RESUMO

With the increase in the elderly population, the number of elderly diabetics is also increasing rapidly. To educate older people with diabetes, we need to understand their characteristics and those of their main caregiver, establish careful and individualized treatment goals, and provide concrete and practical education. Diabetes educators should provide comprehensive geriatric assessment, personalized diabetes education through psychosocial assessment, practical guidance, family education, self-management skills for elderly diabetic patients and caregivers. Diabetes educators should keep in mind to consider elderly diabetic patients can support family or social resources to continue self-management.

METHODS:120 diabetic patients over 60 yeas old in outpatient department of our hospital were randomly selected,self-made questionnaire was conducted, the related factors caused poor medication compliance in elderly diabetic patients were explored by Logistic regression analysis. RESULTS:Krone Bach coefficient of the questionnaire was 0.89. Totally 120 ques-tionnaires were sent out,113 were effectively received, with effective rate of 94.17%. 64 cases(56.64%)were in poor medication compliance in the 113 surveyed patients. Among them,59 cases(92.19%)did not follow the medication prescribed period;54 cas-es(84.38%)did not strictly in accordance with the doses;43 cases(67.19%)took drugs in the wrong time;37 cases(57.81%) freely changed the drugs;18 cases(28.13%)repeated taking drugs. The age, educational background, medical factors, economic factors, adverse effects, medical personnel and treatment regimens had significant effects on medication compliance of the surveyed patients (P<0.05). RESUMO

BACKGROUND: Diabetes has a critical effect on elderly diabetic patients' quality of life. Elderly diabetic patients have many difficulties in diabetes self-management because their physical, psychological and social functions are decreased as a result of the aging process. Therefore, we evaluated the current status of self-management and barriers in elderly diabetic patients METHODS: The sample was 124 elderly diabetic patients (over 61 years) who visited a hospital and two senior welfare centers in Seoul from July to August 2006. The results of the study were analyzed by descriptive statistics, ANOVA, T-test, and hierarchical regression. RESULTS: 1) Diabetes self-management was composed of insulin injection, diet, exercise, foot management, smoking and drinking alcohol. An average score of self-management in the elderly diabetic patients was 4.5784. In demographic variables, only job status showed a significant effect on self-management. 2) Barriers such as 'forgetting taking diet and medication', 'being interrupted by others', and 'lack of family and social support' were significant factors in diabetes self-management. 3) These barriers for diabetes self-management were still significant in the case of controlling demographic and clinical variables. CONCLUSION: Diabetic educator should identify the significant factors that affect patient's self-management such as whether they have a full-time job, or the type of their job. In addition, diabetes education should focus on assertiveness training to deal with various interpersonal barriers and empowerment for enhancing patient's self-efficacy. These approaches would benefit patients who experience barriers of diabetes self-management

RESUMO

In this article,we propose the strategy of rational use of Chinese and Western medicine on elderly diabetic patients, based on their clinical characteristics and different syndromes, combined with glucose control target level, these strategies are described as follows:①To improve patient`s compliance by diabetic education;②To enhance clinical effectiveness through diet and exercise;③To implement individualized treatment with rational use of hypoglycemic drugs;④To simplify medication with easy operability;⑤To focus on drug-induced hypoglycemia;⑥To pay attention to the imcompatibility when various diseases coexist;⑦To avoid repeated use or forgetting to take the hypoglycemic drugs;⑧To highlighted weight loss treatment for overweight or obese patients;⑨To differentiate syndrome and treatment of elderly patients.
