RESUMO

Objective:To analyze the current development status, research hot-spots and development trends of county-level Chinese hospitals in China.Methods:By retrieving the papers of county-level Traditional Chinese Medicine (TCM) hospitals in China National Knowledge Infrastructure (CNKI), Chinese Science and Technology Periodicals Full-text Database (VIP), Wanfang and China Biomedical Database (SinoMed ) with CiteSpace 5.5.R2 software to draw maps of the author, institution and keyword and make analysis.Results:A total of 598 papers are included in this study, and the trend of publications shows an upward trend and then a downward trend. Researchers conduct research in the form of forming research teams. The research institutions are mainly medical colleges and scientific research institutions. The most frequently showed keyword is "medical reform". The keyword knowledge map shows that the research content mainly involves medical reform, problem analysis and countermeasures, economic operation efficiency, TCM service capability, data envelopment analysis and so on.Conclusions:The county-level TCM hospitals generally show sound development trend in China. The research trend is dominated by county-level TCM hospitals. Scale development has been transformed into the study of hospital operation efficiency. County-level TCM hospitals need to start from their own actual conditions, strengthen cooperation in the form of teams and strengthen academic cooperation among institutions, so as to realize the basic TCM services for ordinary people.

Objective To evaluate the present development and status of quality control for intensive care unit (ICU) in Sichuan Provincial traditional Chinese medicine (TCM) hospitals including integrated traditional Chinese and western medicine hospitals and ethnic hospitals, and to provide practical references for improving the service quality of ICU. Methods Supervisory Group of Sichuan Provincial Critical Care Medicine Quality Control Center of TCM was established in September 2018. From September 8th to 17th, 2018, according to the Scoring Criteria of Quality Control and Supervision Project of TCM for Critical Care Medicine, a 10-day quality control professional guidance was hand out to TCM hospitals with independent ICU in Sichuan Province. The service level of different aspects of hospital quality control was evaluated and ranked from equipment and resource support, medical team, service capacity and level, ward quality, completion of critical care core indicators, completion of quality control of TCM, development of new technologies, diagnosis and treatment schemes for dominant diseases. Results There were 52 TCM hospitals across the province that had an ICU. Thirty-three hospitals were third-class (63.5%), while the rest 19 hospitals were second-class (36.5%). Province-level, city-level and county-level hospitals were accounted for 9.6% (5/52), 38.5% (20/52), and 51.9% (27/52), respectively. Average bed ratio of ICU was 1.8%. Doctor-bed and guard-bed ratios were 0.71∶1 and 2.0∶1, respectively. The average annual admission rate of patients and the average daily admission rate of beds were higher, which were basically 1%. Ward quality was high; the incidence of nosocomial infection was controlled below 10%. Compliance rate of septic shock bundle treatment was high. The incidences of ventilator-associated pneumonia (VAP), catheter-related bloodstream infection (CRBSI) and catheter-associated urinary tract infection (CAUTI) were 0.45%, 0.22%, and 0.30%, respectively. Participation rate of TCM was about 83.4%. Average number of new technologies was about 4.4. Average number of disease schemes was about 2.62. Conclusions ICU of Sichuan Provincial TCM hospitals reaches the standard level in service capacity and level, ward quality, critical medicine quality control, and participation rate of TCM treatment. Improvements are required for other prospects, including department scale, medical personnel allocation, new technical development, diagnosis and treatment schemes of dominant diseases.

RESUMO

With the implementation of the comprehensive reform of medical separation in Beijing at April 8 of 2017, more than 3600 medical institutions in the city were involved, including the public hospital of traditional Chinese medicine. ThisIt was an opportunity of public hospitals of traditional Chinese medicine, and it also showed several challenges. This paper analyzed the situation separation of medicine and drug, the opportunity and challenge to put forward reasonable countermeasure and suggestion for the public hospital of traditional Chinese medicine.

Objective To evaluate the effect of the comprehensive reform of medical prices of traditional Chinese medicine hospitals in Jiangsu Province,to provide empirical support for the effectiveness of the reform of government drug prices,and to provide suggestions for further deepening the reform.Methods Based on the effect model of Wei Tang's public policy evaluation model,according to the comprehensive reform goal of medical price,analyzing the effect before and after the implementation of the policy.Results After the reform,the financial subsidy income,medical insurance income and service income have increased,the proportion of drug revenue has dropped by 3.3%,and some medical control fee indicators have reached the policy standards,but the asset liability ratio has increased.Conclusion The reform has achieved some initial results.The income compensation is basically reasonable,but still not enough.The income structure is obviously optimized.The control of medical expenses has improved,but it has not reached the ideal state.

The factors influencing the information service of public WeChat platform in hospitals of traditional Chinese medicine were found by further analyzing and comparing the number of published papers, contents of information service, publication time of papers in such hospitals with Guangdong Provincial Hospital of Traditional Chinese Medicine and Jiangsu Provincial Hospital of Traditional Chinese Medicine as exam-ples. Suggestions were put forward for strengthening the operation and development of public WeChat platform and improving its information service.

Objective To investigate the distribution and drug resistance change of Pseudomonas aeruginosa in the hospital of traditional Chinese medicine to provide a reference for the prevention and control of hospital infections.Methods Various submitted specimens from the inpatients in our hospital from Jan.2012 to Dec.2014 were performed the isolation and culture,bacterial identification and drug susceptibility analysis.Results In these 3 years,the detection rate of Pseudomonas aeruginosa positive strains was gradually increased year by year;the detection rate of sputum specimens was highest;which were mainly isolated from the patients in the respirtion department and cardiology department;the drug resistance rate of non-restricted,restricted and special antibacterial drugs showed an overall downward trend.Conclusion The sseudomonas aeruginosa isolated rate is increased year by year,the antimicrobial drugs classification management achieves initial results,and the overuse or unreasonable use of antibacterial drugs should be prevented.

The management of human resources in the hospital of traditional Chinese medicine should adopt new ideas and system, and change the traditional human resource management mode. Through studying the status of human resource information management in hospital of traditional Chinese medicine, the relevant measures and suggestions for the construction of human resource information management system in hospital of traditional Chinese medicine are put forward in this paper.

Objective To analyze the relationship between measures and economic benefit under the total medical insurance prepaid system in Xuzhou TCM hospital.Methods The trend of financial data of Xuzhou TCM hospital from 2010 to 2013, its incomes, assets, and liabilities were analyzed to evaluate its profitability, operating capacity and solvency.Results Income:The annual average income was 439.81 million yuan, with an annual average growth rate of 12%;the annual average net profit was 24.36 million yuan, with an annual average growth rate of 0.004% (excluding the profit adjustment factors in 2010). Assets:The annual average total assets was 355.82 million yuan, in which 73.21 million yuan was the added value in 2010 compared with 2009 and the growth rate was 17.22%;247.71 million yuan has been reduced in 2011 compared with 2009;45.11 million yuan has increased in 2012 compared with 2011, while in 2013, 35.58 million yuan has increased compared with 2012. The state owned assets has increased significantly. Liabilities:The annual total liabilities were 114.86, 84.03, 112.35, and 120.52 million yuan respectively from 2010 to 2013;the average debt level was 105.61 million yuan. The rate of assets and liabilities were 23.04%, 33.52%, 37.98%, and 36.37% respectively from 2010 to 2013;the annual average debt rate was 29.68%. Profitability:The average net operating margin, net interest rate of total assets and net assets yield rate were 5.54%, 6.85% and 9.73% respectively, which showed the gradually increased profitability. Hospital operating capacity:The average total assets turnover rate was 124% from 2010 to 2013. The solvency of the hospital:asset liability ratio of the hospital was 29.68% from 2010 to 2013 on average, which was in the low level of debt.Conclusion Under the total prepaid system, the measures of the hospital are proper. Economic operation of the hospital is favorable, giving the consideration of combining both economic benefit and social benefit.

Objective To investigate the status of research and research utilization in practice among nurses of hospital of Traditional Chinese Medicine.Methods Two hundred and sixty nurses of hospital of Traditional Chinese Medicine were selected to fill out the questionnaire.Results The rates of involvement of research activities and research utilization among nurses of hospital of Traditional Chinese Medicine were low.The score of every factor ranged from the higher to the lower were communication,organization,research and nurse.Conclusions Involvements of research activities and research utilization among nurses of hospital of Traditional Chinese Medicine are insufficient.Most factors associated with nurses' low extent of research utilization are objective factors.It is pivotal for nursing managers to construct supportive professional environment,improve nurses' research and information skills and the capacity of the research utilization.

A plan of constructing a platform of excavating and protecting characteristic technology of diagnosis and treatment in research-based hospital of TCM was put forward by Shuguang Hospital Affiliated with Shanghai University of Traditional Chinese Medicine and Research Institution of Characteristic Technology of Diagnosis and Treatment Affiliated with Shanghai Academy of Traditional Chinese Medicine. The platform has begun to take shape on the basis of previous theoretical thought and preliminary practice. It has its own management system and working standards and contents with a clear and definite developing direction. Its working contents include excavating and protecting clinical experience of famous and aged practitioner of Chinese medicine, folk proved recipe and technology of diagnosis and treatment of TCM, intangible cultural heritage of TCM. Some problems appeared during the work, such as the evaluation criterion of the folk proved recipe and technology of diagnosis and treatment, the management of clinical proving projects, the issue of intellectual property protection and so on, which must be further solved.

Objective To investigate the awareness of AIDS knowledge among medical staffs and their preventive behaviors among hospitals of traditional Chinese Medicine in Liangshan and to analyze the influential factors so as to provide the scientific basis for the staffs' training as well as for the prevention and control about AIDS. Methods Medical staffs from 7 hospitals of traditional Chinese medicine in Liangshan were surveyed using self-designed questionnaires,among which 276 were valid. Results The total score of the medical staffs' knowledge on AIDS was 34.14 ±6.92 points. Those with the highest score accounted for 16.73%, others with intermediate score accounted for 56.13%and still others with the low score accounted for 27.14%. The total score of the medical staffs' preventive behaviors on AIDS was 35.20 ±9.48 points, with the highest score being 12.64%, the intermediate score being 30.86%and the low score being 56.51%. The major factors to influence the medial staffs' knowledge about AIDS related to his/her educational level, specialty, education background of education, trainings on AIDS and contacts with HIV/AIDS patients and so on (<0.05) . The hospital's level, the specialty, whether or not being trained with AIDS knowledge concerned the level of occupational protection ( <0.05), whereas their ages, titles of a technical post, working years of working were irrelevant. Conclusion The overall level of these medical staffs’knowledge and preventive behaviors has not been satisfactory. Therefore, the various training programs will be necessary to improve the technical level of the medical staffs.

Objective: To analyze economic operation situation of public hospital of traditional Chinese medicine in Jiangsu. Methods: Through the investigation and analysis, to collect situation of hospital health resources, medical service, the resources utilization efficiency and balance. Based on the actual data on the present situation and the development of hospitals of traditional Chinese medicine, the systematic analysis of the income and expenditure was made. Results: The problems in the development of Chinese medicine hospitals and the countermeasures are found, references for the medical institution managers and the basis for policy makers are provided. Conclusion: Hospitals of traditional Chinese medicine need to continue deepening the reform, and actively developing the features of TCM advantages, finding their own positioning goals, trying to explore development service of traditional Chinese medicine and Chinese medicine products, so as to cultivate the self-core competitiveness.

Objective To explore the causes of job burnout in nurses from emergency departments of TCM hospitals.Method Job Burnout of Nurses Scale was adopted to investigate job burnout in 20 nurses from TCM emergency department and 18 from general emergency department.Result The scores on the sources of job pressure,job burnout,the physical and mental symptoms among the nurses from the TCM emergency department were significantly higher than those from the general emergency department(P<0.01). Conclusions The nurses from TCM emergency department may be more susceptible to job burnout than those from the general emergency department for possible reasons like training and shift planning.It is critical for the stabilization of nursing staff to draw attention to their personal development and mental and physical health,improve the work mechanism and raise their occupational self-identity.

RESUMO

OBJECTIVE To investigate the antimicrobial application in a hospital of traditional Chinese medicine(TCM).METHODS Data of antimicrobial application in patients who were discharged within a month were investigated.RESULTS The usage rate of antimicrobials was 62.62%,the surgical departments were higher in antibacterial use than departments of internal medicine;the pathogenic detection rate was 17.28%;the usage rate of combinations accounted for 34.14%;the duration of preoperative and postoperative antimicrobial use was long,some patients didn′t receive rationally preoperative prophylactic antimicrobials.CONCLUSIONS Management of clinical antimicrobial application in hospital of TCM should be strengthened,especially on the indication of antimicrobial and pathogenic detection.
