19.26 g. The decocting method was to crush the medicinal materials into fine powder with 50-80 mesh, add 300 mL of water and decoct to 210 mL for each dose, then remove the dregs and take it warmly. This formula was recorded in ancient books as the main treatment for the cold-deficiency of spleen and stomach, and Qi stagnation in children with vomiting and diarrhea and lack of appetite. It has been flexibly applied by later generations of physicians, and is often used to treat anorexia, inflammation of the digestive tract, diarrhea and other diseases in children.

By consulting the ancient Chinese herbal books, medical books and formularies of the past dynasties, and combining with modern research data, this paper makes a systematic textual research on the name, origin, place of origin, traditional quality evaluation, harvesting and processing of Selaginellae Herba, so as to provide a basis for the development and utilization of the relevant famous classical formulas. According to the textual research, Juanbai is the correct name of the herbal medicine in all dynasties, and there are also aliases such as Baozu, Qiugu, Jiaoshi and Jiusi Huanhuncao. The origin of Selaginellae Herba in the ancient herbal books was Selaginella tamariscina in all dynasties. Since the Republic of China, S. pulvinata has been gradually used as another origin of Selaginellae Herba. In ancient times, the producing area of S. tamariscina was mainly in Shandong, Hebei, Henan, Shaanxi, Jiangsu and Sichuan, etc. Nowadays, it is produced all over the country. S. pulvinata is mainly produced in Guangxi, Fujian, Sichuan, Guizhou, Yunnan, Hebei and so on. Since the recent times, it is concluded that the quality of the green color, complete and unbroken is good. Before the Qing dynasty, it was recorded that the harvesting time of Selaginellae Herba was generally from April to July, and it was expanded to all year round since the Qing dynasty. After harvesting, remove the sediment(sand and mud), cut off the fibrous roots and dry in the shade or in the sun. The processing methods in all dynasties were mainly carbonizing by stir-frying and stir-bake to brown, and some ancient books contained the processing method of brine boiling, which was rarely used in modern times. Based on the results, it is recommended that S. tamariscina should be used as the base material of Selaginellae Herba. Because of more impurities, it should be fully purified to ensure the cleanliness of the herb, and the processing method can be based on the prescription requirements, if the processing requirements are not specified, the raw products can be used, charcoal products is recommended for use as an hemostatic.

By reviewing the ancient and modern literature, the name, origin, scientific name evolution, place of origin, quality, harvesting, processing, efficacy and toxicity of Asteris Radix et Rhizoma(ARR) were systematically sorted out, so as to provide reference for the development and utilization of the relevant famous classical formulas. According to textual research, ARR was first contained in Shennong Bencaojing, all generations are Ziwan for its proper name, and there are still aliases such as Ziyuan, Ziqian and Xiaobianer. Its mainstream origin in successive generations was Aster tataricus, and there are also Ligularia fischeri and others in local area of use. The medicinal parts of ARR are root and rhizome, but in modern times, the rhizome is mostly used for propagation and cultivation, so some of ARR medicinal materials only have the root without the rhizome. The earliest recorded ancient origin of ARR was now Fangxian(Hubei), Zhengding and Handan(Heibei), then the range of production areas gradually expanded, the mainstream production areas from the Song dynasty to the Ming and Qing dynasties included Hebei, Jiangsu, Anhui, Henan and other places, since modern times, two major producing areas have been formed in Anguo, Hebei province and Bozhou, Anhui province. From the quality evaluation, it is clear that from ancient times, flexible roots and purple color are the best. The ancient harvesting was mainly in lunar February or March, and then dried in the shade, and the modern harvesting is mostly in spring and autumn, and the roots are braided into pigtails and then dried in the sun or dried in the sun after 1-2 d. The ancient and modern processing method of ARR are basically the same, mainly honey processing, there are still methods of frying, steaming, vinegar sizzling, etc. Based on the results, it is recommended that the dried roots and rhizomes of A. tataricus should be used in clinical and the development of related famous classical formulas, and those whose original formulas specify the processing requirements can be processed according to the relevant requirements, while whose processing requirements are not specified should be used in the form of raw products.

RESUMO

DNA-encoded chemical library (DEL) links the power of amplifiable genetics and the non-self-replicating chemical phenotypes, generating a diverse chemical world. In analogy with the biological world, the DEL world can evolve by using a chemical central dogma, wherein DNA replicates using the PCR reactions to amplify the genetic codes, DNA sequencing transcripts the genetic information, and DNA-compatible synthesis translates into chemical phenotypes. Importantly, DNA-compatible synthesis is the key to expanding the DEL chemical space. Besides, the evolution-driven selection system pushes the chemicals to evolve under the selective pressure, i.e., desired selection strategies. In this perspective, we summarized recent advances in expanding DEL synthetic toolbox and panning strategies, which will shed light on the drug discovery harnessing in vitro evolution of chemicals via DEL.

The seat of human intelligence is the human cerebral cortex, which is responsible for our exceptional cognitive abilities. Identifying principles that lead to the development of the large-sized human cerebral cortex will shed light on what makes the human brain and species so special. The remarkable increase in the number of human cortical pyramidal neurons and the size of the human cerebral cortex is mainly because human cortical radial glial cells, primary neural stem cells in the cortex, generate cortical pyramidal neurons for more than 130 days, whereas the same process takes only about 7 days in mice. The molecular mechanisms underlying this difference are largely unknown. Here, we found that bone morphogenic protein 7 (BMP7) is expressed by increasing the number of cortical radial glial cells during mammalian evolution (mouse, ferret, monkey, and human). BMP7 expression in cortical radial glial cells promotes neurogenesis, inhibits gliogenesis, and thereby increases the length of the neurogenic period, whereas Sonic Hedgehog (SHH) signaling promotes cortical gliogenesis. We demonstrate that BMP7 signaling and SHH signaling mutually inhibit each other through regulation of GLI3 repressor formation. We propose that BMP7 drives the evolutionary expansion of the mammalian cortex by increasing the length of the neurogenic period.

ObjectiveTo sort out the historical evolution, prescription evolution and modern clinical application of Huagaisan. MethodHuagaisan and its synonym Huagaitang are used as keywords to search the databases of Traditional Chinese Medicine Think Tank, Chinese Medical Dictionary, Airusheng Chinese Medical Database and China National Knowledge Infrastructure(CNKI). According to the inclusion and exclusion criteria, we obtained the information of ancient books and modern clinical research literature related to Huagaisan, and systematically reviewed and analyzed the historical origin, prescription composition, preparation method, dosage, efficacy, medicinal material origin, processing method and modern clinical application of Huagaisan. ResultA total of 198 pieces of ancient book information were included, involving 93 ancient Chinese medicine books. Huagaisan was composed of fried Perillae Fructus, red Poria, fried Mori Cortex, Citri Eoxcarpium Rubrum, stir-fried Armeniacae Semen Amarum, Ephedrae Herba and fried Glycyrrhizae Radix et Rhizoma, which had the efficacy of promoting the lungs and relieving epidemiological symptoms, expelling phlegm and relieving cough, and treating cough with wind-cold bundled epidemiological symptoms and stagnation of phlegm and Qi. The preparation method was suggested as boiling powder, crushing the seven herbs into coarse particles, the dosage of each drug was fried Perillae Fructus of 1.27 g, red Poria of 1.27 g, fried Mori Cortex of 1.27 g, Citri Eoxcarpium Rubrum of 1.27 g, stir-fried Armeniacae Semen Amarum of 1.27 g, Ephedrae Herba of 1.27 g and fried Glycyrrhizae Radix et Rhizoma of 0.64 g, taking 8.26 g when decocting, adding 300 mL of water, decocting to 210 mL, removing the dregs, and taking it warmly after meals. Twenty-one clinical research papers were included to analyze the modern clinical application of Huagaisan, which was mainly used in the treatment of respiratory diseases such as pneumonia, asthma, bronchitis and so on. ConclusionThis paper has verified and summarized the key information of the famous classical formula Huagaisan, which can provide a detailed reference basis for the development and clinical application of its compound preparation.

By consulting ancient herbal books and modern literature, this paper systematically sorted out and researched the processing history, relevant processing norms in recent years, modern processing technology, chemical composition changes of processed products and their pharmacological mechanism of Scutellariae Radix, in order to provide a basis for the further development of Scutellariae Radix decoction pieces. According to the textual research of ancient books, there were many kinds of processing auxiliary materials of Scutellariae Radix, such as wine, vinegar, salt, honey, pig bile and so on, among which the wine processing was the most diverse and detailed, and the processed products such as raw products, stir-fried products, wine-processed products, fried charcoal products were still in use. The modern processing techniques of Scutellariae Radix mainly focus on the processing aspects of softening and slicing, wine processing and charcoal frying, and the research methods are relatively unified. At present, it is found that the changed chemical constituents of Scutellariae Radix after processing are flavonoids, polysaccharides, volatile oils and trace elements, etc. Pharmacological effects of processed products are hemostasis, antibacterial, anti-inflammatory, antioxidant, analgesic and antipyretic, treatment of lung diseases, treatment of colitis, etc. However, in the studies of Scutellariae Radix processing, there is a lack of research on the structural changes of chemical components caused by processing and a comprehensive comparative study on the pharmacological effects of various processed products. Based on this, it is suggested to carry out systematic research on the processing technology to processing mechanism, further explore the relationship between the change rule of material basis and pharmacological action before and after processing of Scutellariae Radix, and deepen the exploration of molecular mechanism and clinical application of processed products of Scutellariae Radix, in order to clarify the scientific connotation of the processing mechanism of Scutellariae Radix, and lay a foundation for the subsequent expansion of the application of Scutellariae Radix decoction pieces and the formulation of processing standards.

Rabies is a fatal zoonotic disease that affects several mammals. Hematophagous bats are recognized hosts of the rabies virus, and their main food source is the blood of other mammals, particularly cattle. During feeding, bats transmit the virus to cattle, which are victims of the disease, contributing to economic losses and increasing the risk of infection for humans. Based on this affinity in the rabies cycle between bats and cattle, the objective of this study was to analyze the phylogenetic relationships of rabies virus samples in cattle and bats. The G gene of the rabies virus was chosen for this study because it is directly related to the infection process. Nucleotide sequences of the viral G gene were selected from GenBank for samples obtained from infected cattle and bats. Maximum parsimony analyses were conducted using the Molecular Evolutionary Genetics Analysis software. The Maxima Parsimony tree indicated a phylogenetic relationship between the G genes of both hosts, indicating that the virus evolved from bats to cattle. Analysis of parsimoniously informative sites revealed that the viral G gene carried specific mutations in each host. Knowledge of the evolutionary relationships between the rabies virus and its hosts is critical for identifying potential new hosts and the possible routes of infection for humans.


A Raiva é uma zoonose fatal que infecta várias espécies de mamíferos. Os morcegos hematófagos são reconhecidos como hospedeiros do vírus da Raiva e sua principal fonte de alimento é o sangue de outros mamíferos, especialmente os bovinos. Quando se alimentam, os morcegos transmitem o vírus para o bovino os quais são vítimas da doença, contribuindo para perdas econômicas e riscos de infecção para humanos. Baseado nesta afinidade do ciclo da Raiva entre morcegos e bovinos, o objetivo deste estudo foi analisar as relações filogenéticas de amostras do vírus da Raiva em ambos os hospedeiros, bovinos e morcegos. O gene G do vírus da Raiva foi escolhido para esta pesquisa porque ele está diretamente relacionado ao processo de infecção. Sequências de nucleotídeos do gene G viral foram selecionadas no GenBank a partir de amostras obtidas de bovinos e morcegos infectados. Análises de Máxima Parcimônia foram conduzidas utilizando o software Molecular Evolutionary Genetics Analysis. A árvore de Máxima Parcimônia indicou uma relação filogenética entre o gene G de ambos os hospedeiros, indicando que o vírus evoluiu dos morcegos para os bovinos. A análise dos sítios parcimoniosamente informativos revelou que o gene G viral apresentou mutações específicas em cada hospedeiro. O conhecimento sobre as relações evolutivas do vírus da Raiva e seus hospedeiros é crucial para identificar nos hospedeiros potenciais e novas rotas possíveis de infecção para humanos.


La rabia es una zoonosis fatal que infecta a varias especies de mamíferos. Los murciélagos hematófagos son reconocidos como huéspedes del virus de la rabia y su principal fuente de alimentación es la sangre de otros mamíferos, especialmente del ganado. Al alimentarse, los murciélagos transmiten el virus al ganado que es víctima de la enfermedad, contribuyendo a pérdidas económicas y riesgos de infección para los humanos. Basado en esta afinidad del ciclo de la rabia entre murciélagos y ganado, el objetivo de este estudio fue analizar las relaciones filogenéticas de las muestras de virus de la rabia tanto en huéspedes, ganado y murciélagos. El gen G del virus de la rabia fue elegido para esta investigación porque está directamente relacionado con el proceso de infección. Las secuencias de nucleótidos del gen G viral se seleccionaron en GenBank a partir de muestras obtenidas de bovinos y murciélagos infectados. Los análisis de parsimonia máxima se realizaron utilizando el software Molecular Evolutionary Genetics Analysis. El árbol de Máxima Parsimônia indicó una relación filogenética entre el gen G de ambos huéspedes, indicando que el virus evolucionó de murciélagos a bovinos. El análisis de los sitios parsimoniosamente informativos reveló que el gen G viral presentaba mutaciones específicas en cada huésped. El conocimiento sobre las relaciones evolutivas del virus de la rabia y sus huéspedes es crucial para identificar huéspedes potenciales y nuevas posibles rutas de infección para humanos.

ABSTRACT Traumatic atlanto-occipital dislocation is a rare, severe, and usually fatal injury. In this paper, we report the case of a 28-year-old patient with multiple trauma, who suffered an atlanto-occipital dislocation (AOD) associated with rotatory dislocation at C1-C2, condyle fracture and traumatic brain injury resulting from a car accident, with a cardiopulmonary arrest at the accident site. The patient had motor and sensory deficits, suffering other complications of clinical severity during hospitalization. After two years of surgical treatment follow-up, the patient had a complete recovery of neuromotor and sensory functions. This case demonstrates that neurological recovery with minimal sequelae is possible, even after an unfavorable prognosis resulting from a severe injury with a high risk of death as this kind of trauma. It is essential for health professionals to have the ability to identify and treat AOD, ensuring improved clinical outcomes, reducing mortality and morbidity, and providing a better life to affected patients. Resumo: A luxação traumática atlanto-occipital (LTAO) é uma lesão rara, grave e com alto índice de mortalidade. Neste artigo relatamos o caso de um paciente de 28 anos, politraumatizado, que sofreu uma LTAO associada à luxação rotatória em C1-C2, à avulsão de côndilo e ao traumatismo crânio encefálico decorrente de acidente automobilístico, com relato de uma parada cardiorrespiratória no local do acidente. O paciente deu entrada no serviço com déficit motor e sensitivo, sofrendo outras intercorrências de gravidade clínica durante a internação. Após dois anos do tratamento cirúrgico, o paciente evoluiu favoravelmente com completa recuperação das alterações neuromotoras e sensitivas. O caso apresentado demonstra que é possível a recuperação neurológica com sequelas mínimas, mesmo após um prognóstico desfavorável decorrente de uma lesão grave e o elevado risco de morte decorrente desse tipo de trauma. É essencial que os profissionais de saúde estejam aptos a identificar e tratar a LTAO, garantindo uma melhorar dos resultados clínicos, redução da mortalidade e morbidade, além de proporcionar uma melhor qualidade de vida aos pacientes afetados. Resumen: La luxación traumática atlanto-occipital (LTAO) es una lesión grave, poco frecuente y con una elevada tasa de mortalidad. En este artículo presentamos el caso de un paciente politraumatizado de 28 años que sufrió una LTAO asociada a luxación rotatoria en C1-C2, a la avulsión de cóndilos y traumatismo craneoencefálico como consecuencia de un accidente de tráfico, con parada cardiorrespiratoria en el lugar del accidente. El paciente presentaba déficits motores y sensoriales y sufrió otras complicaciones clínicas graves durante su hospitalización. Luego de dos años de tratamiento quirúrgico, el paciente evoluciona favorablemente con recuperación completa de las alteraciones neuromotoras y sensitivas. El caso presentado demuestra que la recuperación neurológica con mínimas secuelas es posible, incluso después de un pronóstico desfavorable debido a una lesión grave y al alto riesgo de muerte derivado de este tipo de traumatismos. Es esencial que los profesionales sanitarios sean capaces de identificar y tratar la LTAO, garantizando mejores resultados clínicos, una reducción de la mortalidad y la morbilidad, y una mejor calidad de vida para los pacientes afectados. ABSTRACT The purpose of this review is to highlight the most important aspects of the anatomical and functional uniqueness of the human brain. For this, a comparison is made between our brains and those of our closest ancestors (chimpanzees and bonobos) and human ancestors. During human evolution, several changes occurred in the brain, such as an absolute increase in brain size and number of cortical neurons, in addition to a greater degree of functional lateralization and anatomical asymmetry. Also, the cortical cytoarchitecture became more diversified and there was an increase in the number of intracortical networks and networks extending from the cerebral cortex to subcortical structures, with more neural networks being invested in multisensory and sensory-motor-affective-cognitive integration. These changes permitted more complex, flexible and versatile cognitive abilities and social behavior, such as shared intentionality and symbolic articulated language, which, in turn, made possible the formation of larger social groups and cumulative cultural evolution that are characteristic of our species.


RESUMO Esta revisão se propõe a relatar os aspectos mais importantes da singularidade anatômica e funcional do cérebro humano. Para isso, faz-se uma comparação entre o nosso cérebro e os de nossos parentes evolutivos mais próximos (chimpanzés e bonobos) e os ancestrais humanos. Durante a evolução humana ocorreu aumento absoluto do tamanho do cérebro e do número de neurônios corticais cerebrais, maior grau de lateralização funcional e assimetria anatômica cerebral, citoarquitetura cortical mais diversificada e aumento das redes neurais intracorticais e do córtex cerebral para as estruturas subcorticais acompanhada de mudança em direção ao investimento de redes neurais na integração multissensorial e sensório-motora-afetiva-cognitiva. Essas mudanças possibilitaram capacidades cognitivas e comportamentos sociais complexos, flexíveis e versáteis, destacando-se a intencionalidade compartilhada e a linguagem articulada simbólica, que permitiram a formação de grupos sociais maiores e a evolução cultural cumulativa característica de nossa espécie.

ABSTRACT This study aimed to provide further insight into the evolutionary dynamics of SARS-CoV-2 by analyzing the case of a 40-year-old man who had previously undergone autologous hematopoietic stem cell transplantation due to a diffuse large B-cell lymphoma. He developed a persistent SARS-CoV-2 infection lasting at least 218 days and did not manifest a humoral immune response to the virus during this follow-up period. Whole-genome sequencing and viral cultures confirmed a persistent infection with a replication-positive virus that had undergone genetic variation for at least 196 days after symptom onset.

ABSTRACT Noroviruses are highly infectious, genetically diverse viruses. Global outbreaks occur frequently, making molecular surveillance important for infection monitoring. This cross-sectional descriptive study aimed to monitor cases of norovirus gastroenteritis in the Brazilian Amazon. Fecal samples were tested by immunoenzymatic assay, RT-PCR and genetic sequencing for the ORF1/ORF2 and protease regions. Bayesian inference with a molecular clock was employed to construct the phylogeny. The norovirus prevalence was 25.8%, with a higher positivity rate among children aged 0-24 months. Genogroup GII accounted for 98.1% of the sequenced samples, while GI accounted for 1.9% of them. The GII.P16/GII.4 genotype was the most prevalent, with an evolution rate of 2.87x10−3 and TMRCA estimated in 2012. This study demonstrates that norovirus is a primary causative agent of gastroenteritis and provides data on viral genetic diversity that may facilitate infection surveillance and vaccine development.

This narrative review explores the diverse representations of epilepsy in art across different historical periods, examining the intersection of artistic expression with evolving cultural, medical, and societal perceptions. Beginning with ancient and medieval depictions intertwined with religious symbolism, the paper progresses through the Renaissance, and Baroque, and transitions into modern and avant-garde movements. Each period reflects changing attitudes towards epilepsy, from divine interpretations, and religious concerns to anatomical realism and, later, explorations of psychological themes. The contemporary lens focuses on inclusive and empathetic portrayals, utilizing digital media to challenge stigmas and foster understanding.


Esta revisão narrativa explora as diversas representações da epilepsia na arte em diferentes períodos históricos, examinando a intersecção da expressão artística com a evolução das percepções culturais, médicas e sociais. Começando com representações antigas e medievais entrelaçadas com simbolismo religioso, o artigo avança através da Renascença e do Barroco, e transita para movimentos modernos e de vanguarda. Cada período reflete mudanças de atitudes em relação à epilepsia, desde interpretações divinas e preocupações religiosas até o realismo anatômico e, mais tarde, explorações de temas psicológicos. A lente contemporânea centra-se em representações inclusivas e empáticas, utilizando meios digitais para desafiar estigmas e promover a compreensão.

Introdução: A avaliação fonoaudiológica hospitalar tem um importante papel na prevenção e manejo de pacientes com risco de broncoaspiração. No entanto, nem sempre cabe ao fonoaudiólogo a primeira avaliação e definição da via alimentar nos pacientes hospitalares. Objetivo: Comparar as decisões fonoaudiológicas e médicas quanto à viabilidade da via alimentar em um hospital geral e identificar fatores associados com a melhora da deglutição. Métodos: Trata-se de um estudo retrospectivo de pacientes internados em hospital em Joinville durante março a agosto de 2018. A via alimentar foi considerada com base na Functional Oral Intake Scale (FOIS) sendo a primeira decisão comparada entre o fonoaudiólogo e o médico para o mesmo paciente. Resultados: Dos 171 pacientes, houve maior concordância entre as condutas médica e fonoaudiológica para alimentação por sonda nasoenteral (SNE) (88,7%) e alimentação oral livre (81,9%). No entanto, houve apenas 35% de concordância na definição de dieta adaptada, sendo a concordância geral moderada (Kappa 0,486). Houve evolução na alimentação por via oral em 62 pacientes (36%). Maior limitação da via alimentar, verificado pela necessidade de SNE (OR = 3,17; p = 0,025) e o maior número de atendimentos fonoaudiológicos intra-hospitalares (OR = 1,09; p = 0,020) foram associados com a melhora da disfagia. Conclusão: Encontrou-se concordância entre a avaliação dietética de casos para uso de SNE ou dieta livre entre o fonoaudiólogo e médico. Uso de SNE, como indicador de gravidade do paciente, e o maior número de sessões de fonoterapia foram associados com a melhora da disfagia durante a internação. (AU)


Introduction: Hospital-based speech-language pathology plays a crucial role in preventing and managing patients at risk of bronchoaspiration. However, the initial evaluation and determination of the feeding route in hospitalized patients may not always fall under the responsibility of the speech-language pathologist. Objective: To compare the decisions of speech-language pathologists and medical professionals regarding the feasibility of the feeding route in a general hospital and identify factors associated with swallowing improvement. Methods: This is a retrospective study of patients admitted to a hospital in Joinville from March to August 2018. The feeding route was assessed based on the Functional Oral Intake Scale (FOIS), with the initial decision compared between the speech-language pathologist and the physician for the same patient. Results: Among 171 patients, there was higher agreement between medical and speech-language pathology decisions for nasoenteral tube feeding (88.7%) and oral unrestricted intake (81.9%). However, there was only 35% agreement in defining an adapted diet, with overall moderate agreement (Kappa 0.486). Oral feeding improved in 62 patients (36%). The presence of nasoenteral tube feeding (OR = 3.17; p = 0.025) and a higher number of in-hospital speech-language pathology appointments (OR = 1.09; p = 0.020) were identified as independent predictors for dysphagia improvement. Conclusion: Concordance was found in the dietary assessment for the use of nasoenteral tube feeding or oral unrestricted intake between speech-language pathologists and physicians. The use of nasoenteral tube feeding as an indicator of patient severity and a higher number of speech-language pathology sessions were associated with dysphagia improvement during hospitalization. Sin embargo, no siempre corresponde al fonoaudiólogo la primera evaluación y definición de la vía alimentaria en los pacientes hospitalizados.Objetivo: Comparar las decisiones fonoaudiológicas y médicas sobre la viabilidad de la vía alimentaria en un hospital general e identificar factores asociados con la mejora de la deglución. Métodos: Se trata de un estudio retrospectivo de pacientes hospitalizados en un hospital de Joinville durante marzo a agosto de 2018. La vía alimentaria se evaluó según la Escala Funcional de Ingesta Oral (FOIS), siendo la primera decisión comparada entre el fonoaudiólogo y el médico para el mismo paciente. Resultados: De 171 pacientes, hubo una mayor concordancia entre las decisiones médicas y fonoaudiológicas para la alimentación por sonda nasoenteral (SNE) (88,7%) y la alimentación oral libre (81,9%). Sin embargo, solo hubo un 35% de concordancia en la definición de una dieta adaptada, siendo la concordancia general moderada (Kappa 0,486). La alimentación oral mejoró en 62 pacientes (36%). Una mayor limitación de la vía alimentaria, indicada por la necesidad de SNE (OR = 3,17; p = 0,025), y un mayor número de sesiones fonoaudiológicas intrahospitalarias (OR = 1,09; p = 0,020) se asociaron con la mejora de la disfagia.Conclusión: Se encontró concordancia en la evaluación dietética para el uso de SNE o dieta libre entre el fonoaudiólogo y el médico. Introducción: Las complicaciones, asociadas al brote anormal de los terceros molares, es posible evitarlas con la germenectomía de estos dientes. Objetivo: Caracterizar la evolución de la germenectomía de terceros molares en pacientes con diagnóstico cefalométrico de brote anormal. Métodos: Se realizó un estudio observacional, descriptivo y transversal en el Servicio de Cirugía Maxilofacial del Hospital Provincial "Saturnino Lora Torres", desde febrero de 2019 hasta marzo de 2021. Se estudiaron 32 pacientes de 13 a 15 años de edad, interviniéndose quirúrgicamente 78 terceros molares. Las variables fueron edad, sexo, color de la piel, localización y posición del tercer molar; así como las complicaciones trans y posoperatorias y la evolución. Resultados: Entre los 32 pacientes incluidos en el estudio predominó el sexo femenino (62,5 por ciento) y la edad de 14 años (40,6 por ciento). Las complicaciones ocurridas durante la germenectomía fueron más frecuentes en la arcada inferior. Aunque no fueron detectadas diferencias estadísticamente significativas según localización (valor de p>0,05); con 30 transoperatorias para un 38,4 por ciento y 75 posoperatorias para un 96,2 por ciento. Conclusiones: Se evidencia una evolución marcadamente favorable posterior a la germenectomía de terceros molares en la población de pacientes con diagnóstico cefalométrico de brote anormal. En ellos el sangrado transoperatorio y el dolor posoperatorio constituyen las complicaciones a considerar en este tipo de procedimiento quirúrgico(AU)


Introduction: Complications associated with the abnormal eruption of third molars can be avoided with germenectomy of these teeth. Objective: To characterize the evolution of third molar germenectomy in patients with a cephalometric diagnosis of abnormal bud. Methods: An observational, descriptive and cross-sectional study was carried out in the Maxillofacial Surgery Service at Saturnino Lora Torres Provincial Hospital, from February 2019 to March 2021. Thirty two patients aged 13 to 15 years were studied, with 78 third molars undergoing surgery. The variables were age, sex, skin color, location and position of the third molar; as well as trans and postoperative complications and evolution. Results: Among the 32 patients included in the study, the female sex (62.5 percent) and the age of 14 years (40.6 percent) predominated. Complications that occurred during germenectomy were more frequent in the lower arch. Although no statistically significant differences were perceived according to location (p value > 0.05); with 30 intraoperative for 38.4 percent and 75 postoperative for 96.2 percent. Conclusions: There is evidence of a markedly favorable evolution after germenectomy of third molars in the population of patients with cephalometric diagnosis of abnormal bud. In them, transoperative bleeding and postoperative pain are the complications to consider in this type of surgical procedure(AU)

INTRODUCCIÓN: La evolución de la virulencia ha despertado el interés de la biología evolutiva por décadas. Se trata de co-evolución entre agentes patógenos y sus hospederos. Se han planteado diferentes hipótesis para la evolución de la virulencia, entre ellas la evolución con un compromiso virulencia-transmisión. OBJETIVO: Estudiar la evolución de la letalidad y la transmisión del SARS CoV-2 en Chile, con la hipótesis que ha existido un compromiso con disminución de la letalidad y aumento de la transmisión de esta durante la epidemia. RESULTADOS: La letalidad diaria en Chile disminuyó claramente. Aunque no se encontró correlación entre la letalidad diaria y el número reproductivo efectivo, el número reproductivo efectivo máximo asociado a cada variante viral, presentó un aumento claro desde la aparición de la variante γ en adelante (δ y O) en Chile. CONCLUSIÓN: Este estudio muestra una evolución de la virulencia con un compromiso virulenciatransmisión. Esto puede ser explicado en gran parte por la evolución viral, pero el cambio en letalidad también puede ser afectado por las intervenciones no farmacológicas y farmacológicas realizadas por la población humana.


BACKGROUND: The evolution of virulence has aroused the interest of evolutionary biology for decades. It is about co-evolution between pathogens and their hosts. Different hypotheses have been put forward for the evolution of virulence, including evolution with a virulence-transmission trade-off. AIM: To study the evolution of the fatality rate and transmission of SARS CoV-2 in Chile, with the hypothesis that there has been a trade-off to a decrease in lethality and an increase in its transmission during the epidemic. RESULTS: The daily fatality rate in Chile clearly decreased. Although no correlation was found between daily fatality rate and effective reproductive number, the maximum effective reproductive number associated with each viral variant showed a clear increase from the appearance of the γ variant onwards (δ and O) in Chile. CONCLUSION: This study shows an evolution of virulence with a virulence-transmission trade-off. This can be largely explained by viral evolution, but the change in lethality can also be affected by non-pharmacological and pharmacological interventions carried out by the human population.
