RESUMO

Hobnail hemangioma is a benign vascular tumor that typically presents as a small, solitary lesion on the trunk or limbs of young or middle-aged persons. Characteristically,the tumor has a targetoid appearance where a violaceous papule is surrounded by an ecchymotic rim. Hobnail hemangioma is characterized by a biphasic growth pattern of the neoplastic vascular structures and prominent matchstick-like endothelial cells. Based on the positivity of the endothelial cells for monoclonal antibody D2-40, it has been proposed that this tumor displays lymphatic differentiation. Angiolipomas are benign subcutaneous tumors that consist of mature adipocytes and blood vessels. They are usually multiple and they typically occur in young adults with an obvious male predilection. We describe here a case of hobnail hemangioma that occurred together with angiolipoma in an 8-year-old girl, who had a violaceous papule encircled by a peripheral ecchymotic halo on the left heel.

RESUMO

Targetoid hemosiderotic hemangioma (THH) is a solitary vascular tumor typically occurring in young or middle-aged persons, and affecting the limbs or trunk. Characteristically, the lesion has a "targetoid" appearance where a violaceous papule is surrounded by an ecchymotic or brown ring that can expand or subsequently disappear with the persistence of the central papule. THH is characterized histologically by dilated vascular structures in the superficial dermis lined by prominent hobnail endothelial cells and collagen-dissecting, rather narrow neoplastic vessels in the deeper parts of the lesion. We report a 17-year-old girl who presented showing typical clinical and histological features of targetoid hemosiderotic hemangioma.

The lesion has a "targetoid" appearance, where a violaceous papule is surrounded by an ecchymotic or brown ring. THH is characterized histologically by dilated vascular structures in the superficial dermis lined by prominent hobnail endothelial cells and collagen-dissecting, rather narrow neoplastic vessels in the deeper parts of the lesion. It is important to distinguish this tumor from patch stage of Kaposi's sarcoma, solitary angiokeratoma, and retiform hemangioendothelioma. We report a 12-year-old girl showing typical clinical and histological features of targetoid hemosiderotic hemangioma.

Hobnail hemangioma is a benign vascular tumor that typically presents as a small, single lesion on the skin of the trunk or limb. It occurs mainly in young or middle-aged adults (mean age: 30 years) but rarely in childhood. Histologically, hobnail hemangioma is characterized by a biphasic growth pattern, with dilated vascular spaces lined by hobnail endothelial cells in the superficial dermis, and collagen dissection narrow vascular channels in the deeper dermal parts, as well as inflammatory aggregates, fibrosis, and hemosiderin deposits. We report a case of hobnail hemangioma in a 10-year-old girl who had a violaceous papule surrounded by an ecchymotic halo on the left thigh. She underwent treatment with surgical excision with primary closure. To the best of our knowledge, our case is the first reported case of childhood hobnail hemangioma in Korean dermatological literatures.

Hobnail hemangioma is a benign vascular tumor characterized by a violaceous or brown papule that is surrounded by a thin pale area and a peripheral ecchymotic ring. Histologically the lesion consists of a superficial dermal vascular proliferation. In the upper dermis, dilated vascular spaces often lined by endothelial cells that protrude in to the lumina and closely resemble hobnail. In the deeper dermis, slit-like vascular structures appeared to dissect between collagen bundles. It is important to distinguish these tumors from patch stage of Kaposi's sarcoma, solitary angiokeratomas, retiform hemangioma and progressive lymphangioma. We report a 29-year-old woman showing typical clinical and histological features of hobnail hemangioma.

Hobnail hemangioma(HH) is a benign acquired vascular tumor of endothelial origin which should be differentiated from other malignant vascular neoplasm such as Kaposi's sarcoma or angiosarcoma. We report a case of hobnail hemangioma in a 21-year-old woman who had a dusky-red patch on her left shin. Histologically, ectatic vascular channels with a single layer of plumped endothelial cells were seen and the vascular channels seemed to dissect the collagen bundles. She underwent treatment with surgical excision with primary closure.

Hobnail hemangioma, also known as "targetoid hemosiderotic hemangioma", represents a distinctive benign vascular tumor, characterized histologically by a biphasic growth pattern of dilated vascular structures lined by prominent hobnail endothelial cells in the superficial dermis, and collagen dissection by angulated vessels in deeper parts of the lesion. It is important to distinguish this tumor from patch stage Kaposi's sarcoma, retiform hemangioendothelioma and progressive lymphangioma. We report a 26-year-old man showing typical clinical and histological features of hobnail hemangioma.
