RESUMO

In recent years, natural orifice specimen extraction surgery (NOSES) has been widely used in surgery of colorectal cancer. The rapid development of NOSES is mainly attributed to its own great advantages and values, including the reduction of surgical trauma, the acceleration of postoperative recovery and the reduction of adverse psychological reactions for patients. These advantages of NOSES are also important embodiment and perfect interpretation of the organ functional protection. Organ functional preservation is a hot topic in surgery today, and it is also an inevitable requirement for minimally invasive surgery. Essentially, NOSES and organ functional preservation are proposed in the same background, and the goals are highly compatible. NOSES is an important practitioner of organ functional preservation, and organ functional preservation is also the vane of the development of the theoretical system of NOSES. These two items complement each other and together constitute the important element in the development of modern minimally invasive surgery. In order to comprehensively discuss the relationship between NOSES and organ functional protection, we elaborate the important role and value of functional protection in NOSES from five key procedures of colorectal surgery, namely surgical approach, extent of resection, lymph node dissection, digestive tract reconstruction and specimen extraction.

RESUMO

With the standardized training and spreading,laparoscopic total mesorectal excision for rectal cancer has been spread nationwide.The oncological adequacy and long-term outcomes have also been confirmed by series of randomized controlled trials such as COLOR Ⅱ in Europe.There were still some technical issues,such as safe surgical plane,surgical approach,lymph node dissection,and function-preserving,which were still under debate.In the current article,authors will discuss the issues with clinical experiences about preserving of Denonvilliers facial,preserving of pelvic autonomic nerve,strategy of surgical approach,dissection of lateral lymph nodes,and necessity of protective diverting stoma.

BACKGROUND AND OBJECTIVES: Laryngeal cancer is the most common malignancy in head and neck region. The aim of this study was to evaluate the clinical features and treatment outcomes of laryngeal cancer. MATERIALS AND METHODS: We reviewed medical records of 169 patients from 1984 to 1997 retrospectively. We investigated age, sex distribution, TNM staging in 169 patients and treatment modalities, survival rate in 141 patients who were treated. RESULTS: In all patients, there were 64 glottic, 57 supraglottic, 46 transglottic and 2 subglottic cancers. Among 141 patients, we performed 92 surgical procedures and 49 radiotherapies. Total and near total laryngectomy were performed in 45 cases and conservation laryngectomy in 47 cases. Five year survival rate (YSR) was 88.4% totally. In early cancer cases, 5 YSR was excellent in both surgery and radiotherapy group. Glottic and supraglottic cancer showed much better prognosis than transglottic cancer. There was no statistical differences in the view of 5 YSR between surgery and radiotherapy group. Treatment failures were mainly found at primary sites. Functional preservation of larynx was successful in 79 cases after 3 year-follow up. CONCLUSIONS: Compared with previous studies, our study presented higher proportion of conservation laryngectomy and functional preservation procedues. Early diagnosis and treatment are important and required to improve patient's outcome.
