RESUMO

Objective:To evaluate the effects of single spay of L-menthol (NPO-11) on suppressing gastric peristalsis during upper gastrointestinal endoscopy and the influencing factor.Methods:This study was a multicenter, randomized, double-blind, placebo-parallel controlled study. The eligible patients were randomly divided into two groups by randomized blocks. Patients received local spray of either NPO-11 (160 mg L-menthol) or placebo 20 mL during upper gastrointestinal endoscopy. The gastric peristalsis was recorded and evaluated before, 2 minutes after and at the end of endoscopy. The complexity of the procedure was evaluated by the researchers. The influencing factors for antiperistaltic effect of NPO-11 were analyzed.Results:A total of 220 patients were enrolled from five research centers. There were 109 cases in the NPO-11 group and 111 cases in the placebo group. The baseline data of the two groups were similar and comparable. The proportion of patients with grade 1 peristalsis at 2 minutes after the treatment and at the end of endoscopy was significantly higher in the NPO-11 group than that in the placebo group [40.37% (44/109) VS 16.22% (18/111), χ2=15.93, P<0.001]. Compared with the placebo group, the proportions of weak peristalsis (grade 1 and 2) were higher in the NPO-11 group at 2 minutes after the treatment [67.89% (74/109) VS 46.85% (52/111)] and at the end of endoscopy [79.82% (87/109) VS 48.65% (54/111)]. Subgroup analysis showed that the inhibitory effect of NPO-11 on gastric peristalsis was more significant in Helicobacter pylori antibody positive group. Conclusion:Local spray of NPO-11 can effectively inhibit gastric peristalsis during upper gastrointestinal endoscopy, and its effect is more significant in Helicobacter pylori antibody positive group. It could be recommended for no obvious adverse reactions , its safety, and the convenient procedure.

<b>Introduction</b>: Oxaliplatin is a platinum anticancer drug effective for colorectal cancer. However, it causes acute and cumulative peripheral neuropathy in more than half of colorectal cancer patients, enormously impacting their daily lives. While it is important to manage peripheral neuropathy, no management method has yet been established. <b>Case Report</b>: A woman in her 60s underwent SOX therapy for recurrent colorectal cancer and experienced the onset of peripheral neuropathy in early stage of treatment. She did not respond to continuous administration of pyridoxal phosphate hydrate and goshajinkigan. She was thus instructed at the pharmacy to apply ointment containing 1.35% <i>l</i>-menthol 3 times a day at a dose of approximately 0.5 to 1 g to the affected sites on both feet. The symptoms started to show alleviation from the first day of application. She continued applying the ointment for one month until the end of SOX therapy and had no recurrence of symptoms. <b>Conclusion</b>: It was suggested that the application of ointment containing 1.35% <i>l</i>-menthol was effective for ameliorating peripheral neuropathy.

RESUMO

Objective:To establish a GC method for the determination of phenol and L-menthol in glycerin Zhiyang lotions. Meth-ods:A Zebron ZB-WAX(0. 32 mm × 30. 0 m,0. 50 μm) capillary column was used with an FID detector. The column temperature was 60℃, maintained for 1 min, and then raised to 160℃ at the rate of 8℃·min-1 , and maintained 10 minutes. The inlet tempera-ture was 180℃, the detector temperature was 300℃, and the carrier gas was nitrogen. Results:The linear range of phenol and L-men-thol was 0. 5-10. 0 mg·ml-1(r=0. 999 9) and 0. 25-5. 0 mg·ml-1(r=0. 999 9), respectively. The average recovery of phenol and L-menthol was 99. 01%(RSD=0. 90%,n=9)and 99. 70%(RSD=0. 98%,n=9), respectively. Conclusion: The method is sim-ple, accurate and reliable, and can be used to determine the concentration of phenol and L-menthol in glycerin Zhiyang lotions.

AIM: To study the effect l-menthol pretreated nasal cavity on insulin pharmacological bioavailability(PBA). METHODS: To set up diabetic rats model, nasal cavities were pretreated with l-menthol for two hours, insulin was dripped, blood glucose concentrations of different time were determined, and the area over the curve (AOC) and pharmacological bioavailability (PBA) were calculated. RESULTS: After rats nasal cavity was pretreated with different concentrations l-menthol, nasal cavity absorption of insulin markedly increased between 2 h and 3 h, and PBA of insulin nasal cavity absorption increased respectively 5.77, 7.00 and 3.12 times than control group. CONCLUSION: l-Menthol promotes the nasal cavity absorption of insulin.

AIM: To investigate the effect of l menthol on percutaneous absorption of metronidazole through changing the skin character in rabbits. METHODS: Using rabbit skin with different character as transdermal barrier, and l menthol as transdermal enhancer, the absorbance of metronidazole was determined with a two cells diffusion apparatus in vitro, and its permeability was figured out. RESULTS: The transdermal action of l menthol on percutaneous absorption of metronidazole was very evident when the corneum was eliminated; l menthol and azone could increase the desposit function of metronidazole in the intact skin, but had no effect on the skin without corneum. The result also showed that the percutaneous absorption and intact skins desposit function of metronidazole had no difference between l menthol and azone. CONCLUSION: L menthol can increase the percutaneous absorption of metronidazole. The site of the action is mainly the corneum which has desposit function.

Aim The nasal absorption of furosemide and promotion action of l_menthol was examined. Methods In situ nasal recirculation methods was used to study the does_effect relationship of nasal absorption of furosemide and the effect of l_menthol on nasal absorption of furosemide in rats. Results When the concentration of furosemide in circulation liquid was respectively(4.66?0.33) ?(9.76?0.45) and (18.83?1.89) ?g?ml-1,the absorption rate constant k(min-1) of nasal in situ was correspondingly 0.0067?0.0011,0.0134?0.0024 and 0.0163?0.0037;and urine liquid appearing time was (73.33?1.53)?(52.67?7.02) and (47.00?4.58) min.But the larger the concentration of l_menthol, the little the absorption rate constant k. Conclusion Furosemide can be absorbed through the nasal mucous, and l_menthol can promote its absorption.

AIM: To investigate the effect of l menthol on the absorption of ciprofloxacin (Cip) via the mucosa. METHODS: Mucosa absorption experiment and deposit effect were made on a two cells diffusion apparatus in vitro to calculate permeation coefficients. RESULTS: With increasing of the concentration of Cip, the permeation coefficient of mucosa absorption showed a increasing tendency, but deposit effect declined significantly. RESUMO

AIM: To study the effects of l-menthol and its binary systems on percutaneous penetration and deposit effects of 5-fluorouracil (5-FU). METHODS: Percutaneous absorption experiment was performed on the apparatus of isolated skin, penetrated amounts of different times of 5-FU were determined, the cumulate penetrated amounts and permeation rates and increased permeation times were calculated, and the deposit effect of 5-FU was investigated. RESULTS: L-menthol of different concentrations could significantly enhance the percutaneous penetration of 5-FU, its increased permeation times were 1.25 and 1.45 and 1.37 times respectively,and the action of 5-FU with 2% l-menthol was the strongest. 5-FU with 2% and 4% l-menthol had a marked deposit effect. When some kinds of enhancers were used alone or combined with l-menthol, the percutaneous penetration of 5-FU had a significantly increase. CONCLUSION: L-menthol and its binary systems can markedly promote the percutaneous penetration and the deposit effect of 5-FU.

OBJECTIVE:Indomethacinum(Ind) and diclofenac sodium(Dcs) were taken as model drugs to study the enhancing effect of l-menthol on percutaneous absorption of two compounds with different characteristics,and to probe into the effect of combined use of l-menthol with azone.METHODS:Percutaneous penetration experiment was performed on in vitro device and deposit effect was observed with vivoperception.RESULTS:l-menthol could significantly enhance the percutaneous absorption and shorten the lag times(T1ag) of both Ind and Dcs.The enhancing effects of l-menthol,used either alone or in combination with azone,were several times stronger on Ind than on Dcs.Combined use of two enhances was more effective in enhancing penetration and longer in T1ag than single use of l-menthol,especially for Dcs.The study on deposit effect showed that l-menthol could increase the penetration rate than control agent(P
