RESUMO

Eccrine hidrocystomas are small benign cystic lesions that normally affect only adult females. Eccrine hidrocystoma is characterized by tense vesicles that are predominately located on the face and particularly on the periorbital areas. Histologically, eccrine hidrocystoma consists of one or more partially collapsed unilocular cysts in the dermis, and these cysts are often situated adjacent to normal eccrine ducts. An 8-year-old boy presented with multiple skin-colored to bluish tiny papules on his nose. Histopathologically, the dilated partial cystic structures in the middermis were lined by one or two layers of flattened or cuboidal epithelial cells. We report here on an atypical presentation of multiple eccrine hidrocystomas that were localized on the nose of a child.

RESUMO

Eccrine hidrocystomas are benign cystic dilatations with eccrine differentiation. The clinical presentation usually occurs as a solitary tense vesicle or translucent papule located predominantly on the face, whereas multiple eccrine hidrocystomas are unusual. Histologically, eccrine hidrocystoma consists of one or more partially collapsed unilocular cysts in the dermis, which show flattened epithelium that is partially lined by two layer of cuboidal epithelium. We report here on a case of the patient with multiple eccrine hidrocystomas that showed an especially generalized presentation that included the face, neck, anterior chest and both arms.

RESUMO

We report a case of intratumoral fat in neurofibroma and coexistance of eccrine hidrocystoma on the occipital area of the scalp for one year duration. There are several diseases showing fatty change histopathologically. Among them, few cases of neurofibroma showing fatty change had been reported. The mechanism of fatty change is unclear, but several hypotheses are proposed. Here we report a case of neurofibroma showing fatty change and coexistence of eccrine hidrocystoma, and discuss the pathomechanism of fatty change and its relationship with disease.

RESUMO

Eccrine hidrocystoma is a cyst origined from the secretory portion of the eccrine sweat gland and tends to occur as a solitary facial lesion. Clinically, an eccrine hidrocystoma presents as a small, translucent and cystic swelling(often having a bluish hue) which measures 1 to 3 mm in diameter. In some patients, the mass is appeared with numerous lesions. The mass has the tendency that the number of cyst increases in warm weather and decreases during winter. The histologic finding is an intradermal cyst with a thin wall composed of two layers of eosinophilic- staining cuboidal cells. Eccrine hidrocystoma has not yet reported in the journal of korean society of plastic and reconstructive surgeons. Thus, we report a case of 65-years-old female patient with eccrine hidrocystoma, which presents 2.1 x 1.6 x 1.6 cm in size and bluish hue color, occurring on the nasal tip.

BACKGROUND: Eccrine hidrocystoma (EH) represents benign cystic dilatations of previously normal dermal eccrine ducts. It is characterized by tense vesicles or translucent papules most often involving the face, predominantly in a periorbital distribution. There were no data on the clinical and histologic features of EH in Koreans. OBJECTIVE: The purpose of this study is to investigate clinical and histopathological features of EH in Koreans. METHOD: We reviewed the medical records and biopsy slides of 19 cases diagnosed previously as EH in 3 hospitals of Catholic Medical Center from August 1991 to May 2003. RESULT: We could compile clinical data and histologic informations on 15 of the total 19 patients with EH. 1. The mean age at diagnosis was 51.6 years, 54.0 years in females and 32.5 years in males. EH developed predominantly in females as the sex ratio of 1: 6.5. 2. Most patients were asymptomatic, but 4 and I patients complained of mild itching and one of a mild burning sensation. The predilection sites were the lower eyelids, nose and lateral canthus. Only one patient had a concomitant Graves' disease. 3. The ratio of Robinson to Smith types was 14: 1. The Robinson type had multiple, pinhead-sized, translucent skin-colored lesions, whereas one smith type had two, pea-sized, dark brownish lesions. The former only had seasonal variation, aggravated in warm, humid weather and improved by a cold environment. 4. Histopathologic examination revealed all cases had a single cystic cavity located in the dermis. In 12 of 15 patients, there were eccrine secretory tubules and ducts in close approximation to a cystic cavity, but neither papillary projections of cyst wall nor connections between the epidermis and the cystic cavity could be found in all cases. 5. Various treatments, such as CO2 laser, atropine ointment, cryotherapy or punch excision were tried, but all failed. CONCLUSION: EHs in this study appeared on the face, especially on the lower eyelids of middle-aged women. In contrast to the western literatures in which most EHs are presented as a solitary lesion, Robinson type is much more common in Koreans. There have been no effective treatments for EHs.

RESUMO

We report a case of multiple eccrine hidrocystoma (MEH) in a 57-year-old woman who had asymptomatic, discrete, skin-colored, shiny, firm papulovesicles on the face. These lesions enlarged during summer or when the patient did housework, and decreased in sue during winter or rest. Histologic examination showed a dilated, unilocular, cystic, invaginated structure within the middle dermis. The wall of the cyst generally consisted of two layers of Rat or cuboida1 epithelial cells. Decapitation secretion and myoepithelial cells were not observed. The lesions improved markedly with 1% topical atropine sulfate.

RESUMO

Nevus sebaceus has a well-documented neoplastic potential, and multiple tumors arising in nevus sebaceous have been reported. However, the development of three or more tumors is extremely rare. We report a case of nevus sebaceus in a 56-year-old male associated with basal cell carcinoma, verrucous carcinoma and eccrine hidrocystoma.

RESUMO

Eccrine hidrocystoma is a benign eccrine duct-derived cyst which may be solitary or multiple. We report two cases of multiple eccrine hidrocystoma, which have two points of sameness in middle aged women and worsening of the condition in summer and improvement in winter. Recently, the incidence of multiple eccrine hidrocystoma seems to be decreasing due to well-developed cooling devices in summer. In spite of its decreasing incidence, we think that it is still noteworthy to keep in mind the possibility of eccrine hidrocystoma when a patient presented with multiple facial papules in hot season.

RESUMO

We report two cases of eccrine hidrocystoma, one in a 13-year-old-female and the other a 56-year-old male. In the first case, rnultiple pale blue, translucent, discrete yellow papules of one years duration were noted on the nose tip of the nose. The second case presented with a tiny asympt,omatic papule superimposed on a slightly erythematous 2cm-sized, round patch on the forehead for 2 months. In both cases, the histopathology demonstrated a cystic space filed with eosinophilic amorphous material. The cyst wall was lined by two layers of cuboidal pithelial cells with eosinophilic cytoplasm. PAS staining showed PAS-positive materials in he cytoplasm of cyst wall epithelium, lumen, and eccrine gland cells. Topical application of 10% atropine sulfate and surgical excision were performed in the each cases with improvernent.
