ABSTRACT Introduction: Prostate cancer (PC) is the second most commonly diagnosed cancer in males. 68Ga-PSMA PET/CT, a non-invasive diagnostic tool to evaluate PC with prostate-specific membrane antigen (PSMA) expression, has emerged as a more accurate alternative to assess disease staging. We aimed to identify predictors of positive 68Ga-PSMA PET and the accuracy of this technique. Materials and methods: Diagnostic accuracy cross-sectional study with prospective and retrospective approaches. We performed a comprehensive literature search on PubMed, Cochrane Library, and Embase database in search of studies including PC patients submitted to radical prostatectomy or radiotherapy with curative intent and presented biochemical recurrence following ASTRO 1996 criteria. A total of 35 studies involving 3910 patients submitted to 68-Ga-PSMA PET were included and independently assessed by two authors: 8 studies on diagnosis, four on staging, and 23 studies on restaging purposes. The significance level was α=0.05. Results: pooled sensitivity and specificity were 0.90 (0.86-0.93) and 0.90 (0.82-0.96), respectively, for diagnostic purposes; as for staging, pooled sensitivity and specificity were 0.93 (0.86-0.98) and 0.96 (0.92-0.99), respectively. In the restaging scenario, pooled sensitivity and specificity were 0.76 (0.74-0.78) and 0.45 (0.27-0.58), respectively, considering the identification of prostate cancer in each described situation. We also obtained specificity and sensitivity results for PSA subdivisions. Conclusion: 68Ga-PSMA PET provides higher sensitivity and specificity than traditional imaging for prostate cancer.

RESUMO

Deamidation is one of the most common degradation impurities in protein and peptide drugs. The deamidation of glutamine and asparagine in the protein sequence can lead to changes in the chemical and biological properties of the protein. However, the rutine trypsin-based pretreatment process can significantly increase the artificial deamidation impurities during the digestion process, resulting in high determination level. In this study, after optimizing the conditions of Glu-C enzymatic hydrolysis, we obtained the best enzymatic conditions under acidic condition and the artificial deamidation impurities significantly reduced in digestion process, identified the deamidation site (N48). Through the methodological investigation and comparison of the measurement results of different methods, the specificity, reproducibility and accuracy of the method are verified. The method established in this research has laid a solid foundation for the accurate determination of deamidation impurities in cobratide and its similar protein peptide biochemical drugs.

BACKGROUND: To investigate the thalamic neurotransmitters and functional connections in the development of chronic constriction injury (CCI)-induced neuropathic pain. METHODS: The paw withdrawal threshold was measured by mechanical stimulation the right hind paw with the von frey hair in the rats of CCI-induced neuropathic pain. The N-acetylaspartate (NAA) and Glutamate (Glu) in thalamus were detected by magnetic resonance spectrum (MRS) process. The thalamic functional connectivity with other brain regions was scanned by functional magnetic resonance image (fMRI). RESULTS: The paw withdrawal threshold of the ipsilateral side showed a noticeable decline during the pathological process. Increased concentrations of Glu and decreased levels of NAA in the thalamus were significantly correlated with mechanical allodynia in the neuropathic pain states. The thalamic regional homogeneity (ReHo) decreased during the process of neuropathic pain. The functional connectivity among the thalamus with the insula and somatosensory cortex were significantly increased at different time points (7, 14, 21 days) after CCI surgery. CONCLUSION: Our study suggests that dynamic changes in thalamic NAA and Glu levels contribute to the thalamic functional connection hyper-excitation during CCI-induced neuropathic pain. Enhanced thalamus-insula functional connection might have a significant effect on the occurrence of neuropathic pain.

OBJECTIVE: To study the chemical constituents of bamboo juice. METHODS: The compounds were isolated and purified by column chromatography on silica gel, ODS, macroporous resin, MPLC and HPLC. Their structures were elucidated by NMR spectroscopic evidence and compared with those in literature. RESULTS: Fifteen compounds were isolated and identified as (+)-lyoniresinol-3α-O-β-D-glucopyranoside (1), 5, 5'-dimethoxylariciresinol-4'-O-β-D-glucopyranoside (2), syringaresinol 4'-O-β-D-glucopyroside (3), (-)-4-epi-lyoniresinol (4), (-)-lyoniresinol -2α-O-β-D-glucopyranoside (5), (+)-lyoniresinol (6), 8, 8'-bisdihydrosiringenin glucoside (7), 2, 4, 6-trimethoxyphenyl-1-O-β-D-glucopyranoside (8), threo-8S-7-meth-oxysyringylglycerol (9), salidroside (10), (7S, 8R)syringoylglycerol (11), syringaldehyde (12), 2, 6-dimethoxy-1, 4-benzoquinone (13), n-butyl-β-D-fructopyranoside (14), and n-butyl pyroglutamate (15).CONCLUSION: Compounds 1-11 and 14-15 are isolated from bamboo juice for the first time. Compounds 14 and 15 are β-D-fructopyranoside and pyroglutamic acid artifacts produced during the extraction.

Resumen La leishmaniosis cutánea es una enfermedad causada por un parásito pro tozoo intracelular; el glucantime es una opción terapéutica, aunque está asociado con alteraciones cardiovasculares, siendo la más frecuente la prolongación del intervalo QTc que se presenta entre el 17,8 % y 19 % de los pacientes. Si este efecto no es detectado a tiempo puede causar una arritmia fatal por torsade de pointes. Se presenta el caso de una paciente de 77 años quien se encontraba en tratamiento con glucantime intramus cular como tratamiento de leishmaniosis cutánea e ingresó por un cuadro clínico de hipocalemia severa refractaria y episodios de torsade de pointes; posteriormente, presentó fibrilación ventricular que no respondió a des fibrilación y reanimación. Las alteraciones en la repolarización cardiaca producidas por este medicamento se consideran secundarias a la acu mulación de compuestos pentavalentes y trivalentes en el miocardio. No existe tratamiento específico para esta situación, pero siempre se debe realizar manejo de soporte, evitar fármacos que prolonguen el intervalo QT, normalizar los niveles de potasio y de magnesio, elevar la frecuencia cardiaca con isoproterenol e implantar marcapaso transvenoso para lograr sobre-estimulación y reducción de los periodos refractarios.


Abstract Cutaneous leishmaniasis is a disease caused by an intracellular protozoan parasite; Glucantime is a therapeutic option, although it is associated with cardiovascular alterations, the most frequent being the prolongation of the QTc interval, that occurs between 17.8% and 19% of patients. If this effect is not early recognized, it can cause a fatal arrhythmia due to torsade de pointes. The case of a 77-year-old patient who was receiving intramuscu lar glucantime as treatment for cutaneous leishmaniasis is presented, the patient was admitted with severe refractory hypokalemia and episodes of torsade de pointes; subsequently, presented ventricular fibrillation that did not respond to defibrillation and resuscitation. The alterations in cardiac repolarization produced by this medicine are considered secondary to the accumulation of pentavalent and trivalent compounds in the myocardium. There is no specific treatment for this situation, but supportive manage ment should always be performed, avoid drugs that prolong the QT inter val, normalize potassium and magnesium levels, raise the heart rate with isoproterenol and implant transvenous pacemaker to achieve over-stimulation and reduction of refractory periods.

OBJECTIVE: To explore the precise mechanism of electroacupuncture (EA) to delay cognitive impairment in Alzheimer disease. Methods N -Acetylaspartate (NAA), glutamate (Glu) and myoinositol (mI) metabolism were measured by magnetic resonance spectroscopy, learning and memory of APP/PS1 mouse was evaluated by the Morris water maze test and the step-down avoidance test, neuron survival number and neuronal structure in the hippocampus were observed by Nissl staining, and BDNF and phosphorylated TrkB detected by Western blot. RESULTS: EA at DU20 acupuncture significantly improve learning and memory in behavioral tests, up-regulate NAA, Glu and mI metabolism, increase the surviving neurons in hippocampus, and promote the expression of BDNF and TrkB in the APP/PS1 transgenic mice. CONCLUSION: These findings suggested that EA is a potential therapeutic for ameliorate cognitive dysfunction, and it might be due to EA could improve NAA and Glu metabolism by upregulation of BDNF in APP/PS1 mice.

AIM:To explore the protective effect of morinda officinalis oligosaccharides monomer HexB on hy -poxia/reoxygenation(H/R)-induced injury in human umbilical vein endothelia cells(HUVECs).METHODS:HUVECs were treated with HexB,4-phenylbutyric acid(4-PBA)and thapsigargin(TG),respectively.The cells were divided into control group,HexB group,H/R group,HexB+H/R group,4-PBA+H/R group,TG group and HexB+TG group.The cell viability was measured by CCK-8 assay.The apoptotic rate was detected by flow cytometry.Western blot was used to determine the protein levels of endoplasmic reticulum stress(ERS)related molecules chaperone protein glucose-regulated protein 78(GRP78),C/EBP homologous protein(CHOP),apoptosis-related protein caspase-12 and phosphorylated c-Jun NH2-terminal kinase(p-JNK).RESULTS: The viability of HUVECs was reduced in H/R group and TG group(P<0.05),increased in HexB+H/R,4-PBA+H/R and HexB+TG group(P<0.05).The apoptotic rate,the protein levels of GRP78,CHOP,caspase-12 and p-JNK were increased in H/R group and TG group(P<0.05),weakened in the HexB+H/R group(P<0.05),4-PBA+H/R group and HexB+TG group(P<0.05).No significant change in the apoptotic rate,cell viability,protein levels of GRP78, CHOP, caspase-12, p-JNK between HexB +H/R group and 4-PBA+H/R group was observed.CONCLUSION:HexB attenuates HUVECs injury caused by H/R through suppressing ERS and ap-optosis.The possible mechanism may be involved in the apoptotic pathways related to GRP 78,CHOP,caspase-12 and p-JNK.

Objective: To investigate the effects of Cbp/p300 interacting transactivator with Glu/Asp rich carboxy-terminal domain 1 (CITED1) gene silencing on the proliferation, apoptosis and migration of papillary thyroid carcinoma K1 cells. Methods: The recombinant lentivirus carrying CITED1-shRNA or the negative control (NC)-shRNA was successfully infected into thyroid cancer K1 cells. The silencing efficiency of CITED1 gene was detected by real-time fluorescent quantitative PCR and Western blotting, respectively. The cell proliferation was detected by CCK-8 method, the cell cycle distribution and apoptosis were detected by FCM, and the cell migration ability was detected by scratch wound healing assay. Results: After the recombinant lentivirus carrying CITED1-shRNA was successfully infected into papillary thyroid carcinoma K1 cells, the expression levels of CITED1 mRNA and protein were significantly decreased (both P < 0.01), which suggested that the K1 cells with CITED1 gene silencing were successfully obtained. After silencing CITED1 gene expression, the cell proliferation was significantly inhibited (P < 0.01), the apoptosis rate was significantly increased (P = 0.001), the proportion of G0/G1-phase cells was significanlty increased (P = 0.007), the proportion of G2/M- and S-phase cells was significanlty decreased (both P < 0.05), and the cell migration abilities at 12 h and 24 h were significantly decreased (both P < 0.01). Conclusion: Silencing CITED1 gene expression can inhibit the proliferation and migration of papillary thyroid carcinoma K1 cells, and promote the apoptosis of tumor cells.

Objective To investigate the neuroprotective mechanisms of Cerebroprotein Hydrolysate for Injection (Ⅰ) on vascular dementia in rats.Method The rat vascular dementia model was prepared using an improved two-vessel occlusion method,and the common carotid artery was only isolated but not blocked in sham group.Rats were randomly divided into sham group,model group,Cerebroprotein Hydrolysate for Injection (Ⅰ) groups with low,medium and high dose (5,10,20 mg/kg) and Cerebroprotein Hydrolysate Injection group (Cerebrolysin,Positive drug,10 mg/kg).The drug was administered by iv injection of rat tail vein once a day for two weeks,while the same volume of saline was administered in sham and model group.At the end of administration,the plasma was collected through abdominal aorta to separate serum,and rat cortex was isolated to prepare homogenate.The levels of nerve growth factor (NGF) and insulin-like growth factor 2 (IGF-2) in serum and level of gamma-aminobutyric acid (GABA) in cortex were detected by ELISA.Level of glutamate (Glu) in cortex of VaD rats was detected by colorimetry.Results Compared with model group,levels of NGF and IGF-2 in the serum of VaD rats and level of GABA in cortex were significantly increased,while level of Glu in cortex was significantly decreased after administration of Cerebroprotein Hydrolysate for Injection (Ⅰ).The increased IGF-2 and GABA levels by Cerebroprotein Hydrolysate for Injection (Ⅰ) were significantly higher than that of Cerebrolysin at same dose.Conclusion The mechanisms underlying the increased leaming and memory ability of VaD rats by Cerebroprotein Hydrolysate for Injection (Ⅰ),are possibly related to the increased levels of NGF and IGF-2 in body and a regulation of the balance between excitatory and inhibitory amino acid neurotransmitters.

Objective To observe the clinical efficacy of Huoxue Rongluo Particles combined with acupuncture in the eight confluent points of spastic cerebral infarction paralysis and its effects on Glu and Asp levels of serum. Methods Totally 60 patients were divided into two groups: the experimental group and the control group. Both groups received basic Western medicine treatment, and the experimental group received Huoxue Rongluo Particles additionally, one dose a day for two times orally taken; Acupuncture was on eight confluent acupoint, every two days. 15 d is a treatment course, with 6 courses in total. Clinical spasticity index (CSI) and TCM symptom scores before treatment and the treatment of half month, 1 month, 3 months were observed. The levels of Glu and ASP in serum were detected, and TCM clinical efficacy was observed. Results The CSI score, levels of Glu and Asp in serum and TCM symptom scores of the patients after half-month, one-month, and three-month treatment were lower than those before treatment (P0.05). The TCM symptom scores in experimental group was lower than the control group after half-month, one-month and three-month treatment (P<0.05). The total TCM effective rate was 86.67% (26/30) in experimental group, and 80.00% (24/30) in the control group, with the experimental group better than the control group (P<0.05). Conclusion Huoxue Rongluo Particles combined with acupuncture eight confluence acupoints in the treatment of spastic cerebral infarction paralyzed patients can relieve spasm degree, improve TCM clinical symptoms, which mechanism may be related to reducing serum excitatory neurotransmitters.

Objective To compare the effects of glutathione and tiopronin in chemo -induced liver injury respectively.Methods 122 patients with gastrointestinal cancer who were diagnosed with chemo -induced liver injure were chosen.According to random number table,they were divided into treatment group(63 cases)and control group(59 cases).The control group were given glutathione 1.8g/d intravenously,while the treatment group were given tiopronin 0.2g/d.Both treatments lasted a week.Liver function indexes before /after the treatment were observed respectively,as well as the symptoms of every patient.The adverse drug reactions of both treatments were also observed.Results The remission ratio of the treatment group and the control group was 90.48% and 81.35%, respectively,the difference of the two groups was statistically significant(χ2 =7.65,P 0.05);after both treatment the indexes were significantly improved,and ALT,TBIL,AST and ALP between the two groups were significantly different(t =4.67,6.13,4.76,6.90,all P 0.05),and no severe adverse drug reactions were observed in our research.Conclusion Tiopronin is more effective in treating chemo -induced liver injure caused by capetcitabine /5-FU and oxaliplatin,when compared with glutathione.And no severe adverse drug reaction were observed in our research.

Objective:To investigate the pharmacokinetics of mycophenolic acid( MPA)and its metabolites in different stages af-ter the administration of enteric-coated mycophenolate sodium( EC-MPS)tablets in Chinese liver transplant recipients. Methods:The blood samples of 24 patients were collected in 0-12h of the 1st and 3rd week after the administration of EC-MPS. The concentrations of MPA,AcMPAG and MPAG in plasma were measured by LC-MS/MS developed in our lab. The pharmacokinetic parameters of MPA and its metabolites were estimated by non-compartmental method. Results:After 1-and 3-week therapy with EC-MPS,Cmax ,AUC0-12 and t1/2 was(18. 1 ± 8. 75)and(20. 7 ± 16. 0)μg ml-1 ,(42. 7 ± 17. 5)and(47. 1 ± 23. 9)μg·h·ml-1 ,(3. 33 ± 2. 81)and (3.30 ±1.89)h for MPA;(2.50 ±1.86)and(1.78 ±1.72)μg·ml-1,(14.5 ±11.7)and(6.97 ±6.57)μg·h·ml-1, (4. 48 ± 2. 53)and(3. 76 ± 1. 8)h for AcMPAG;(171. 6 ± 135. 4)and(152. 2 ± 115. 9)μg·ml-1 ,(1299 ± 1 204)and(1 051 ± 561)μg·h·ml-1 ,(8. 73 ± 4. 25)and(7. 75 ± 2. 87)h for MPAG,respectively. There was no significant difference in the PK parameters of MPA after the 1-and 3-week therapy. The Cmax ,Tmax and t1/2 of MPA in the patients received EC-MPS were significantly higher than those in the patients received MMF(P<0. 05). Cmax and AUC0-12 of MPAG in the patients received EC-MPS were signifi-cantly higher than those in the patients received MMF after the 3-week therapy(P<0. 05). Conclusion:There is no significant accu-mulation of MPA after the therapy with EC-MPS at different stages. The absorption of MPA is delayed after the therapy with EC-MPS compared with that with MMF. There is no difference in MPA exposure between EC-MPS and MMF in Chinese liver transplant patients.

OBJECTIVE: To investigate the flavonoid glycosides in Clematis connata DC. METHODS: Colum chromatography with different materials, such as RP-18, Sephadex LH-20 and silica gel, and semi preparative high performance liquid chromatography were used to isolate and purify the chemical constituents. Their structures were identified by spectroscopic analysis. RESULTS: Eleven flavonoid glycosides were isolated and identified as kaempferol-3-O-β-D-glucopyranosyl methyl ester (1), isovitexin (2), kaempferol-3-O-β-D-glucopyranoside-7-α-L-rhamnopyranoside (3), kaempferol-3-O-α-L-rhamnopyrano-side-7-α-L-rhamnopyranoside (4), kaempferol-3-O-α-L-(4-O-acetyl) rhamnopyranoside-7-α-L-rhamnopyranoside (5), kaempferol-3-O-(2-β-D-glucopyranosyl)-α-L-rhamnopyranoside-7-O-α-L-rhamnopyranoside (6), genistein-7-O-β-Z)-apiofurano-syl-(1→6)-O-β-D-glucopyranoside(7), lanceolarin(8), quercetin-3-O-β-D-(6″-n-butyl) glucuronide(9), quercetin-3-O-β-D-glucopyranosyl methyl ester(10), and isoorientin (11). CONCLUSION: Compounds 1-11 are isolated from Clematis connata DC for the first time.

Objective:To sduty the effect of β-lactam antibiotics efiriaxone on the levels of glutamate and glutamate transporter subtype-1 in hippocampus following traumatic brain injury in rats.Methods: All rats were divided randomly into three groups:sham group;TBI group and CTX group.The rat model of TBI were made by modified Feeney method ,and treated with ceftriaxone immediately after injury ( 200 mg/kg ).Wet-dry weight method was used to evaluate brain edema.The content of glutamate in hippocampus was measured with the high performance liquid chromatography.The expression of GLT-1 in hippocampus areas was tested by immunohisto-chemical and Western blot methods.Results:Compared with TBI group ,TBI-induced cerebral edema was significantly attenuated ( P<0.05 ) , the content of glutamate in hippocampus was were significantly decreased ( P<0.05 ) , the level of GLT-1 were significantly increased in CTX group ( P<0.05 ).Conclusion: β-lactam antibiotics ceftriaxone can block the excitatory neurotoxicity , reduce the extent of brain edema.

Aims To establish a UPLC-MS method for quantifying protocatechuie acid,kaempferol-3-O-β-D-glucoside and quercitrin and to investigate the pharma-cokinetics of three indix components in flower of Poly-gonumorientale L.in rat plasma.Methods The anal-ysis was achieved by BEH C18 column (2.1 mm ×100 mm,1.7 μm)with a mobile phase composed of 0.1%formic acid using step gradient elution.A TQD tandem mass spectrometry equipped with electrospray ionization source was used as detector and operated by selected ion recording(SIR)mode.Results In the selected linear range,calibration curves of the three markers components showed good linearity.Extraction recovery rate,precision,accuracy and stability reached the de-termination request.The parameters of Tmax (h ) in three index components were 0.46 ±0.1,0.79 ±0.33 and 2.63 ±4.6,respectively;Cmax (μg·L -1 )in three index components were 463.8 ±207.81,18.53 ±7.82 and 137.38 ±71.09,respectively.Conclusion The fully validated UPLC-MS method has been successfully applied to the pharmacokinetic study of the three index components in flower of Polygonumorientale L.in rat plasma.

[Summary] MS ,which is highly related tothe occurrence of cardiovascular disease and T2DM ,is characterized by glucose and fat metabolic dysregulation ,central obesity ,hypertension and hyperuricemia.11β‐hydroxysteroid dehydrogenase type 1 (11β‐HSD1) is regarded as a new therapeutic target for MS.11β‐HSD1 converts inactive glucocorticoid to active glucocorticoid. 11β‐HSD1 knockout improves obesity , hyperlipidemia andhyperglycemia. The inhibition of 11β‐HSD1 alleviates IR in human and rodents ,but the application of 11β‐HSD1 inhibitors is limited by the side effects.

Objective To evaluate the efficacy of intratympanic steroid therapy compared with systemic ster‐oid therapy on the treatment of idiopathic sudden sensorineural hearing loss (ISSNHL) patients with damaged glu‐cose tolerance .Methods Fifty first -diagnosed unilateral ISSNHL patients with damaged glucose tolerance were randomized devided equally to the intervention group (intratympanic steroid therapy) or the control group (systemic steroid therapy) ,all patients received conventional drug therapy simultaneously .Pure-tone hearing threshold tests were performed in all patients every 3 days after the first time ,and repeated measures anova was used to assess effects of hearing recovery accompanied with time .Results The mean hearing threshold in the control group de‐creased from 85 .4 ± 5 .6 dB to 48 .2 ± 4 .9 dB ,while in the intervention group it decreased from 84 .8 ± 5 .6 dB to 31 .7 ± 4 .6 dB .Total effective rate in the intervention group (84 .00% ,21/95) was higher than that in the control group (68 .00% ,17/25)(P<0 .05) .Conclusion The intratympanic steroid therapy is more effective than systemic steroid therapy in the treatment of ISSNHL patients with damaged glucose tolerance .

DEAD (Asp-Glu-Ala-Asp) box polypeptide 43 (DDX43) plays an important role in the malignant proliferation,drug resistance of tumor cells as well as neoplasm immunotherapy.The overexpression of DDX43 has been found in various solid tumors and some hematologic malignancies.The hypomethylation of DDX43 gene promoter is identified in chronic myeloid leukemia,acute myeloid leukemia and myelodysplastic syndrome,which is correlated with DDX43 overexpression and the prognosis of these diseases.

Objective To explore the change characteristics of early renal blood infusion in patients with diabetes and its relation-ship with fasting blood sugar by using multi-slice spiral CT (MSCT)perfusion scan.Methods Thirty cases of T2DM patients within five years of disease course that meet clinical diagnostic criteria (poor DN glycemic control group and good DN glycemic control group with 1 5 cases in each group)and 1 5 cases in the control group underwent bilateral renal perfusion scan using 64-detector spiral CT,thus obtaning their cortical perfusion parameters of bilateral kidneys,including blood flow (BF),blood volume (BV),mean transit time (MTT)and capillary permeability surface (PS).At the same time,for each case,fasting glucose,blood urea nitrogen, serum creatinine and blood uric acid value on the third days after and before perfusion were also measured;the glomemlar filtration rate (C-GFR)was estimated.Statistical analysis was performed on all of these obtained values.Results (1).For the poor DN gly-cemic control group,the average BF value,average BV value and average PS value were reduced,average MTT was prolonged sig-nificantly,and compared with normal group,average BF value and average MTT were statistically significant (P 0.05)for both normal control group and DN groups in the third day before and after renal CT perfusion imaging examination.Conclusion BF,BV and MTT of MSCT perfusion scan can reflect the characteris-ticsof early renal blood infusion in patients with diabetes.And changes of fasting blood sugar in patients with diabetes may influence mean BF and mean MTT of kidney.
