RESUMO

Objective To explore the effects of immune-enhanced enteral nutrition support on the clinical outcome and nutritional status of patients with acute stress. Methods From December 2014 to August 2015,120 patients with acute stress were enrolled in this study in the First Affiliated Hospital of Zhengzhou University. The patients were randomly divided into the control group and the experimental group,60 cases in each group.Besides of medical treatment, the control group received routine enteral nutrition, while the experimental group received immune-enhanced nutrition for 8 days. The main parameters related to nutritional status, immune function, blood glucose, infection control of patients were collected. Results No significance of parameters listed above were observed between patients in the two groups at baseline. After intervention, the levels of prealbumin and lymphocyte counts in the experimental group were higher than those in the control group [(162.5±29.7) mg/L and (136.6±15.1) mg/ L,(1.86±0.9) × 109/L and (1.45±0.710) × 109/L,P=0.021 and P=0.012].The levels of C-reactive protein in the experimental group were lower than those in the control group [(47.2±22.1) mg/L and (82.6±13.4) mg/L,P--0.043].Moreover,the level of blood glucose in the experimental group was lower than that in the control group [(5.4±1.7) mmol/L and (6.6±3.5) mmol/L,P=0.009].The patients in the experimental group had better intestinal tolerance (8.3% vs 25%,P=0.014) but lower mortality (6.7% vs 20%,P=0.032) than those in the control group. Conclusions Immune-enhanced enteral nutrition can reduce level of blood glucose and alleviate inflammatory responses of patients with acute stress, thus improving intestinal tolerance, and reducing mortality.

RESUMO

Objective To evaluate the effect of immune enhanced enteral nutrition on immune function in postoperative patients with gastric cancer by using meta-analysis. Methods The databases of PubMed, the Cochrane Library, CNKI, CBM, VIP and Wanfang Data were used to search randomized controlled trials (RCT) about the effect of immune enhanced enteral nutrition and conventional enteral nutrition on immune function in postoperative patients with gastric cancer. The retrieval time span was from inception to Sept 1, 2016. The studies were screened according to the inclusion and exclusion criteria, the data were extracted and the quality was evaluated by 2 reviewers independently. Besides, the meta-analysis was conducted by using RevMan 5.3 software. Results Meta-analysis included 5 RCT, 216 cases in total. The results showed that the immune parameters of immune enhanced enteral nutrition such as IgA (MD=0.18, 95 % CI 0.09-0.27, P0.05). Regarding to cellular factor, the IL-6 level of immune enhanced enteral nutrition was lower than conventional enteral nutrition after 9 days of gastric surgery (MD=-77.40, 95 %CI-112.25 - -42.55, P 0.05). Conclusion Immune enhanced enteral nutrition for postoperative patients with gastric cancer can improve and enhance the immune function under stress, and reduce the excessive inflammatory reaction, which is more conducive to the safety of patients with gastric cancer.

RESUMO

Objective:The aim of this study was to explore the applicative effects of immune-enhanced enteral nutrition(Supportan) in preoperative preparation for patients with colorectal carcinoma.Methods:56 colorectal carcinoma patients with mid-severe malnutrition were randomly divided into two groups, experiment group and control group, each of 23.Experiment group was administrated with immune-enhanced enteral nutrition(Supportan).Control group was administrated with common enteral nutrition(Nutrison Fibre).Nutritional status, immunological function, inflammatory factor and the satisfaction of colon were observed. Results:After enteral nutrition, the plasma albumin,prealbumin,IgG,IgM,CD4+ T cells and the ratio of CD4+/ CD8+ were higher in experiment group than in control group and TNF-?,IL-6 and PGE2 were lower in experiment group than in control group.Conclusions:Immune-enhanced enteral nutrition(supportan) can improve nutritional status, strengthen immunological function, and decrease inflammatory reaction in the colorectal carcinoma patients with mid-severe malnutrition.
