RESUMO

This paper summarized professor ZHANG Lei′s experience in treating scleroderma from “extraordinary pathogens entering collaterals”. The basic pathogenesis of scleroderma is “extraordinary pathogens entering the collaterals”, and extraordinary pathogens can be divided into external and internal categories. External extraordinary pathogens are mostly exogenous wind, cold and damp pathogens, and the pathogenesis is wind, cold and damp invading skin stria and retaining collaterals. Most of the endogenous extraordinary pathogens are turbid phlegm and blood stasis, and the pathogenesis is endogenous phlegm and stasis leaving the channels and overflowing the collaterals, and blocking the collaterals. Blocked by extraordinary pathogens for a long time, the long illness will lead to deficiency and develop into a syndrome of collaterals excess and channels deficiency. Therefore, professor ZHANG creats Tengluo Beverage (藤络饮) as the basic formula to unblock collaterals and dispel pathogens, and recommends to add or subtract it according to the different syndrome and pathogenic characteristics of the edema stage, sclerosis stage, and atrophy stage. In the edema stage, it is advised to expel wind, remove dampness and unblock the collaterals, while in the sclerosis stage, the method of dissolving phlegm, expelling stasis and unblocking collaterals should be used; in the atrophic stage, it is suggested to differentiate the deficiency of qi, blood, yin and yang, and eliminate extraordinary pathogens on the basis of reinforcing healthy qi .

RESUMO

Objective:To explore whether or not radioactive wastewater leakage or overflow exists in the nuclear medicine decay pool after the 7·20 extraordinary rainstorm disaster and to analyze its causes, so as to provide scientific basis for the design, construction and maintenance of the nuclear medicine decay pool and the radiation protection management of nuclear medicine in the future. Methods:Three hospitals A, B and C where 131I therapy are being performed were selected in Zhengzhou city, Henan province. Sampling points were set based on the monitoring result of the environmental radiation level around the nuclear medical decay pool in line with the relevent standard method. Soil samples were collected at different depths. The 131I radioactivity level in the soil was measured using HPGe gamma spectrometer, with the measured result being processed and analyzed. Results:Except for Hospital B where no 131I was detected, different 131I activity concentrations were detected in the soil samples collected around the nuclear medicine decay pool at Hospitals A and C. The 131I activity concentrations ranged from 16.4 to 98 111.8 Bq/kg at Hospital A and from 10.6 to 7 176.6 Bq/kg at Hospital C. After a time period of decay, the repeated detection has shown that the 131I activity concentrations ranged from 1.3 to 17.0 Bq/kg at Hospital A and from 3.9 to 7.1 Bq/kg at Hospital C. The 131I activity concentration in the soil at the depth of 0-5 cm was higher than that at the depth of 5-10 cm soil at the same sampling point. The ratios of activity concentration of 131I between different depths ranged from 1.3 to 13.1, with the median value of 5.9. Conclusions:After the 7·20 extraordinary rainstorm disaster, the surrounding environment of nuclear medicine decay pools at a few hospitals was contamminated by 131I to some extent. To prevent the risk of leakage of radioactive wastewater, the safety evaluation and management of radiation protection should be made better with increased awareness during the design, evaluation, construction and use of nuclear medicine decay pools.

RESUMO

OBJECTIVE@#To observe the effect of Dong's extraordinary point needling technique on postoperative complications of anal fistula.@*METHODS@#A total of 241 patients undergoing anal fistula surgery were randomly divided into an observation group (121 cases, 3 cases dropped off) and a control group (120 cases, 2 cases dropped off). The patients in the control group were treated with intramuscular injection of compound diclofenac sodium injection and oral administration of tamsulosin hydrochloride sustained release capsules. In addition to the treatment in the control group, the patients in the observation group were treated with Daoma needling technique at the "Sanqi points" (Qimen point, Qijiao point, and Qizheng point) combined with Dongqi needling technique at "Sanhuang points" (sub-Tianhuang point, Dihuang point, Renhuang point), with each session lasting 30 min. The treatment in the two groups both started on the first day after surgery, and was given once daily for 14 consecutive days. Visual analog scale (VAS) score was compared between the two groups on postoperative day 1, 7, and 14; bladder residual urine volume, spontaneous voiding volume, and urinary catheterization frequency were assessed after treatment on postoperative day 1; and anorectal dynamic indexes (anal canal resting pressure, rectal resting pressure, maximum squeeze pressure of the anal canal, and minimum rectal sensory threshold) were evaluated before surgery and on postoperative day 4. Clinical efficacy was assessed in both groups one month after surgery.@*RESULTS@#On postoperative day 7 and 14, the VAS scores of both groups were lower than those on postoperative day 1 (P<0.05), and the VAS scores in the observation group were lower than those in the control group (P<0.05). The bladder residual urine volume and urinary catheterization frequency in the observation group were lower than those in the control group (P<0.05), while the spontaneous voiding volume was higher than that in the control group (P<0.05). On postoperative day 4, the anal canal resting pressure, maximum squeeze pressure of the anal canal, and the minimum rectal sensory threshold were lower than preoperative values (P<0.05), while the rectal resting pressure was higher than preoperative value (P<0.05) in both groups. The anal canal resting pressure, maximum squeeze pressure of the anal canal, and minimum rectal sensory threshold were lower than those in the control group, and the rectal resting pressure was higher than that in the control group (P<0.05). The effective rate was 93.2% (110/118) in the observation group, which was higher than 84.7% (100/118) in the control group (P<0.05).@*CONCLUSION@#Dong's extraordinary point needling technique could reduce postoperative pain, alleviate urinary retention, and improve defecation in patients undergoing anal fistula surgery.

Abstract Paranormal beliefs, regardless of their veracity, clearly constitute an important dimension of human psychology. Survey work across the world reveals that around 50 % of people hold at least one paranormal belief. This figure is surprisingly high considering the lack of robust scientific evidence in support of the referred phenomena, and suggests the existence of resilient psychological mechanisms at the basis of the development and maintenance of such beliefs. Researchers in the area have proposed a diversity of explanations, exploring a large number of individual factors. However, despite the generous effort, we have not been able to develop a comprehensive explanatory model capable of accommodating the body of empirical findings. With this study we aimed to contribute towards a better understanding of paranormal belief as a multicausal phenomenon. In order to achieve this, we explored a set of variables related to the main perspectives in the literature. We used a sample of 184 subjects of very diverse nationalities, ages, and cultural backgrounds. We surveyed paranormal beliefs among our participants and observed that 71 % of the sample showed traditional religious beliefs, 34 % believed in psychic phenomena such as extrasensory perception or psychokinesis, 31 % in superstition, 23 % in witchcraft, and 39 % in anomalous natural phenomena such as astrology or extraordinary life forms. Eighty four per cent of the subjects responded positively to at least one of the 26 items of paranormal beliefs evaluated in the scale. A correlation analysis was conducted between the individuals' traits and paranormal belief scores. Schizotypy (rxy = .23, p < .001), unusual experiences (rxy = .37, p < .001), introvertive anhedonia (rxy= .28, p < .001), conditional reasoning in tasks with paranormal content (rxy = .23, p = .001), need for affiliation (rxy = .21, p < .002), and report of subjective extraordinary experiences (rxy = .35, p < .001) correlated significantly with belief in the paranormal. These results are consistent with Russell and Jones' (1980) theory of compensation of basic needs and suggest that paranormal belief may rise in combination with premorbid signs of psychopathology. However, they suggest that belief in the paranormal may not be a consequence of a cognitive deficit in this type of reasoning. A multiple regression analysis revealed a five variable solution. In the equation, subjective extraordinary experiences, unusual experiences (from the schizotypy scale), need for autonomy, need for achievement, and introvertive anhedonia contributed standardized coefficients of .3, .23, .24, .22 and .13 respectively. The model explained 27 % of the variance. The findings of this study are consistent with a multicausal conception of paranormal belief. A model is proposed in which paranormal beliefs can stem from a variety of factors and, as they develop, a reciprocal interaction between experience and belief arises, contributing to their definitive accommodation in the individual's psychology. The identification and exploration of contributing factors to the development and maintenance of the paranormal belief has been very fruitful in the last decades. In addition, the explanatory hypotheses raised by the researchers do not seem to be mutually exclusive. Therefore, the simultaneous consideration of these could be a successful approach in future studies. The elaboration of an explanatory model that contemplates both the multicausality and the multidimensionality of the phenomenon could provide a qualitative leap in the area.

RESUMO

Through sorting out the development of meridian points and the standardization of extraordinary points in dynasties, two objectives are discovered in the standardization of extraordinary points starting from the Tang Dynasty. The points with extraordinary effect are selected points and transferred to meridian points and the commonly-used key points are selected and developed to be standardized acupoints. Hence, 3 approaches are summarized. Approach No.1: a large number of "meridian-outside" points that are the potential supplementation but not included yet in meridian points are supplemented to meridian points. Approach No.2: based on the specified requirement, the key points are selected from the acupoints with determined locations and names, which is developed to be the standard of acupoints. Approach No.3: the acupoints with extraordinary effect are selected from the "meridian-outside" points and these points are collected for the formulation of the standard of extraordinary points. The latter two approaches provide for the development direction of teaching and standardization in the future.

RESUMO

Professor Shao Jing-ming had practiced Chinese medicine for more than 80 years with rich clinical experience and exquisite acupuncture techniques. From his clinical experience, Professor Shao's clinical features can be summarized as the followings: attaching importance to the theory of meridians and collaterals, combining pattern identification and disease identification, using fewer acupoints for treatment, and using Ashi point and Hegu (LI 4) to treat goiter. He emphasized the priority and sequence in acupoint-selection and manipulation. In treatment of epilepsy, he proposed to treat it according to the situation and paid great attention to the special function of the extraordinary acupoints. During the onset, it should be managed by calming the mind and controlling the symptoms; during the remission period, acupuncture and drugs should be applied simultaneously to regulate qi-blood and yin-yang, so as to reduce the frequency of attacks. He believed that acupuncture manipulation be one of the key factors in achieving the efficacy. In treatment of the motive diseases, such as convulsions and cramps, acupuncture with static and longer needle-retaining time should be adopted to control the limb movement, to strengthen the stimulation and to obtain a long-term efficacy. Professor Shao Jing-ming's medical records range from internal medicine, external medicine, gynecology and pediatrics, listing various refractory diseases effectively treated by him. Professor Shao was a moral and erudite Chinese medicine master, and his clinical experience is worthy of inheritance and development.

RESUMO

The discovery and application of Lanwei (EX-LE 7) was reviewed to elaborate its enlightenment on the inheritance and innovation of acupuncture. Literature published before April of 2017 was searched in CNKI and VIP database with"Lanwei (EX-LE 7)"and"acupuncture"as key words. Related references and publications were retrieved. According to the purpose, literature was screened and key information was extracted to perform systematic review and analysis. The results indicated Lanwei (EX-LE 7), reflection of relationship between organ and body surface under the guidance of acupuncture and meridians theory, was not a point, but an area. The discovery and application of Lanwei (EX- LE 7) was the product of western and Chinese combination. The best indication was simple appendicitis, but operation should be applied once adhesion, purulent and perforation occurred. It is indicated to improve clinical efficacy and avoid accident, the reasoned core of theory system should be insisted and the communication with modern medicine should be reinforced.

RESUMO

Burns caused by uncommon agents means those caused by other agents except heating power, with special characteristics existing in traumatogenesis, pathophysiology, and clinical manifestation. With the development of social economy, various new techniques and new equipments are springing up. In the practical use, improper operations would become traumatogenic agents and cause various special types of trauma. In addition, some special injuries emerged with the changes in people′s lifestyle. For battle injury, some new war wounds, which are different from fire-arm injuries in the past, appeared with the emergence of acoustic wave, light wave, electrical and magnetic weapons. Extraordinary regions are those located on body surface with anatomic and physiological particularity. Injuries caused by uncommon traumatogenic agent or in extraordinary region are different from those ordinary burns and trauma, and their clinical treatments have special characteristics. Clinical treatments were studied aiming at these special characteristics, and some achievements in treatment of high-voltage electrical burn, hydrofluoric acid burn, wounds on special regions, and new types of burns and trauma have been made. However, a doctor′s duty is not only to cure the diseases and save the patients′ lives, but also to prevent the diseases. The suitable treatment and precautionary measures for the new types of burns and trauma that differ from ordinary burns and trauma in the past remain to be explored.

RESUMO

Point Dingchuan (Ex-B1) pertains to extraordinary points and often produces a good therapeutic effect on respiratory system diseases. Ancient Chinese medical books do not record point Dingchuan. Modern Chinese medical textbooks describe the location and efficacy of and indications for this point but do not state their provenances. In order to further popularize the clinical application of point Dingchuan, this article verifies the origin and development of the name and location of point Dingchuan so as to provide a certain reference for clinically correct selection and use of this point. The results show that point Dingchuan followed the course of development from ashi point to new point and then to extraordinary point. The development of point Dingchuan to the same acupoint name and location as now international use went mainly through four stages, that is, similar name and different location, similar name and location, same name and different location, and same name and location.

RESUMO

ABSTRACT: This study attempts an exposition of different perceptions of obligation to medical treatment that have emerged from the Islamic theological understanding and how they contribute to diversity of options and flexibility in clinical practice. Particularly, an attempt is made to formulate an Islamic perspective on ordinary and extraordinary means of medical treatment. This distinction is of practical significance in clinical practice, and its right understanding is also important to public funded healthcare authorities, guardians of the patients, health and life insurance institutions, and employers who provide health care coverage to their employees. Not only these parties, but also lawyers and justice administration functionaries such as public prosecutors and judges are in need of understanding this distinction to deal with relevant litigations. The distinction could be made regarding terminally ill patients and non-terminally ill patients separately. The essential factors that matter in making the distinction between ordinary and extraordinary means of treatment are: (1) patient capacity (2) expert advice, and (3) nature of medication. Regarding terminally ill patients, medical treatment can become extraordinary because of (1) patient capacity and (2) nature of medication. In both these case the deciding condition applies: the expert advice taken from a group of physicians. In regards to non- terminally ill patients, extraordinary medical treatment includes three cases: (1) treatment that is known to be useless and futile, (2) treatment that endangers the life or cause more harm than what it removes, and (3) useful treatment, but the patient is unable to bear the cost.

RESUMO

According to ancient and present Chinese sources the Eight Confluent Points are essentials of acupuncture and can treat all kinds of diseases. They exert a fundamental influence on the Twelve Regular Meridians and their mobilization can be seen as a necessity for a successful acupuncture treatment. For this they should play a major role in acupuncture. In European acupuncture teachings there is also the opinion that by too much needling of the Eight Confluent Points the qi of the organism might get exhausted which will deteriorate the patient's symptoms and eventually will create new symptoms. Here it will be shown that deterioration as well as the creation of new symptoms due to acupuncture is caused by not observing the contraindications in acupuncture as set up in ancient China provided that syndrome differentiation, point selection and stimulation have been correct. In opposition to this, all of the Eight Extraordinary Channels are to be activated at first and then the Twelve Regular Meridians are to be mobilized in order to achieve therapeutic effects.
