RESUMO

Objective:To explore the application experience of autologous fat transplantation in improving the facial contour of young cosmetic patients.Methods:From October 2017 to October 2020, the plastic surgery department of Beijing Tsinghua Changgung Hospital admitted 10 young cosmetic patients with poor facial contours, including 1 male and 9 females, aged 18-35 years, with an average of 28 years. Autologous subcutaneous fat was harvested by liposuction and static purification and then injected into the areas with poor facial contour with an amount of 10%-30% over.Results:The facial contour of 10 young cosmetic patients was well improved, and there were no postoperative complications such as facial asymmetry, local uneven skin, skin infection and necrosis or fat embolism. 8 cases were filled once and 2 cases were filled twice. The patients were followed up for 6-24 months and the postoperative effect was good. The excellent and good rate evaluated by patients, plastic surgeon and the third party doctor was more than 80%.Conclusions:The use of autologous fat to improve the facial contour of young cosmetic patients is easy to operate with less trauma and good effect, which is worthy of promotion.

RESUMO

Intraoral facial contour surgery is one of the most popular craniofacial cosmetic surgeries in China, in spite of its challenging surgical technique and serious complication, such as fracture and bleeding. In addition, the key point, such as inadequate tissue exposure and high soft tissue tension cannot be simulated using traditional three-dimensional model. Consequently, craniomaxillofacial surgery requires a long learning curve for young surgeons. This article describes the establishment and application of a new model for intraoral facial contour surgery. Compared to young surgeons trained using traditional three-dimensional model, the authors confirmed this new surgical model is more reliable and effective. Therefore, it is worthy of being popularized.

Objective@#To present the overall therapy to correct mandible deformities and modify facial contour of patients who have been diagnosed as auriculo—condylar syndrome (ACS) in our hospital.@*Methods@#We formulate effective therapeutic plan for every single patient based on digital surgical techniques. Three patient received individualized therapy, which include bilateral Distraction osteogenesis, orthodontic treatment, genioplasty, LeFort Ⅰ osteotomy, Chin augmentation with porous polyethylene implant, or mandibular contour correction with porous polyethylene implant.@*Results@#All three patients gained satisfactory surgical results. Occlusal function and sleeping quality in 2 severe patients was significantly improved.@*Conclusions@#Clinical features of ACS is complex, which may range from mild to severe in different patients. Individualized therapeutic plan of comprehensive treatment should be established after facial deformity evaluation and surgical simulation based on surgical techniques.

Objective To investigate how to improve the survival rate,and to avoid the serious complications in fat grafting.Methods Under comprehensive design of facial filling area,the fat was harvested by concentration in mesh;under the micro autologous fat transplantation gun with blunt needle injection technique,the fat was filled into the face to achieve the effect of improving the facial contour and the rejuvenation.From June 2014 to July 2015,78 patients with facial contour and facial soft tissue relaxation depression were treatment by the technique.Results The follow-up of 2 months to 1 year showed that 64 patients had the soft,young,significantly enhanced facial contour;7 patients had second times grafts and the remaining were satisfied.7 patients gave up the second times grafts because the other reasons.The surgical complications such as facial nerve injury and infection and embolism were not seen.Conclusions This technique by comprehensive design and purification of autogenous fat granules achieves a better effect on facial rejuvenation with fewer complications.

Objective To analyze the difference of soft and hard tissues between beautiful face and ordinary face in young Han women for providing data basis for modification of lower facial contour.Methods 38 cases of young Han women were selected with craniofacial spiral CT scanning,three-dimensional reconstruction and histomorphometry of the mandibular angle region (12 cases of beautiful face and 26 cases of ordinary face).Results The difference of bony bigonial breadth,lower face width,angle of mandibular angle,mandibular branch width and height,the distance between the mandibular angle point and pogonion in mandibular angle region had statistical significance (P < 0.05);the difference of mandibular angle thickness had no statistical significance (P>0.05);the difference of masseter muscle length,width and thickness had statistical significance (P < 0.05).Conclusions In clinical lower facial contour modification practice,we can refer to the data of soft and hard tissue of mandibular angle region of beautiful face,combining with the specific circumstances of the pursuit of beautiful people and select the appropriate surgical approach for lower facial contour modification.

RESUMO

Objective:To assess the outcome for prevention the gustatory sweating syndrome and facial contour deformity with sternocleidomastoid muscle flaps in the parotidectomy. Method:It was a prospective study. Eighty-one patients who underwent parotidectomy were randomly divided into 2 groups, sternocleidomastoid muscle flaps group and control group. They were tested with the subjective and objective function testing in 3, 6, 9, 12 months after operation. We assessed the result. Result:All of the result of the sternocleidomastoid muscle flaps group was obviously better than the control group. There was significant difference between the 2 groups (P<0.05). Conclusion:It effectively prevented the gustatory sweating syndrome and facial contour deformity that we used the sternocleidomastoid muscle flaps to fill the cavity in the parotidectomy.

Aim: The aim of this study was to assess the esthetic perception of adults with primary, secondary and higher education and recent dental graduates towards different combinations of facial contours and upper central incisor shapes. Methods: Photographs of 6 individuals with square, tapered and ovoid facial contours (2 per type) were modified to have the 3 different types of tooth shapes (square, tapered and ovoid) in each facial contour (total of 18 photographs). The 195 participants rated each photograph using visual analogue scales. Comparison between groups was performed using the Chi-square t-test ('alfa'=0.05). Results: No statistically significant differences (p>0.05) were found in the esthetic perception towards the frontal facial outline and upper incisor shape by participants’ educational level. In addition, the shapes of teeth perceived as the most esthetically pleasing were not always similar to the form of frontal facial outline evaluated. Conclusions: No general agreement was found in the esthetic perception towards the frontalfacial outline and upper incisor shape by educational level. Dentists tend to prefer ovoid shape tooth for almost all frontal facial outline.

Objective To evaluate the efficacy of endoscopic techniques for corrections of facial contour deformities.MethodsA total of 58 patients(48 women and 10 men)with square face(38),prominent zygoma(12),or crooked noses(8)were treated by endoscopic-assisted technique in our hospital.Results The operation time ranged from 40 to 86 min(mean,65 min),and intraoperative blood loss was 60-120 ml(mean,78 ml).The patients were discharged from the hospital in 3 to 7 days(mean 4)after the surgery,and was followed up for 3 to 24 months.During the period,no one had unexpected fracture,injuries to the mental and facial nerves and vessels,or infection.All the patients were satisfied with the surgical outcomes.ConclusionsEndoscopic-assisted correction of facial contour deformities is feasible,safe,and effective.

Autologous fat injection had not gained an attention in aesthetic plastic surgery, because of high reabsorption. Recently, fatty tissue was obtained by the low-power technique and centrifuged, results of autologous fat injection was improved. The authors performed autologous microfat injections on 277 patients from March 1997 through December 2002 to correct soft tissue facial contour such as aging, depressed facial contour, thin and long faces as well as postoperative complications and scars. The fatty tissues remaining from the first injection were kept at -20 degrees C up to twelve months, and were used for repeated injections in the overly- absorbed or inadequate areas. Of those 277 patients, 59% received an injection once, 35% received an injection twice, 5% received an injection three times and 1% received an injection up to four times. Follow-ups and postoperative photo confirmations were possible only in 119 out of 277 cases. 81.5% showed satisfied results, and development of complications were insignificant occurring at 6.2%. Owing to advances in tissue engineering and frozen storage method, autologous microfat used for injection will be developed as a better filler, which may substitute soft tissues.
