Abstract The aim of this study was to analyze the association between adherence to school meals and the co-occurrence of the regular consumption of healthy and unhealthy eating markers among Brazilian adolescents. Data from 67,881 adolescents in Brazilian public schools who participated in the 2015 National School Health Survey, were used. From the 7-day FFQ, the dependent variable was constructed, co-occurrence of regular consumption (≥ 5x/week) of healthy and unhealthy food markers, which was categorized as regular consumption of none, one or two, or three eating markers. We performed an ordinal logistic regression with adjustment for sociodemographic, eating habits outside of school, and school characteristics variables. The prevalence of the co-occurrence of the regular consumption of three healthy eating markers was 14.5%, and that of three unhealthy markers was 4.9%. High adherence to school meals (every day) was positively associated with regular consumption of healthy eating markers and inversely associated with regular consumption of unhealthy eating markers. The school meals provided by PNAE contribute to the promotion of healthy eating habits among Brazilian adolescents.


Resumo Este estudo tem como objetivo analisar a associação entre a adesão à alimentação escolar e a coocorrência do consumo regular de marcadores de alimentação saudável e não saudável entre adolescentes brasileiros. Foram avaliados 67.881 adolescentes de escolas públicas brasileiras participantes da Pesquisa Nacional de Saúde do Escolar (PeNSE) de 2015. A partir do QFA de sete dias, construiu-se a variável dependente, coocorrência do consumo regular (≥ 5x/semana) de marcadores de alimentação saudável e não saudável, que foi categorizada em consumo regular de nenhum; um ou dois; ou três marcadores de alimentação. Realizou-se regressão logística ordinal com ajuste para variáveis sociodemográficas, hábitos alimentares fora da escola e características da escola. A prevalência da coocorrência do consumo regular de três marcadores de alimentação saudável foi de 14,5%, e de três marcadores de alimentação não saudável foi de 4,9%. A alta adesão à alimentação escolar (todos os dias) foi positivamente associada ao consumo regular de marcadores de alimentação saudável e inversamente associada ao consumo regular de marcadores de alimentação não saudável. A alimentação escolar fornecida pelo PNAE contribui para a promoção de hábitos alimentares saudáveis entre os adolescentes brasileiros.

Abstract Introduction Historically, the difficulty of referrals to specialized health services has been discussed. Objective To analyze the factors associated with waiting time for specialized physiotherapy services. Methods Cross-sectional, multilevel study, with secondary data, based on the external evaluation of the second cycle of the Access and Quality Improvement Program and the United Nations Development Program. The explanatory variables for the outcome "Estimated waiting time of users for specialized consultations - Physiotherapy" were grouped according to the characteristics of the family health teams and contextual factors. In the statistical analysis, Poisson's Multilevel Regression was used. A significance level of 5% was adopted (p < 0.05). Results Observed association of the time of referral to physiotherapy with the health teams that receive support for the planning of the work process (n = 25.476; 83,4%; p < 0.0001), that the management provides information about the health situation (n = 26.505; 86,8%; p < 0.0016), receives support for the discussion of monitoring data (n = 24.149; 79,1%; p < 0.0001), receives permanent institutional support (n = 25.140; 82,3%; p < 0.0001), gets feedback from the evaluation carried out by the specialists (n = 22.801; 76,6%; p < 0.0001) and in the teams that are supported by the NASF physiotherapist (n = 5.666; 18,6%; p < 0.0001), with an association for the Gini Index (p < 0.044) and the HDI (p < 0.0001). Conclusion The referral time to the specialized physiotherapy service was shown to be associated with both system organization and management factors, as well as contextual variables.


Resumo Introdução Historicamente, discute-se a dificuldade de encaminhamentos para o serviço especializado de saúde. Objetivo Analisar os fatores associados ao tempo de espera para os serviços especializados de fisioterapia. Métodos Estudo transversal, multinível, realizado a partir do Programa de Melhoria do Acesso e da Qualidade e do Programa das Nações Unidas para o Desenvolvimento. Adotou-se a variável desfecho: tempo estimado de espera dos usuários para atendimento especializado de consultas de fisioterapia. Na análise estatística, utilizou-se a regressão multível de Poisson e adotou-se nível de significância de 5% (p < 0,05). Resultados Observou-se associação do tempo de encaminhamento para fisioterapia com as equipes de saúde que recebem apoio para o planejamento do processo de trabalho (n = 25,48; 83,4%; p < 0,0001), como também para a gestão que disponibiliza informações sobre a situação de saúde (n = 26,55; 86,8%; p < 0,0016), recebe apoio para a discussão dos dados de monitoramento (n = 24,149; 79,1%; p < 0,0001), recebe apoio institucional permanente (n = 25,14; 82,3%; p < 0,0001) e obtém retorno da avaliação realizada pelos especialistas (n = 22,80; 76,6%; p < 0,0001). Nas equipes que são apoiadas por fisioterapeutas do Núcleo de Apoio à Saúde da Família (n = 5,67; 18,6%; p < 0,0001), verificou-se associação entre o Índice de Gini (p < 0,044) e o Índice de Desenvolvimento Humano (p < 0,0001). Conclusão O tempo de encaminhamento para o serviço especializado de fisioterapia mostrou-se associado tanto com fatores de organização e gestão do sistema quanto com variáveis contextuais.

Abstract Background: Cardiovascular risk in adolescence is a public health problem that has grown along with the increase in soft drink consumption. Objective: To investigate the association between cardiovascular risk factors and daily consumption of soft drinks in Brazilian adolescents. Methods: We conducted a cross-sectional, national, school-based study of 36,956 Brazilian adolescents aged 12 to 17 years. Daily soft drink consumption was estimated using a 24-hour dietary recall. Cardiovascular risk was categorized as overweight, obesity, hypertension, hyperglycemia, and dyslipidemia. The survey command of Stata 14.0® was used to analyze data from a complex sample. The chi-square test was used to assess differences in soft drink consumption and other variables in the descriptive analysis. The odds ratio of cardiovascular risk factors and their respective 95% confidence intervals were estimated, considering sociodemographic and behavioral variables in the Mantel-Haenszel model. Statistical significance was set at p < 0.05. Results: Daily consumption of soft drinks was common among adolescents. A daily serving ≥ 450 mL was significantly associated with overweight and hypertension. Results associated with the consumption of regular soft drinks show the possibility of reverse causality. Consumption of diet soft drinks in adolescence should be considered a cardiovascular risk factor. Conclusion: Daily consumption of soft drinks can be understood as a relevant risk factor in the epidemiological scenario. Improper eating habits are multifactorial in nature and need to be better understood in the context of adolescent health and further explored in national surveys.

Objective To investigate the influencing factors on serum homocysteine level in healthy retired-elderly people in Tianjin,in order to provide theoretical basis and guidance for clinical intervention therapy.Methods A total of 184 participants meeting the standard of Chinese healthy elderly people (2013) were selected from 1247 retired-elderly people who took healthy physical examination in three grade Ⅲ-A hospitals in Tianjin from June to August in 2014.They were divided into two groups based on plasma Hcy level:hyper-homocystinemia group (≥15 μmol/L) and normal plasma Hcy level group (<15μmol/L).The influencing factors on plasma Hcy level were analyzed by multiple stepwise regression and logistic regression analysis.Results The proportion of healthy retired-elderly people accounted for 14.76 %,with men at 82.61% and women at 17.39 %,mean age (78.37±6.88) years.The Hcy level was higher in healthy males than in healthy females [(16.90± 6.86) μmol/L vs.(12.89 ± 5.97) μmol/L,P<0.005].The proportion of people with hyper-homocystinemia accounted for 45.65%,and the mean Hcy concentration was (21.39±6.98) μmol/L.The age of people with hyper-homocystinemia was higher than that with normal Hcy [(80.58±6.09) years vs.(76.56±7.04) years,P<0.005].There were no statistically significant differences in blood pressure,lipid and blood glucose levels and the proportion of patients with coronary heart disease,hypertension,type 2 diabetes mellitus,gout,periodontitis and other diseases between the two groups (all P>0.05).Spearman correlation analysis showed that the level of serum Hcy was positively correlated with serum uric acid levels (P=0.000).Multiple stepwise regression and logistic regression analysis showed that age,gender and serum uric acid level were risk factors for plasma Hcy level in healthy elderly people.Conclusions Age,gender and serum uric acid level are risk factors for serum Hey level in healthy elderly people.
