RESUMO

ABSTRACT Introduction: There are different possibilities of orthodontic planning for cases with congenital absence of maxillary lateral incisors. This subject divides the opinion of orthodontists and oral rehabilitation clinicians, due to the advantages and disadvantages of each treatment option, which may involve opening spaces for future implants and/or prosthetic restorations, or closing the spaces by positioning the maxillary canines in the place of lateral incisors. The correct diagnosis and careful evaluation of each patient allow to determine the best therapeutic approach. This paper discusses the main topics to be considered when planning these cases. Objectives: To evaluate the main aspects related to orthodontic treatment planning in cases of congenital absence of maxillary lateral incisors, to aid the decision-making, with clinical and scientific basis.


RESUMO Introdução: Existem diferentes possibilidades de planejamento ortodôntico para os casos que apresentam ausência congênita de incisivos laterais superiores. Esse é um assunto que divide a opinião de ortodontistas e reabilitadores orais, devido às vantagens e desvantagens de cada uma das opções de tratamento, as quais podem envolver a abertura de espaços para futuros implantes e/ou restaurações protéticas ou o fechamento dos espaços, com posicionamento dos caninos superiores no lugar dos incisivos laterais. O correto diagnóstico e uma criteriosa avaliação de cada paciente permitem determinar a melhor abordagem terapêutica. Nesse artigo, serão discutidos os principais tópicos a serem considerados no planejamento desses casos. Objetivos: Avaliar os principais aspectos relacionados ao planejamento do tratamento ortodôntico nos casos de ausência congênita de incisivos laterais superiores, de maneira a auxiliar nas tomadas de decisão, com embasamento clínico e científico.

RESUMO

PURPOSE: Most of the former studies about the occlusal contact patterns during the mandibular movement focused on foreigner. The purpose of this study is analyzing the occlusal contacts of young Koreans by using T-Scan system. MATERIALS AND METHODS: The sample size was 87 and the occlusal contacts of each right and left lateral movements were measured from the maximum intercuspation to the 3mm excursive position for three times respectively. All of the occlusal contacts were double checked through the thin metal foil. The results were categorized as two; 1) considering occlusal contact patterns on working side only, 2) considering occlusal contact patterns on working and nonworking sides. RESULTS: The results showed that the nonworking side occlusal contacts play major roles in the lateral mandibular movement. In both cases of considering with and without the nonworking side occlusal contacts, the group function was the most prevalent. In the working side, the contacts were the most frequent in canine and the frequency of contacts was decreased as the distance was increased from canine to molar. In the nonworking side, the contacts were the most frequent in second molar. And the gender factor was statistically significant (alpha=.05), as females have more nonworking side occlusal contacts in this study. CONCLUSION: Among the three factors of anterior guidance, the group function was the most dominant factor. The analysis of 87 samples showed that canine contact was the most frequent in the working side and second molar contact was the most common in the nonworking side.

Introduction: The prevalence of group function and canine guided occlusion is common in natural dentition, but cross arch balance is not often seen. Hence, a study was carried out with following objectives: Analysis of incidence of cross arch balance in different age groups and working side occlusal wear facets in following subjects. Materials and Methods: A total of 200 patients with natural dentition were included in the study. Subjects with cross arch balance were analyzed for working side occlusal wear facets. Data were analyzed statistically by Chi-square test. Results: The incidence of cross arch balance in the study group was 0.7%. On examination, subjects with group function on the working side and cross arch balance on the non-working side had more occlusal wear facets. Conclusion: From the study, it was observed that there was no significant variation in the number and extent of wear facets between the cross arch and without cross arch subjects due the smaller sample size hence it was difficult to conclude the relationship non-working side contact as interference and its influence in wearing of dentition in this study.

PURPOSE: To study the distribution of various occlusal schemes as well as associated static occlusal relationship among Thais. MATERIALS AND METHODS: The subjects' occlusal schemes on the left and right sides were classified as canine protected occlusion, group function, or unclassified according to the definition from Glossary of Prosthodontic Terms (8th edition). In addition, the presence of occlusal interferences during mandibular excursions were recorded. The measurement of the horizontal overlap and vertical overlap was also performed. Chi-square, One-way ANAVA and Mann-Whitney U statistical tests were used with level of significance set at P=.05. RESULTS: Total of 104 subjects were included in this study. The ratio for male to female was 1.8 to 1. Average age of the population was 25.01+/-6.87 years old. The mean vertical overlap and horizontal overlap were 1.94+/-1.20 and 2.41+/-1.32 mm respectively. The majority of the populations (68.3%) possessed group function occlusal scheme. For the remaining, 17.3% possess canine protected occlusion and 12.5% possess combination of both occlusal schemes. We also found that occlusal interference was presented in 20.2% of the population. The most common was protrusive interference (57.14%), the second was balancing interference (38.1%) and the third was working interference (4.1%). CONCLUSION: Among Thais, the most common occlusal scheme was group function, however there were no significant occlusal factors related to any particular occlusal scheme.

A variety of functional tooth contact patterns exist in population of young adult patients.The purpose of the present study is to find the relationship between different lateral tooth contact patterns and dental morphology and their effect on masticatory efficiency. Forty young dentulous subjects were selected and their impression with irreversible hydrocolloid was made. Polyvinyl silicone bite registration paste was used to record lateral tooth contact patterns in functional range of mandibular movement. Overjet and overbite including mesiodistal relation of tooth recoded with cast secured in the intercuspal position and masticatory efficiency was determined by calorimetric method by using raw carrot. Among all subjects 15% having canine protection, 55% group function, and 30% balance occlusion. There is positive correlation between age and masticatory efficiency [r=+0.656, p<.01] and negative correlation between overjet and masticatory efficiency [r=-0.409, p<0.01]. In the present study Canine protection occlusion had significantly lower mean masticatory efficiency than group function and balanced occlusion group. This signifies that the masticatory efficiency is dependent on lateral tooth contact patterns.

La sedación intravenosa se ha convertido en una de las técnicas más comúnmente empleadas para evitar traumas psicológicos a los pacientes que necesiten de algún tipo de cirugía bucal. Pues como todos sabemos el término Cirugía está asociado a miedo y ansiedad, y si se agrega la palabra Bucal que implica Odontología, para muchos es sinónimo de dolor y angustia. La necesidad de manejar el paciente que se somete a una cirugía bucal sin ansiedad, tensión, miedo, ni dolor, plantea la posibilidad de usar la sedación intravenosa como método para eliminar esas situaciones ante el procedimiento quirúrgico pero es muy importante la presencia del Anestesiólogo como integrante indispensable junto con el Cirujano para poder ser utilizada, además del ayudante del cirujano y el personal auxiliar. En el presente trabajo se revisarán las funciones de cada uno de los integrantes del equipo multidisciplinario que se necesita para que éste tipo de alternativa de tratamiento quirúrgico se aplique coordinada y eficazmente, permitiendo reducir al mínimo o evitando el riesgo que pudiera alguna vez ocasionar su aplicación, brindando al paciente la vigilancia de parámetros vitales y el suficiente confort antes, durante y después de la cirugía.


Intravenous sedation has become the most common procedure to avoid psychological trauma in the oral surgery patients. As we know the word Surgery is associated with fear and anxiety and if we add the word Oral that imply Dentistry for the majority of the people is synonymous of pain and suffer. The importance of management these feelings bring the possibility of use the intravenous sedation as a technique that controls and avoid those situations but it is very important to use an anesthesiologist as a indispensably member with the surgeon besides the surgical assistant and the hygienist. We are going to expose the specific functions of each member of the multidisciplinary team that is needed to apply this kind of surgical treatment efficiently and allowing to minimize or avoid the risks that in any case it should occur, taking good care of the patient before, during and after the surgery.
