RESUMO

Objective:Health education program based on information-motivation-behavioral skills model (IMB model) was developed, and its application effect in functional exercise after autogenous arteriovenous fistula (AVF) in hemodialysis patients was explored.Methods:Using the convenient sampling method, 100 patients undergoing AVF surgery in hemodialysis center of Qingdao Municipal Hospital from June 2020 to June 2021 were selected as the study objects. The patients were numbered according to the order of visit. The single number was the observation group, and the double number was the control group. There were 50 patients in each group. The control group was given routine AVF function exercise education, and the observation group was given AVF function exercise education based on the IMB model. Patients′ compliance with functional exercise, cephalic vein diameter, fistula maturation time, and the incidence of complications were compared between the two groups.Results:Finally, 49 cases in the control group and 50 cases in the observation group finished the research. The compliance of function exercise of the observation group was 94.0%(47/50), which was higher than 73.5%(36/49) in the control group, and the difference was statistically significant ( χ2=7.70, P<0.05). The maturation time of plasty in the observation group was (5.18 ± 1.14) weeks, shorter than that in the control group (5.94 ± 1.39) weeks, and the difference was statistically significant ( t=2.98, P<0.05). The cephalic vein diameter and blood flow controlled by hemodialysis pump in the observation group were (5.19 ± 0.28) mm and (218.40 ± 24.19) ml/min, respectively, higher than those in the control group (4.99 ± 0.34) mm and (200.41 ± 23.89) ml/min, the differences were statistically significant ( t=-3.21, -3.72, both P<0.05). The incidence of fistula complications in the observation group was 6.0%(3/50), lower than 22.4%(11/49) in the control group, and the difference was statistically significant ( χ2=5.52, P<0.05). Conclusions:The application of IMB model in hemodialysis patients with functional exercise after AVF plasty can effectively improve patients′ compliance, increase the diameter of cephalic vein and the blood flow controlled by dialysis pump, shorten the maturation time of internal fistula, and reduce the incidence of complications of AVF.

RESUMO

To develop an intelligent lower limb rehabilitation instrument which could realize the quantification and visualization of lower limbs' raising angle and frequency, using the smart client to realize the remote control, autonomous data acquisition and the establishment of database. Doctors had the access to the database in order to adjust the rehabilitation program in time to meet the individual requirement. We realized the design of intelligent lower limb instrument based on the Andriod smartphone, which is suitable for clinical and family use.

@#Gut microbiota, as a "microbial organ" in human body, has important physiological functions and participates in regulating body's homeostasis and health. Exercise can improve metabolism and immune regulation of body by promoting the composition and structure of gut microbiota, and even affects interaction of the "microbiota-gut-brain axis". This article reviewed the effects of various functions of gut microbiota on body and relationship between gut microbiota and exercise.

Objective To explore the effect of group management mode on compliance of pelvic floor function exercise for patients with post partum urinary incontinence (PPUI). Methods A total of 80 cases of patients with PPUI were randomly divided into the observation group and the control group. Patients in the control group received one-to-one routine health guidance. Patients in the observation group received group management mode including special subject teaching guidance, induction and communication between patients. Three months after the intervention, two groups of patients were evaluated at six months after postpartum by the pelvic floor muscle strength, one-hour urine pad test and pelvic floor muscle function exercise compliance. Results Six months after postpartum, the cure rate of the pelvic floor muscle strength was 100%(40/40) for typeⅠmuscle, 100%(40/40) for typeⅡmuscle in the observation group. In the control group, the cure rate for typeⅠmuscle was 70%(28/40) and 65%(26/40) for type Ⅱmuscle. The difference between these two groups was statistically significant (χ2=14.118, 16.970, P=0.000). The total effective rate of urinary incontinence was 100.0%(40/40) in the observation group, 67.5% (16/40) in the control group, and there was statistical significance (χ2=25.232, P=0.000). Evaluation of the compliance of pelvic floor function exercise showed that the rates were 72.5% (29/40) for complete compliance, 27.5%(11/40) for incomplete compliance and 0 for total non-compliance in the observation group. In the control group, these rates were 2.5%(1/40), 55.0%(22/40), 42.5%(17/40). And there was statistical significance as well (χ2=54.847, P=0.000). Conclusions Group management mode can improve the compliance of pelvic floor muscle function exercise and the strength of pelvic floor muscle, and improve the degree of urinary incontinence in postpartum patients with urinary incontinence.

Objective To explore the influence of acupuncture point massage combined with limb function exercise on ankle brachial index (ABI) and pulse wave velocity (PWV) in patients with coronary heart disease (CHD). Methods According to scores by grace score scale, 180 CHD patients were divided into three groups: low risk group (n=58), moderate risk group (n=68) and high risk group (n=54). Within the three groups, the patients were divided into the experiment group and the control group by using the random digital table. The control group was treated with routine nursing intervention , while the experiment groups accepted acupuncture point massage and limb function exercise training on the basis of control groups. We collected the values of ABI and PWV at four points-in-time: before intervention, 7 days after intervention, 30 days after intervention and 90 days after intervention. Results Repeated measurement data analysis of the experiment group and control group suggested that:in the moderate and high risk groups, there was statistically significant difference (P 0.05) in time and group interaction effect. The difference between the experiment group and control group was statistically significant (P0.05) in ABI&PWV interaction effect at the four time points between the experiment group and control group. In the low-risk group,the differences in time points compared with the main effect were insignificant (all P>0.05). In comparison of main effect at all the four time points, there was significant different in the moderate and high risk group (P0.05) of ABI&PWV before and 90-days after intervention. Conclusion Acupuncture point massage combined with limb function exercise can effectively improve the peripheral artery blood supply in CHD patients, lower ABI and promote PWV.

Objective To explore the influence of social support on compliance of respiratory function exercise and rehabilitation in patients with coronary artery bypass graft surgery. Methods A total of 102 patients with coronary artery bypass graft surgery were evaluated by General Data Survey Scale,Social Support Questionnaire and Compliance of Respiratory Function Exercise Scale.According to the situation of patients′social support, patients were divided into high score group,medium score group and low score group, then compliance of respiratory function exercise,postoperative ICU stay time, postoperative ventilation time, postoperative hospital stay, 6-min walking test (6MWD) and left ventricular ejection fraction (LVEF) were compared among three groups. Results The social support of patients with coronary artery bypass graft surgery in medium level,the score was (34.70 ±7.46) points. The postoperative ventilation time, postoperative ICU stay time in high score group were less than those in medium score group and low score group:(17.79±5.30) h vs. (42.41±14.18), (80.00±11.15) h, (24.29±6.00) h vs. (55.66±16.31), (92.46±10.21) h, and there were significant differences, F=47.340,110.252,P<0.01. The compliance of respiratory function exercise,6MWD and LVEF in high score group were better than those in medium score group and low score group:(38.93±24.37) points vs.(30.26±4.79), (19.85±4.45) points, (376.07±55.92) m vs. (328.83±59.27), (302.04±60.24) m, (60±4)%vs. (53±9)%, (51±10)%, and there were significant differences, F=286.040, 55.228, 20.159,P < 0.01. Conclusions Social support can improve the patients′adherence to treatment, and enhance the patients′ability to cope with stressful events,then accelerate the process of postoperative rehabilitation of patients effectively,and improve quality of life.

Objective To explore the influence of uncertainty in illness in compliance of respiratory function exer-cise and rehabilitation after surgery in patients with coronary artery bypass graft (CABG) surgery. Methods Mishel uncer-tainty in illness scale and compliance of respiratory function exercise scale were used to collect data in 108 patients with CABG. The correlation of the uncertainty in illness and compliance of respiratory function exercise was analyzed. Patients were divided into three groups (high score, medium score and low score groups) according to the detection of uncertainty in illness. Values of compliance of respiratory function exercise, postoperative care time, postoperative ventilation time, postop-erative hospital stay duration, 6-min walking test (6MWD) and left ventricular ejection fraction (LVEF) were compared be-tween three groups. Results The uncertainty in illness was in medium level (95.51±9.48) in patients with CABG surgery, which was negatively correlated with compliance of respiratory function exercise (r=-0.926, P<0.01). The postoperative venti-lation time and postoperative care time were less in low score group than those of medium score group and high score group. The compliance of respiratory function exercise, 6-min walking test and postoperative LVEF were better in low score group than those of medium score group and high score group. Conclusion Medical personnel should take targeted interventions to reduce the uncertainty in illness in patients, enhance the compliance of respiratory function exercise, reduce the postopera-tive complications and promote early recovery of patients.

Objective To evaluate the effects of clinical nursing path on respiratory function exercise of patients with esophageal carcinoma.Methods A total of 60 postoperative patients with esophageal cancer were divided into the observation group and the control group according to the time period with 30 cases in each group.The observation group was given the clinical nursing pathway care in their respiratory exercise,and the control group routine nursing before and after the operation. The two groups were compared in terms of blood gas analysis implement on 5th day,pulmonary function on 10 th day and pulmonary complications after operation.Results PaO2,VC,MVV,FEV1 and FEV1/FVC in the observation group were much better than those of the control group.The incidence of complications was obviously lower than that of the control group(P<0.05). Conclusion Clinical nursing path used in respiratory function training of perioperative patients is effective for the improvement of pulmonary function and reduction of postoperative complications.

Pulmonary rehabilitation identifies and treats the systemic effects of the disease and the positive outcomes are realised without demonstrable improvements in lung function. It is multidisciplinary, patient centred and provides a comprehensive assessment upon which the three components of exercise training, self-management education, and psychosocial/ behaviour intervention are conducted over a period lasting 6 to 12 weeks. Pulmonary rehabilitation administered after hospital admission for an exacerbation was shown to be able to improve quality of life, improve exercise capacity, and also reduce re-exacerbation and hospital admission. Self-management education may promote long-term adherence to the exercise program.

Objective To know the influence of preoperative respiratory function exercise on the postoperative pulmonary function among patients with lung cancer. Methods Divided 80 patients with lung cancer into the observation group and the control group, there were 40 cases in the each group. Routine perioperative nursing cares was used in the control group, while a two weeks respiratory function exercise was used in the observation group. Compared the incidence rate of complications be-tween the two groups after the intervention, some indexes which can indicate the respiratory function were compared before and after the exercise in the observation group. Results The indexes such as PaO_2, SpO_2, MVV, TCL and FEV in the observation group after the respiratory function exercise were better than before significantly. The incidence rate of complications in the control group was more than in the observa-tion group. Conclusions Strengthen respiratory function exercise can effective improve the respiratory func-tion for patients with lung cancer when cured by thoractomy, which can promote patients' compliance, and then avoid certain postoperative complications.

The operation methods, clinical classification, postoperative function exercise of gluteal muscles contracture were investigated. Clinically and retrospectively, treatment of 1280 patients with gluteal muscles contracture, being subjected to a "Z-shaped" release lengthening operation and efficiency exercise, was clearly standardized. All the cases were followed up from 3 months to 2 years with the effective rate being 100%, the cure rate being 98.5%, the recent complications being 5% , and the far complications being 0.2%. It was concluded that the clear diagnosis combined with standarized operation and efficiency functional exercise could greatly improve the therapeutic effects of gluteal muscles contracture.
