Objective: This systematic review aims to compare the clinical longevity of metallic and fiberglass intraradicular posts in teeth with severely compromised crowns, utilizing randomized clinical trials and case reports published in the last eleven years. Material and Methods: The research was conducted on PubMed, Medline, Lilacs, and BBO databases using the first search strategy with the descriptors 'dental posts,' 'longevity,' 'fiberglass-reinforced posts' or 'metallic posts.' The second search strategy involved analyzing the references of articles identified by the first search. Both studies were carried out with stringent inclusion and exclusion criteria. Results: No statistically significant and clinically relevant differences in longevity were observed between metallic and fiberglass posts. Conclusion: The clinical success and durability of the restorative procedure using intraradicular posts primarily depend on the remaining amount around the post (ferrule), the type and position of the tooth in the arch (impacting the masticatory forces exerted on the restored tooth), and the correct application of the cementation technique. It is noteworthy that fiberglass posts offer substantial advantages by presenting a modulus of elasticity similar to dental structure, ensuring a more homogeneous distribution of masticatory forces and reducing the risk of fractures. These findings have practical implications for material selection in restorative procedures involving intraradicular posts.(AU)


Objetivo: Esta revisão sistemática visa comparar a longevidade clínica de pinos intrarradiculares metálicos e de fibra de vidro em dentes com coroas severamente comprometidas, através de estudos clínicos randomizados e relatos de casos, publicados nos últimos onze anos. Material e Métodos: A pesquisa foi conduzida nas bases de dados PubMed, Medline, Lilacs e BBO, utilizando a primeira estratégia de busca com os s 'pinos dentais' e 'longevidade' e 'pinos reforçados com fibra de vidro' ou 'pinos metálicos'. A segunda estratégia de busca consistiu na análise das referências dos artigos identificados pela primeira estratégia. Ambos os estudos foram delineados com critérios rigorosos de inclusão e exclusão. Resultados: Não se constataram diferenças estatisticamente significativas e clinicamente relevantes na longevidade entre pinos metálicos e de fibra de vidro. Conclusão: O êxito clínico e a durabilidade do procedimento restaurador empregando pinos intrarradiculares dependem primordialmente da quantidade de remanescente ao redor do pino (férula), do tipo e posição do dente na arcada (o que impacta nas forças mastigatórias exercidas sobre o dente restaurado) e da aplicação correta da técnica de cimentação.Destaca-se que o pino de fibra de vidro oferece vantagens substanciais ao apresentar módulo de elasticidade similar à estrutura dentária, assegurando uma distribuição mais homogênea das forças mastigatórias e reduzindo o risco de fraturas (AU)

Introducción: El uso de postes de fibra de vidrio es el método actual más utilizado para la reconstrucción postendodóncica. Sin embargo, su adhesión sigue siendo impredecible debido al control limitado de su técnica de cementación. Las resinas bulk-fill revelan valores de fuerza flexural similares a la dentina y su empleo en la reconstrucción postendodóncica podría ser una alternativa viable y conservadora. Objetivo: Comparar tres técnicas de reconstrucción postendodóncica al medir su resistencia a fuerzas de desalojo, clasificar el tipo de fractura y falla adhesiva, evaluar su costo y tiempo de trabajo. Material y métodos: Se realizan 18 muestras divididas en tres grupos: grupo 1: poste más cemento de un paso, grupo 2: poste más cemento de tres pasos y grupo 3: reconstrucción postendodóncica con resina bulk-fill. Resultados: En la evaluación de la fuerza al desalojo no se encontró diferencia estadísticamente significativa en los tres grupos. Respecto al comportamiento y falla adhesiva, se encontró una diferencia significativa p < 0.001, favoreciendo a los grupos 2 y 3, por su adhesión en la porción radicular. El tipo de fractura fue restaurable para los tres grupos. Respecto al tiempo y costo, fueron significativamente menores para el grupo 3. Conclusión: En nuestro estudio no existió diferencia significativa en la fuerza al desalojo en los tres grupos; sin embargo, la hubo en el comportamiento, falla adhesiva, costo y tiempo, favoreciendo mayormente al grupo 3. La técnica de reconstrucción con resina puede ser una técnica predecible, suficientemente resistente, conservadora, rápida y de menor costo


Introduction: The use of fiberglass posts is the current method most used for post-endodontic reconstruction. However, its adhesion remains unpredictable due to the limited control of its cementing technique. Bulkfill resins reveal values of flexural strength similar to dentin and their use in post-endodontic reconstruction could be a viable and conservative alternative. Objective: Compare three post-endodontic reconstruction techniques by measuring their resistance to eviction forces, classifying the type of fracture and adhesive failure, evaluating their cost and time of work. Material and methods: 18 samples are divided into 3 groups: group 1: post plus cement of 1 step, group 2: post plus cement of 3 steps and group 3: post-endodontic reconstruction with bulk-fill resin. Results: In the evaluation of the force to the eviction, no statistically significant difference was found in the 3 groups. Regarding the behavior and adhesive failure, a significant difference was found p < 0.001, favoring groups 2 and 3, due to its adhesion in the root portion. The type of fracture was restorable for the 3 groups. Regarding time and cost, they were significantly lower for group 3. Conclusion: In our study there was no significant difference in the force to the eviction in the three groups, however there was in the behavior, adhesive failure, cost and time, favoring mainly the group 3. The technique of reconstruction with resin can be a predictable technique, Sufficiently resistant, conservative, fast and of lower cost (AU)

Introducción: durante el proceso de identificación forense de cadáveres y restos humanos quemados, carbonizados o incinerados, el análisis de los materiales empleados en los diferentes tratamientos odontológicos se constituye en un marcador fehaciente para lograr una identificación positiva. El objetivo de este trabajo fue describir los cambios radiográficos de postes de titanio y de fibra de vidrio cementados, en premolares humanos sometidos a altas temperaturas con fines forenses. Métodos: estudio pseudo-experimental in vitro que observó los cambios radiográficos de los tejidos dentales (esmalte, dentina y cemento), de los materiales de uso endodóncico gutapercha (Wave One® de Dentsply Maillefer®), material de obturación (Top Seal® de Dentsply Maillefer®), material de cementación de los postes (Relyx TM ARC de 3M ESPE®) y de los postes de titanio (Tenax® Endodontic Post System de Coltene®) y fibra de vidrio (Tenax® Fiber Trans de Coltene®) en 30 dientes humanos, sometidos a cinco rangos de temperatura 200, 400, 600, 800 y 1000ºC. Resultados: los tejidos y los materiales dentales estudiados presentan gran resistencia a las altas temperaturas, sin variar considerablemente su macro-estructura (fisuras, grietas, fracturas y fragmentación), de tal manera que los cambios físicos pueden identificarse a través de radiografía convencional, y asociarse a cada rango de temperatura específico. Conclusiones: los postes cementados en premolares humanos presentan gran resistencia a la acción de altas temperaturas, razón por la cual pueden ser visualizados a través de radiografía convencional, contribuyendo con el proceso de identificación y documentación de la necropsia médico-legal de un cadáver o restos humanos que hayan resultado quemados, incinerados o carbonizados


IIntroduction: during the process of forensic identification of corpses and burnt, carbonized, or incinerated human remains, the analysis of materials used in dental treatments is a reliable marker for accurate identification. The objective of this study was to describe the radiographic changes of titanium and fiberglass posts cemented in human premolars subjected to high temperatures for forensic purposes. Methods: this was a pseudo-experimental in vitro study to observe the radiographic changes of dental tissues (enamel, dentin, and cement) and the following endodontic materials: gutta-percha (Wave One® by Dentsply Maillefer®), obturation material (Top Seal® by Dentsply Maillefer®), post cementation material (Relyx TM ARC by 3M ESPE®), titanium posts (Tenax® Endodontic Post System by Coltene®), and fiberglass posts (Tenax® Fiber Trans by Coltene®) in 30 human teeth subject to five temperature ranges: 200, 400, 600, 800, and 1000° C. Results: the studied dental tissues and materials are highly resistant to high temperatures without significantly changing their macrostructure (fissures, cracks, fractures, and fragmentation), in such a way that the physical changes can be identified through conventional radiography and associated with each specific temperature range. Conclusions: posts cemented in human premolars have great resistance to the action of high temperatures, so they can be observed through conventional radiography thus contributing to the process of identification and documentation of the medico-legal autopsy of a corpse or human remains that have been burned, incinerated, or carbonized

This study evaluated the influence of the surface treatments of fiberglass posts on bond strength to root dentin using push-out test. Forty bovine incisor roots were endodontically treated. The surface of the fiberglass posts (Exacto #2, Angelus) were treated using 4 different protocols (n=10): Control - 70% ethanol for 1 min; 37% phosphoric acid for 1 min; 10% hydrofluoric acid for 1 min; and 24% hydrogen peroxide for 1 min. After a silane coupling agent was applied for 1 min and all posts were cemented using self-adhesive resin cement (RelyX Unicem, 3M-ESPE). The roots were sectioned and two 1-mm-thick slices were obtained from each third: cervical, middle and apical. The specimens were subjected to the push-out test with a crosshead speed of 0.5 mm/min. Data were analyzed by repeated measures ANOVA followed by Tukey's HSD tests (=0.05). The surface treatment (p<0.001) and root third region (p=0.007) factors were significant. The retention to root canal was affected by surface treatment type. The post surface treatment with 24% hydrogen peroxide for 1 min yielded significantly higher bond strength when the fiberglass posts were cemented with RelyX Unicem.


Este estudo avaliou a influência de tratamentos de superfície de pinos de fibra de vidro na resistência de união à dentina radicular por meio do teste de push-out. Quarenta raízes de incisivos bovinos foram submetidas a tratamento endodôntico. A superfície dos pinos de fibra de vidro (Exacto #2, Angelus) foram tratadas com 4 protocolos diferentes (n=10): Controle - 70 % de etanol durante 1 min; 37 % de ácido fosfórico durante 1 min, 10% de ácido fluorídrico durante 1 min e 24 % de peróxido de hidrogênio durante 1 min. Depois foi aplicado agente de união silano por 1 min e todos os pinos foram cimentados com cimento resinoso auto-adesivo (RelyX Unicem, 3M- ESPE). As raízes foram seccionadas e foram obtidas duas fatias de 1 mm de espessura em cada terço: cervical, médio e apical. Os espécimes foram submetidos ao teste de push-out com uma velocidade de 0.5 mm/min. Os dados foram analisados pelo teste ANOVA com medidas repetidas, seguido pelo teste de Tukey HSD (=0,05). Os fatores tratamento de superfície (p<0,001) e região do terço radicular (p=0,007) foram significantes; no entanto, a interação entre os dois fatores não foi significante (p=0,827). A retenção ao canal radicular foi afetada pelo tipo de tratamento de superfície. O tratamento de superfície com 24% de peróxido de hidrogênio por 1 min rendeu significativamente maior resistência de união quando os pinos de fibra de vidro foram cimentados com RelyX Unicem.

RESUMO

Aim: The aim of this study was to evaluate the influence of different regions of dentin within the post space on the retention of fiber posts. Materials and Methods: Ten human incisors with straight roots were selected for this study. Endodontic treatment of the specimens was done. The post spaces were created immediately after obturation and the posts were luted with dual-cure resin cement. Approximately 2.5-mm-thick sections were made from the coronal, middle, and apical thirds of the post space and thus we had three groups: Group I: Cervical, Group II: Middle, Group III: Apical. The specimens were tested on a universal testing machine. Statistical analysis was done using the unpaired Student's t-test and one-way ANOVA test. Result: The best push-out strength was obtained with the apical sections (14.69±0.298 MPa), followed by the middle (10.66±0.34 MPa) and cervical sections (9.73±0.42 MPa). Conclusion: highest pust out strengths were obtained in apical sections followed by middle and coronal.
