Resolução simplificada pela conversão de uma prótese parcial fixa/removível de extensão distal em uma overdenture maxilar retida por implantes / A simple treatment way by conversion of a distal extension fixed/removable bridge into an implant-retained maxillary overdenture: a case report

Este trabalho relata um caso clínico onde uma paciente de 82 anos de idade queixava-se da instabilidade da porção removível de sua prótese maxilar híbrida. Dois implantes foram colocados na região posterior e a nova prótese passou a funcionar como uma overdenture retida por encaixes tipo O-ring. As extremidades distais dos encaixes nas coroas metalocerâmicas dos dentes 13 e 23 não puderam ser capturadas. Mesmo em função das limitações técnicas e econômicas, o tratamento proposto satisfez as exigências estéticas e funcionais da paciente.

This work reports a clinical case where a 82 years-old patient had complaints about the removable portion of an hybrid maxillary prosthesis. Two dental implants were inserted in the posterior region and the new reconstruction was designed as an O-ring retained overdenture. The female portions of the metalloceramic crowns of teeth 13 and 23 could not be captured in this way. Even thus, given the technical and financial limitations, the proposed treatment addressed all esthetic and functional patient demands

Prótese fixa associada à PPR com encaixe extracoronário uma alternativa de planejamento em reabilitação oral / Fixed prosthesis associated with RPD extracoronal attachment an alternative planning in oral rehabilitation

Atualmente as próteses implantossuportadas são a primeira alternativa de tratamento na reabilitação de arcos desdentados parciais ou totais, porém muitas vezes, existem limitações de ordem anatômica, sistêmica ou econômica que impossibilitam esse tipo de tratamento. Um recurso que pode ser considerado, quando não é possível este tipo de tratamento é o uso de próteses fixas conjugadas a PPR com encaixes (attachments). Esses são componentes alternativos aos grampos convencionais e sua indicação é baseada nas particularidades de cada caso e na disponibilidade, normalmente os encaixes rígidos são indicados para casos próteses conjugadas dentossuportadas enquanto que os semirígidos ou resilientes para os casos de extremidades livres. Com base nessas considerações o presente trabalho tem como objetivo apresentar dois casos clínicos de reabilitação com PPR retida por encaixes resilientes de semiprecisão associados à prótese fixa. Obteve-se resultados satisfatórios, quanto a estética e funcionalidade, destacando-se a importância de um bom planejamento para o sucesso do tratamento

Currently implant supported prostheses are the first choice of treatment in rehabilitation of partially or totally edentulous arches, but there are often limitations in anatomic, systemic or economic precluding this type of treatment. One feature that may be considered when this kind of treatment is not possible the use of bridges combined with the RPD attachments. These components are alternatives to conventional clips and its indication is based on the particularities of each case and the availability, attachments normally rigid prostheses are indicated for cases combined dental-supported while the semi rigid or resilient to the cases of free ends. Based on these considerations this paper aims to present a case of rehabilitation with RPDs retained by resilient attachments for semi precision associated with the fixed prosthesis. We obtained satisfactory results, as the aesthetics and functionality, highlighting the importance of good planningfor successful treatment

Utilização de encaixe resiliente extra-coronário na obtenção de retenção, estabilidade e proteção dos pilares em prótese parcial removível / Extracoronal resilient attachment use in the attainment of retention, stability and protection of the abutments in removable partial denture

Grande parte dos pacientes que apresentam perdas dentárias bilaterais posteriores queixa-se da estética deficiente exibida pelas próteses parciais removíveis a grampo. O objetivo deste estudo foi demonstrar, através do relato de um caso clínico, a associação de prótese parcial removível (PPR) a próteses parciais fixas (PPF), através do uso de encaixes extra-coronários resilientes, como forma de se obter bom funcionamento do sistema mastigatório e proteção das estruturas envolvidas. Ao final do tratamento, o planejamento protético utilizado proporcionou à paciente o uso de uma prótese estética, confortável e estável.

Great part of the patients that presented posterior bilateral dental losses complains that the removable partial dentures retained by clasps have poor aesthetics. The aim of this study is to demonstrate, through the presentation of a clinic case, the association of removable partial dentures and fixed partial dentures as a way of obtain good masticatory function and protection of the involved structures. At the end of the treatment, the prosthetics planning selected for the case led the patient to the use of an aesthetic, comfortable and stable prosthesis.
