@#OBJECTIVE: It is common to get lost during a comminuted proximal humerus surgery, and the pectoralis major insertion is always a constant. Therefore, this study aimed to do a cadaveric study on the Filipino population to assess the distance from the pectoralis major tendon to the top of the humeral head (PMT) as a reference during proximal humerus surgery. METHODS: This study dissected the shoulders of cadavers. The distance from the pectoralis major tendon insertion to the top of the humeral head (PMT) was measured using a caliper. This PMT distance was also correlated to the cadaver's height and sex. RESULTS: This study dissected 110 shoulders (55 cadavers | 24 females, 31 males). The median PMT was 5.40 cm for males and 4.90 cm for females, with a combined value of 5.40 cm overall. There was a direct and moderate correlation between the PMT with overall height. Height and PMT of both the left and right shoulder were significantly longer among males compared to females. The study showed that for every centimeter increase in the height of males, there was a corresponding 0.02 cm increase in the PMT, adding the constant factor of 1.83. A corresponding 0.04 cm increase in the PMT for females added the constant factor of -0.81. CONCLUSION; The pectoralis major tendon insertion is a consistent landmark that can accurately restore humeral length when reconstructing complex proximal humerus fractures where landmarks are otherwise lost because of comminution.

Background: This study was carried out to evaluate humeral length among children in Southern Nigeria and to describe its sexual dimorphism among study population.Methods: This study involved 450 children (230 males and 220 females) between 3-14 years old in Southern Nigerian.  The humeral length was measured as distance between lateral epicondyle distally palpated when elbow was flexed to 90 degrees and acromion proximally palpated lateral end of clavicle. Definitive right and left humeral lengths (RHL and LHL) were derived by subtracting 2.5 mm from measured length. Average values of RHL and LHL were evaluated as morphological humeral length (MHL) for each subject. Data were analyzed using IBM-SPSS version 20 and statistical comparisons done using t-test with p < 0.05 regarded as level of significant difference.Results: In all age groups, non-significant bilateral variation was observed with the RHL higher than the LHL among both male and female subjects. In addition, the results showed significant (p<0.05) sexual dimorphism in all age groups with mean±SEM of MHL among 3-6 years old higher in females (19.45±0.81) than in males (18.63±0.83).  However, the reverse was observed among higher age groups with mean±SEM of MHL in 7-10- and 11-14-years old males (24.43±0.95 and 28.75±0.94) significantly higher than in 7-10 years old and 11-14 years old females (22.85±0.91 and 26.73±0.84) respectively.Conclusions: Based on findings of this study, humeral morphometrics particularly the humeral length can be applied as a significant indicator of sexual dimorphism among the study population.

OBJECTIVE: We investigated the usefulness of shortening of the fetal femur length (FL) and humeral length (HL) to predict Down syndrome at the middle gestation of pregnancy in Korean subjects. METHODS: This retrospective study involved 41 fetuses with Down syndrome and 328 fetuses with normal chromosome between 14+0 and 28+6 weeks of gestation. The expected FL and HL for any biparietal diameter (BPD) was calculated based on the control group data. The odds ratios for measure to expected FL and HL in comparison between normal fetuses and Down syndrome fetuses were calculated. The sensitivities of short FL and HL to predict Down syndrome were analyzed at a fixed false positive rate of 5%. RESULTS: The lengths of femur and humerus long bone for any BPD in Down syndrome group were significantly shorter than the lengths in control group (P<0.001). A low ratio of measured to expected FL and HL increased the risk of fetal Down syndrome (P<0.001). At a fixed false positive rate of 5%, the sensitivities were 21.3% (95% confidence interval [CI] 0.698~0.852, P<0.001) in FL and 29.9% (95% CI 0.773~0.914, P<0.001) in HL. CONCLUSION: Both of short FL and HL are poor marker for Down syndrome at the middle gestation of pregnancy in Korean population though the sensitivity of HL was higher than FL.
