RESUMO

Objective To observe the clinical efficacy of scalp acupuncture combined with electroacupuncture in the treatment of aphasia after ischemic stroke. Methods Totally 90 patients with aphasia after ischemic stroke were randomly divided into the control group and the treatment group, with 45 cases in each group. The control group received Schuell language training, 30 min each time, while the treatment group received scalp acupuncture plus electroacupuncture (Yifeng, Baihui, Houxi and Tongli), 20 min each time. The treatment for both group was 5 times a week and lasted for 16 weeks. The clinical efficacy was evaluated by aphasia degree, Chinese functional communication profile (CFCP) score and Western aphasia battery (WAB). Results There was statistical significance between the treatment group and the control group in the level of aphasia degree (P<0.05). Compared with before treatment, the total scores of CFCP and WAB in both groups increased, with statistical significance (P<0.05). The total scores of CFCP and WAB in treatment group were higher than control group, with statistical significance (P<0.05). The total effective rate of the therapeutic effect was 91.11% (41/45) in the treatment group and 75.56% (34/45) in the control group, with statistical significance (P<0.05). Conclusion Scalp acupuncture combined with electroacupuncture can obviously improve the language understanding and expression ability and language communication ability of aphasic patients after ischemic stroke.

RESUMO

Army Medical University has taken the mission of medical aiding to African for three years, which requires high needs for the aiding doctors, especially the communication abilities of listening and spoken English. Doctors' English training methods and contents are monotony, training period is short and inefficient in the past. For the aiding doctors, whose English abilities are disturbed by local African accents, may lead to the failure of communication. For the difficulties above, under the new training strate-gies, the communicative and task-based teaching methods through needs analysis theory can be applied for training doctors. Through learning African English accents, doctors take accent English exercises; situation teaching, and case analysis discussion could be taken, so that English communication practices and cross-cultural exchanges can be engaged. Through those ways, the training for aiding African doctors would be effectiveness and diversified, which can also improve doctors' aiding work.

Ohjective To explore the impact of constraint-induced aphasia therapy (CIAT) on language function and neural activity in patients with chronic Broca's aphasia.Methods Two chronic aphasics whose use of language was recovering after standard language therapy were selected to receive 1.5 h of CIAT twice daily for two weeks (30 hours in total).Before and after the CIAT they were tested using a block-designed picture-naming task,fMRIs were taken and their use of language was examined.Results The language function assessments showed relatively large improvements in the subjects' use of language after 4 weeks of conventional language training,but not much further change after 8 and 12 weeks.After the two weeks of CIAT,their language function improved further to a certain extent.fMRI showed increased activation in the left inferior frontal gyrus with or without enhanced activation in the left hemisphere,and reduced activation in the right inferior frontal gyrus.Conclusions Functional reorganization induced in the brain by CIAT was associated with up-regulation of the left inferior frontal gyrus or down-regulation of the right inferior frontal gyrus.Even the whole language network may have been modified.

Objective:To study the scalp electroacupuncture combined with language training on the language function of post-stroke aphasia patients.Method:Ninety patients were divided into experimental group and control group by random number table method.The patients in the experimental group were treated by language rehabilitation training combined with electroacupuncture,while the control group was treated only by verbal training.Schuell stimulation was performed by one to one training and different types of aphasia patients focued on the different types of training.A course of treatment consisted of 2 weeks training with 30mins for each time,5 times a week.For acupuncture treatment,scalp acupuncture points were employed with motor area for motor aphasia,sensory area for sensory aphasia and three language areas for complete language aphasia,also combined with Baihui,wind House,dumb gate and other points.All patients received a course of treatment.The language function was evaluated by using Chinese Aphasia Test and the scores of spontaneous speech,language comprehension,repetition and nomenclature were compared before and after treatment.The degree of aphasia was assessed by the Boston Diagnostic Aphasia Examination (BDAE).The total efficacy was compared by Boston Diagnostic Aphasia.Result:There were significant differences in spontaneous speech,verbal comprehension,repetition,nomenclature and aphasia scores in the experimental group before and after treatment(P=0.01,0.03,0.02,0.02,0.02,P<0.05)There were significant differences(P=0.02,0.03,0.01,0.02,0.03,P<0.05) in the spontaneous speech,verbal comprehension,repetition and nomenclature and aphasia scores in the control group before and after treatment.There were significant differences in spontaneous speech,verbal comprehension,repetition,nomenclature and aphasia scores between the two groups(P=0.03,0.03,0.03,0.02,0.03,P<0.05).There was significant difference in the total curative effect between the two groups(P=0.03,P<0.05).Conclusion:The scalp electroacupuncture combined with language training can significantly improve spontaneous speech,verbal comprehension,repetition and naming degree score and aphasia for post-stroke patients,and has better curative effect than pure language training.

Objective:To explore the application value of transcranial magnetic stimulation combined with language training for children with language retardation.Methods:100 children with language retardation who were treated in our hospital from March 2015 to March 2016 were selected as the research object.They were divided into control group and observation group according to the random number table method,50 cases in each group.The control group was treated with routine language training,while the observation group was treated with transcranial magnetic stimulation combined with language training.The treatment of the two groups were 3 months.The therapeutic effects were evaluated by the language development in Chinese children assessment method and the changes of developmental quotient before and after treatment were evaluated by the neuropsychological development test for children.Results:The effective rate of the observation group was 98.0%,which was significantly higher than 87.0% of the control group (P<0.05).Compared with before treatment,the language development quotient and development quotient of the two groups after treatment for 1,2 and 3 months were significantly improved,and the observation group was significantly better than the control group (P<0.05).The normal rate of the observation group was 80.0%,which was significantly higher than 66.0% of the control group (P<0.05).Conclusion:The effect oftranscranial magnetic stimulation combined with language training is ideal,which can effectively improve the developmental quotient of children with language retardation,and it is worth promoting in clinical practice.

@#Objective To screen the factors that affect severity of language disorder in subcortex aphasia, and analyze its characteristics and therapeutic effect. Methods 10 moderate degree and 12 severe degree subcortex aphasics were evaluated with Chinese Rehabilitation Research Center Aphasia Examination (CRRCAE) and the Boston Diagnostic Aphasia Examination (BDAE) before and 4 weeks after language training. The factors affecting severity of subcortex aphasia and characteristics were analyzed before training, and effect of language training was examined before and after training. Results Severity of subcortex aphasia was related to age and the complication of apraxia of speech. There was a relatively high level in listening comprehension, reading, repeating and reading aloud abilities. After training, all kinds of language abilities improved significantly in moderate degree subcortex aphasics. Whereas, comprehension and verbal communication capabilities increased obviously, but written communication abilities advanced rarely in severe degree subcortex aphasics. Conclusion Age and the complication of aphasia of speech may be important factors influencing severity of language disorder in subcortex aphasia. There are specific clinical features in subcortex aphasia. Almost all language abilities can be improved by language training, but therapeutic effect is differrent between moderate degree and severe degree subcortex aphasics.

@#Objective To observe the clinical effect of acupuncture combined with language training on subcortical aphasia.Methods 32 cases in treatment group and 26 cases in control group were given routine medication,in addition,treatment goup accepted acupuncture and language training.Results After 4 weeks of treatment,patients' language function improved significantly in treatment group compared with that of control(P<0.05).The total effective rate was 87.5% in treatment group,while it was 61.5% in control group(P<0.05).Conclusion Acupuncture combined with language training is effective on subcortical aphasia.

Objective To observe the effect of Xingnao Kaiqiao(Brain-resuscitating)Acupuncture plus language rehabilitation training for motor aphasia caused by cerebral infarction.Methods Totally 90 patients with motor aphasia were randomized into acupuncture plus language training group,single acupuncture group,and language training group(control group),with 30 in each.The Western Aphasia Battery(WAB)was adopted to evaluate the speaking functions of the patients.The aphasia quotient(AQ)of the patients,four basic speaking functions including spontaneous talk,spoken language understanding,retelling,and naming,and communicative ability in daily life(CADL)were observed.Results After treatment,AQ,CADL and score of spontaneous talk,spoken language understanding,retelling,and naming of the three groups were all significantly improved(P0.05).Conclusion Xingnao Kaiqiao Acupuncture plus language rehabilitation training for motor aphasia caused by cerebral infarction is effective with the superiority in improving the ability of spontaneous talk,retelling,naming and CADL.
