RESUMO

Objective: To observe the effects of acupoint sticking therapy of different dosages and durations on the subjective and objective sleep indicators in insomnia patients.Methods: Ninety-six patients with insomnia due to liver-Qi stagnation and spleen deficiency were divided into a high-dosage 7 d group (25 cases), a high-dosage 14 d group (22 cases), a low-dosage 7 d group (21 cases), and a low-dosage 14 d group (28 cases) using the random numbers generated in a stratified and stage-by-stage manner in combination with the visiting sequence. The four groups all received the same acupuncture treatment, but acupoint sticking therapy varied in dosage and duration. Before and after treatment, the actigraphic readings (total time in bed, total sleep time, sleep efficiency, number of wake bouts, length of wakes after asleep, and sleep latency), Pittsburgh sleep quality index (PSQI) score, and symptoms score of traditional Chinese medicine (TCM) were observed. A correlation analysis was conducted among the subjective and objective indicators. Results: The PSQI score was positively correlated with the total time in bed and total sleep time (P<0.05). After treatment, the sleep latency, PSQI scores, and TCM symptoms scores changed significantly in the four groups (P<0.05). The total sleep time and sleep efficiency gained improvements after treatment in the high-dosage 14 d and low-dosage 14 d groups (P<0.05). The high-dosage acupoint sticking groups had longer total sleep time compared with the low-dosage groups of the same treatment duration (P<0.05). After treatment, the length of wakes after asleep, PSQI scores, and TCM symptoms scores were better in the 14 d groups than in the 7 d groups of the same acupoint sticking dosage (P<0.05). Conclusion: Given the same acupuncture treatment, acupoint sticking therapy of different treatment durations produces different effects on the length of wakes after asleep, PSQI score, and TCM symptoms score in insomnia patients, and the 14-day acupoint sticking treatment is superior to the 7-day treatment.

Objective: To observe the efficacy of acupoint sticking with Jianpi Tongjing Zhitong ointment in the treatment of functional dyspepsia due to liver-qi stagnation and spleen deficiency and its effect on serum 5-hydroxytryptamine (5-HT) and ghrelin contents. Methods: One hundred patients with functional dyspepsia due to liver-qi stagnation and spleen deficiency were divided into a treatment group and a control group by the random number table method, with 50 cases in each group. The treatment group received acupoint sticking with Jianpi Tongjing Zhitong ointment and the control group was treated with mosapride citrate orally. Patients were treated for 4 weeks as a course. The therapeutic efficacy was compared after one-course treatment and the differences in gastric emptying rate, and serum 5-HT and ghrelin contents between groups were compared before and after treatment. Results: The total effective rate was 79.6% in the control group and 89.4% in the treatment group, showing significantly different between groups (P<0.05). After treatment, the gastric emptying rate and serum ghrelin content of the two groups increased significantly, and the serum 5-HT content decreased significantly, the intra-group differences were significant (all P<0.01). After treatment, the gastric emptying rate and serum ghrelin content were significantly higher in the treatment group than those in the control group, while the serum 5-HT was significant lower in the treatment group, the inter-group differences were significant (all P<0.05). A negative correlation (r=-0.59) was observed between serum 5-HT content and gastric emptying rate, and a positive correlation (r=0.64) was observed between serum ghrelin content and gastric emptying rate, showing statistical significance (all P<0.01). Conclusion: Acupoint sticking with Jianpi Tongjing Zhitong ointment has a remarkable clinical efficacy in treating patients with functional dyspepsia due to liver-qi stagnation and spleen deficiency and is able to influence the secretion of serum 5-HT and ghrelin. Improving the gastrointestinal motility through the regulation of related brain-gut peptides is suggested as an underlying mechanism for this therapy.

Objective: To observe the clinical efficacy of acupuncture plus medicine in treating laryngopharyngeal reflux due to liver-qi stagnation and spleen deficiency. Methods: A total of 70 patients were divided into a control group and an observation group by the random number table method, with 35 cases in each group. Both groups were treated with conventional medications, and the observation group was treated with additional acupuncture therapy. The reflux symptom index (RSI) and reflux finding score (RFS) were evaluated. Esophageal motility indicators such as lower esophageal sphincter pressure (LESP) and upper esophageal sphincter pressure (UESP), and salivary pepsin level were measured. The clinical efficacy was also compared. Results: The total effective rate of the observation group was higher than that of the control group (P<0.05). After treatment, the RSI and RFS scores in both groups decreased significantly (all P<0.05), and the RSI and RFS scores in the observation group were significantly lower than those in the control group (both P<0.05). There were no significant changes in the LESP and UESP in the control group (both P>0.05), while LESP and UESP in the observation group increased significantly (both P<0.05), and higher than those in the control group (both P<0.05). The salivary pepsin levels in both groups decreased (both P<0.05), and the salivary pepsin level in the observation group was significantly lower than that in the control group (P<0.05). Conclusion: Acupuncture plus medicine can improve symptoms and signs in patients with laryngopharyngeal reflux due to liver-qi stagnation and spleen deficiency, and regulate esophageal motility and salivary pepsin level. Its efficacy is more significant compared with medicine alone.

Objective: To observe the clinical efficacy difference in treating irritable bowel syndrome-diarrhea (IBS-D) of liver-qi stagnation and spleen-deficiency pattern with different treatment protocols, and the effects on serum levels of 5-hydroxytryptamine (5-HT), substance P (SP) and vasoactive intestinal peptide (VIP), for unveiling the mechanism of intradermal needle therapy plus pinaverium bromide in treating IBS-D. Methods: A total of 123 IBS-D patients were divided into an observation group, a Western medication group and an integrated Western and Chinese medication group using the random number table method, with 41 cases in each group. The Western medication group was given oral pinaverium bromide, 50 mg each time and 3 times a day. The integrated Western and Chinese medication group was given additional Chinese herbal medicine Tong Xie Yao Fang, one dose each day. The observation group was given additional intradermal needle therapy on the basis of the Western medication group. The whole intervention lasted for 6 weeks. Before and after treatment, the scores of gastrointestinal symptoms, traditional Chinese medicine (TCM) symptoms, irritable bowel syndrome (IBS) symptom severity scale (IBS-SSS) and IBS quality of life (IBS-QOL) questionnaire, as well as the serum levels of 5-HT, SP and VIP were observed. The clinical efficacy was estimated. Results: The total effective rate was 92.7% in the observation group, 68.3% in the Western medication group and 78.1% in the integrated Western and Chinese medication group. The total effective rate was higher in the observation group than in the other two groups, and higher in the integrated Western and Chinese medication group than in the Western medication group, showing statistical significance (all P<0.05). After treatment, the scores of gastrointestinal symptoms, TCM symptoms and IBS-SSS showed significant decreases in the three groups, presenting statistical significance compared with the baseline (all P<0.05); the scores of gastrointestinal symptoms, TCM symptoms and IBS-SSS were notably lower in the observation group than in the other two groups (all P<0.05), and lower in the integrated Western and Chinese medication group than in the Western medication group (all P<0.05). After treatment, the eight component scores of IBS-QOL showed significant increases in the three groups compared with the baseline (all P<0.05); the eight component scores in IBS-QOL were significantly higher in the observation group than in the other two groups (all P<0.05), and higher in the integrated Western and Chinese medication group than in the Western medication group (all P<0.05). After treatment, the serum levels of 5-HT, SP and VIP decreased markedly in the three groups compared with the baseline (all P<0.05); the serum levels of 5-HT, SP and VIP were significantly lower in the observation group than in the other two groups (all P<0.05), and lower in the integrated Western and Chinese medication group than in the Western medication group (P<0.05). Conclusion: Treatment with intradermal needle therapy plus pinaverium bromide results in significant improvements in the gastrointestinal symptoms and quality of life in patients with IBS-D of liver-qi stagnation and spleen deficiency pattern, and effectively regulates the gastrointestinal hormone production.

Liver-qi stagnation and spleen deficiency syndrome can be seen in a variety of clinical diseases,such as chronic hepatitis,liver cirrhosis,chronic gastritis,peptic ulcer,irritable bowel syndrome (IBS),p.sychosis and so on.Disease characteristics determine the symptom characteristic and criterion of syndrome differentiation and treatment.Therefore,different diseases with liver-qistagnation and spleen deficiency syndrome have different clinical manifestation and diagnostic criteria.This paper summarized the modern biological basis of liver-qistagnation and spleen deficiency syndrome from the nervous system,endocrine system,digestive system,circulatory system,immune system and metabolic system,in order to provide reference for researches on modern biology basis of liver-qistagnation and spleen deficiency syndrome.

Objective To observe the clinical effect on sleep improvement about acupoint application in treating the chronic insomnia of Liver-stagnation and Spleen-deficiency Syndrome.Methods A total of 68 patients with chronic insomnia with Liver-stagnation and Spleen-deficiency Syndrome were randomly and blindly divided into treatment group and control group by registration order. In the treatment process, 1 was eliminated and 33 were completed in the treatment group, 2 were eliminated and 32 were completed in the control group. The treatment group underwentAn Mian Tietherapy half an hour before bed and the control group underwent placebo therapy in the same way of treatment group. Two groups were treated for 40 days and followed-up visit six months. The change of Pittsburgh Sleep Quality (PSQI), Index Insomnia Severity Index (ISI) and TCM Syndromes Scale were detected.Results The clinical total effective rate of treatment group was 72.7% and the control group was 9.4%, and the difference was statistically significant (χ2=46.977,P<0.01). The PSQI scores after treatment (7.55 ± 1.52vs. 13.90 ± 2.44,t=148.165), and follow up 1 month (8.97 ± 2.51vs. 13.17 ± 2.79,t=37.926) in the treatment group were significantly lower than those in the control group (P<0.01). The ISI scores after treatment (7.03 ± 3.37vs. 20.89 ± 4.40,t=73.75), and follow up 1 month (9.81 ± 3.16vs. 19.41 ± 3.66,t=40.79) in the observation group were significantly lower than those in the control group (P<0.01). The TCM Syndromes Scale scores after treatment (2.05 ± 1.09vs. 6.98 ± 1.23,t=17.116), and follow up 1 month (4.06 ± 1.59vs. 6.83 ± 0.91,t=68.055) and follow up 6 month (5.12 ± 1.84vs. 7.19 ± 1.07,t=27.716) in the observation group were significantly lower than those in the control group (P<0.01).Conclusions Acupoint application could obviously change the sleep quality and Chinese medicine symptom in chronic insomnia of Liver-stagnation and Spleen-deficiency Syndrome.

Objective To discuss the effect ofQiqin decoction combined with lamivudine for the patients with hepatitis B and the TCM syndrome of liver stagnation and spleen deficiency and damp heat. Methods A total of 70 patients with chronic hepatitis B were divided into two groups by random number table method, 35 patients in each group. The patients in the control group were treated with lamivudine, and the patients in the observation group used lamivudine combined withQiqin decoction. The HBV-DNA was detected by fluorescence quantitative PCR, and the levels of serum ALT and AST were detected by full automatic biochemical analyzer. The TCM syndrome score of two groups were compared, and the drug resistance in the treatment process was recorded, and the clinical effect rate was evaluated.Results After treatment, the HBV-DNA negative rate of observation group was 77.1% (27/35), and the control group was 54.3% (19/35). There were statistically significant difference of the negative rate of HBV-DNA between two groups (χ2=4.136, P=0.041). The drug resistance rate of the observation group was 11.4% (4/35), and the control group was 31.4% (11/35). There were statistically significant difference of the drug resistance rate between two groups (χ2=4.297,P=0.043). After treatment, the serum ALT (29.1 ± 13.2 U/Lvs. 121.8 ± 50.2 U/L,t=5.982), the AST (28.5 ± 10.9 U/Lvs. 98.3 ± 46.1 U/L,t=8.263), the TCM syndrome scores (4.8 ± 3.1vs. 11.5 ± 7.9, t=5.982) in the observation group were significantly lower than those in the control group (P<0.01). The total effect rate of observation group was 88.6% (31/35), and the control group was 57.2% (20/35). There was significant difference between two groups (χ2=4.248,P=0.003).Conclusions The Qiqin decoction combined with lamivudine could reduce the levels of HBV-DNA, serum ALT levels and AST levels of patients with chronic hepatitis B, and improve the clinical efficacy.

Objective To discuss the common nature of live depression and spleen qi deficiency and liver disease and the changes after the intervention ofXiaoyaopower by the metabonomics method. Methods A total of 40 rats were randomly devided into the normal control group, the liver depression and spleen qi deficiency model group, the liver depression and spleen qi deficiency treatment group, the liver disease group, the liver disease treatment group, 8 in each group. The liver depression and spleen qi deficiency model rat was established by tail-clamping infuriation and Senna gavage method, and the liver disease model rat was established by the CCl4-induced liver injury method. The rats of the liver depression and spleen qi deficiency treatment group and the liver disease treatment group were given 2 g/mlXiaoyao powder, and other groups were given the equal-volume saline. All the treatment lasted 28 days. Twelve hours after the last administration, rat metabolite profiling and contents in the serum were detected by GC-MS, the change of rat liver tissue was observed through HE staining.Results The rats of liver depression and spleen deficiency pattern and liver disease model group appeared liver cell nuclei atrophy, dissolution, degeneration necrosis and lymphocyte infiltration, and at the same time the contents of Glycine, Butanediodic acid, Propanoic acid, Methionine, Glutamine, Phenylalanine in serum were all higher than the normal control group (P<0.05). AndXiaoyao powder could reduce the content of the contents of Glycine, Butanediodic acid, Propanoic acid, Methionine,Glutamine, Phenylalanine in serum (P<0.05).ConclusionXiaoyao powder may play the role of protecting liver by the regulation of one carbon unit, sulfur containing amino acids, aromatic amino acids and amino acid of special metabolism and three tricarboxylic acid cycle.

Objective To evaluate the curative effect of Jiejiu-Hugan decoction combined with polyene phosphatidylcholine capsules on the patients with alcoholic liver disease of liver-qi stagnation and spleen deficiency syndrome. Methods A total of 60 ALD patients were divided by random number table into two groups:combined treatment group (35 cases) and conventional treatment group (25 cases). Both groups received treatment according to the changes of symptomatic treatment. Besides, the conventional treatment group was treated with polyene phosphatidylcholine capsules, and combined treatment group was with both polyene phosphatidylcholine capsules and Jiejiu-Hugan decoction. Treatment last for 3 months. The improvement of liver function, blood lipid and the clinical symptoms and signs were observed. Results The total effective rate of the combined treatment group was 94.28%, whilethe conventional treatment group was 68%. The difference between two groups was statistically significant (χ2=7.255, P=0.007). After the treatment, the levels of AST (52.60 ± 9.48 U/L vs. 63.28 ± 19.92 U/L, t=2.964), TBil (18.91 ± 7.23 U/L vs. 27.41 ± 8.53 U/L, t=4.678) in the combined treatment group were significantly lower than that in the conventional treatment group (P<0.01). And TG (2.70 ± 0.52 mmol/L vs. 3.65 ± 0.74 mmol/L, t=4.809), LDL-C (3.56 ± 0.41 mmol/L vs. 4.20 ± 0.40 mmol/L, t=3.810) in the combined treatment group were significantly lower than that in the conventional treatment group (P<0.01). Conclusions Jiejiu-Hugan decoction combined with polyene phosphatidylcholine capsule can effectively improve ALD patients' liver function;reduce the level of blood lipid. The combined treatment curative effect is better than the only western medicine conventional therapy.

Objective:To observe the clinical effects of acupoint thread-embedding method for non-alcoholic fatty liver disease (NAFLD) in pattern of liver qi stagnation and spleen deficiency. Methods: A total of180 cases in conformity with the diagnostic criteria of NAFLD in pattern of liver qi stagnation and spleen deficiency were randomly divided into a treatment group and a control group, 90 cases in each group, and finally 88 cases in the treatment group and 90 cases in the control group were recruited for statistics. The treatment group was managed by acupoint thread-embedding method, and the control group was treated by oral administration of Polyene Phosphatidylcholine capsules. Both groups were treated for 6 months. Before and after the treatment, liver enzymology, blood fat test, abdominal B ultrasonic examination, and assessment of clinical effects were respectively processed. Results:After treatment, alanine transaminase (ALT), aspartate transaminase (AST), total cholesterol (TC), and triglyceride (TG) decreased in the treatment group, and were significantly different from those in the control group after treatment (allP0.05). After treatment, the improvement degree of fatty liver in the treatment group was more significant than that in the control group (P Conclusion:The acupoint thread-embedding method can reduce the levels of ALT, AST, TC and TG in NAFLD patients, and can improve the pathologic degree of fatty liver and its clinical effects are remarkably better than that of oral medications.
