RESUMO

We described for the first time the contrast enhancement of a giant fibrovascular esophageal polyp using ultrasound contrast agent, Sonovue® (Bracco, Milan, Italy) during echoendoscopy. Fine Doppler was unsuccessful in showing vascularization due to the mobile characteristic of the tumor. In contrast, via Sonovue®, tissue microcirculation was highlighted inside the entire head of the polyp, leading to better appreciate the risk of bleeding related to its resection. In a second part, we showed the feasibility of classic polypectomy for this giant polyp (5×5 cm) without complication and results of control endoscopy at 3 months. The present case is summarized in a video.

RESUMO

Fibrovascular polyps are rare benign intraluminal tumors that usually arise from the cervical esophagus. These often present as very large sized pedunculated polyps and cause symptoms including dysphagia and respiratory distress. Generally, large polyps are surgically excised, while endoscopic resection is limited to smaller polyps. Herein, we present a giant fibrovascular polyp of the esophagus treated successfully by endoscopic resection.

RESUMO

Giant fibrovascular polyps of the esophagus and hypopharynx are rare, benign, and tumor-like lesions of the upper digestive tract. Due to the initial lack of symptoms, these polyps are often clinically undiagnosed or misdiagnosed until they are significant in size. Most of them can be surgically removed by a cervical incision approach or endoscopic approach. However, we experienced a case of 37 years old male patient, who had a giant fibrovascular polyp of the hypopharynx removed by transoral approach because of the location and size of the stalk. We present this case of a giant fibrovascular polyp of the hypopharynx with a brief review of literature.

RESUMO

A five month female was referred complaining of intermittent vomiting with protrusion of a sausage-like mass through the oral cavity. Esophageal endoscopy and esophagogram revealed a mass in the upper esophagus, which was diagnosed as a fibrovascular polyp. Under general anesthesia, the mass was grasped through the oral cavity with a forcep and ligated and excised at the base, where a stump arose from the posterior wall of the cervical esophagus. The pathology was confirmed as a fibrovascular polyp, which is a rare benign esophageal lesion occurring mostly in adult males, and has not been reported in infancy.

RESUMO

Benign tumor of the esophagus are rare, and the true incidence rate is unknown. Fibrovascular polyp is the most common intraluminal tumor, although its occurrence is rare. The fibrovascular polyp has been called variously fibroma, fibrolipoma, myxofibroma, polyp, and pedunclated lipoma. The majority of fibrovascular polyps are solitary, but cases of multiple polyps have been reported. Recently the authors have experienced a case of fibrovascular polyp of the upper esophagus, regurgitated into the hypopharyngeal cavity which might been mistaken for laryngo-pharyngeal mass. We report this case with brief review of the literatures.
