RESUMO

Objective To analyze the influence of early treatment of pregnancy combined with subclinical hypothyroidism (SCH) during pregnancy on pregnancy outcome.Methods From January 2013 to June 2015,128 pregnant women who were diagnosed as SCH in the Maternal and Child Health Hospital of Beilun District were randomly divided into the observation group and control group according to the digital table,with 64 cases in each group.The observation group was given levothyroxine early treatment,while the control group adopted conventional hospitalization,unexposed.Pregnancy outcomes of the two groups were analyzed.Results After treatment,the incidence rates of complications in the observation group:preeclampsia (4.69%),poor fetal growth (1.56%),premature rupture of membranes (7.81%),less amniotic fluid volume (4.69%),placental abruption (0%),postpartum hemorrhage (3.12%);the incidence rates of complications in the control group:preeclampsia (17.19%),poor fetal growth (10.94%),premature rupture of membranes (21.88%),less amniotic fluid volume (15.63%),placental abruption (6.25%),postpartum hemorrhage (12.50%),and there were statistically significant differences between two groups (x2 =5.133,4.800,5.006,4.195,4.129,3.905,P =0.023,0.028,0.025,0.041,0.042.0.048).After treatment,the incidence rates of adverse pregnancy outcomes:preterm birth (3.12%),cesarean section (60.94%),abortion (0.00%),fetal distress (4.69%),perinatal death (0.00%);the incidence rates of adverse pregnancy outcomes in the control group:preterm birth (12.50%),cesarean section (79.69%),abortion (7.81%),fetal distress (15.63%),perinatal death (6.25%),and the differences between the two groups were statistically significant (x2 =3.905,5.389,5.203,4.195,4.129,P =0.048,0.020,0.023,0.041,0.042).Conclusion Pregnancy complicated with SCH has different degree influence on patients with obstetric complications and outcome of pregnancy,early effective drug treatment can significantly reduce the incidence of obstetric complications and adverse pregnancy outcomes.

RESUMO

A screening of thyroid function was performed in 2 320 pregnant women whose gestational age was less than 12 weeks.The incidence of subclinical thyroid disease and its relationship with gestational diabetes mellitus were analyzed.The results showed that there were 8 patients (0.34%) with clinical hypothyroidism,226 (9.74%) with subclinical hypothyroidism,and 14 patients (0.6%) with hyperthyroidism among 2 320 pregnant women.The prevalence of gestational diabetes mellitus in pregnant women with subclinical hypothyroidism was apparently higher than that in normal pregnant women (33.3% vs 12.9%,P<0.05),suggesting that the increased incidence of gestational diabetes mellitus seems to be related to subclinical hypothyroidism during early pregnancy.Therefore,it is important to screen the function of thyroid during early pregnancy

RESUMO

Objective To investigate the prevalence and epidemiologic characteristics of hypothyroidism among community population in Jiangsu province. Methods The residents who had lived for at least five years and aged more than 20 years old were sampled from six layers in urban and rural districts of Jiangsu province by a stratified cluster sampling technique. Serum was sampled from 7 122 subjects and sTSH was measured by chemiluminescence immunoassay, and FT3 and FT4 were determined in the subjects with abnormal sTSH level. Results (1) The crude prevalences of overt hypothyroidism and subclinical hypothyroidism were 0.66% and 7.53% respectively in total population, with the respective standardized rates of 0.43% and 6.28%. (2)The prevalences of hypothyroidism and subclinical hypothyroidism were significantly higher in females than in males (both P <0.05). (3) The prevalence of subclinical hypothyroidism was significantly increased with advancing age in both female and males (P<0.05). Conclusion Comparing with hypothyroidism, subclinical hypothyroidism shows higher prevalence in Jiangsu province, and more attention should be paid to the follow-up and diagnosis of subclinical hypothyroidism.

RESUMO

hypothyroidism is a common subclinical disease in endocrinology. Its diagnosis is based on an elevated serum TSH but normal FT 4 levels. The adverse outcome of subclinical hypothyroidism is progressing to overt hypothyroidism with an increased risk for atherosclerosis and myocardial infarction . Two factors may affect the development of overt hypothyroidism: the levels of serum TSH and thyroid autoantibodies and both factors have additional effects on the outcome. The benefits of treatment with T 4for preventing the development of overt hypothyroidism is not yet certain; subclinical hypothyroidism is a strong indicator of risk for atherosclerosis and myocardial infarction, just like other known major risk fators such as hypercholestrolemia, hypertension, smoking and diabetes (odds ratio for aortic atherosclerosis 1.9; odds ratio for myocardial infarction 3.1). The treatment with T 4 for the patients could reduce the levels of serum cholesterol. The effects of subclinical hypothyroidism in pregnant women on the IQ of their children have been widely focused. According to the results from a published epidemiological study, the normal reference values of TSH, thyroid peroxidase antibody (TPOAb) and thyroglobulin antibody (TgAb) detected by immunochemiluminesence assay (ICMA) method and the cut-off values of TPOAb and TgAb associated with thyroid diseases have been proposed.
