Abstract Psychological empowerment is a construct with a multifaceted character composed of four basic cognitions (impact, competence, meaning, and self-determination), which goes beyond a simple delegation of power. The impact of empowerment in different fields of Psychology has been important due to the proven relations maintained with other well-founded psychological variables in the scientific literature, and because of the positive consequences on the results and well-being of people. Although there are different instruments for the measurement of this construct, the Psychological Empowerment Instrument (PEI) by Spreitzer (1995) has been traditionally the one most employed to measure the psychological empowerment in the area of Organizational Psychology, counting with more than one thousand publications, and articles as well as translations and adaptations to different languages. The original questionnaire elaborated by Spreitzer consists of 16 items organised in four factors following the four basic cognitions that were described above. Therefore, the aim of this article is the adaptation to Spanish, validation of the PEI, and to analyse its factor structure. In order to do so, two independent studies were carried out. The specific objective of the first study was to translate into Spanish and to adapt culturally such questionnaire. In the second study, the objective was to validate the resulting questionnaire and to analyse the factor structure and its goodness and with other psychological and organisational variables as well as the external validity in a sample of workers. In order to do so, in the first study it was carried out an inverse translation and adaptation to Spanish with a sample of 242 university students to subsequently analyse the factorial structure of the items and its reliability. The resulting translated questionnaire confirmed the factor structure in four dimensions (Competence, Meaning, Self-determination, and Impact), although the number of items was reduced from the original 16 to 13, which would explain the 74.08 % of the sampling variance. To confirm the psychometric characteristics of this new questionnaire, we performed a second study with 317 workers of a Spanish service company in order to analyse through confirmative procedures the internal structure of this new instrument, verify its adequacy and goodness of fit of the resulting factors as well as assessing the relation and external validity in a sample of workers and concurrency with other variables (structural empowerment, affective commitment and intention to leave), which are known to have a relation with psychological empowerment from previous research. From the application of this new questionnaire translated to Spanish into this sample of workers, we confirmed again the structure of 13 items in four factors from the first study. Furthermore, we obtained good fit indexes (GFI, AGF, and NNFI), with values between .90 and .96. Additionally, through regression analysis we obtained positive and statistically significant results between psychological empowerment and other variables included in the study (structural empowerment, affective commitment, and intention to leave), testing the interrelations among them. Therefore, those people with a greater cognition on their empowerment developed a greater engagement and attachment towards the organisation, and consequently, their wish to leave was lower. Our study concludes with a discussion about the importance of the adapted instrument and its possible applications.

RESUMO

It is a type of quantitative documentary research of descriptive and exploratory content in which studied the profile of adolescents with self-injurious behavior and the variables of risk and protection regarding the suicidal intent, reported in a Children and Youth Psychosocial Care Center (CAPS IJ) from a metropolitan region in the south of Brazil. Data from 139 assisted adolescents, admitted for self-injury, reported that self-injuries occurred predominantly at home (M=14,36 years; SD=1,63), with multiple episodes, using sharp objects with suicidal intent. The hierarchical binary logistic regression results point out that experiencing abuse in the present -using non-sharps objects- having severe injuries and practicing them in different places are risk variables. Whereas the existence of community and school social support networks are protective factors. There is a necessity of investment in the studies that explore the etiology of self-injuries and that provide support for cases of prevention, detection, and treatment. (AU)

(AU)

RESUMO

In order to improve the motion fluency and coordination of lower extremity exoskeleton robots and wearers, a pace recognition method of exoskeleton wearer is proposed base on inertial sensors. Firstly, the triaxial acceleration and triaxial angular velocity signals at the thigh and calf were collected by inertial sensors. Then the signal segment of 0.5 seconds before the current time was extracted by the time window method. And the Fourier transform coefficients in the frequency domain signal were used as eigenvalues. Then the support vector machine (SVM) and hidden Markov model (HMM) were combined as a classification model, which was trained and tested for pace recognition. Finally, the pace change rule and the human-machine interaction force were combined in this model and the current pace was predicted by the model. The experimental results showed that the pace intention of the lower extremity exoskeleton wearer could be effectively identified by the method proposed in this article. And the recognition rate of the seven pace patterns could reach 92.14%. It provides a new way for the smooth control of the exoskeleton.

RESUMO

Background: The elements of job satisfaction can be categorized into intrinsic and extrinsic factors. The presence of a higher level of intrinsic factors will result in increased motivation amongst employees, whilst extrinsic factors will result in job dissatisfaction. Decreased job satisfaction levels amongst healthcare professionals are known to create an intent to leave. Hence the need to explore these factors amongst radiographers employed by tertiary hospitals in the Gauteng province of South Africa. Objective: To determine the influence of intrinsic and extrinsic factors of job satisfaction on intent to leave amongst radiographers employed by public tertiary hospitals in the Gauteng province. Methods: A quantitative cross-sectional survey guided the study, and a self-administered questionnaire was used to collect data. The sampling technique used for this study was disproportional stratified sampling. Results: The study had a response rate of 62%. A significant number of the participants (50%) were between the ages of 21-33 years. Also, worth noting that 51% of the participants were newly qualified, 28% were employed for a period of 10-20 years and only 20% were employed for a period greater than 20 years. Diagnostic radiography had the most number of participants at 55%, followed by radiation therapist at 24%, nuclear medicine radiographers at 13%, mammography radiographers at 5% and only 3% were sonographers. Pearson's correlation showed a significant negative correlation with the following extrinsic factors: supervision, r= -.344, p=.000; satisfaction with PMDS, r=-.302, p=.000; human resources processes, r=-.249, p=.001; infrastructure, r=-.236, p=.001; the OSD policy, r=-.233, p=.002; satisfaction with remuneration, r=-.202, p=.006; satisfaction with CPD activities, r=-.201, p=.007; and satisfaction with equipment, r=-.163, p=.029. Conclusion: Both intrinsic and extrinsic factors are associated with an intent to leave amongst radiographers employed by public tertiary hospitals in the Gauteng province

RESUMO

Objective:To study the risk factors of positive surgical margins in patients with advanced hilar cholangiocarcinoma (HCCA) undergoing curative-intent resection with the aim to provide references for clinical decision-making.Methods:The clinical pathological data of 126 patients with advanced HCCA who underwent curative-intent resection at the Chinese PLA General Hospital from January 2011 to December 2014 was retrospective analyzed. There were 78 males and 48 females, with an average age of 55 years. The patients were divided into two groups: the resection margin positive group (positive for residual tumor at any surgical margin, n=29) and the negative resection margin group ( n=97). Multivariate logistic regression analysis, in reference to the results of univariate analysis, was applied to the relevant variables to study independent risk factors of positive resection margin. Receiver operating characteristic (ROC) curves were drawn to evaluate the independent and joint predictive values of the relevant indicators. Results:Univariate analysis showed vascular involvement, tumor diameter volume ≥20.94 cm 3, plasma fibrinogen ≥3.36 g/L, and DRR≤0.61 were associated with positive resection margins (all P<0.05). A history of cholecystectomy was potentially associated with positive resection margins ( P<0.1). These variables were included in the multivariate logistic regression analysis which showed vascular involvement ( OR=4.134, 95% CI: 1.545-11.060, P=0.005), tumor size ≥ 20.94 cm 3 ( OR=2.926, 95% CI: 1.107-7.733, P=0.030) and DRR≤0.61 ( OR=3.170, 95% CI: 1.126-8.928, P=0.029) were independent risk factors of positive margins after curative resection in patients with advanced HCCA. ROC curve analysis was used to evaluate the impact of the combination of the above variables in predicting positive surgical margins. Sensitivity and specificity were calculated to be 86.2% and 65.7% respectively, and the area under the curve was 0.771. Conclusion:Vascular involvement, tumor size ≥20.94 cm 3 and DRR≤0.61 were independent risk factors of positive surfical margins in patients with advanced HCCA undergoing curative-intent resection. The combination of the above predictive indicators provided some references for treatment decisions.

RESUMO

Background: The incidence of cancer in India is alarming. However, many patients discontinued their treatment protocols resulting in higher mortality rate. This study aims to find out different patterns of treatment defaults in cancer patients receiving external beam radiation in the radiotherapy department of a cancer center in the southern part of Odisha state. Methods: It is a retrospective epidemiological study carried out in cancer patients receiving external beam radiotherapy for their cancer treatment but had not completed the full course of treatment during the study period from January 2018 to May 2019. Patterns of failure in relation to various demographic and socio-economic statuses were analyzed. Results: One hundred seven defaulter patients were included in this study. The mean age of the patients was 50.93±14.3 years (range 12 to 90 years) and 66(61.7%) were being females. Out of 107 patients, 53.2% patients were receiving treatment in curative intent, 28.0% patients were treated in adjuvant intent and 18.2% in palliative intent. Forty-two (39.2%) patients were from a distance >50 km from the study center, 74.7% of patients belong to low socio-economic status and 60.7% patients were illiterate. Only 33.3% of patients completed more than 15 fractions of radiation excluding the palliative cases. The comparison of various factors such as distance from study center (p=0.759), education (p=0.2428), socio-economic status (p=0.6628) and acute radiation toxicity (p=0.9359) among the groups of patients receiving more or less than 15 fractions of radiation did not showed any statistical significant differences. Conclusions: Radiation induced acute toxicity, distance from study center, education and socio-economic status might be responsible for the treatment discontinuation. There is no particular association of age, disease site and intent of treatment with default among patients.

At present, assistive devices for human movement such as exoskeletons are widely used to solve ergonomic problems in tasks such as repetitive work, rehabilitation, etc., this allows maintaining or improving the level of quality of life of the user which allows new movements or reduces fatigue at the end of a work day. The study and development of these exoskeletons largely requires external parameters (operating conditions and purpose) and internal movement parameters. These requirements and characteristics are fundamental aspects to detect what kind of action you want to perform in the process of body movement. The control of the exoskeletons that are currently used is usually done manually or the reaction to the movement detected, causing problems of delays in the realization of the movement or the discomfort of carrying out the movement, so studies are currently carried out to faithfully identify the action that the user tries to perform and support the movement from before it began. The objective of the project is to identify the intention of movement of people by means of electroencephalography and surface electromyography signals as a starting point for future exoskeleton control methods. As a result of the study, the design and implementation of a system was obtained to obtain, process and identify electrophysiological signals to predict the intention of movement of the lower limbs with success rates greater than 86.66 percent(AU)

RESUMO

Background: Large number of patients with head-and-neck cancer presents with factors such as advanced disease, poor general condition, and associated comorbidities due to which radical treatment is not recommended in these patients. In this scenario, the aim of the present study is to assess the role and feasibility of hypofractionated palliative radiotherapy in these patients. Subjects and Methods: This study was conducted on patients with histopathologically proven cases of squamous cell carcinoma of the head-and-neck region who were surgically unresectable. The quality of life (QOL) was assessed before and after 1 month of radiotherapy using University of Washington Quality of Life questionnaire version 4. All patients received 40 Gy in 10 fractions, twice weekly by two lateral fields covering primary and secondary disease. Response evaluation criteria in solid tumor criteria were used to assess the tumor response. Toxicity was assessed weekly using radiation therapy oncology group criteria. Results: A total of 50 patients were enrolled in this study, out of which 46 completed the planned treatment of 40 Gy in 10 fractions. Common complaints were distressing pain at the primary site (42%), neck swelling (30%), difficult in swallowing (18%), and change in voice (10%). Statistically significant improvements were observed in overall QOL (26.9 ± 9.63 to 55.65 ± 19.28) and none of them experienced Grade IV mucositis or skin toxicity. Good objective response was seen in 82.6% and 84.7% of patients at primary and nodal sites, respectively. Conclusions: This hypofractionated palliative radiotherapy regimen is a good treatment option in patients with Stage IV head-and-neck cancer, who are not fit for treatment with curative intent.

RESUMO

Objective To investigate the status of the nurse work environment, organizational commitment and intent to stay, and explore the relationships among them, ultimately provide some references for the managers. Methods 1 168 registered nurses were recruited from seven hospitals in Zhejiang province. They were investigated with Nursing Work Environment Scale, Organizational Commitment Scale and Intent to Stay Scale. Multiple regression analysis utilized the intent to stay of nurses as the dependent variable, and the demographic data, the dimensions of work environment and dimensions of organizational commitment as the independent variables. Results The mean scores of work environment, organizational commitment and intent to stay were 2.78 ± 0.39, 2.42 ± 0.43, 3.30 ± 0.67, respectively. Among nurse work environment, organizational commitment and intent to stay, there was a significant positive correlation between any two of them. Duty, affective commitment, position of work, sufficient manpower and resources, normative commitment, leadership and ability of manager, gender, shift, housing condition (rent house), employment nature and education are the main factors for the intent to stay, which can explain 40.3% of the variations of nurse retention. Conclusions The results of the study showed that the nurse work environment and intent to work of the 1168 registered nurses were at a medium level, and the organization′s commitment was at a lower-middle level. Managers can refer to the factors affecting the intent of nurses to stay, develop retention strategies, and increase their intent to stay.

Despite multi-sector efforts and continuous quality improvement mechanisms, national newborn screening coverage remained low for several years.</p><p style="text-align: justify;"><strong>OBJECTIVE:</strong> The study determined factors that influence Newborn Screening (NBS) uptake from various perspectives: mothers, health providers, and program administrators.</p><p style="text-align: justify;"><strong>METHODS:</strong> Framework analysis of NBS program documents, 25 focus group discussions and 37 key informant interviews of mothers, health providers and program administrators were done in purposively selected communities in the Cordillera Administrative Region and Region V.</p><p style="text-align: justify;"><strong>RESULTS AND CONCLUSIONS:</strong> Findings showed the need to disseminate correct NBS procedures, especially upon obtaining positive results. Financing issues were addressed innovatively, but system administrators and health providers required a common understanding of program implementation. Monitoring geographically hard-to-reach areas remained a challenge. Barriers outside the system adversely affected filter cards availability, specimen transport, and release of results. Improved online and paper-based educational campaign, greater local government unit support, streamlined PhilHealth processes, a workload-based manpower complement for monitoring, and continuity clinics to handle positive findings can increase NBS uptake.</p>

RESUMO

PURPOSE: This study was performed to measure nursing professionalism, organizational communication, and intent to stay, and investigate the relationship among these three variables in geriatric hospital nurses. METHODS: A cross-sectional survey was done with the participation of 195 nurses working at 23 geriatric hospitals in B city of Gyeongsangnam-do, Korea. Data were collected from January 19 to 25, 2018 using structured questionnaires and were analyzed using SPSS/WIN program. RESULTS: The geriatric hospital nurses' scores for nursing professionalism, organizational communication, and intent to stay were relatively lower than those of general hospital nurses. Each of these three variables was differently perceived according to participants' general characteristics but there was a positive correlation between them. The factors influencing intent to stay were organizational communication, age and through newspapers/advertising (one of motivations for working at geriatric hospitals). These three variables explained 47% of intent to stay. CONCLUSION: To increase intent to stay in geriatric hospital nurses, nursing or hospital managers must develop and apply programs that help improve nurses' professionalism and organizational communication. Especially geriatric hospital nursing managers should try to provide organizational communication enhancement programs for young and inexperienced nurses.

RESUMO

ABSTRACT: Santo Giorno cheese, obtained from raw milk and selected autochthonous starters, is emerging as the newest typical food product from the Southwestern region of Paraná, Brazil. The objective of this study was to evaluate the acceptance of the cheese with two ripening times of 60 and 180 days, produced in two dairy factories, testing two starters and two preservatives. Subjective sensory evaluation was applied using 129 consumers and hedonic scales for the attributes and for purchasing intent. A questionnaire involving the rate of cheese consumption was also used. The results suggested good reproducibility of the cheese preparation by the dairy factories, with no significant differences between the type of preservative used and the suitability of the two types of starter tested. An inverse relationship between hedonic scores for attributes (appearance, color, odor, texture, flavor) and ripening time was observed, except for texture. By applying multinomial logistic regression and a box plot analysis, a significant effect of age on cheese purchasing intent was verified, with a mean acceptance rate of 87.8% for 60 days ageing and 81.8% for 180 days ageing. Results revealed a possible Santo Giorno cheese public of older consumers who had a cheese consuming habit.
