RESUMO

The advantages and disadvantages of different treatment methods of traditional tibial fracture were summarized. The research progress on the treatment of tibial fracture with internal fixation plates as external fixators was introduced from two aspects of biomechanics and clinical application. It was proved that internal fixation plates as external fixators could reduce the structural stability, but this technique was still controversial to meet the mechanical requirements of fracture healing on biomechanics.Internal fixation plates as external fixators,it had the advantages of minimal invasion,high healing rate, less complications and favorable activities. It's pointed out clinical trials on its feasibility and availability as well as simulating body mechanics environment should be carried out to provide basis for relevant biomechanics researches.

RESUMO

Objective To design a medical fixation plate with simple structure and easy operation which can be applied to hemodialysis and IV transfusion patients. Methods The plate was composed of a low-temperature thermoplastic board with semi-cylindric shape and net-like structure and an elastic fixation bandage, which was used for the treatment of hemodialysis and IV transfusion patients. Totally 40 hemodialysis patients including 22 males and 18 females from July to December 2016 were divided into an observation group and a control group randomly and equally. There were 400 IV transfusion patients including 188 males and 212 females enrolled into an observation group and a control group randomly and equally. The observation group applied the developed plate, and then compared with the control group on the rate for needle dropping off, the rate for needle displacement, nurse satisfaction and patient satisfaction. Results For the hemodialysis patients with 2020 times of punctures, there were no cases for needle dropping off and only 1 case of needle displacement due to improper fixation;the patient and nurse satisfaction (higher than 99%) in the observation group were obviously higher than that in the control group (P<0.001). For the IV transfusion patients, there were no cases of needle dropping off and 2 cases of needle displacement due to the patients' abusing of the developed plate;the patient and nurse satisfaction in the observation group was obviously higher than that in the control group (P<0.05). Conclusion The plate fixes the limbs of the hemodialysis and IV transfusion patients efficiently, decreases the incidences of needle dropping off and displacement, reduces the workload of medical staffs, enhances the patient and nurse satisfaction, and thus is worthy promoting practically.

Objective: Application of tomotherapy with megavoltage CT (MVCT)image-guided technique into craniospinal radiotherapy to analyzethe fixation effect of whole-body fixation plate.Methods: A retrospective analysis of 10 cases receiving craniospinalradiotherapy was performed. All patients were in supine position and were fixed with a whole-body fixation plate combined with thermoplastic mesh. The MVCTimages acquired before tomography were matched with the kilovoltage CT (KVCT) images, sothat the setup errors in six directions were detected. The tomography was performed if thematching result was satisfactory, otherwise the patient needed to be setup for the secondtime when the matching result was not accepted by the clinical radiation therapists.Results: A total of 159 MVCT scans was conducted in 10 patients. The average setup errorsin left-right (LR), cranial-caudal (CC), anterior-posterior (AP), pitch, roll and yaw directionswere (1.94 ± 1.44) mm, (2.98 ± 2.51) mm, (1.71 ± 1.87) mm, (0.73 ± 0.64) degrees, (0.30 ±0.26) degrees and (0.34 ± 0.31) degrees, respectively. The probability of second setup was3.14% (5/159).Conclusion: MVCT image-guided technique can significantly reduce the setup errors incraniospinal tomography. The fixation effect of the whole-body fixation plate is acceptable.

PURPOSE: The purpose of this study was to compare the mechanical stability of three types of plate systems for opening wedge high tibial osteotomy. MATERIALS AND METHODS: Forty-eight fresh frozen porcine tibia specimens were assigned to three different fixation device groups: Aescular group (16 specimens) was fixed with Aescular plates; Puddu group (16 specimens) with a Puddu plate, and TomoFix group (16 specimens) with a TomoFix plate. We compared axial displacements under compression loads from 200 to 2,000 N and maximal loads at failure among 8 specimens per group. We also compared displacements under cyclic load after 100 cycles at a compressive load of 2,000 N among 8 specimens per group. RESULTS: In all three groups, displacement under compression load increased with the increase in the axial compressive load; however, no significant intergroup differences were observed in the mean values under tested loading conditions. The mean maximal loads at failure were not significantly different (6,055, 6,798, and 6,973 N in the Aescular, Puddu, and TomoFix groups, respectively; p=0.41). While the TomoFix group showed less extension and strain during the cyclic load test, the mean values showed no significant differences among groups. CONCLUSIONS: All three plate systems were found to provide fixation stability suitable for bearing axial compression and cyclic loads while walking.

RESUMO

[Objective]A new device of gear-distraction plate (GDP) for spine reduction and fixation was designed and its biomechanical characteristics was evaluated.[Method]The GDP implants were made of titanium alloy (TC4,Ti6AL4V) and the instruments were made of stainless steel after design.Eighteen fresh calf lumbar specimens were randomly divided into three groups, the GDP group had a biomechanical test contrast to control group(CD, Steffee) in load-responsive change, load-displacement, strength, stiffness, torsion intensity and ultimate strength.[Result]Results It is better than control group in load-responsive change, load-displacement, strength, stiffness, torsion intensity and ultimate strength, there was a significant difference between them (P
